From trauma to fibrosis: a case of ulnar nerve entrapment in Guyon’s canal

Guyon's canal syndrome is an uncommon distal ulnar neuropathy caused by compression within the wrist. We report a case of delayed ulnar nerve entrapment secondary to post-traumatic fibrosis, diagnosed nine years after wrist injury and surgery. High-resolution ultrasonography demonstrated focal nerve compression within Guyon's canal adjacent to fibrotic tissue. The patient presented with chronic sensory deficits and hypothenar atrophy. Conservative treatment led to partial symptom control. This case highlights delayed post-surgical fibrosis as a potential cause of Guyon's canal syndrome and und

The Risk of Acute Carpal Tunnel Syndrome in Lunate and Perilunate Dislocations.

PURPOSE: Lunate and perilunate dislocations are uncommon wrist injuries that have a high reported association with acute median neuropathy, frequently referred to as acute carpal tunnel syndrome (ACTS). Previous investigations into the incidence of lunate and perilunate dislocation and acute median neuropathy have often been limited by small sample sizes. The purpose of our study was to report a relatively large cohort examining the incidence of ACTS in lunate and perilunate dislocations and assess predictive risk factors. METHODS: A retrospective review was performed of all lunate and perilu

SN-centered triple-network reorganization in carpal tunnel syndrome: A multimodal fMRI study of salience-driven network bias.

Carpal tunnel syndrome (CTS), a median nerve compression disorder, offers a model of chronic afferent deprivation, yet its systems-level impact on intrinsic brain networks remains unclear. We tested a tri-network hypothesis that dysregulated interactions among the salience (SN), sensorimotor (SMN), and default mode (DMN) networks underlie persistent functional impairment in CTS. Resting-state fMRI data were acquired from 52 CTS patients and 30 matched healthy controls. We examined static and dynamic functional connectivity among SN, SMN, and DMN, directional effective connectivity using networ

Comparison of the effects of intensities and sessions of extracorporeal shock wave therapy on nerve regeneration in rats with experimentally induced sciatic nerve injury.

Extracorporeal shock wave therapy (ESWT) has emerged as a promising treatment for neurological disorders but optimal protocols regarding energy density and session frequency remain unclear. This study aimed to evaluate the effects of different ESWT energy levels and treatment sessions on peripheral nerve regeneration in rats with sciatic nerve injury. Fifty rats were randomized into five groups: four experimental groups received ESWT at low energy (0.098 mJ/mm2) or high energy (0.229 mJ/mm2), either once or three times, and one control group received no treatment. Assessments included protein

Carpal Tunnel Syndrome Diagnosis: A Narrative Review of Complementary Roles of Neuromuscular Ultrasound and Electrodiagnostic Studies.

Carpal tunnel syndrome (CTS), the most prevalent entrapment neuropathy of the upper extremities, presents diagnostic challenges due to nonspecific clinical symptoms and discrepancies between subjective experiences and objective findings. Traditional reliance on electrodiagnostic studies (EDX), including nerve conduction studies (NCS) and electromyography, as the traditional reference method for assessing median nerve function is limited by invasiveness, operator dependency, and reduced sensitivity in mild cases. Neuromuscular ultrasound (NMUS) has emerged as a non-invasive complementary imagin

Double Crush Ulnar Nerve Palsy with Vascular Compression Causing First Dorsal Interosseous and Adductor Deficit: A Case Report.

CASE: We present a case of a 30-year-old man with severe ulnar nerve neuropraxia and first dorsal interosseous muscle wasting. Intraoperatively, a large venous plexus compressing the terminal division of the deep motor branch of the ulnar nerve was identified as a contributing factor. CONCLUSION: This is the first documented instance of compression of the terminal branch of the ulnar nerve caused by a venous plexus, resulting in neurapraxia. This rare anatomical variant caused progressive functional impairment and was successfully treated with ligation of vessels distally along with proximal

Development of a machine learning model using structured and unstructured features for predicting surgery among patients with carpal tunnel syndrome: development and validation.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy and often requires surgery when symptoms persist. However, identifying patients requiring surgery remains challenging. This study aimed to develop a predictive model for surgery in CTS patients by integrating standardized common data model (CDM)-based structured data with unstructured data from electromyography (EMG) reports and radiology reports. We retrospectively analyzed 3602 adults diagnosed with CTS at Seoul Metropolitan Boramae Medical Center, of whom 696 (19.3%) underwent surgery within 365 days. Structured CDM data

Morphological-biomechanical dual-cornerstone framework: quantitative ultrasound for electrodiagnostically negative carpal tunnel syndrome.

BACKGROUND: The diagnosis of carpal tunnel syndrome (CTS) is frequently challenging when typical clinical presentations conflict with negative electrodiagnostic (EDX) findings. Quantitative ultrasound (QUS) can detect early neuropathophysiological changes that precede electrophysiological abnormalities; however, its clinical application is limited by the lack of a systematic diagnostic framework. OBJECTIVE: To construct and validate an integrative QUS diagnostic framework for patients with clinically suspected CTS but negative or inconclusive EDX results. METHODS: We conducted a comprehensiv

Hand-arm vibration exposure and outcomes after cubital tunnel surgery.

BACKGROUND: Cubital tunnel syndrome is the second most common upper limb compressive neuropathy after carpal tunnel syndrome. Occupational hand-arm vibration exposure (HAV) has been linked to peripheral nerve compression, though a causal relationship remains unproven. Prior studies have shown that vibration exposure negatively affects outcomes following carpal tunnel surgery, but its impact on cubital tunnel surgery outcomes remains unclear. AIMS: This study aimed to evaluate the effect of previous HAV exposure on patient-reported outcome measures (PROMs) following cubital tunnel release. Sec

Ponseti-Based Hybrid Approach for Bilateral Teratologic Clubfoot With Congenital Peroneal Nerve Dysfunction: A Case Report.

CASE: We report the case of a newborn boy with bilateral clubfoot associated with congenital peroneal nerve dysfunction who responded poorly to the standard Ponseti method. Successful initial correction was achieved through multiple tenotomies of the Achilles, posterior tibialis, flexor digitorum longus, and flexor hallucis longus tendons. Recurrence of equinus deformity without hindfoot varus was treated with posterior soft tissue release, during which all previously transected tendons were found to have completely regenerated. CONCLUSION: This case highlights the value of a Ponseti-based tr

Secondary Carpal Tunnel Syndrome Due to Flexor Tenosynovitis in a Diabetic Patient.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper limb. Although many cases are idiopathic, structural or inflammatory pathology of the flexor tendon sheath can result in secondary median nerve compression. Diabetes mellitus predisposes to such changes due to chronic connective tissue alterations, but secondary CTS arising solely from noninfective flexor tenosynovitis remains uncommon. A 49-year-old woman with recently diagnosed type 2 diabetes presented with a 2-month history of progressive right-wrist pain, paresthesia, and volar swelling. Magnetic resonance

Rare Common Peroneal Nerve Neuropraxia after Removal of Upper Tibial Pin Traction.

Common peroneal nerve (CPN) injury is an uncommon complication associated with skeletal traction. Neurological deficits occurring after removal of upper tibial pin traction (UTPT) are exceedingly rare and sparsely reported in the literature. A 27-year-old male sustained a closed left subtrochanteric femur fracture following a road traffic accident. Preoperative UTPT was applied with no immediate neurological deficit. UTPT was removed intraoperatively before definitive fixation. Postoperatively, the patient developed left-sided foot drop. Electromyography and nerve conduction velocity studies a

Schwann Cells in Development, Regeneration, and Therapeutic Applications: Insights From the Sciatic Nerve Model.

The sciatic nerve is the largest nerve in the human body and is frequently affected by lesions, causing pain and even loss of sensitivity. The sciatic nerve injury model provides a robust platform to study the degeneration-regeneration process. In this context, Schwann cells (SC) are central regulators of peripheral nervous system (PNS) development, homeostasis, and regeneration. During development, SC arise from neural crest-derived precursors and undergo tightly regulated lineage progression, culminating in myelinating and non-myelinating phenotypes. This process is orchestrated by coordinat

InSafeLOCK intramedullary nailing versus plate fixation for humeral diaphyseal fractures: a retrospective cohort study with 24-month outcomes.

OBJECTIVE: This study evaluated, in humeral diaphyseal fractures, how the InSafeLOCK intramedullary nail (IMN) compares with limited-contact dynamic compression plating (LC-DCP) in terms of radiologic and functional outcomes. The InSafeLOCK incorporates an internal, unicortical distal-locking feature intended to streamline distal fixation and potentially reduce neurovascular risk; however, its clinical performance has not previously been directly compared with conventional plating. METHODS: We retrospectively reviewed 93 adults who underwent surgery for humeral diaphyseal fractures between Jan

Anti-GD1b Disorder Presenting as Multiple Mononeuropathies.

Isolated anti GD1b disorders are quite rare. They usually present with ophthalmoplegia, sensory ataxia, paraproteinemia, and elevated cold agglutinants. The response to immunosuppressant treatment does not seem to be very effective. We report a case of isolated mononeuropathies associated with isolated anti-GD1b antibodies, without ataxia or paraproteinemia with dramatic response to short course of steroid treatment.

Afferent-driven cortical remodeling: multimodal neuroimaging insights into carpal tunnel syndrome-related plasticity.

Carpal tunnel syndrome (CTS), the most common peripheral entrapment neuropathy, often leads to persistent hand dysfunction. Given the dual role of the human hand as both a motor effector and a cognitive interface, understanding network-level brain remodeling is crucial for developing effective rehabilitation strategies. This cross-sectional study investigated CTS-related cortical reorganization using resting-state functional magnetic resonance imaging (fMRI) in 52 patients with bilateral CTS and 30 matched healthy controls. All participants underwent standardized clinical and neuroimaging eval
