PHARC (Polyneuropathy, Hearing Loss, Ataxia, Retinitis Pigmentosa and Cataract) - A Case Report and Clinical-Focused Literature Review.

Polyneuropathy, hearing loss, ataxia, retinitis pigmentosa, and cataract (PHARC) is a rare condition caused by mutations in ABHD12. We present the first documented case of PHARC in a Brazilian patient. Describe the clinical and genetic aspects of patients diagnosed with PHARC through a literature review. A literature review was conducted in February 2024 using Pubmed/Medline database. We also report a 37-year-old Brazilian woman diagnosed with PHARC. Between 38 patients diagnosed with this condition, the majority were male (74.35%) and the median age was 35.7 years. The most common symptom rep

[Peripheral Nerve: Cause and Pathology of Inherited Peripheral Neuropathy].

Inherited peripheral neuropathies (IPN) are classified as hereditary motor and sensory neuropathy (HMSN), hereditary motor neuropathy (HMN), or hereditary sensory and autonomic neuropathy (HSAN) based on clinical symptoms. Many IPN are accompanied by central nervous system disorders, such as spastic paraplegia and cerebellar ataxia. With the development of genetic analysis, the causes of IPN continue to be elucidated. This article describes the causative genes and pathologies identified to date.

Management of Small Fiber Neuropathy: A Clinical Perspective.

Small fiber neuropathy (SFN) is a common neurological diagnosis with a multitude of symptoms, including dysautonomia symptoms (e.g., orthostatic dizziness) as well as sensory disturbances, such as a tingling sensation or burning pain. The disease can be due to metabolic, autoimmune, or hereditary factors, and treatment plans stem from SFN etiology. Diabetes, autoimmune disease, infection, vitamin deficiencies, post-vaccination syndromes can all cause SFN. Needle electromyography and nerve conduction studies can be performed to exclude other neuromuscular conditions by ruling out large fiber ne

The Role of Visual Electrophysiology in Systemic Hereditary Syndromes.

Visual electrophysiology is a valuable tool for evaluating the visual system in various systemic syndromes. This review highlights its clinical application in a selection of syndromes associated with hearing loss, mitochondrial dysfunction, obesity, and other multisystem disorders. Techniques such as full-field electroretinography (ffERG), multifocal electroretinography (mfERG), pattern electroretinography (PERG), visual evoked potentials (VEP), and electrooculography (EOG) offer insights into retinal and optic nerve function, often detecting abnormalities before clinical symptoms manifest. Acute adrenal insufficiency-induced hypotension following spinal surgery in a pediatric patient with Charcot-Marie-Tooth disease and severe scoliosis: a case report.

BACKGROUND: Acute adrenocortical insufficiency (adrenal crisis) as a cause of postoperative hypotension is an uncommon and underreported condition in pediatric patients. Charcot-Marie-Tooth (CMT) disease, a hereditary motor and sensory neuropathy, frequently coexists with scoliosis. Surgical correction of severe scoliosis involves significant stress due to considerable surgical trauma and prolonged operative time. CASE DESCRIPTION: A 13-year-old female diagnosed with CMT and severe scoliosis (thoracic curvature Cobb angle 92°) underwent posterior osteotomy and corrective surgery. On postoperat

Plantar Pressure Distribution in Charcot-Marie-Tooth Disease: A Systematic Review.

Background: Charcot-Marie-Tooth (CMT) disease is a hereditary motor and sensory neuropathy that affects foot morphology and gait patterns, potentially leading to abnormal plantar pressure distribution. This systematic review synthesizes the existing literature examining plantar pressure characteristics in CMT patients. Methods: A comprehensive search was conducted across PubMed, Scopus, and Web of Science databases. Risk of bias was assessed using the Newcastle-Ottawa Scale. Results: Six studies comprising 146 patients were included. Four studies employed dynamic baropodometry, and two used in

Isolated peripheral neuropathy in a young adult: the role of comprehensive genetic testing.

Leukoencephalopathy with brainstem and spinal cord involvement and lactate elevation is a disorder with an autosomal recessive mode of inheritance. It is caused by mutations of the DARS2 gene that encodes aspartyl-transfer RNA (tRNA) synthetase enzymes responsible for the accurate charging of aspartate-specific tRNA with aspartate. Clinical features include seizures, cognitive impairment, cerebellar signs, upper motor neuron type of weakness, peripheral neuropathy, muscle weakness, cramps and deformities. Exclusive involvement of the peripheral nervous system has not been reported so far. We d

French guidelines for the diagnosis and management of pure hereditary spastic paraplegia.

INTRODUCTION: Hereditary spastic paraplegia (HSP) describes a group of rare genetic neurological diseases characterized by a common clinical presentation made of progressive motor weakness and spasticity of the lower limbs. The global prevalence is estimated at 3.6 individuals per 100,000 inhabitants. CLINICAL: The age of onset of first symptoms, the severity and the rate of progression are highly variable between different HSP subtypes, ranging from juvenile forms that present in the first years of life, to the later-onset forms, sometimes presenting after the age of 60. The medical history

Impaired Kidney Function With Kidney Tubular Vacuolization and Crystals in Adult Refsum Disease: A Case Report.

Adult Refsum disease is a rare hereditary peroxisomal disorder characterized by impaired phytanic acid metabolism. The accumulation of phytanic acid in tissues, especially adipose and neural, is believed to contribute to disease manifestations. Clinically apparent kidney dysfunction has been reported in only 2 cases. We describe a 55-year-old woman with adult Refsum disease, who was referred to nephrology for impaired kidney function without proteinuria. Kidney biopsy showed vacuolization of proximal tubular epithelium and unremarkable glomeruli. Focally, distal tubular cytoplasm showed variab

Charcot-Marie-Tooth Disease With Severe Hand Contractures and Respiratory Failure Requiring Long-Term Ventilator Support: A Case Report.

Charcot-Marie-Tooth (CMT) disease is the most common hereditary motor and sensory neuropathy, predominantly affecting distal limb muscles, particularly in the lower extremities. Respiratory muscle involvement is rare and not typically associated with ventilator dependence. We report a case of a 76-year-old man with a longstanding history of CMT who developed severe bilateral hand contractures and progressive respiratory failure following a brainstem stroke. Despite multiple weaning attempts, he remained ventilator dependent due to neuromuscular weakness and recurrent respiratory infections. Th

Digital biomechanical assessment of gait in patients with peripheral neuropathies.

BACKGROUND: The clinical status and treatment response of patients with peripheral neuropathies (PNs) rely on subjective and inaccurate clinical scales. Wearable sensors have been evaluated successfully in other neurological conditions to study gait and balance. Our aim was to explore the ability of biomechanical analysis using wearable technology to monitor disease activity in PN. METHODS: We conducted a single-center, longitudinal study to analyze gait parameters in PN patients and healthy controls using wearable biomechanical sensors. We used a novel technology that registers and integrate

Inherited Peripheral Neuropathies.

Inherited peripheral neuropathies are a diverse group of genetic disorders marked by progressive degeneration of peripheral motor, sensory, and autonomic nerves. While next-generation sequencing has improved diagnosis, challenges remain, including resolving variants of uncertain significance and addressing 20% to 50% of inherited neuropathy cases without clear diagnoses. Current care for inherited neuropathies focuses on supportive therapies to preserve functional range of motion, strength, and balance. Research into disease-modifying treatments is advancing, but accelerating progress requires

Long-term benefits of TUDCA supplement in ARSACS zebrafish model.

Autosomal recessive spastic ataxia of Charlevoix-Saguenay (ARSACS) is an early-onset neurodevelopmental and neurodegenerative disorder characterized by ataxia, spasticity, and peripheral neuropathy. However, several studies have highlighted that some patients also experience cognitive, emotional and social deficits, suggesting a more complex clinical picture that extends beyond motor symptoms. Building on these findings, this study aimed to: (i) investigate locomotor, social and cognitive deficits in adult sacs-/- zebrafish versus wild-type (WT) controls through behavioural tests; (ii) identif

Synthesis of stable isotope labeled analogs of phytanic acid for separate and combined tracing of alpha-, beta- and omega-oxidation.

Phytanic acid (3,7,11,15-tetramethylhexadecanoic acid) is a branched very long chain fatty acid (VLCFA) derived in humans from the diet and catabolized via peroxisomal metabolism. Catabolism of phytanic acid is important in humans as certain inborn errors of peroxisomal metabolism, including Refsum Disease, manifest when catabolism of diet derived phytanic acid is impaired. We developed a novel, linear, unified, unoptimized synthesis of all-racemic phytanic acid along with its 2,3,6,7,10,11,14,15-d8, 1,2-13C2, and 1,2-13C2-2,3,6,7,10,11,14,15-d8 stable isotope labeled analogs from commercially

Hereditary Motor and Sensory Neuropathies (HMSNs) With Conduction Block.

Dysmyelinating forms of hereditary motor and sensory neuropathy (HMSN) typically exhibit uniform conduction slowing on electrophysiological studies. However, this has been challenged in the past few decades with the discovery of novel gene mutations that induce nonuniform conduction slowing resembling acquired demyelinating neuropathies. Conduction block (CB) and/or excessive temporal dispersion (TD) are characteristic of segmental changes. Nevertheless, there is no consistent definition for CB and TD in inherited neuropathies. Pathological changes at paranodal regions or segmental demyelinati

Hypertrophic cardiomyopathy with ataxic gait: a cardiac clue to a neurologic diagnosis.

Hypertrophic cardiomyopathy (HCM) is an autosomal dominant cardiac myocyte disease characterised by left ventricular hypertrophy that develops in the absence of causative haemodynamic factors. In addition to mutations in sarcomeric genes, which are the most significant cause of HCM in adults and children, paediatric HCM also exhibits a high prevalence of non-sarcomeric causes. These include inherited metabolic disorders, malformation syndromes, neuromuscular diseases and mitochondrial diseases collectively accounting for about 35% of cases. In this case report, we describe a case of non-sarcom

A unique case of microspherophakia in adult Refsum disease.

Adult Refsum disease (RD) is a rare autosomal recessive peroxisomal disorder with an estimated prevalence of fewer than 1 in 1 million. The ocular manifestations result from the accumulation of phytanic acid, leading to retinitis pigmentosa, attenuated retinal vessels, bone spicule pigmentation, optic disc pallor, and macular involvement in advanced stages. To date, there has been no documented association between RD and microspherophakia, a rare lens abnormality more commonly linked to Weill-Marchesani syndrome. Microspherophakia is characterized by a small lens diameter, increased anteropost
