Misophonia is a disorder characterized by exaggerated emotional responses and heightened autonomic arousal triggered by specific sounds. Despite evidence that higher-level cognitive processes contribute to misophonia, it remains uncertain whether they can be used to modulate misophonic responses to trigger sounds. Based on the observation that individuals with misophonia often report listening to music as a coping mechanism, we investigated how directing attention to music could modulate physiological responses of individuals with misophonia in the presence of trigger sounds. Participants with

A 29-year-old woman presented with fever, malar rash, and alopecia. Laboratory evaluation demonstrated a high-titre antinuclear antibody of 1:5120, hypocomplementaemia, and positivity for anti-double-stranded DNA and anti-Smith antibodies, leading to a diagnosis of systemic lupus erythematosus. During the clinical course, she developed progressive peripheral neuropathy with inability to walk. Intravenous methylprednisolone pulse therapy and intravenous immunoglobulin were administered, but neurological improvement was limited, and intravenous cyclophosphamide therapy was initiated. Subsequentl

Neurotoxicity refers to injury of the central or peripheral nervous system caused by harmful stimuli including environmental chemicals, disease-associated endogenous stressors and other exposures that disturb neuronal homeostasis. A common outcome of neurotoxic stress is progressive disruption of synaptic function, redox balance and energy metabolism which together increase vulnerability to regulated cell-death programs. This review focuses on oxidative stress as core driver that shift reactive species from physiological signalling to direct chemical damage of lipids, proteins and DNA with mit

Autoimmune neurological diseases affecting the central nervous system, peripheral nervous system and neuromuscular junction are frequently refractory to conventional B-cell-depleting therapies such as CD20-directed monoclonal antibodies, highlighting a need for more effective treatment approaches. Chimeric antigen receptor T (CAR-T) cell therapy, initially developed for haematological malignancy, has been repurposed to more comprehensively deplete pathogenic B-cell and plasma cell populations in autoimmune disease, raising the possibility of durable, treatment-free remission. This scoping revi

INTRODUCTION: Facial diplegia is an uncommon regional variant of Guillain-Barre syndrome (GBS). It may initially resemble acute cerebrovascular disease when facial weakness appears unilateral or central in pattern.
CASE PRESENTATION: A 48-year-old Sri Lankan man with untreated hypertension, obesity, and newly recognized diabetes presented approximately 18 h after onset of slurred speech and persistent left facial weakness. Initial examination suggested a left central facial palsy, although incomplete left eye closure was also present. Non-contrast computed tomography and same-day brain magnet

OBJECTIVE: This article describes the clinical features, genetic causes, and diagnostic strategies of inherited peripheral neuropathies, and highlights how recent developments are reshaping the management of these disorders.
LATEST DEVELOPMENTS: Multiple new genetic causes of hereditary neuropathies have been identified, deepening insight into peripheral nerve biology and enabling the first clinical trial targeting a recessive form of genetic neuropathy. Concurrent advances in next-generation sequencing have substantially improved diagnostic yield. In parallel, several disease-modifying thera

Neurons extend long axons that traverse distinct microenvironments, yet how these subcellular compartments acquire and maintain specialized identities remains unclear. Here, we use spatial translatomics to define the local translatomes of somatosensory dorsal root ganglion (DRG) neurons. Translating Ribosome Affinity Purification and RNA sequencing (TRAP-seq) reveal thousands of mRNAs preferentially translated within central axons, peripheral axons, or DRG somata, establishing compartment-specific translational programs. Many of these transcripts encode ion channels and neurotransmitter recept

BACKGROUND: Sleep complaints and autonomic dysfunction are common non-motor manifestations of Parkinson's disease (PD). We examined the association between subjective sleep quality and nocturnal cardiac autonomic modulation in a retrospective cross-sectional PD cohort.
METHODS: In this retrospective cross-sectional study, we retrospectively analyzed routinely collected standardized clinical assessments and 24-hour ambulatory electrocardiography data from 86 patients with early-to-mid-stage PD. Participants were categorized as having poorer subjective sleep quality (Pittsburgh Sleep Quality In

OBJECTIVE: Glycosylphosphatidylinositol (GPI)-anchored proteins play critical roles in nervous system function. Pathogenic variants in genes involved in GPI-anchor biosynthesis cause early-onset multisystem disorders known as inherited GPI deficiencies. We describe a novel neuromuscular phenotype associated with PIGB deficiency.
METHODS: Patients with neuromuscular disease carrying PIGB variants were identified, and clinical data collected. GPI-anchored protein and free-GPI expression were assessed by flow cytometry and variants validated in a PIGB knockout cellular model.
RESULTS: Biallelic

Spinal Muscular Atrophy (SMA) is a neuromuscular disorder associated with motor neuron degeneration, yet the role of the well-conserved Survival of Motor Neuron (SMN) protein in muscle remains insufficiently understood. Using the Drosophila neuromuscular junction (NMJ) as a model, we combined tissue-specific Smn depletion with transcriptomic profiling and prior genetic modifier screens to identify SMN-dependent genes that contribute to synaptic architecture. Functional screening of candidate genes significantly and selectively effected in muscle identified genes whose knockdown phenocopied NMJ

BACKGROUND: Localized scleroderma (LoS) is an autoimmune disorder leading to skin sclerosis and subcutaneous tissue atrophy. Apart from the disfiguring craniofacial lesions, the extracutaneous involvement in LoS has been increasingly recognized and studied. Among all, neurological involvement is common and worth special attention.
OBJECTIVES: This systematic review summarizes the neurological symptoms, neuroimaging findings, and other complementary examination results of LoS.
METHODS: Records were retrieved using keywords related to the disease, neurological symptoms, and neurological examin

BACKGROUND: Aggregation of amyloid-β (Aβ) and tau is central to Alzheimer's disease (AD) pathogenesis and forms the basis of established cerebrospinal fluid (CSF) and imaging biomarkers. However, increasing evidence suggests that AD involves systemic processes beyond the central nervous system. We previously demonstrated the presence of fecal Aβ aggregates. Here, we assessed the detectability of tau aggregates in human feces and examined the diagnostic value of combined fecal Aβ and tau quantification in cognitive impairment.

BACKGROUND: Active central nervous system (CNS) infection can resemble acute immune polyradiculoneuropathy, and the two processes may coexist. We describe a patient with neurosyphilis, rapidly progressive paraparetic weakness, and anti-GT1a IgG detected in cerebrospinal fluid (CSF) but not serum by a qualitative line immunoblot.
CASE PRESENTATION: A 41-year-old man developed rapidly progressive paraparesis, urinary retention, and left abducens palsy. On hospital day 2, CSF opening pressure was 260 mmH2O, protein 1.56 g/L, white-cell count 272 × 106/L, CSF TPPA was reactive, and CSF RPR was 1:

Migraine is a highly disabling neurological disorder in which activation of the trigeminovascular system and the ensuing peripheral and central sensitization are major pathological foundations for pain initiation, recurrent attacks, and disease chronification. In recent years, innate immune-mediated sterile neuroinflammation has emerged as a key mechanism linking trigeminal neurovascular system activation to maladaptive remodeling of nociceptive pathways. During the early phase of a migraine attack, trigeminal nerve endings release calcitonin gene-related peptide, pituitary adenylate cyclase-a

Guillain-Barré syndrome (GBS) is an acute immune-mediated peripheral neuropathy characterized by rapidly progressive symmetric limb weakness and diminished tendon reflexes, which may be complicated by respiratory muscle paralysis in severe cases. Current first-line treatments primarily consist of intravenous immunoglobulin (IVIg) and plasma exchange (PE), but approximately 20% of patients have an incomplete response to standard therapies. Additionally, issues such as limited supply of blood products, adverse reactions, and contraindications may limit conventional treatments. Efgartigimod alfa

INTRODUCTION: Diffuse large B-cell lymphoma (DLBCL) comprises biologically distinct subtypes, but whether cell-of-origin-related features remain stable over long disease courses is unclear.
CASE DESCRIPTION: A woman was initially diagnosed with CD20-positive B-cell lymphoma in 1988 and treated with cyclophosphamide, vincristine and prednisolone (COP) chemotherapy. She experienced a first relapse in 1998 and received cyclophosphamide, doxorubicin, vincristine and prednisolone (CHOP) chemotherapy. The second relapse occurred in 2008 and was managed with rituximab. At the third relapse in 2011,

Malignant peripheral nerve sheath tumors (MPNST) are aggressive sarcomas of Schwann cell lineage with poor prognosis in both humans and dogs. While rare in humans, MPNSTs occur more frequently in dogs and share histomorphological and clinical features. Recent methylome and transcriptome analyses have identified two molecular subgroups of human MPNST with distinct oncogenic signaling pathways and prognostic implications; however, it remains unclear if these subgroups also exist in canines. Given their higher incidence and biological similarities to human disease, canine MPNSTs represent a promi

INTRODUCTION: Regional anesthesia is frequently used for upper limb surgeries and postoperative pain control. Different approaches to brachial plexus blocks are similarly effective but may differ in the frequency and severity of iatrogenesis. We, therefore, examined large-scale registry data to explore the risks of typical complications among different brachial plexus block sites for regional anesthesia.
METHODS: 26,947 qualifying adult brachial plexus blocks (2007-2022) from the Network for Safety in Regional Anesthesia and Acute Pain Therapy registry were included in a retrospective cohort

BACKGROUND: Blue Cone Monochromacy is a rare X-linked retinal disorder characterized by the selective loss of long-(L) and medium-(M) wavelength-sensitive cone function. While typically caused by large-scale genomic deletions or rearrangements within the OPN1LW/OPN1MW gene cluster, missense mutations are less common. Because blue cone monochromacy is often considered a stationary, congenital condition, sporadic cases lacking a family history can often be misidentified.
