[The first case of Miller Fisher syndrome associated with anti-GM3 IgG antibodies: Classic triad and an atypical immune response].

Miller-Fisher syndrome (MFS) is a rare variant of Guillain-Barré Syndrome (GBS), an acute immune-mediated peripheral neuropathy most preceded by infections, particularly Campylobacter jejuni, through a mechanism of molecular mimicry that underlies its predominantly autoimmune pathogenesis. Anti-ganglioside antibodies play a significant role in the pathogenesis of MFS, with a particularly high prevalence of anti-GQ1b antibodies among affected individuals. However, the presence of alternative anti-ganglioside antibodies does not exclude the diagnosis. We report the case of a 72-year-old male who

Comparative Effects of Monocular and Stereopsis Training on Visual Acuity and Stereoacuity in Children With Amblyopia.

PURPOSE: To evaluate changes in visual acuity (VA) and stereoacuity in children with amblyopia following monocular or stereopsis training delivered alongside standard clinical care and to examine factors associated with training-related improvements in visual outcomes. METHODS: Forty-three children with amblyopia (7.3-14.8 years; 37 anisometropic, five strabismic, and one mixed) completed approximately 29 sessions of either monocular training (grating acuity or motion discrimination targeting the amblyopic eye, n = 23) or binocular stereopsis training (dichoptic disparity discrimination, n =

Estimating Risk of Guillain-Barré Syndrome in US Medicare-enrolled Older Adults Following Medically Attended Respiratory Syncytial Virus Disease: A Self-controlled Case Series Analysis.

BACKGROUND: Guillain-Barré syndrome (GBS) is considered a rare complication of viral respiratory infections (eg, influenza, severe acute respiratory syndrome coronavirus 2); however, its association with respiratory syncytial virus (RSV) infection has not been studied. We performed a self-controlled case series analysis to estimate GBS risk following medically attended RSV disease among older adults. METHODS: We identified adults aged ≥65 years with medically attended RSV disease and incident inpatient GBS using specific International Classification of Diseases (ICD) codes, excluding those wi

Severe lupus vasculitic neuropathy.

A 31-year-old woman presented with generalised pain and weakness due to a severe axonal sensorimotor neuropathy, rapidly worsening over 2 weeks. For 6 months she had experienced transient joint symptoms, rash and hair loss. A vasculitic process was considered. Blood tests taken following intravenous immunoglobulin (IVIG) (given for suspected Guillain-Barré syndrome) noted strongly positive lupus and antiphospholipid antibody titres. She was diagnosed with severe multisystem lupus vasculitis involving the central and peripheral nervous system and started on cyclophosphamide and prednisolone, wi

Patient Outcomes Among Medicare Beneficiaries With Guillain-Barré Syndrome.

BACKGROUND AND AIMS: Understanding of population-level outcomes for patients with Guillain-Barré syndrome (GBS) remains limited. We identified which GBS patients are most likely to experience worse outcomes using the largest and most current GBS cohort in the United States. METHODS: This retrospective cohort study used 2005-2020 fee-for-service Medicare claims to identify individuals newly diagnosed with GBS (N = 16 280). We used three person-level modified Poisson regressions to estimate hospital and intensive care unit (ICU) lengths of stay and GBS episode length as a function of sociodemog

Miller Fisher Variant of Guillain-Barré Syndrome Presenting With Dysphagia and Ophthalmoplegia.

Miller Fisher syndrome (MFS) is a rare variant of Guillain-Barré syndrome (GBS). It is characterized by a triad of ataxia, areflexia, and ophthalmoparesis. Akin to GBS, it is often triggered by a respiratory infection, causing immune cross-activation against pathogenic proteins and gangliosides. Symptoms typically occur in chronological progression, beginning with ophthalmoparesis, then ataxia, and finally areflexia. Our case is of a patient presenting with dysphagia and ophthalmoplegia as the initial manifestations. There currently exist only a limited number of reported cases of MFS presenti

Serum Galectin-3 as a biomarker in acute inflammatory polyradiculoneuropathies: a cohort study.

BACKGROUND AND AIMS: Guillain-Barré Syndrome (GBS) refers to a group of acute inflammatory polyradiculopathies characterized by a diffuse inflammatory attack on the peripheral nervous system, often triggered by a preceding infection. Despite significant progress in both diagnosis and treatment, only a few prognostic systems and useful biomarker have been proposed. In this study, we propose galectin-3 (Gal-3), a novel serum molecule identified expressed by Schwann cells and macrophages, as a potential biomarker with both prognostic and diagnostic significance in patients with GBS. METHODS: Ser

Utility of the "Modified Erasmus GBS Respiratory Insufficiency Score" in Axonal and Demyelinating Guillain-Barré Syndrome.

BACKGROUND: The modified Erasmus GBS Respiratory Insufficiency Score (mEGRIS) has been proposed to be a useful tool for predicting the risk of mechanical ventilation (MV) in Guillain-Barré syndrome (GBS), whereas most of the patients included in previous studies had classical demyelinating GBS. This study validated the utility of the mEGRIS in axonal, as well as demyelinating GBS, defined by electrophysiologic criteria in Japan. METHODS: Data from 214 consecutive patients diagnosed with GBS at our institution within 28 days from disease onset between 1998 and 2023 were reviewed and 200 patien

Intravenous immunoglobulin use in neurology.

In the past 3 decades, intravenous immunoglobulin (IVIg) - pooled IgG (IgG) obtained from the plasma of several thousands of healthy donors - has had a major impact in the successful treatment of previously poorly controlled autoimmune neurologic disorders. Based on randomized controlled trials, IVIg is approved for the treatment of Guillain-Barré syndrome, chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), multifocal motor neuropathy (MMN), and dermatomyositis. It has been effective in myasthenia gravis exacerbations and stiff-person syndrome, and exhibits convincing efficacy i

Guillain-Barré syndrome with unilateral oculomotor nerve palsy and anti-GM3 IgG antibodies.

Guillain-Barré syndrome (GBS) is a life-threatening acute paralytic neuropathy characterized by rapidly progressive limb weakness and bilateral cranial nerve involvement. We report an early 70s male with no relevant medical history diagnosed with anti-GM3 IgG-positive GBS, who developed unilateral oculomotor nerve palsy and autonomic dysfunction during intravenous immunoglobulin (IVIG) therapy, with spontaneous symptom resolution following IVIG completion. Six-month follow-up assessments confirmed complete remission. This case reinforces the therapeutic value of IVIG in GBS patients with speci

Longitudinal Monitoring of Systemic Cytokines After Mild Zika Virus Infection Revealed an Association Between Th17 Polarization and Clinical and Serological Outcomes.

Zika virus (ZIKV) is a neurotropic virus that can cause a variety of neurological manifestations, ranging from mild forms to severe disorders like Guillain-Barré syndrome and congenital Zika syndrome. The pathophysiology of these complications is not fully understood, but they have been linked to host immune responses, particularly a proinflammatory Th1/Th17 profile. In this study, the kinetics of 14 cytokines were characterized in ZIKV-infected patients recruited in French Guiana in 2016-2017. Cytokine concentrations were quantified using a multiplexed bead-based immunoassay in serum samples

Patterns of Seasonality and Subtype-Linked Outcomes of Pediatric Guillain-Barré Syndrome ICU Admissions: A 10-Year Audit from Southern India.

OBJECTIVE: To evaluate the seasonal variation in Guillain-Barré Syndrome (GBS) subtype distribution and its association with clinical outcomes in pediatric patients requiring intensive care unit (ICU) admission. METHODS: An audit of pediatric patients with GBS admitted to the neuro-ICU of a tertiary center in Southern India between 2008 and 2018 was conducted. Seasonal distribution was defined as per meteorological classification: summer (March-May), monsoon (June-September), post-monsoon (October-November), and winter (December-February). Clinical subtypes, treatment, and outcomes, including

Patient Perspective: Words That Matter: Reflections on a Patient's Experiences With Guillain-Barré Syndrome.

Guillain-Barré syndrome (GBS) is a rare immune-mediated polyradiculoneuropathy. When severe, GBS can result in near-total paralysis and respiratory failure. Psychological symptoms are common but under-recognized complications of GBS and may develop because of physical disability and social isolation. In this article, we share the patient's perspective of the psychological effects of GBS and how words of encouragement, a listening ear, communication aids, and dedicated caregiving made the difference in the patient's physical and emotional recovery.

Case Report: Guillain-Barré syndrome temporally associated with levofloxacin exposure and improvement following efgartigimod treatment.

Levofloxacin, the L-isomer of the racemic fluoroquinolone ofloxacin, is a broad-spectrum antibiotic with broad clinical applications in both prophylactic and therapeutic indications. However, it has been associated with various adverse reactions, including Guillain-Barré syndrome (GBS). GBS is an immune-mediated peripheral neuropathy that typically presents with acute-onset symmetrical flaccid paralysis. Here, we report a case of GBS temporally associated with levofloxacin exposure that improved after initiation of efgartigimod, following limited response to plasma exchange, methylprednisolone

Incidence of Guillain-Barré Syndrome in Chile: A Population-Based Study Between 2013 and 2022.

BACKGROUND AND AIMS: Guillain-Barré syndrome (GBS) is the leading cause of acute flaccid paralysis. A Chilean study for 2001-2012 reported an age-standardized incidence of 2.10 per 100 000. We updated nationwide GBS incidence for 2013-2022 by sex, age, and macrozone. METHODS: We conducted a retrospective, population-based analysis of the Chilean Department of Statistics and Health Information (DEIS) hospital-discharge database. Cases were identified with the ICD-10 code G61.0 and deduplicated. Incidence rates (IRs) per 100 000, using official mid-year populations, were age-standardized to the

Utilizing internal analytics and physician-director collaborations to enhance neurological health communication on YouTube.

The dissemination of health information on YouTube presents a paradox where expert-generated content, despite high clinical accuracy, often lacks the engagement seen in non-expert patient narratives. Recent evaluations of Guillain-Barré Syndrome (GBS) videos confirm that while physician-produced content achieves superior objective quality scores, it frequently suffers from low visibility and fails to correlate popularity with scientific validity. This manuscript proposes a shift toward utilizing internal analytics, specifically Average View Duration (AVD) and Click-Through Rate (CTR), alongsid
