Carpal tunnel syndrome, resulting from median nerve compression at the wrist, is a common and often disabling mononeuropathy. Risk factors include female sex, family history, repetitive hand use, obesity, pregnancy and a variety of medical comorbidities including diabetes mellitus, rheumatoid arthritis, and other connective tissue diseases. In many cases, an accurate diagnosis can be reached on the basis of clinical history and supportive examination findings alone. Neurophysiological investigations are essential for confirming the diagnosis, assessing severity and excluding more generalised n

OBJECTIVE: This prospective diagnostic test study evaluated the relationship between interpretation criteria and accuracy of the Upper Limb Neurodynamic Test 1 (ULNT1) in the diagnosis of carpal tunnel syndrome.
METHODS: A blind comparison with a reference criterion of typical clinical presentation and abnormal median nerve conduction was used. All subjects were first tested with nerve conduction studies and then with ULNT1. Each examiner was blinded to the results collected by the other examiners.
RESULTS: We analyzed 47 subjects and 84 limbs. Considering ULNT1 as positive in the presence o

UNLABELLED: Defects of the median, ulnar or radial nerves in the forearm, can be treated by conventional nerve grafts, or by interposing a synthetic guide such as nerve conduits. Wounds without nerve loss treated with simple suture may be supplemented by sleeving to prevent the nerve irritation symptoms using a nerve conduit or a vein sleeve. We studied the results of nerve conduits in both cases in a single-center retrospective study.
PATIENTS AND METHODS: Four patients underwent surgery with placement of a nerve conduit in the forearm, between May 2007 and January 2011. All patients were re

Opponoplasty using tendon transfer is a useful reconstructive procedure that restores lost thenar muscle function. Tendon transfers, however, require postoperative immobilization periods of up to four weeks before the sutured tendons reach required strength. We developed an opponoplasty procedure using α-TCP (alpha-tricalcium phosphate) cement that does not require postoperative immobilization and was applied to nine hands out of nine cases. The procedure is performed under local anesthesia without a pneumatic tourniquet and on an outpatient basis. In this procedure, the flexor digitorum super

BACKGROUND: The purpose of this study was to determine the prevalence of carpal tunnel syndrome (CTS) among dairy workers.
METHODS: Sixty-six dairy parlor workers and 58 non-parlor workers at dairies in Texas, New Mexico, and Colorado participated in structured interviews regarding demographics, work history, and hand symptoms. All participants had nerve conduction studies performed on both hands across the carpal tunnel. A CTS case definition was based on the presence of characteristic CTS symptoms and an abnormal median mononeuropathy across the carpal tunnel.
RESULTS: The prevalence of CT

The anterior tarsal tunnel syndrome (ATTS) has first been described by Kopell and Thompson in 1963. The anterior tarsal tunnel is formed by the fascia lining the inferior extensor retinaculum and talus as well as the navicular bone. Many ATTS cases with various etiologies have been reported since the first description. We report here an ATSS case resulting from a fibro-osseous structure that occurred after a missed talus fracture. The ATTS diagnosis can be made with a comprehensive clinical neurological examination and electrophysiological study. The treatment is based on the underlying etiolo

The differential diagnosis for chronic ankle pain is quite broad. Ankle pain can be caused by intra-articular or extra-articular pathology and may be a result of a traumatic or nontraumatic event. A detailed patient history and physical examination, coupled with judicious selection of the appropriate imaging modalities, are vital in making an accurate diagnosis and providing effective treatment. Chronic ankle pain can affect all age groups, ranging from young athletes to elderly patients with degenerative joint and soft-tissue disorders. It has been estimated that 23,000 ankle sprains occur ea

Neuropathies can be a cause of chronic foot and ankle pain. The diagnosis can be elusive given the sometimes nonspecific clinical presentation. Although electrodiagnostic studies are primarily relied on for the diagnosis of nerve impairment, imaging is sometimes helpful in helping define the exact site of the entrapment and whether any masses are present. It is critical for the imager to understand the complex anatomy of these nerves and their adjacent structures, to know the most common locations for their entrapments or injury, and to select the proper imaging modality to improve detection o

Tarsal tunnel syndrome, unlike its similar sounding counterpart in the hand, is a significantly misunderstood clinical entity. Confusion concerning the anatomy involved, the presenting symptomatology, the appropriateness and significance of various diagnostic tests, conservative and surgical management, and, finally, the variability of reported results of surgical intervention attests to the lack of consensus surrounding this condition. The terminology involved in various diagnoses for chronic heel pain is also a hodgepodge of poorly understood entities.

Failed surgical releases of the tarsal tunnel may be due to numerous causes. Many of the failures are due to lack of appreciation of the involved anatomy or inadequate technique. When an insufficient release is done, a revision simply completes the necessary steps. When external scarring is the problem, barrier materials may be used to help protect the nerve after neurolysis. When intrinsic damage is the problem, nerve wrapping, reconstruction, conduits, and nerve stimulators all play a role to restore function or ameliorate pain.

OBJECTIVE: This study was carried out with the aim of investigating the contribution of nerve mobilization exercises to the conservative treatment of tarsal tunnel syndrome.
METHODS: In this clinical trial, 28 patients were randomly allocated into 2 groups. The control group was composed of 14 patients who were treated conservatively with a program consisting of physiotherapy and supportive inserts, whereas 14 patients in the study group were given nerve mobilization exercises in addition to the same treatment. Allpatients were followed up for 6 weeks. Before treatment, subjects were evaluate

Excessive abnormal strain or tension on the posterior tibial nerve in feet exhibiting talotarsal instability has been considered one of the possible etiologic factors of tarsal tunnel syndrome. The suggested treatment options in such cases include stabilization of the talotarsal joint complex in a corrected position, which might help minimize the abnormal forces placed on the posterior tibial nerve due to over stretching. The primary goal of this study was to quantify strain on the posterior tibial nerve in feet exhibiting hyperpronation caused by talotarsal instability, before and after an ex

UNLABELLED: The flexor digitorum accessory longus (FDAL) muscle is one of the most commonly encountered anomalous muscles in the foot and ankle. Literature has documented the prevalence of the FDAL anywhere from 4% to 12%, based on cadaveric limb dissection. The variability of the origin, insertion, size, and location of the FDAL muscle can cause a wide array of foot and ankle pathologies, most notably, tarsal tunnel syndrome and flexor hallucis longus syndrome. Accessory musculature should be included in the list of differential diagnoses for foot and ankle pain until proven otherwise. This r

Tarsal tunnel syndrome is a condition that is caused by compression of the tibial nerve or its associated branches. Diagnosis is based on clinical findings but imaging is performed to exclude a cause of compression, identified in 60 to 80% of cases. Ultrasound is a useful examination because of its high spatial resolution and ability to rapidly perform an axial survey of the nerves. The ultrasound imaging features of the tarsal tunnel are described. The etiologies and different types are illustrated through a review of clinical cases.

BACKGROUND: Tarsal tunnel syndrome (TTS) is an entrapment neuropathy of the posterior tibial nerve or its branches within its fibro-osseous tunnel beneath the flexor retinaculum on the medial side of the ankle. It is a rare but important condition which is regularly under diagnosed leading to a range of symptoms affecting the plantar aspect of the foot. Management of this entrapment neuropathy remains a challenge and we have therefore reviewed the published literature in an attempt to clarify aspects of initial presentation, investigation and definitive treatment including surgical decompressi

Patients with Bertolotti's syndrome have characteristic lumbosacral anomalies and often have severe sciatica. We describe a patient with this syndrome in whom standard decompression of the affected nerve root failed, but endoscopic lumbosacral extraforaminal decompression relieved the symptoms. We suggest that the intractable sciatica in this syndrome could arise from impingement of the nerve root extraforaminally by compression caused by the enlarged transverse process.

We report on magnetic resonance neurography (MRN) as a supplementary diagnostic tool in sciatic nerve injection injury. The object of the study was to test if T2-weighted (w) contrast within the sciatic nerve serves as an objective criterion for sciatic injection injury. Three patients presented with acute sensory and/or motor complaints in the distribution of the sciatic nerve after dorsogluteal injection and underwent MRN covering gluteal, thigh and knee levels. Native and contrast-enhanced T1-w images were employed to identify the tibial and peroneal division of the sciatic nerve while T2-w

Although carpal tunnel syndrome (CTS) is the most common compressive neuropathy seen in the upper extremity of adults, it is rarely seen in children. Several reports have shown that mucopolysaccharidosis type II (Hunter syndrome), a rare genetic disorder, is one of the causes of CTS in children. Usual symptoms of CTS are pain, weakness, and paresthesias in the hand and digits. However, the diagnosis of CTS in Hunter syndrome is often delayed or unrecognized because of atypical symptoms and cognitive impairment. Here, we report the prevalence, clinical manifestation, and nerve conduction profil
