Expansion repeat disorders are caused by abnormal expansions of short tandem repeats (STRs). We evaluated targeted long-read nanopore sequencing for comprehensive characterization of STR expansions in 132 participants, including patients with myotonic dystrophy type 1 (DM1, n = 51) and DM2 (n = 3) already characterized by Southern blot, C9orf72-associated amyotrophic lateral sclerosis (ALS n = 11) evaluated by repeat primed polymerase chain reaction (RP-PCR; validation group), suspected spinocerebellar ataxia (SCA, n = 46), cerebellar ataxia with neuropathy and vestibular areflexia syndrome (C

Variants that disrupt pre-mRNA splicing represent a historically underrecognized proportion of pathogenic alleles in Mendelian neurological disorders, accounting for an estimated 15-30% of disease-causing variants. Conventional clinical genomic pipelines frequently overlook these variants because analysis and reporting have traditionally focused on protein-coding regions, with intronic and synonymous positions assigned lower priority. The central practical problem this review addresses is therefore not whether splicing matters in neurogenetic disease - that is well established - but how a spli

Hereditary transthyretin amyloidosis is an autosomal dominant disease caused by mutations in the TTR gene, which encodes the protein transthyretin. It is characterized by the deposit of mutated transthyretin in multiple organs and systems, mainly the heart and peripheral nervous system, causing different forms of polyneuropathy. It is a progressive, disabling and fatal disease. Early diagnosis of hereditary amyloidosis, including amyloidotic neuropathy, is currently relevant, as there are specific treatments that modify the natural course of the disease. The purpose of this consensus is to pro

Charcot-Marie-Tooth disease (CMT), caused by dominant loss-of-function mutations in DNM2, encoding the GTPase dynamin-2, impairs motor and sensory function. However, the respective contributions of muscle and nerve pathology, and the therapeutic potential of increasing DNM2 expression, remain unresolved. We evaluated tissue-targeted and systemic approaches to increase DNM2 in a mouse model carrying the common K562E-CMT mutation. Muscle-specific DNM2 overexpression from embryogenesis in Dnm2K562E/+ mice ameliorated desmin and integrin mislocalization, membrane trafficking defects, mitochondrial

Ankle-foot deformities, such as clubfoot, pose treatment challenges due to complex and patient-specific biomechanics. Accurate assessment of muscle force behavior is essential for tailoring interventions, yet conventional gait analysis is limited in isolating muscle-specific function. Intraoperative force measurements via the muscle's tendon offer a unique opportunity to directly quantify muscle mechanics. This study aimed to characterize the passive and active force-angle relationship of the tibialis anterior (TA) muscle, intraoperatively. Ten patients (ages 3-62) undergoing corrective foot s

Adult Refsum disease (ARD, OMIM #266510) is an autosomal recessive condition, resulting in phytanic acid (PA) accumulation in plasma and tissue. The management of ARD relies on a low PA diet, but the importance of adequate energy and carbohydrate provision in reducing circulating PA levels and preventing metabolic crisis is less well described. Two patients recently diagnosed with ARD initiated on the low PA diet leading to a 65% reduction in circulating PA levels, one case achieving this reduction in 5 months. Four patients already established on the low PA diet for > 10 years experienced a r

BACKGROUND: PDXK-related neuropathy is a rare form of hereditary motor and sensory neuropathy with optic atrophy, caused by biallelic variants in the PDXK gene. To date, only four missense variants have been reported in 13 patients from six families. Unlike most inherited neuropathies, which currently have only symptomatic treatments, this type of neuropathy can be managed with pyridoxal 5'-phosphate supplementation.
METHODS: The clinical presentation of the patient was evaluated twice, at ages 14 and 15 years. The whole-exome sequencing was performed for the proband followed by functional st

Charcot-Marie-Tooth (CMT) disease comprises a genetically diverse range of disorders affecting the peripheral nervous system. We report an axonal CMT (CMT 2) case from India with a novel heterozygous variant of uncertain significance detected in the ganglioside-induced differentiation-associated protein 1 (GDAP1) gene. A 12-year-old male patient presented with severe distal lower limb weakness (bilateral foot drop) progressing proximally. Bilateral pes-cavus, high-steppage gait, reduced power in bilateral lower limbs, bilaterally absent ankle jerks, and no cerebellar or pyramidal signs/vocal c

BACKGROUND: PMP22 duplications cause Charcot-Marie-Tooth disease (CMT), whereas PMP22 deletion results in hereditary neuropathy with liability to pressure palsies (HNPP). Although PMP22 point mutations can cause a spectrum of neuropathies, including hereditary motor and sensory neuropathy (HMSN) I, congenital hypomyelinating neuropathy, HMSN III, and HNPP, they are rare, and their features in Japanese patients have not been well characterized. Therefore, we compared the genetic, clinical, and electrophysiological characteristics of patients with PMP22 point mutations.

Adult Refsum disease (ARD; OMIM 266510) is a degenerative autosomal recessive condition typically diagnosed in adulthood. It affects visual, auditory and nervous system function. It is characterised by plasma, neuro-ophthalmological and adipose tissue accumulation of the dietary-derived phytanic acid (PA). This guidance reviews the clinical aspects of ARD and discusses interventions to address various co-morbidities of the disease. This GRADE-aligned guidance is based on a review of the literature and a consensus statement reflecting the conclusions of professionals with experience in the diag

BACKGROUND: Neuropathological examinations in spinocerebellar ataxia type 3 (SCA3) have demonstrated peripheral and autonomic nervous system degeneration, but the impact of associated symptoms on genetically affected individuals at different disease stages remains understudied.
OBJECTIVE: To investigate the clinical burden of peripheral and autonomic nervous system involvement in SCA3 mutation carriers across the disease spectrum.
METHODS: Forty SCA3 mutation carriers, including ten pre-ataxic individuals, completed questionnaires about muscle cramps, neuropathic pain, autonomic symptoms, ac

Primary mitochondrial diseases (PMDs) affect ∼1 in 4300 individuals, yet mitochondrial dysfunction is also a hallmark of common inherited and acquired disorders. Although advances in genomics now allow molecular diagnosis in the majority of mitochondrial diseases, treatment remains largely supportive, leading to progressive disability and early mortality. Despite progress in gene-modifying approaches, no approved therapies exist for the majority of mitochondrial diseases, and none of the recent trials has met its primary end point, underlining the urgent need for innovative therapeutic strateg

ObjectiveCharcot-Marie-Tooth disease (CMT) is the most common inherited peripheral neuropathy and usually manifests in the first two decades of life. However, prospective data on the natural course during childhood remain scarce.MethodsThis two-year, prospective, observational cohort study was conducted at two German pediatric neuromuscular centers. Annual standardized clinical assessments were performed. Sixty-eight pediatric patients with CMT (age range: 4-18 years; 35 females) were included. The primary outcome was the CMTPedS score, assessed at baseline and after a mean interval of 2.03 ±

Pediatric Charcot-Marie-Tooth (CMT) disease comprises a heterogeneous group of inherited peripheral nervous system neuropathies that manifest during childhood or adolescence. Clinically, it is characterized by progressive distal muscle weakness, atrophy, areflexia, sensory impairment, and orthopedic deformities such as pes cavus or scoliosis. In early-onset cases, hypotonia, motor delay, and limited independent ambulation may be observed. From a genetic perspective, the most frequent subtypes in the pediatric population include PMP22 duplication (CMT1A), mutations in MPZ (CMT1B), mutations in

Autoimmune etiologies of seizures have been increasingly recognized with the discovery of novel autoantibodies to cell surface and intracellular antigens. GAD65 neurologic autoimmunity has been associated with stiff person syndrome, cerebellar ataxia, and chronic focal epilepsy, usually of temporal regional origin. While musicogenic reflex seizures have been described in multiple GAD65 IgG seropositive patients, non-musicogenic acoustic reflex seizures have not been described. We present a case of a patient with GAD65 antibody associated epilepsy with a long-standing history of right temporal

Dominant loss-of-function mutations in DNM2 cause Charcot-Marie-Tooth neuropathy characterized by sensory and motor deficits associated with myelin and/or axonal abnormalities and muscle atrophy. Increasing DNM2 activity from embryogenesis has been reported to ameliorate neuromuscular phenotypes in the Dnm2K562E/+ Charcot-Marie-Tooth mouse; however, this model displays predominantly muscle pathology and limited nerve involvement, precluding rigorous evaluation of neuropathic mechanisms and potential therapies. Here, we performed comprehensive behavioural, electrophysiological, histological and

Biallelic pathogenic variants of the Reticulon 4 interacting protein 1 (RTN4IP1) gene are responsible for optic atrophy, either isolated or associated with ataxia, mental retardation, and seizures. They are identified as a cause of hereditary optic neuropathy in 7% of patients diagnosed before the age of 20. We have built a dataset for this gene by collating all the clinical cases available in the literature, and unpublished patients diagnosed at our centre, using standard nomenclature to describe both the molecular and phenotypic features. We performed a comprehensive data analysis, based on

BACKGROUND: Spinocerebellar ataxia 4 (SCA4) is a late-onset dominant ataxia with neuropathy caused by exonic GGC repeat expansion in the ZFHX3 gene thought to originate from a Swedish founder event. The GC-rich expansion is highly thermodynamically stable, posing challenges for standard clinical genetic testing methods. Development of a high-throughput relatively inexpensive detection method would benefit both clinical diagnostic testing and large-scale research applications.
OBJECTIVE: Using a cost-effective high-throughput polymerase chain reaction (PCR) assay, we assessed the frequency of

BACKGROUND: Pathogenic variants of the COQ7 gene result in a spectrum of neurological diseases, mainly distal hereditary motor neuropathy (dHMN). We herein report cases of dHMN related to biallelic p.Met1? (c.3G > T [NM_016138]). We compare phenotypes among different COQ7variants reported in the literature.
OBJECTIVE: To describe and analyze our case series, review COQ7-related diseases, and compare our case series with the literature reports.
METHODS: We described 5 dHMN-p.Met1? patients and searched dHMN AND Brazil and COQ7 in the PubMed/MEDLINE, SciELO and Scopus databases. The categorica

ATP7A is a P-type ATPase copper transporter and a central component of the intracellular network to maintain copper (Cu) homeostasis, trafficking between the trans-Golgi network (TGN) and the cell periphery in response to intracellular Cu levels. Pathogenic ATP7A variants cause Menkes disease (MNK), occipital horn syndrome (OHS) and X-linked distal hereditary motor neuropathy (HMNX), with the clinical severity inversely related to residual ATP7A function. In MNK and OHS, variants result in loss of ATP7A function due to absent (MNK) or markedly reduced (OHS) protein levels, leading to abolished
