Simultaneous Carpal Tunnel Release With Forearm Fasciotomy: Are We Doing Enough?

BACKGROUND: Simultaneous carpal tunnel release (sCTR) with emergent forearm fasciotomy in the setting of acute compartment syndrome (ACS) has been reported, but the national incidence and cost of delayed carpal tunnel release (CTR) remain unknown. This large database study aims to evaluate how often sCTR is performed, the frequency of delayed CTR when not performed simultaneously, and the associated costs. METHODS: The PearlDiver Mariner database was queried for patients who underwent forearm fasciotomies between January 1, 2015, and October 31, 2022. Patients were grouped based on CTR timing

Changes in peripheral sensory afference do not alter predictive motor planning: evidence from carpal tunnel syndrome.

Anticipatory control organises motor output prior to predictable perturbations and is expressed in multi-digit tasks as anticipatory synergy adjustments (ASAs), which coordinate digit forces before movement onset. Whether such feedforward coordination depends on peripheral sensory input remains unclear. Carpal tunnel syndrome provides a model of altered median nerve afference with within-subject restoration following surgical decompression. We quantified ASA onset and amplitude in eleven individuals with carpal tunnel syndrome performing a multi-finger grasp-and-release task before and three w

Ulnar neuropathy at the elbow: from diagnosis to rehabilitation.

BACKGROUND: Ulnar neuropathy at the elbow (UNE) is the second most common entrapment neuropathy of the upper limb, involving physical medicine and rehabilitation, neurology, orthopedics, and plastic surgery. It results from static or dynamic ulnar nerve instability, cubital tunnel syndrome, or space-occupying lesions. Accurate diagnosis requires thorough history, clinical examination, and selection of appropriate diagnostic tests. SEARCH STRATEGY: PubMed/MEDLINE, Scopus and Web of Science were searched (January 2000-August 2026) using combinations of "ulnar neuropathy", "cubital tunnel syndro

Fibrolipomatous Hamartoma of the Median Nerve Misdiagnosed as a Palmar Space Abscess: A Case Report.

BACKGROUND: Fibrolipomatous hamartoma (FLH), also categorized under lipomatosis of nerve, is a rare, is an uncommon benign fibrofatty lesion of the peripheral nerves that most commonly affects the median nerve. Its slow progression and atypical presentation often lead to misdiagnoses and improper surgical management. CASE PRESENTATION: A 17-year-old female presented with a 5-year history of progressive left-hand swelling and paraesthesia and a 2-year history of pain and secondary reduction in grip strength. The mass was previously misdiagnosed as an acute mid-palmar space abscess, which resul

Gelsemine attenuates neuropathic pain in a rat model of sciatic nerve chronic constriction injury.

ObjectiveThe effect of Gelsemine on neuropathic pain was investigated using a sciatic nerve (SN) chronic constriction injury (CCI) rat model.MethodsNetwork pharmacology analysis was performed to identify potential molecular targets of Gelsemine associated with neuropathic pain, followed by molecular docking analysis of the identified targets. Neuropathic pain was induced in male rats by SN-CCI. The rats were divided into four groups: sham, CCI, Gelsemine 2 mg/kg, and Gelsemine 4 mg/kg. Gelsemine (2 or 4 mg/kg, i.p.) was administered to the treatment groups for 30 consecutive days. The effect o

Ultrasound-Guided Treatment of Peripheral Entrapment Mononeuropathies.

High-resolution neuromuscular ultrasound (US) is a valuable tool for the localization, diagnosis, and provision of conservative treatment to peripheral entrapment mononeuropathies. US-guided interventions require careful coordination of transducer and needle movement along with a detailed understanding of sonoanatomy. Preprocedural planning and positioning are instrumental in performing these interventions and should not be overlooked. Minimally invasive procedures, such as corticosteroid injections, aspiration of ganglia, orthobiologics, and hydrodissection, are useful options for the nonsurg

Clinical outcomes of low versus high intensity laser therapy in patients with subacute carpal tunnel syndrome: A randomized, single-blinded, controlled trial.

A common entrapment of median nerve that causes pain, numbness, and functional impairment is called Carpal Tunnel Syndrome (CTS). Laser therapy is a noninvasive intervention for neural repair and pain modulation, but comparative evidence between Low-Level Laser Therapy (LLLT) and High-Intensity Laser Therapy (HILT) in subacute CTS is limited. The aims of this study were to compare the effects of LLLT and HILT on pain, functional status, electrophysiological, and ultrasonographic parameters in subacute CTS. A single-blinded randomized controlled trial (Reg. No. IRCT20241111063665N1) was conduct

Surgery to treat autonomic dysreflexia due to cubital tunnel syndrome in cervical spinal cord injury: a case report.

INTRODUCTION: Autonomic dysreflexia (AD) is a potentially fatal syndrome in people with spinal cord injury (SCI) marked by episodic hypertension with flushing, diaphoresis, and bradycardia. Common etiologies include bladder distension and bowel impaction. Compressive neuropathies lack attention as a potential precipitant of AD. This report describes an individual with cervical SCI and with recurrent AD treated with ulnar nerve decompression surgery. CASE PRESENTATION: A 74-year-old woman with a remote history of C6 SCI (American Spinal Injury Association Impairment Scale Grade B) presented to

Swimming Exercise Ameliorates Neuropathic Pain and Is Associated with Hippocampal H19, miR-22, and Wnt/β-Catenin Signaling.

Physical activity is known to ameliorate neuropathic pain (NP); however, the underlying mechanisms, particularly those involving long non-coding RNAs (lncRNAs) in the hippocampus, remain poorly understood. In this study, we assessed the impact of swimming training on NP in a mouse model of sciatic nerve injury (SNI), with a focus on the lncRNA H19 and its associated signaling pathway. A male C57BL/6 mouse model of SNI was established and randomly divided into five groups: naïve, sham, sham + swim, SNI, and SNI + swim. The swimming groups underwent a 6-week training regimen. Pain thresholds wer

[Carpal Tunnel Syndrome and Lumbar Spinal Stenosis: When to Suspect Amyloidosis and How to Proceed with the Diagnostic Work-up].

Transthyretin (TTR) amyloidosis, particularly the wild-type form (ATTRwt), is an age-related systemic disease characterized by the deposition of misfolded TTR in tendons and ligaments. Patients therefore frequently present initially to orthopaedic or hand surgeons with carpal tunnel syndrome (CTS) or lumbar spinal stenosis (LSS) rather than to cardiologists or neurologists. These musculoskeletal manifestations may precede ATTR cardiomyopathy (ATTR-CM) by several years to more than a decade, serving as "red flags" that provide an opportunity for early diagnosis, particularly now that disease-mo

Change in Carpal Tunnel Pressure With Dynamic Wrist Flexion/Extension and Radioulnar Deviation: A Cadaveric Study.

Carpal tunnel pressure (CTP) is affected by flexion/extension and radioulnar deviation of the wrist. The purpose of this study was to investigate the elevations in CTP as a function of dynamic wrist motion in cadaveric hands. A goniometer was attached on hand dorsal side of cadaveric hands (n = 5). A micro-tip catheter pressure transducer was placed inside the carpal tunnel. With a baseline pressure set around 30 mmHg, the wrist was dynamically and randomly moved in its flexion/extension and radioulnar workspace while position and CTP were simultaneously recorded. CTP as a function of flexion/

Efficacy of US-guided Hydrodissection for Carpal Tunnel Syndrome: A Prospective Randomized Controlled Trial.

Background US-guided hydrodissection is a minimally invasive treatment for nerve entrapment syndromes. However, its independent therapeutic efficacy remains unproven because most studies combine hydrodissection with corticosteroids or dextrose. Purpose To evaluate and compare the clinical and sonographic outcomes and complication rates of three US-guided hydrodissection techniques for the treatment of carpal tunnel syndrome (CTS) and to assess the relative efficacy of hydrodissection. Materials and Methods Adults with refractory CTS were included in this prospective randomized controlled trial

Comparative Analysis of Patient Perspectives on Surgical Centers: A Study of Carpal Tunnel Releases Performed At An Outpatient Surgery Center Versus Clinical Procedural Suite.

BACKGROUND: This study compares outcomes of carpal tunnel release (CTR) performed in a clinic procedural suite (PS) versus an ambulatory surgery center (ASC). CTR in the PS is performed using the wide-awake local anesthesia no tourniquet (WALANT) technique and does not require NPO compliance. In contrast, CTR in an ASC is performed in an operating room under either WALANT or monitored anesthesia care (MAC), the latter requiring preoperative NPO restrictions. Prior studies demonstrate that WALANT CTR performed in a PS is associated with significantly reduced in-house time, lower healthcare cost

Beyond Floristic Quality: A Data-Driven Approach to Assessing Coastal Wetland Condition Using Plants.

Escherichia coli is an enteric bacterium that is capable of growing over a wide range of pH values (pH 5-9) and, incredibly, is able to survive extreme acid stresses including passage through the mammalian stomach where the pH can fall to as low as pH 1-2. To enable such a broad range of acidic pH survival, E. coli possesses four different inducible amino acid decarboxylases that decarboxylate their substrate amino acids in a proton-dependent manner thus raising the internal pH. The decarboxylases include the glutamic acid decarboxylases GadA and GadB, the arginine decarboxylase AdiA, the lysi
