Nanopore sequencing combined with adaptive sampling and NanoExpansion enables accurate characterization of repeat expansion disorders.

Expansion repeat disorders are caused by abnormal expansions of short tandem repeats (STRs). We evaluated targeted long-read nanopore sequencing for comprehensive characterization of STR expansions in 132 participants, including patients with myotonic dystrophy type 1 (DM1, n = 51) and DM2 (n = 3) already characterized by Southern blot, C9orf72-associated amyotrophic lateral sclerosis (ALS n = 11) evaluated by repeat primed polymerase chain reaction (RP-PCR; validation group), suspected spinocerebellar ataxia (SCA, n = 46), cerebellar ataxia with neuropathy and vestibular areflexia syndrome (C

Splice-site variants in neurology: from molecular mechanisms to clinical interpretation - a focused review.

Variants that disrupt pre-mRNA splicing represent a historically underrecognized proportion of pathogenic alleles in Mendelian neurological disorders, accounting for an estimated 15-30% of disease-causing variants. Conventional clinical genomic pipelines frequently overlook these variants because analysis and reporting have traditionally focused on protein-coding regions, with intronic and synonymous positions assigned lower priority. The central practical problem this review addresses is therefore not whether splicing matters in neurogenetic disease - that is well established - but how a spli

Peripheral and autonomic nervous system involvement in spinocerebellar ataxia type 3: unveiling an invisible burden.

BACKGROUND: Neuropathological examinations in spinocerebellar ataxia type 3 (SCA3) have demonstrated peripheral and autonomic nervous system degeneration, but the impact of associated symptoms on genetically affected individuals at different disease stages remains understudied. OBJECTIVE: To investigate the clinical burden of peripheral and autonomic nervous system involvement in SCA3 mutation carriers across the disease spectrum. METHODS: Forty SCA3 mutation carriers, including ten pre-ataxic individuals, completed questionnaires about muscle cramps, neuropathic pain, autonomic symptoms, ac

Mitochondrial DNA release and inflammation in mitochondrial disease pathogenesis.

Primary mitochondrial diseases (PMDs) affect ∼1 in 4300 individuals, yet mitochondrial dysfunction is also a hallmark of common inherited and acquired disorders. Although advances in genomics now allow molecular diagnosis in the majority of mitochondrial diseases, treatment remains largely supportive, leading to progressive disability and early mortality. Despite progress in gene-modifying approaches, no approved therapies exist for the majority of mitochondrial diseases, and none of the recent trials has met its primary end point, underlining the urgent need for innovative therapeutic strateg

Duolingo-induced seizures in GAD65 IgG associated autoimmune epilepsy.

Autoimmune etiologies of seizures have been increasingly recognized with the discovery of novel autoantibodies to cell surface and intracellular antigens. GAD65 neurologic autoimmunity has been associated with stiff person syndrome, cerebellar ataxia, and chronic focal epilepsy, usually of temporal regional origin. While musicogenic reflex seizures have been described in multiple GAD65 IgG seropositive patients, non-musicogenic acoustic reflex seizures have not been described. We present a case of a patient with GAD65 antibody associated epilepsy with a long-standing history of right temporal

A dataset of patients with isolated and syndromic optic neuropathies linked to RTN4IP1 genetic variants.

Biallelic pathogenic variants of the Reticulon 4 interacting protein 1 (RTN4IP1) gene are responsible for optic atrophy, either isolated or associated with ataxia, mental retardation, and seizures. They are identified as a cause of hereditary optic neuropathy in 7% of patients diagnosed before the age of 20. We have built a dataset for this gene by collating all the clinical cases available in the literature, and unpublished patients diagnosed at our centre, using standard nomenclature to describe both the molecular and phenotypic features. We performed a comprehensive data analysis, based on

Frequency of ZFHX3-Mediated Spinocerebellar Ataxia 4 in a US Undiagnosed Ataxia Cohort.

BACKGROUND: Spinocerebellar ataxia 4 (SCA4) is a late-onset dominant ataxia with neuropathy caused by exonic GGC repeat expansion in the ZFHX3 gene thought to originate from a Swedish founder event. The GC-rich expansion is highly thermodynamically stable, posing challenges for standard clinical genetic testing methods. Development of a high-throughput relatively inexpensive detection method would benefit both clinical diagnostic testing and large-scale research applications. OBJECTIVE: Using a cost-effective high-throughput polymerase chain reaction (PCR) assay, we assessed the frequency of

The weight of genotype on the clinical presentation of COQ7-related hereditary motor axonal neuropathy: a case series and literature review.

BACKGROUND: Pathogenic variants of the COQ7 gene result in a spectrum of neurological diseases, mainly distal hereditary motor neuropathy (dHMN). We herein report cases of dHMN related to biallelic p.Met1? (c.3G > T [NM_016138]). We compare phenotypes among different COQ7variants reported in the literature. OBJECTIVE: To describe and analyze our case series, review COQ7-related diseases, and compare our case series with the literature reports. METHODS: We described 5 dHMN-p.Met1? patients and searched dHMN AND Brazil and COQ7 in the PubMed/MEDLINE, SciELO and Scopus databases. The categorica

A functional framework in patient fibroblasts informs ATP7A variant pathogenicity and identifies p.Q990P as a novel cause of distal motor neuropathy.

ATP7A is a P-type ATPase copper transporter and a central component of the intracellular network to maintain copper (Cu) homeostasis, trafficking between the trans-Golgi network (TGN) and the cell periphery in response to intracellular Cu levels. Pathogenic ATP7A variants cause Menkes disease (MNK), occipital horn syndrome (OHS) and X-linked distal hereditary motor neuropathy (HMNX), with the clinical severity inversely related to residual ATP7A function. In MNK and OHS, variants result in loss of ATP7A function due to absent (MNK) or markedly reduced (OHS) protein levels, leading to abolished

Disease-Associated Mutations Impact DNA Methyltransferase 1 Function through Dynamic Allosteric Effects and Solvent Exposure.

Mutations in the DNA methyltransferase 1 (DNMT1) enzyme can lead to neurodegenerative diseases, including autosomal dominant cerebellar ataxia-deafness and neuropathy (ADCA-DN) and hereditary sensory and autonomic neuropathy type 1E (HSAN1E). The impact of these mutations on the structural dynamics of DNMT1 remains largely unknown. Here, we present an extensive molecular dynamics investigation of wild-type (WT) DNMT1 and its mutants associated with ADCA-DN (A554V, G589A, and V590F) and HSAN1E (D490E-P491Y and Y495C). The first group of mutants increases structural flexibility and disrupts the

Novel KIF5A variant in a patient with early-onset levodopa-responsive Parkinson's syndrome.

We present the case of a male in his mid-30s with a progressive complex neurological phenotype primarily characterised by levodopa-responsive parkinsonism with motor fluctuations as well as gait ataxia, peripheral neuropathy and finally also spastic paraplegia. Genetic analysis identified a novel heterozygous variant in the KIF5A gene: c.937G>A (p.Glu313Lys). This variant is genetically classified as likely pathogenic. Other pathogenic mutations in the KIF5A gene are associated with hereditary spastic paraplegia type 10, Charcot-Marie-Tooth disease type 2 and amyotrophic lateral sclerosis. We

Instrumented Three-Dimensional Gait Analysis in an Adult With KIF1A-Associated Neurological Disorder: A Case Report.

KIF1A-associated neurological disorder (KAND) encompasses recessive and dominant variants with wide clinical variability. Several de novo variants in the KIF1A gene have been reported to cause a complicated form of hereditary spastic paraplegia (HSP), frequently accompanied by peripheral neuropathy, cerebellar ataxia, and cognitive impairment. Instrumented three-dimensional (3D) gait analysis is underutilized in this population, and comparative data between KAND and pure HSP remain limited. We performed a comprehensive instrumented 3D gait analysis in a 19-year-old woman with KAND caused by a

ARSACS: Clinical Features, Pathophysiology and iPS-Derived Models.

Autosomal-recessive spastic ataxia of Charlevoix-Saguenay (ARSACS) is an early-onset neurodegenerative disease caused by mutations in the SACS gene. The first two mutations were identified in French Canadian populations 20 years ago. The disease is now known as one of the most frequent recessive ataxias worldwide. Prominent features include cerebellar ataxia, pyramidal spasticity, and neuropathy. Neuropathological findings revealed cerebellar atrophy of the superior cerebellar vermis and the anterior vermis associated with Purkinje cell death, pyramidal degeneration, cortical atrophy, loss of

Identification of genetic mechanisms of non-isolated auditory neuropathy with various phenotypes in Chinese families.

BACKGROUND: Non-isolated auditory neuropathy (AN), or syndromic AN, is marked by AN along with additional systemic manifestations. The diagnostic process is challenging due to its varied symptoms and overlap with other syndromes. This study focuses on two mitochondrial function-related genes which result in non-isolated AN, FDXR and TWNK, providing a summary and enrichment analysis of genes associated with non-isolated AN to elucidate the genotype-phenotype correlation and underlying mechanisms. METHODS: Seven independent Chinese Han patients with mutations in FDXR and TWNK underwent comprehe

The Role of Visual Electrophysiology in Systemic Hereditary Syndromes.

Visual electrophysiology is a valuable tool for evaluating the visual system in various systemic syndromes. This review highlights its clinical application in a selection of syndromes associated with hearing loss, mitochondrial dysfunction, obesity, and other multisystem disorders. Techniques such as full-field electroretinography (ffERG), multifocal electroretinography (mfERG), pattern electroretinography (PERG), visual evoked potentials (VEP), and electrooculography (EOG) offer insights into retinal and optic nerve function, often detecting abnormalities before clinical symptoms manifest.
