Charcot-Marie-Tooth Disease With Severe Hand Contractures and Respiratory Failure Requiring Long-Term Ventilator Support: A Case Report.

Charcot-Marie-Tooth (CMT) disease is the most common hereditary motor and sensory neuropathy, predominantly affecting distal limb muscles, particularly in the lower extremities. Respiratory muscle involvement is rare and not typically associated with ventilator dependence. We report a case of a 76-year-old man with a longstanding history of CMT who developed severe bilateral hand contractures and progressive respiratory failure following a brainstem stroke. Despite multiple weaning attempts, he remained ventilator dependent due to neuromuscular weakness and recurrent respiratory infections. Th

Inherited Peripheral Neuropathies.

Inherited peripheral neuropathies are a diverse group of genetic disorders marked by progressive degeneration of peripheral motor, sensory, and autonomic nerves. While next-generation sequencing has improved diagnosis, challenges remain, including resolving variants of uncertain significance and addressing 20% to 50% of inherited neuropathy cases without clear diagnoses. Current care for inherited neuropathies focuses on supportive therapies to preserve functional range of motion, strength, and balance. Research into disease-modifying treatments is advancing, but accelerating progress requires

Hereditary Motor and Sensory Neuropathies (HMSNs) With Conduction Block.

Dysmyelinating forms of hereditary motor and sensory neuropathy (HMSN) typically exhibit uniform conduction slowing on electrophysiological studies. However, this has been challenged in the past few decades with the discovery of novel gene mutations that induce nonuniform conduction slowing resembling acquired demyelinating neuropathies. Conduction block (CB) and/or excessive temporal dispersion (TD) are characteristic of segmental changes. Nevertheless, there is no consistent definition for CB and TD in inherited neuropathies. Pathological changes at paranodal regions or segmental demyelinati

[Charcot-Marie-Tooth Disease: Historical Evolution and Present Understanding].

Charcot-Marie-Tooth disease is a hereditary motor and sensory neuropathy characterized by progressive distal muscle weakness, atrophy, and sensory impairment. In this article, we review the original reports published in 1886 by Charcot, Marie, and Tooth, analyzing the clinical features they used to distinguish Charcot-Marie-Tooth disease from other forms of muscle atrophy. We then trace the historical development of the disease concept and examine evolving classification systems based on clinical, electrophysiological, and genetic findings. The identification of more than 140 causative genes h

Novel TFG mutation causes autosomal-dominant spastic paraplegia and defects in autophagy.

BACKGROUND: Mutations in the tropomyosin receptor kinase fused (TFG) gene are associated with various neurological disorders, including autosomal recessive hereditary spastic paraplegia (HSP), autosomal dominant hereditary motor and sensory neuropathy with proximal dominant involvement (HMSN-P) and autosomal dominant type of Charcot-Marie-Tooth disease type 2. METHODS: Whole genome sequencing and whole-exome sequencing were used, followed by Sanger sequencing for validation. Haplotype analysis was performed to confirm the inheritance mode of the novel TFG mutation in a large Chinese family wi

Hereditary motor and sensory neuropathy Okinawa type mimicking proximal myopathy.

Hereditary motor and sensory neuropathy with proximal dominant involvement (HMSN-P), or, Okinawa type, is a rare neuromuscular disorder characterized by proximal dominant neurogenic atrophy and distal sensory alterations with an autosomal dominant inheritance pattern. We present a case of a Brazilian woman of Okinawan ancestry, with symmetrical proximal weakness, fasciculations, absent patellar reflexes and positive familial history for the same symptoms. These findings led to genetic testing, which identified a variant in the TFG gene (c.854 C>T;p.(Pro285Leu), confirming the diagnosis of HMSN

Trk-fused gene plays a critical role in diet-induced adipose tissue expansion and is also involved in thyroid hormone action.

Mutations in the Trk-fused gene (TFG) cause hereditary motor and sensory neuropathy with proximal dominant involvement, which reportedly has high co-incidences with diabetes and dyslipidemia, suggesting critical roles of the TFG in metabolism as well. We found that TFG expression levels in white adipose tissues (WATs) were elevated in both genetically and diet-induced obese mice and that TFG deletion in preadipocytes from the stromal vascular fraction (SVF) markedly inhibited adipogenesis. To investigate its role in vivo, we generated tamoxifen-inducible adipocyte-specific TFG knockout (AiTFG

Genetic Landscape of SH3TC2 variants in Russian patients with Charcot-Marie-Tooth disease.

INTRODUCTION: Charcot-Marie-Tooth disease type 4C (CMT4C) OMIM#601596 stands out as one of the most prevalent forms of recessive motor sensory neuropathy worldwide. This disorder results from biallelic pathogenic variants in the SH3TC2 gene. METHODS: Within a cohort comprising 700 unrelated Russian patients diagnosed with Charcot-Marie-Tooth disease, we conducted a gene panel analysis encompassing 21 genes associated with hereditary neuropathies. Among the cohort, 394 individuals exhibited demyelinating motor and sensory neuropathy. RESULTS AND DISCUSSION: Notably, 10 cases of CMT4C were ide

Electrophysiological and radiological diagnosis of hereditary motor and sensory polyneuropathy.

Hereditary motor and sensory neuropathy (HMSN), also known as Charcot-Marie-Tooth disease (CMT), is a member of the inherited neuropathy family with specific clinical and genetical manifestations. More than twenty genes have been linked to HMSN, and the number might increase. Regarding diagnosis, a healthcare provider should be suspicious if the patient is young with a family history. Integrative diagnosis, which includes electrophysiological, radiological, and genetic screening, is of great value to exclude metabolic, nutritive-toxic, infectious, and inflammatory or autoimmunological causes a

Expanding the genetic spectrum of hereditary motor sensory neuropathies in Pakistan.

BACKGROUND: Hereditary motor and sensory neuropathy (HMSN) refers to a group of inherited progressive peripheral neuropathies characterized by reduced nerve conduction velocity with chronic segmental demyelination and/or axonal degeneration. HMSN is highly clinically and genetically heterogeneous with multiple inheritance patterns and phenotypic overlap with other inherited neuropathies and neurodegenerative diseases. Due to this high complexity and genetic heterogeneity, this study aimed to elucidate the genetic causes of HMSN in Pakistani families using Whole Exome Sequencing (WES) for varia

A comparison of lower body gait kinematics and kinetics between Theia3D markerless and marker-based models in healthy subjects and clinical patients.

Three-dimensional (3D) marker-based motion capture is the current gold standard to assess and monitor pathological gait in a clinical setting. However, 3D markerless motion capture based on pose estimation is advancing into the field of gait analysis. This study aims at evaluating the lower-body 3D gait kinematics and kinetics from synchronously recorded Theia3D markerless and CAST marker-based systems. Twelve healthy individuals and 34 clinical patients aged 8-61 years walked at self-selected speed over a 13 m long walkway. Similarity between models was statistically analysed using inter-tria

Brain magnetic resonance imaging of patients with spinal muscular atrophy type 2 and 3.

BACKGROUND AND OBJECTIVE: Proximal spinal muscular atrophy (SMA) is caused by deficiency of the ubiquitously expressed survival motor neuron protein. Although primarily a hereditary lower motor neuron disease, it is probably also characterized by abnormalities in other organs. Brain abnormalities and cognitive impairment have been reported in severe SMA. We aimed to systematically investigate brain structure in SMA using MRI. METHODS: We acquired high-resolution T1-weighted images of treatment-naive patients with SMA, age- and sex-matched healthy and disease controls with other neuromuscular

Late-onset sensory-motor axonal neuropathy, a novel SLC12A6-related phenotype.

We describe five families from different regions in Norway with a late-onset autosomal-dominant hereditary polyneuropathy sharing a heterozygous variant in the SLC12A6 gene. Mutations in the same gene have previously been described in infants with autosomal-recessive hereditary motor and sensory neuropathy with corpus callosum agenesis and mental retardation (Andermann syndrome), and in a few case reports describing dominantly acting de novo mutations, most of them with onset in childhood. The phenotypes in our families demonstrated heterogeneity. Some of our patients only had subtle to modera

The Impact of a Late Diagnosis: A Case of Charcot-Marie-Tooth Type 1.

Charcot-Marie-Tooth (CMT) is a hereditary motor and sensory neuropathy. The disease consists of a spectrum of inherited disorders caused by pathogenic variants in genes, which lead to multiple different clinical phenotypes. It is one of the most common inherited neuromuscular disorders. This disease most commonly presents with symptoms of distal weakness and muscular atrophy, which then lead to foot drop and pés cavus. In this article, we describe the case of a patient who developed muscle atrophy and distal weakness over the course of his 52 years of life, leading to gait impairment and foot

Differential effects of Mendelian GDAP1 clinical variants on mitochondria-lysosome membrane contacts sites.

GDAP1 pathogenic variants cause Charcot-Marie-Tooth (CMT) disease, the most common hereditary motor and sensory neuropathy. CMT-GDAP1 can be axonal or demyelinating, with autosomal dominant or recessive inheritance, leading to phenotypic heterogeneity. Recessive GDAP1 variants cause a severe phenotype, whereas dominant variants are associated with a milder disease course. GDAP1 is an outer mitochondrial membrane protein involved in mitochondrial membrane contact sites (MCSs) with the plasmatic membrane, the endoplasmic reticulum (ER), and lysosomes. In GDAP1-deficient models, the pathophysiolo
