BACKGROUND: Mononeuritis multiplex (MM) is a severe and clinically heterogeneous form of peripheral neuropathy, most commonly arising in the context of systemic vasculitis in rheumatology practice. Despite its potential to cause substantial functional impairment, data on its clinical spectrum, management, and outcomes remain limited.
OBJECTIVES: This study aimed to comprehensively evaluate the clinical characteristics, underlying etiologies, treatment approaches, and outcomes of MM in a nationwide multicenter rheumatology cohort.
DESIGN: Retrospective, multicenter observational study.

Eosinophilic granulomatosis with polyangiitis (EGPA) frequently presents with asthma and sinusitis, making otorhinolaryngologic manifestations potential early indicators of systemic vasculitis. However, these manifestations are often misdiagnosed as common inflammatory diseases. We report two cases of EGPA diagnosed based on distinct rhinologic presentations. The first case involved a 52-year-old woman who developed mononeuritis multiplex and cutaneous vasculitis following surgery for nasal polyps, which ultimately led to a definitive diagnosis of EGPA. The second case involved a 39-year-old w

Microscopic polyangiitis (MPA) is a primary systemic small-vessel vasculitis associated with antineutrophil cytoplasmic antibodies (ANCA). The condition is uncommon among the UK population, although it increases with age. MPA manifests heterogeneously and may affect any organ, although glomerulonephritis is almost always present. This case report describes a rare presentation of MPA with isolated mononeuritis multiplex, without glomerulonephritis or other hallmark features, and reflects the varied manifestations of MPA and challenges in recognising the condition. Awareness of MPA is essential

Eosinophilic granulomatosis with polyangiitis (EGPA) is an antineutrophil cytoplasmic antibody-associated vasculitis, affecting various organs. Although ocular involvement occurred in 6.8-11.1% of patients, anterior ischaemic optic neuropathy (AION) is rare. We present the case of a 57-year-old woman with an 11-year history of asthma who developed sudden, painless visual loss in her left eye. On admission (3 days after symptom onset), visual acuity was counting fingers in the left eye and 20/20 in the right eye. Funduscopy showed left optic disc oedema. Magnetic resonance imaging revealed intr

Giant cell arteritis is the most common vasculitis in those over 50 years old with a peak incidence between 70 and 80. Symptoms include a new-onset headache, scalp tenderness, jaw/tongue claudication, and visual symptoms, which can be sight-threatening. We report a case of an 82-year-old lady who presented with typical symptoms of giant cell arteritis and bilateral halo sign on an ultrasound scan. Despite initial steroid treatment she deteriorated and developed neurological symptoms including mononeuritis multiplex. Following further investigations, it was found that she was suffering from gra

UNLABELLED: Background/Objectives: Nontuberculous mycobacterial pulmonary disease (NTM-PD) has been associated with myeloperoxidase anti-neutrophil cytoplasmic antibody (MPO-ANCA)-positive ANCA-associated vasculitis (AAV). However, clinical factors associated with MPO-ANCA positivity in patients with NTM-PD remain unclear. This study determined the frequency of MPO-ANCA positivity among tested patients with NTM-PD and explored the clinical factors associated with MPO-ANCA positivity.
METHODS: This study retrospectively analyzed patients aged ≥15 years old with NTM-PD who were treated at Tokyo

Systemic relapse remains a major cause of morbidity in eosinophilic granulomatosis with polyangiitis (EGPA), but widely available prognostic markers are lacking. We retrospectively assessed 83 EGPA patients with rheumatoid factor (RF) tested at diagnosis. Systemic relapse was defined as new or worsening systemic disease activity and treatment escalation, excluding isolated asthma and rhinosinusitis exacerbations. RF-positive patients (45.8%) had higher C-reactive protein levels, more frequent mononeuritis multiplex, and more frequent systemic relapses (39.5% vs. 2.2%, q = 0.002). In the penali

BACKGROUND: Vasculitic neuropathy (VN) is a diverse group of immune-mediated peripheral neuropathies that result in systemic or non-systemic vasculitic neuropathy. There are limited studies that characterize the neurological outcome of VN patients. We aimed to describe the clinical features, diagnostic workup, and neurological outcomes of VN patients.
METHODS: We retrospectively reviewed adults with VN evaluated at the Hospital of the University of Pennsylvania between October 2020 and December 2025. Diagnostic classification followed the Peripheral Nerve Society 2010 criteria and the 2022 Am

Pustulotic arthro-osteitis (PAO) is a chronic inflammatory disorder associated with palmoplantar pustulosis (PPP) and characterised by osteoarticular manifestations. Although several immune-mediated comorbidities have been reported in association with PPP/PAO, polyarteritis nodosa occurring in patients with PPP/PAO has not been described previously. We report a case of a 56-year-old woman with a long-standing history of PPP and PAO who developed worsening osteoarticular symptoms in close temporal proximity to purpura, lower-extremity pain, and sensory disturbance, followed by paraesthesia in t

PURPOSE: Deepwater Horizon oil spill (DWHOS) responders potentially experienced multiple stressors. We examined associations between DWHOS deployment-related stressor patterns and neurological and endocrine/metabolic conditions among U.S. Coast Guard active duty responders who completed post-deployment surveys (n = 3102).
METHODS: Deployment-related stressor patterns were identified using principal component analysis of self-reported stressors. Neurological and endocrine/metabolic outcomes were classified from military health encounter records. Associations between principal components (PCs)

Delayed neurological toxicity, distinct from acute immune effector cell-associated neurotoxicity (ICANS), is an emerging complication of ciltacabtagene autoleucel (cilta-cel), a B-cell maturation antigen (BCMA)-targeting CAR T-cell therapy. Peripheral nerve (PN) involvement has been described as cranial nerve palsies or Guillain-Barré-like syndromes, yet the underlying mechanisms remain poorly defined. We describe a case of asymmetric, immune-mediated multifocal PN involvement, clinically resembling autoimmune, T-cell-mediated mononeuritis multiplex (MNM), occurring after cilta-cel therapy. Se

BACKGROUND: Mononeuritis multiplex is a recognized, though sometimes overlooked, clinical feature of myeloperoxidase antineutrophil cytoplasmic antibody (MPO-ANCA)-associated vasculitis. When renal or pulmonary involvement is initially mild, the condition may mimic common neurological disorders and diagnosis can be delayed.
CASE PRESENTATION: A 51-year-old woman presented with a five-month history of progressive bilateral lower-limb pain and edema, followed by three weeks of rapidly progressive asymmetric weakness affecting all four limbs. Nerve conduction studies showed severe multifocal axo

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) is an anti-neutrophil cytoplasmic antibody (ANCA)-associated systemic vasculitis characterized by asthma, eosinophilia and inflammation of small-tomedium vessels. Although respiratory and neurological manifestations are predominant, cardiovascular involvement and ischemic complications may significantly affect the prognosis.
CASE PRESENTATION: We report a case of a 37-year-old woman, diagnosed with EGPA based on a history of bronchial asthma, marked eosinophilia, mononeuritis multiplex progressing to sensorimotor polyneuropathy,

BACKGROUND: Pure neuritic leprosy (PNL) is a distinct form of leprosy characterized by peripheral nerve involvement without skin lesions. Diagnosis is often delayed, and 'silent neuritis' contributes to progressive and irreversible nerve damage. Despite appropriate therapy, many patients develop long-term disability. This study describes the clinical profile, diagnostic patterns, therapeutic outcomes, and predictors of disability in biopsy-confirmed PNL.
METHODS: We conducted a retrospective observational cohort study at a tertiary referral centre in South India, enrolling biopsy-confirmed PN

A 62-year-old man with a history of diabetes mellitus was hospitalised with numbness of lower limbs, bullous lesions of the whole body, kidney dysfunction, presence of eosinophils, and elevated antineutrophil cytoplasmic antibodies to myeloperoxidase and anti-bullous pemphigoid 180 antibodies and was diagnosed with mononeuritis multiplex. Kidney and muscle biopsies showed vasculitis with fibrinoid necrosis, whereas skin biopsies showed only blister formation between the epidermis and dermis; a high eosinophilic infiltrate was present in all three tissues. These findings led to a diagnosis of e

Rapidly progressive neurologic symptoms can be alarming to both the patient and the clinician, given the concern for serious pathologies. In this case, a 67-year-old patient presents with painful sensorimotor symptoms that progress rapidly within days of onset. Proper localization and consideration of medical co-morbidities is essential for accurate diagnosis. This case highlights the importance of a detailed neurologic examination to aid in the localization and differential diagnosis. The case will take readers through a stepwise approach to this complex neurologic presentation, work-up and m

A 33-year-old man had 1 month of right-sided headache with multiple right-sided cranial neuropathies. He had a history of recurrent sinusitis, elevated inflammatory markers and positive cytoplasmic anti-neutrophilic cytoplasmic antigens (c-ANCA) anti-proteinase 3 (anti-PR3). MR scans of the brain and cervical spine identified an inflammatory mass at the skull base with hypertrophic pachymeningitis and a sigmoid sinus thrombosis. Cerebrospinal fluid examination excluded infection and a meningeal biopsy confirmed vasculitis. CT scan of the chest showed multiple lung nodules with cavitation. We g

This systematic review comprehensively analyzes the neurological complications associated with Sjögren's Syndrome (SS), focusing on peripheral neuropathy, central nervous system (CNS) involvement, cognitive dysfunction, and autonomic dysregulation. Eight studies, published between 2010 and 2024, were meticulously selected, encompassing a range of study designs, patient populations, and diagnostic methodologies. The findings highlight the substantial burden of neurological manifestations in SS, with peripheral neuropathy identified as the most prevalent complication, followed by cognitive impai

We present the case of an ANCA+ EGPA patient with a long disease history and remission for over 8 years (between 2015 and 2023), who developed a life-threatening relapse marked by diffuse alveolar haemorrhage and mononeuritis multiplex despite low dose mepolizumab (100 mg q4w) maintenance therapy. She was treated with pulse dose corticosteroids, rituximab, and mepolizumab at an increased dose of 300 mg q4w. This treatment regimen led to clinical and serological improvement, but she has persistent neuropathic pain and reduced handgrip strength. Use of low dose mepolizumab in high risk EGPA pati
