A Case of Challenging Polyarteritis Nodosa With Critical Limb Ischemia.

Polyarteritis nodosa (PAN) is a systemic vasculitis with necrotizing inflammation of medium and small-sized arteries. Critical limb ischemia is a rare complication of PAN. We present a case report of a 64-year-old male who presented initially with constitutional symptoms, cutaneous lesions and bilateral orchitis. After admission, he developed critical ischemia of his right lower limb with milder ischemia in his other limbs. After failure to respond to intravascular thrombolysis, he ultimately required a right below-knee amputation. He had symptoms and signs of mononeuritis multiplex and devel

Pure Neuritic Leprosy In Children With Grade 2 Disability On Hands And Feet: A Rare Case And Literature Review.

Pure neuritic leprosy (PNL) is a rare clinical variant of leprosy, especially in children. This variant of leprosy only involves peripheral nerves without skin involvement or acid-fast bacilli (AFB) found on slit-skin smear examination. The challenging diagnosis and establishment of PNL due to the absence of skin lesions often lead to disability. A case of PNL with grade 2 disability of hands and feet in a 16-year-old boy was reported. The patient has been living in a leprosy-endemic area and has a history of contact with people who have symptoms of leprosy. On physical examination, there were

Eosinophilic Granulomatosis With Polyangiitis Presenting as a Mononeuritis Multiplex Mimicking Guillain-Barré Syndrome.

A 41-year-old Malay woman with a background of allergic rhinitis, nasal polyps, and eczema presented with bilateral foot numbness and weakness. Neurological examination revealed bilateral foot drop and sensory loss over the L5-S1 dermatomes. Nerve conduction studies showed bilateral peroneal and tibial axonal neuropathy. She was initially diagnosed with Guillain-Barré syndrome (GBS) and treated with intravenous immunoglobulin (IVIG), but showed no clinical improvement. Further evaluation revealed pansinusitis on CT imaging, vasculitic rashes, marked eosinophilia of 8.7 × 10⁹/L, and positive p-

Eosinophilic Granulomatosis With Polyangiitis Presenting as Mononeuritis Multiplex in a Diabetic Patient: Diagnostic and Therapeutic Challenges.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare antineutrophil cytoplasmic antibody (ANCA)-associated small-vessel vasculitis that may present with asthma, eosinophilia, pulmonary involvement, and peripheral neuropathy. Distinguishing EGPA-related vasculitic neuropathy from diabetic neuropathy can be particularly challenging in patients with long-standing diabetes mellitus. We present a case of a 64-year-old man with type 2 diabetes, chronic kidney disease, and asthma, who developed systemic symptoms, mononeuritis multiplex, and pulmonary haemorrhage. Laboratory tests revealed m

EGPAware: a European Delphi consensus study on red flags for suspicion of eosinophilic granulomatosis with polyangiitis.

OBJECTIVES: Diagnosing eosinophilic granulomatosis with polyangiitis (EGPA) is challenging due to its rarity, multisystem involvement, and varied clinical presentation, often resulting in delayed diagnoses and associated complications. EGPAware project aimed to establish a first European Delphi consensus on red flags for raising suspicion of EGPA to assist the prompt identification, referral, and diagnosis of these patients. METHODS: EGPAware was a European Delphi study led by a multidisciplinary committee that developed the survey based on the red flags from a previously published Spanish sy

Microscopic Polyangiitis With a Twist: A Rare Case of CRAO and Brainstem Infarction.

Microscopic polyangiitis (MPA) is a rare autoimmune vasculitis of small vessels. It is often linked with ANCA. Central retinal artery occlusion (CRAO) is characterized by sudden, painless vision loss and is typically connected with embolic or inflammatory causes. However, limited literature exists on its association with MPA. This case report uniquely highlights a 50 year old patient diagnosed with MPA exhibiting CRAO and brainstem infarct. The report stresses the importance of early recognition of uncommon vascular manifestations, multidisciplinary diagnostic approaches, and the integration o

Isolated Pectoralis Major and Tricep Atrophy Secondary to Neuralgic Amyotrophy.

Neuralgic amyotrophy (NA), or Parsonage-Turner syndrome, describes a spectrum of relatively rare peripheral neuropathies characterized by acute pain followed by patchy, multifocal sensory disturbances, weakness, and atrophy, predominantly in the upper extremity. While typically affecting the upper brachial plexus, NA can also present with selective involvement of distal nerve branches. This case report details a unique presentation of NA in a 39-year-old male bodybuilder who developed isolated weakness and atrophy of the left pectoralis major and a single head of the left triceps following a p

Spontaneous accessory renal artery aneurysm rupture as a first presentation of polyarteritis nodosa: a case report and review of literature.

INTRODUCTION: Polyarteritis nodosa (PAN) is a systemic necrotizing vasculitis primarily affecting medium-sized vessels. It has several clinical manifestations, including renal, gastrointestinal, cutaneous, neurologic, and general symptoms, but it is not associated with pulmonary manifestations. PAN mainly affects individuals aged 40-60 years, with a male predominance. Although the underlying cause of this disease remains unclear, several triggers can be associated with it such as hepatitis B virus. Diagnosis typically requires an organ biopsy or angiography revealing microaneurysms or stenotic

Rhinocerebral Mucormycosis in a Patient With Diabetes: A Rare but Critical Infection in the United Arab Emirates.

A middle-aged man with no chronic medical conditions presented to the emergency department with flu-like symptoms, breathlessness, and vomiting. Arterial blood gas analysis revealed high blood glucose and an elevated anion-gap metabolic acidosis, suggestive of diabetic ketoacidosis, prompting the initiation of treatment. Neurological examination revealed multiple cranial nerve palsies with cranial mononeuritis multiplex, raising suspicion of rhino-orbito-cerebral mucormycosis. Liposomal amphotericin B was started empirically. Nasal endoscopy and biopsy, along with microbiological evidence, con

IgA Vasculitis With Nephritis Following Controlled Ovarian Stimulation and Oocyte Donation.

A woman in her 20s developed IgA vasculitis with nephritis (IgAVN), which started four days after oocyte donation (for in-vitro fertilisation (IVF) and transfer to another recipient). Prior to oocyte retrieval, the patient received a course of controlled ovarian stimulation (COS) with menotrophin, ganirelix acetate and choriogonadotropin alfa. Four days after COS, she developed a painful purpuric rash alongside knee pain and swelling. Prednisolone was started for presumed IgA vasculitis. Soon after, she developed de novo mononeuritis multiplex and renal dysfunction characterised by haematoprot

A Diagnostic Challenge in Progressive Limb Weakness: A Case of Myasthenia Gravis With Atypical Distal and Cranial Nerve Involvement.

Myasthenia gravis (MG) is an autoimmune neuromuscular disorder commonly characterised by fatigable muscle weakness, often presenting with ocular symptoms and proximal weakness, though atypical presentations can also occur, especially in patients with overlapping autoimmune conditions. Prompt diagnosis is essential to initiate effective treatment and prevent complications. We report a case of a 39-year-old male with type 1 diabetes mellitus (T1DM) who presented with a six-week history of progressive limb weakness, imbalance, and intermittent diplopia. Examination revealed bilateral distal upper

[Eosinophilic small vessel vasculites (EGPA) a hidden severe asthma comorbitity].

A 72-year-old woman presented to the emergency department due to worsening dyspnea. She had been diagnosed with asthma a year earlier. At arrival, her oxygen saturation was only 84%. During lung auscultation, wheezing was noted over all lung fields. A blood test showed a significant increase in eosinophils in peripheral blood, highest value of 1.4 x 10E9/L. Further investigations in the respiratory ward showed a positive MPO-ANCA, which, together with clinical features of asthma, chronic rhinosinusitis with polyps, mononeuritis multiplex and eosinophilia, led to the diagnosis of eosinophilic g

IgG4 disease with multiorgan involvement: a case report.

BACKGROUND: IgG4-related disease (IgG4RD) is a rare fibroinflammatory disease with multiorgan involvement. It presents insidiously over several years and can be a diagnostic enigma. Delays in diagnosis occur due to failure to consider IgG4 as a differential diagnosis, atypical presentations, and an insidious clinical course. CASE PRESENTATION: We report the case of a 70-year-old Sri Lankan man with pulmonary, renal, and neurological involvement of IgG4-related disease. Clinical manifestations evolved over a 4-year period and included exertional shortness of breath and dysesthesia of extremiti

Idiopathic membranous nephropathy and synchronous mononeuritis multiplex secondary to idiopathic small vessel vasculitis.

Membranous nephropathy has been associated with demyelinating polyneuropathies and antiglomerular membrane disease; however, an association with vasculitic neuropathy has not been described. This case describes a patient with biopsy-proven idiopathic membranous nephropathy and synchronous mononeuritis multiplex secondary to idiopathic small vessel vasculitis, who presented with lower limb microvascular ischaemia, peripheral neuropathy and active urinary sediment. Her extensive non-invasive screening for immunological disease and radiological investigations for occult malignancy were unremarkab

From periphery to center, untold story of pure neuritic leprosy: an electrophysiological study.

BACKGROUND: Pure neuritic leprosy (PNL) is uncommon form of leprosy involving peripheral nerves. Some isolated case reports have shown imaging changes in the central nervous system (CNS) and also impairment in visual evoked potential (VEP), somatosensory evoked potential (SSEP) and brain stem auditory-evoked potentials (BAEPs) parameters in PNL, but there is lack of large study. This prospective observational study evaluates impairment in these central conduction studies among PNL patients. METHODS: We screened patients with leprosy presenting with features of neuropathy and/or thickened nerv

Eosinophilic Granulomatosis With Polyangiitis Gradually Worsening With Consecutive Pregnancies: A Case Report.

This case report elucidates the diagnosis and management of eosinophilic granulomatosis with polyangiitis (EGPA), a form of systemic vasculitis, in a 32-year-old female presenting with progressive dermal, respiratory, and gastrointestinal symptoms following multiple pregnancies. The patient's history of allergic reactions and pregnancies suggested a gradual progression of EGPA, a condition rarely associated with pregnancy. Initial symptoms were misinterpreted as allergic reactions and acute gastroenteritis, delaying the correct diagnosis. Laboratory findings included eosinophilia and elevated

Eosinophilic Granulomatosis With Polyangiitis Presenting With Purpura, Mononeuritis Multiplex, and Cerebral Infarction.

We present a case of a 52-year-old woman who had transient speech impediment and progressive numbness, weakness, and a purpuric rash affecting her limbs, with severe joint pains. Because she had a chest infection two weeks prior, her clinical presentation gave rise to a suspicion of post-infective vasculitis or post-infective polyneuritis. Further investigation proved this to be eosinophilic granulomatosis with polyangiitis (EGPA) presenting with purpura, mononeuritis multiplex, and cerebral infarction. Treatment with glucocorticoids and cyclophosphamide led to rapid remission. This case highl

A Rare Presentation of Non-systemic Vasculitic Neuropathy Mimicking Guillain-Barré Syndrome: A Case Report.

Vasculitic neuropathy typically presents as a painful, asymmetrical sensory-motor polyneuropathy, more commonly demonstrating a mononeuritis multiplex. We present the case of a 63-year-old woman who experienced acute-onset flaccid weakness in all four limbs following an episode of diarrhea. Guillain-Barré syndrome (GBS) was considered, which supported acute motor axonal neuropathy (AMAN) in the nerve conduction study (NCS). On the second day of treatment with intravenous immunoglobulin (IVIG), a vasculitic-type rash appeared along with limb pain. Furthermore, the asymmetrical sensory and motor
