Unveiling a Regional Variant of Demyelinating Guillain-Barré Syndrome: Nerve Conduction Study Evidence in Bifacial Weakness With Paresthesias.

BACKGROUND AND AIMS: Guillain-Barré syndrome (GBS) is categorized into acute inflammatory demyelinating polyneuropathy (AIDP) and axonal GBS. Bifacial weakness with paresthesias (BFP) is regarded as a rare regional variant of AIDP, yet its electrophysiological characteristics remain inadequately defined. This study aimed to clarify the pathophysiological basis of BFP and its classification under AIDP through serial nerve conduction study (NCS) analysis. METHODS: NCS data of six patients diagnosed with BFP were retrospectively analyzed. Four patients underwent serial NCSs, while two received a

Predictive value of neuroelectrophysiological parameters for respiratory failure in Guillain-Barré syndrome patients.

OBJECTIVE: To evaluate the predictive value of parameters derived from nerve conduction studies (NCS) for respiratory failure in patients with Guillain-Barré syndrome (GBS). METHODS: We conducted a retrospective analysis of 213 patients with GBS admitted between January 2020 and August 2023. Patients were categorized into respiratory failure (n = 92) and non-respiratory failure groups (n = 121). Demographic, clinical, and neuro-electrophysiological parameters were compared. Multivariate logistic regression analysis was performed to identify independent predictors of respiratory failure. The m

Neuromuscular Scoliosis as a Rare Manifestation of Guillain-Barré Syndrome in a Child.

Guillain-Barré syndrome (GBS) is an acute autoimmune polyneuropathy that may lead to long-term neuromuscular deficits, though the development of scoliosis as a late complication is rare, especially in children. We present the case of a nine-year-old boy with progressive thoracic scoliosis following a severe episode of GBS diagnosed at 18 months of age, which resulted in persistent flaccid paralysis, bilateral hip dislocation, and long-term tracheostomy-assisted respiratory support. Radiographs revealed a 71° thoracic curve, and the patient underwent correction using a minimally invasive bipola

Anti-GM1 antibodies in Bickerstaff's brainstem encephalitis.

Here, we present a case of Bickerstaff's brain stem encephalitis secondary to suspected campylobacter infection. Diagnosis was based on the classical presentation; ophthalmoplegia, ataxia, reduced consciousness and hyperreflexia, exclusion of other pathology and was supported by MRI, cerebrospinal fluid and serological testing. Despite the strong association of anti-GM1 antibodies in campylobacter infection and subsequent Guillain-Barré syndrome, this is the first reported case of brainstem encephalitis secondary to campylobacter infection demonstrating positive antiganglioside IgG GM1 serolog

Two Frontlines, One Diagnosis: Family Medicine-Neurology Collaboration Enables Early Recognition and Treatment of Acute Inflammatory Demyelinating Polyneuropathy in a Diabetic Patient.

Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy that often presents with rapidly progressive, symmetrical weakness and areflexia, typically following an infectious illness. Its recognition may be delayed in patients with underlying diabetic neuropathy due to overlapping sensory symptoms. We present the case of a 36-year-old woman with type 2 diabetes mellitus and chronic distal neuropathic pain who developed acute bilateral lower-extremity paralysis after a self-limited viral illness. Cerebrospinal fluid (CSF) revealed albuminocytologic dissociation, and magne

Reply to The outcome of severe Guillain-Barré syndrome  after robotic or conventional rehabilitation also depends on the triggering agent and the neurophysiological subtype.

Dear Editor, We read the interesting letter to the Editor by Dr. Finsterer and appreciated the comments on our work. In this paper we aim at answering to the questions raised. For the first point, Dr Finsterer suggests to add information about Nerve Conduction Studies (NCS) and needle Electromyography (EMG); unfortunately, we are not able to give the neurophysiological investigations acquired during the first admission at the hospital. Anyway, as regards the type of GBS, we can point out that he was diagnosed with an Acute Motor Axonal Neuropathy (AMAN), as stated by previous medical documents

Pyridostigmine Treatment for Pediatric Axonal Guillain-Barré Syndrome.

INTRODUCTION/AIMS: Direct involvement of the neuromuscular junction (NMJ) in the inflammatory process of Guillain-Barré syndrome (GBS) has been described. Despite this, the NMJ very rarely serves as a target for direct medical intervention in GBS. Here, we report the use of an acetylcholinesterase inhibitor, pyridostigmine, in four pediatric patients with axonal GBS. METHODS: All patients received standard immune therapy. Pyridostigmine treatment was started 7 weeks to 5 months after disease onset, and 5-11 weeks from reaching the plateau phase. Treatment efficacy was monitored by the 6-min w

Acute Polyradiculoneuropathy in a Patient With Squamous Cell Carcinoma of the Cervix: A Diagnostic Challenge.

Acute polyradiculoneuropathies are a heterogeneous group of disorders that may present with rapidly progressive, symmetrical weakness and can mimic Guillain-Barré syndrome (GBS). We report the case of a 60-year-old woman who presented with acute ascending flaccid weakness of all four limbs, consistent with a severe acute polyradiculoneuropathy clinically resembling GBS. During the diagnostic workup, a previously unrecognized squamous cell carcinoma of the cervix was identified. This case report aims to describe a rare temporal association between an acute polyradiculoneuropathy with GBS-like f

Clinical and Electrophysiological Spectrum of Guillain-Barré Syndrome in Syria: A Prospective Cohort Study Highlighting Atypical Presentations.

BACKGROUND: Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyneuropathy that presents significant diagnostic and therapeutic challenges, particularly in low-resource settings. While GBS has been extensively studied in high-income countries, data from conflict-affected and resource-limited regions such as Syria remain scarce. Understanding regional disease patterns and atypical clinical variants is essential for timely diagnosis and management. METHODS: This prospective study was conducted at the National and Al-Mowasat University Hospitals in Damascus between September 2023 and

Dynamics of Nerve Conduction Studies in Patients With Guillain-Barré Syndrome.

INTRODUCTION/AIMS: The value of electrodiagnostic subtyping of Guillain-Barré syndrome (GBS) is still debated. This study aimed to determine the diagnostic yield, timing, and changes of the electrodiagnostic subtyping in patients with GBS in serial nerve conduction studies (NCS). METHODS: Data were extracted from the International GBS Outcome Study (IGOS) database. Serial NCS were available for 469 patients. For the serial NCS analysis, the intervals between the first and second study were defined as ≥ 7 and ≤ 42 days after onset of weakness. All NCS were classified according to the electrodi

Fusion of molecular mimicry, epigenetic predisposition, and new onset GBS: a narrative review of current understanding and future directions.

Guillain-Barré syndrome (GBS) is a severe immune-driven polyneuropathy marked by the acute onset of flaccid paralysis, areflexia, and in severe cases, life-threatening autonomic or respiratory failure. Although the clinical presentation and diagnostic criteria are widely established, the precise mechanisms underlying GBS are complex and poorly understood. This review summarizes current literature on the interplay of post-infectious triggers, molecular mimicry, and host susceptibility as influenced by genetic and epigenetic variables. Infectious pathogens such as Campylobacter jejuni, cytomegal

Exploring the Role of Nuclear Factor Kappa B in the Immunobiology of Inflammatory Neuropathies.

BACKGROUND AND OBJECTIVES: Guillain-Barré syndrome (GBS) and chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) are inflammatory conditions, representing prototype examples of immune-mediated disorders of the peripheral nervous system. Activation of the nuclear factor-κB (NF-κB) pathway is crucial for generating immune and inflammatory responses and creating an inflammatory milieu; however, this has not been studied in inflammatory neuropathies. We aimed to quantify the expression levels of two critical genes in the NF-κB pathway ( Nfkb1 and Nfkb2 ), together with a prototype inf

Case Report: Absent F-Waves in a Case of Spinal Ischemia - A Potential Source of Diagnostic Error.

INTRODUCTION: Rapidly progressive para- or tetraparesis and early loss of F-waves on electrodiagnostic studies usually indicate acute inflammatory demyelinating polyneuropathy. A less common alternative etiology is acute spinal cord pathology. CASE PRESENTATION: We describe here a case of a 65-year-old woman with progressive motor deficits and absent F-waves early in the disease course due to anterior spinal artery syndrome. We provide serial electrodiagnostic findings with a focus on the basic electrodiagnostic parameters as well as imaging correlates during the inpatient management. CONCLU
