BACKGROUND AND OBJECTIVES: Bladder augmentation in children can have significant benefits in terms of continence and social function. In an attempt to overcome the problems associated with using foreign mucosa in the urinary tract, techniques to increase to bladder volume and improve compliance by resecting the detrusor muscle alone have been described. Here we describe our experience using an omental-backed detrusorectomy augmentation and examine possible reasons for a poor outcome observed in some patients in the longer term.
METHODS: This was a retrospective review covering an eight-year p

Autoimmune neuropathies are common and treatable disorders of the peripheral nerves, which should be properly recognized. This article discusses their diagnosis, differential diagnosis and proper treatment.

Guillain-Barré syndrome associated with acute hepatitis B virus infection is rare. We report a 56-year-old man who presented as Guillain-Barré syndrome in the absence of icterus and was subsequently shown to have acute hepatitis B. He improved on conservative therapy.

Since the hepatitis B vaccine are on the market in France, until the end of 2002, 1211 observations of demyelinating disease of the central nervous system (1109 cases of which 895 multiple sclerosis) or peripheral (102 cases of which 49 Guillain Barre Syndrome), have been reported to the french network of pharmacovigilance and to the AFSSAPS. It is not possible to singularize these observations, neither from a clinical nor an epidemiological point of view. No risk factor has been detected. Only the chronology could suggest a causal relationship, the vaccine preceding the pathology in all the c

Guillain-Barre syndrome is a postinfectious disorder caused by an aberrant immune response to an infectious pathogen, resulting in an autoimmune disease. As with other autoimmune diseases of infectious nature, the intricate balance of the numerous factors involved in the immune response may determine the outcome of the interaction between the microbe and host. Recent studies focusing on the role of cytokines and its network of related mediators and receptors suggest that any imbalance may make a significant contribution to the outcome of the infectious disease process. Better understanding of

A retrospective study of 120 cases of acute polyradiculoneuropathy hospitalized in the National Institute of Neurology, since 1974. We analysed the clinical and neurophysiological pictures, the course of the disease and the prognosis. All ages and sex were affected. The mean age of onset was 34 years. There were a predominance of male 2.5:1. Weakness, particularly of the lower limbs, was the initial complaint of patients. A rise in the level of cerebro-spinal fluid protein in the absence of pleiocytosis was found in 80% of patients. Electrophysiological data showed most frequent sensory motor

We report an 11-year-old boy with Bickerstaff's brainstem encephalitis (BBE). He had gait disturbance, disturbed consciousness, and diplopia after upper respiratory tract infection. On admission, he showed multiple cranial nerve palsy, muscle weakness of arms, cerebeller ataxia and generalized areflexia. The cerebrospinal fluid on day 7 revealed albuminocytologic dissociation. IgG antibodies against GQ1b and GT1a were detected in the serum. Immunoglobulin was administered intravenously from day 11, and then his symptoms gradually diminished. When he was discharged on day 27, he had neither con

The patient was a 34-year-old male with chronic pure motor neuropathy with such acute onset as seen in Guillain-Barré syndrome. Neurological symptoms were gradually progressive for 4 weeks, and predominantly noted in the left side. Deep reflexes were normal and the distribution of muscle weakness was uneven. Plasma exchange reduced neurological symptoms. Four weeks later, right drop foot was relapsed. High dose intravenous immunoglobulin was effective. Serial electrophysiological studies indicated the asymmetric reduction of CMAP. Repeated assays of anti-GalNAc-GD1a IgM antibody were positive.

OBJECTIVE: To report two patients with an acute exclusively motor neuropathy with conduction blocks.
METHODS: Serial electrophysiologic studies were carried out.
RESULTS: Two patients developed symmetric proximal and distal weakness without sensory abnormalities after enteritis. Tendon reflexes were normal in one patient and brisk in the other. One patient had high titer immunoglobulin G to GD1a and GM1, and the other to GD1b, GD1a, and GM1 and a recent Campylobacter jejuni infection. Electrophysiology showed early partial motor conduction block in intermediate and distal nerve segments, nor

OBJECTIVE: Multifocal motor neuropathy with persistent conduction blocks is classically described as a chronic neuropathy with progressive onset, and acute forms have not previously been characterised. We report four cases of severe motor impairment with acute and generalised onset and with persistent motor conduction blocks.
PATIENTS AND RESULTS: An acute tetraparesis with diffuse areflexia but little or no sensory disturbance was the clinical picture. Serial electrophysiological tests showed persistent multifocal motor conduction blocks with absent F waves in most tested motor nerves. No or

A database search was performed to identify all patients presenting to a Rehabilitation Centre over a five-year period with a confirmed diagnosis of Guillain-Barré syndrome. Retrospective analysis of acute and rehabilitation hospital notes was performed and information on demographics, management and outcome obtained. There were equal numbers of males and females with an average age of 36 years. 75% had a prodromal illness and 62.5% presented with distal paraesthesia. 75% exhibited cranial nerve signs and 62.5% required artificial ventilation for a prolonged period. The investigations performe

Visceral leishmaniasis is an endemic parasitic infection rarely observed in association with Guillain-Barré syndrome in immunocompetent patients. A 40-year-old immunocompetent woman was admitted to our unit with recent onset difficulty in walking. The neurological examination and electrophysiological study led to the diagnosis of Guillain-Barré syndrome. During hospitalization, she developed cytopenia involving all three lines revealing visceral leishmaniasis. A few cases of visceral leishmaniasis with neuropathy have been reported, mainly in tropical regions. Neuropathological manifestations

We described a patient with ataxic Guillain-Barré syndrome who subsequently developed facial diplegia. A 38-year-old man developed ataxia, distal limb paresthesias, mild dysphagia, urinary retention and orthostatic hypotension a week after an episode of laryngitis. He had high titers of serum anti-GQ1b, anti-GD1b, anti-GM1b, anti-GT1a, and anti-GD1a IgG antibodies during the acute phase. Although the initial symptoms markedly improved by intravenous immunoglobulin therapy, asymmetric facial diplegia subsequently occurred and remained longer than ataxia. Similar course of facial nerve palsy has
