BACKGROUND: Among human immunodeficiency virus (HIV)-infected men who have sex with men (MSM) taking combination antiretroviral therapy (cART), the impact of rectal sexually transmitted infections (STIs) on rectal HIV-1 shedding is unknown.
METHODS: Human immunodeficiency virus type 1 (HIV-1) RNA was quantified from rectal swabs collected for Neisseria gonorrhoeae (GC) and Chlamydia trachomatis (CT) screening of HIV-1-infected MSM. Correlations of STIs with rectal viral load were explored using multinomial regression modeling. HIV-1 coreceptor tropism was predicted from sequencing in a subset

A recent increase of sexually transmitted infections (STI) of rectum and anus has been observed in France, particularly in men who have sex with men. Symptoms of STI are not specific and sometimes patients have no symptom, which implies a high risk of contamination. Ulcerations, vegetant lesions and proctitis are the main anorectal lesions observed. Infection by multiple germs is frequent. STI, particulary those including ulceration, facilitate the transmission of HIV. New therapeutic strategies have to be established as antimicrobial resistance of several sexually transmitted pathogens is inc

Little is known about the prevalence of rectal chlamydial infection amongst men who have sex with men (MSM). Previous studies using culture methods reported this to be between 4-6%. The emergence of nucleic acid amplification tests has significantly increased the sensitivity and specificity for chlamydial detection, making it possible to estimate the prevalence of rectal infection more accurately. A prospective cross sectional study involving 443 MSM who were screened for sexually transmitted infections (STIs) between May 1999 and January 2002. Rectal swabs for chlamydiae were obtained in addi

OBJECTIVE: To describe an outbreak of gonorrhoea caused by penicillin sensitive Wild type 1B2-Wt/1B2(FS), in homosexual men in Sydney.
DESIGN AND SETTING: The study sample comprised all gonococcal isolates referred to the Gonococcal Reference Laboratory (GRL), New South Wales, Australia between 1 January 1990 and 30 June 1992. Demographic data on Wt/1B2(FS) were sought by review of all request forms accompanying specimens to the GRL. Detailed review was undertaken of the clinical records of all men with gonorrhoea which had been differentiated by auxotype and serotype (A/S) from two large STD

Over a 13-year period non-gonococcal neisseriae (NGN) were isolated from 114 of 88,670 patients (0.13%) screened for anogenital gonorrhoea at a Genitourinary Medicine Unit. During the same period there were approximately 9000 anogenital gonococcal infections (10%). The prevalence of NGN was 0.09% (27/31,500) in women, 0.04% (20/52,800) in heterosexual men and 1.5% (67/4370) in homosexual men: the differences in prevalence between women and heterosexual men (P < 0.01) and between heterosexual patients and homosexual men (P < 0.001) are highly significant. Neisseria meningitidis was isolated mos

Heterosexual anal intercourse is rarely discussed in the scientific literature. Review of the literature suggests the silence is linked to ethnocentric discomfort about it among researchers and health care providers, coupled with the misconception that anal sex is a homosexual male practice, not heterosexual. Review of surveys of sexual practices suggest that heterosexual anal intercourse is far more common than generally realized, more than 10% of American women and their male consorts engaging in the act with some regularity. Sexually transmitted disease (STD) data, especially where only the

During the past two decades, an explosive growth in both the prevalence and types of sexually transmitted diseases has occurred. Up to 55 percent of homosexual men with anorectal complaints have gonorrhea; 80 percent of the patients with syphilis are homosexuals. Chlamydia is found in 15 percent of asymptomatic homosexual men, and up to one third of homosexuals have active anorectal herpes simplex virus. In addition, a host of parasites, bacterial, viral, and protozoan are all rampant in the homosexual population. Furthermore, the global epidemic of AIDS has produced a plethora of colorectal m

Diseases of the anus and rectum are frequently the outcome of proctogenital and oral-anal sexual activities. These sexually transmitted diseases are more common among homosexual and bisexual men than among heterosexuals. A variety of infectious agents are responsible including viruses, bacteria, spirochetes, helminths, and protozoa. Anal warts, herpetic ulcers, and syphilitic chancres are common anal STDs. Gonorrhea, herpes, and chlamydial organisms are common causes of venereal acute proctitis. Enteric infections such as shigellosis, amebiasis, giardiasis and pinworms can be transmitted by or

It is unclear whether anorectal gonorrhea in women is more difficult to cure than endocervical gonorrhea or whether anorectal test-of-cure cultures are always indicated. Using endocervical and anorectal test-of-cure cultures, the authors evaluated 1,124 women treated for culture-positive endocervical gonorrhea with a recommended regimen of ampicillin (n = 868), procaine penicillin G (n = 78), spectinomycin (n = 45), or tetracycline (n = 133). There were 35 failures (3.1%) among women with anogenital gonorrhea, including 11 (1.0%) among those with anorectal infection. Only eight failures (0.7%)

Physicians treating adolescents should take a complete sexual history, including sexual orientation and practices, to determine whether their patients are homosexually active. Lesbians are at very low risk for sexually transmitted diseases, but they do have other health concerns. Four general groups of conditions may be encountered in homosexually active men: classical sexually transmitted diseases (gonorrhea, infections with Chlamydia trachomatis, syphilis, herpes simplex infections, genital warts, pubic lice, scabies); enteric diseases (infections with Shigella species, Campylobacter jejuni,

The histology of the rectal mucosa in 57 men with culturally-proven rectal gonorrhoea is described and the results compared with the findings in biopsies from 57 non-infected men. An acute inflammatory cell infiltration of the rectal mucosa was found in three (5.3%) and two (3.5%) infected and non-infected men respectively. Twenty-one (36.8%) men with rectal gonorrhoea and nine (15.8%) non-infected men had an increased number of chronic inflammatory cells in the lamina propria.

Certain enteric ailments are particularly common among homosexual men. They are primarily infectious diseases and include not only such common venereal diseases as gonorrhea and syphilis but also infections not usually regarded as being sexually transmitted. Among the latter are shigellosis, salmonellosis, giardiasis, and amebiasis. Patients' symptoms are non-specific and seldom helpful in diagnosing particular diseases. The practitioner must be prepared to identify a number of infections with similar presentations that may occur singly or together in gay men. Gonorrhea is probably the most co

Thirty-nine homosexual men seeking treatment for symptomatic proctitis were examined for anorectal gonorrhea by study of gram-stained smears obtained via an anoscope and by a blindly obtained anorectal swab specimen. Anorectal specimens for culture were obtained from all men. The sensitivity of the anoscopically obtained smears was 79% as compared with 53% for the blindly obtained smears. It was concluded that gram-stained smears obtained through an anoscope can provide rapid diagnosis of most cases of symptomatic anorectal gonorrhea. All patients with symptomatic proctitis should have routine

About 2-2 1/2 million people do, or will repeatedly, participate in ano-rectal sexual intercourse in Britain alone. The anus and the rectum of these individuals are thus vulnerable to all the sexually transmitted diseases (except trichomoniasis). Male homosexuals appear to be more prone to these conditions than female heterosexuals, possibly because a large minority is indiscriminately promiscuous. Over the last 20 years homosexually acquired infections--particularly syphilis--have increased in Britain; these are now more readily admitted to because of changes in the law and in public opinion.

The infectious etiology of symptomatic anorectal disease was studied in 52 homosexual men who did not have gonococci on initial Gram stain of anorectal exudate. Herpes simplex virus (HSV) was isolated from the anal canal or rectum in 15 of the 52 (29 percent) men and characteristically caused severe anorectal pain and focal ulcerations visible on sigmoidoscopy. Despite negative initial Gram stains, seven men (14 percent) had anorectal gonococcal infection. Six (12 percent) had syphilis, including two with dark-field positive anal lesions. Four were infected with enteric pathogens, including Gi

A retrospective assessment of 159 female patients who had undergone proctoscopy was carried out between January and September 1977. One hundred and twenty-seven (80%) were known contacts of gonorrhoea; of these, 63 (49.6%) were found to be infected with Neisseria gonorrhoeae. Of these, 29 (46%) harboured gonococci in the rectum as well as in the urethra and cervix while four (6.3%) harboured gonococci only in the rectum. Gram-stained smears gave positive results in only 12 of the 29 cases of rectal gonorrhoea, which indicates the importance of culturing rectal material. It is recommended that

Prevalence and site-pathogen studies of pathogenic Neisseria were conducted in 815 homosexual men over a 12-month period. Neisseria meningitidis was isolated from more patients than N gonorrhoeae (43.1% vs 33.1%). A high oropharyngeal carriage of n meningitidis (42.5%) was present, with much lower urethral (0.7%) and rectal (2.0%) colonization. Although N meningitidis urethral isolates were associated with urethral discharge in five of six patients, N meningitidis in the oropharynx or rectum was not usually associated with clinical illness. Neisseria meningitidis was usually isolated from only

The results of Gram stains and cultures from 145 women with uncomplicated ano-genital gonorrhoea are presented. The site which gave the highest yield of gonococci was the cervix. Equally good results were obtained with Stuart's transport medium and direct plating in the clinic. Positive results from rectal specimens alone were obtained from 8.4% of the 119 women in whom the rectum was examined. In 89 women the results of rectal specimens taken through a proctoscope were compared with those of swabs passed blindly up the anal canal. When Stuart's transport medium was used swabs taken through a
