Severe Acute Pancreatitis Complicated by Guillain-Barré Syndrome in the Intensive Care Unit.

Severe acute pancreatitis (SAP) rarely causes immune-mediated neurological complications. We report Guillain-Barré syndrome (GBS), acute motor and sensory axonal neuropathy subtype, occurring during SAP requiring extracorporeal membrane oxygenation and continuous renal replacement therapy. A 39-year-old woman with hyperlipidemic pancreatitis and autoimmune diseases developed abrupt symmetric flaccid quadriplegia after sedation withdrawal in the intensive care unit. Electrophysiological studies and cerebrospinal fluid analysis confirmed the diagnosis of GBS. Intravenous immunoglobulin therapy r

Machine learning-based identification of concomitant stroke and prognostic analysis in patients with Guillain-Barré syndrome: a retrospective study.

BACKGROUND: Guillain-Barré syndrome (GBS) constitutes an immune-mediated inflammatory polyradiculoneuropathy. Stroke may coexist with GBS during the same clinical episode, but the associated clinical predictors remain insufficiently characterized. The present investigation therefore sought to construct a machine learning-based model for identifying concomitant stroke in patients with GBS. METHODS: This retrospective cohort study included 260 patients with GBS who received care at the Second Affiliated Hospital of Army Medical University from January 1, 2015, to December 31, 2024. All candidat

Prognostic value of modified Erasmus Guillaiun Barre' syndrome outcome score (mEGOS) in an Italian cohort of patients: Challenges, insights and limitations.

BACKGROUND: Clinical course and outcome of Guillain Barre' syndrome (GBS) are highly variable. among countries. The modified Erasmus GBS Outcome Score (mEGOS) is a prediction model proposed to predict risk of walking inability within six months of the disease. Our study aimed to validate the mEGOS in an Italian cohort of patients. METHODS: We used dataset from 230 adults GBS admitted to Neurology Unit of Modena over 22 years. We studied whether mEGOS applied at entry and at week 1 to patients unable to walk could predict the to restoration of independent ambulation at 4 and 24 weeks. Model pe

Guillain-Barré syndrome presenting with unilateral facial weakness.

Acute immune system-mediated neuropathy, referred to as Guillain-Barré syndrome, is a common etiology of acutely acquired weakness that often presents with proximally spreading initial distal extremity weakness and paralysis. Less common manifestations include delayed facial nerve palsy and weakness, occurring in approximately 15% of patients. This case report highlights a rarer initial presentation of Guillain-Barré syndrome in a 43-year-old man initially presenting with unilateral upper facial weakness whose symptoms progressed to bilateral hand and foot numbness over a 2-week period.

Disproportionality Analysis and Timing of Drug-Associated Guillain-Barré Syndrome Onset Based on the Japanese Adverse Drug Event Report Database.

Background: Guillain-Barré syndrome (GBS) is an autoimmune peripheral neuropathy that can lead to paralysis and respiratory failure. In addition to infections, several drugs have been suggested as potential triggers of GBS. This study investigated drug-associated GBS using a spontaneous adverse event reporting database through disproportionality analysis for signal detection and time-to-onset analysis. Methods: The Japanese Adverse Drug Event Report (JADER) database was analyzed to assess more than 4000 drugs for potential associations with GBS. Signal detection was performed using reporting o

Lessons from a Severe Case of Fulminant Guillain-Barré Syndrome: A Case Report.

Background and Clinical Significance: Guillain-Barré syndrome (GBS) can rarely progress to fulminant paralysis with loss of brainstem reflexes, mimicking coma or brain death despite preserved cortical function. Case Presentation: A 38-year-old man developed rapidly progressive weakness following a diarrheal illness, culminating in quadriplegia, areflexia, respiratory failure, and complete loss of brainstem reflexes within 72 h. Neuroimaging was unrevealing. EEG demonstrated preserved cerebral activity with an alpha coma pattern. Despite initial intravenous immunoglobulin therapy, neurological

Upper limb-predominant variant of Guillain-Barre Syndrome (GBS) following diarrhoea.

Guillain-Barre syndrome (GBS) is an acute immune-mediated polyneuropathy classically presenting with ascending lower limb weakness; however, rare regional variants may pose diagnostic challenges. We report a case of an unusual upper limb-predominant variant of GBS following a self-limiting diarrhoeal illness, presenting with acute shoulder pain, progressive bilateral hand paraesthesia and upper limb weakness without lower limb, bulbar or cranial nerve involvement. Neurological examination demonstrated proximal and distal upper limb weakness with upper limb hyporeflexia. Cerebrospinal fluid ana

Disease-specific divergence of inflammatory and metabolic biomarkers in neurocritical neuromuscular disorders.

BACKGROUND: Myasthenia gravis (MG) and Guillain-Barré syndrome (GBS) are immune mediated neuromuscular disorders that may require intensive immunotherapy and respiratory support. Although inflammatory biomarkers have been explored in both conditions, their diagnosis specific prognostic value remains unclear. We aimed to compare hemogram-derived inflammatory indices and metabolic injury-related biomarkers in hospitalized MG and GBS patients and to evaluate their associations with disease severity and clinical outcomes. METHODS: This retrospective cohort study included 162 patients (88 MG, 74 G

Establishing a Safe and Effective Therapeutic Plasma Exchange Program in a Resource-Constrained Transplant Center: A 6-Year Nigerian Experience.

Therapeutic plasma exchange (TPE) is essential for kidney transplant desensitization and management of immune-mediated disorders, yet access remains limited across sub-Saharan Africa due to infrastructure, workforce, and supply chain constraints. While clinical efficacy is well established, there is limited literature describing how such programs can be successfully implemented in resource-constrained environments. This study describes the systematic development, implementation, and operational outcomes of a sustainable TPE program in Nigeria, with emphasis on practical strategies for replicat

Anti-GQ1b-Positive Miller Fisher Syndrome Following Pfizer Bivalent COVID-19 Vaccination.

Miller Fisher syndrome (MFS) is a rare, immune-mediated variant of Guillain-Barré syndrome characterized by acute onset of ataxia, areflexia, and ophthalmoplegia. It typically develops following an antecedent event, such as an infection or, in rare instances, a vaccination, which triggers the production of anti-ganglioside antibodies. While cases have been anecdotally linked to COVID-19 vaccination, serologically confirmed reports remain uncommon. We report a case of a 51-year-old, previously independent male who presented with acute neurological deterioration, including ascending numbness, se

Use of Therapeutic Plasma Exchange in Autoimmune Neuromuscular Disorders.

Therapeutic plasma exchange (TPE) plays a pivotal role in the management of autoimmune neuromuscular disorders by facilitating the removal of pathogenic autoantibodies, immune complexes, and proinflammatory mediators, often resulting in short-term symptomatic improvement. This monograph provides a comprehensive review of the indications, mechanisms, clinical applications, and evidence supporting TPE across a spectrum of autoimmune neuromuscular conditions. TPE is utilized in both acute and maintenance settings, particularly when rapid immunomodulation is required. However, sustained disease co

Prevalence and Patterns of Cranial Nerve Involvement in CIDP, Autoimmune Nodopathy, MMN, and Anti-MAG Neuropathy: A Multicenter Korea/UK Study of 582 Patients.

BACKGROUND: Cranial nerve involvement is a well-recognized feature in Guillain-Barré syndrome (GBS) but remains less well understood in chronic forms of autoimmune neuropathies. Earlier studies of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) were conducted before updated diagnostic criteria and the recognition of autoimmune nodopathy (AN), which may limit the interpretation of their findings. METHODS: We retrospectively analyzed 582 patients with chronic autoimmune neuropathies-CIDP (n = 431), multifocal motor neuropathy (MMN) (n = 64), anti-myelin-associated glycoprotein
