Drinking habits and peripheral alcoholic neuropathy.

Drinking habits of 156 consecutive polyneuropathic and 106 consecutive pressure palsy patients were evaluated in retrospect. Respectively, 46 patients (30%) had alcohol polyneuropathy and 32 (30%) got pressure neuropathy while being drunk and these patients were analyzed in more detail. Most of the patients with alcoholic neuropathies were men, those with polyneuropathy being older than those having pressure palsies. Pressure neuropathy coincided with alcoholic polyneuropathy in 13 patients (28%). Other medical complication of heavy alcohol drinking (i.e. liver diseases, seizures and cerebella

Electromyographic study of diabetic and alcoholic polyneuropathic patients treated with gangliosides.

A double-blind, randomized electromyographic investigation was conducted of the effects of cerebral ganglioside treatment on patients suffering from diabetic or alcoholic polyneuropathy. Cerebral gangliosides (50 mg once a day) administered to 15 diabetic and to 15 alcoholic neuropathic patients for 40 days, facilitated the reappearance of sensory potentials and significantly increased the MAP amplitude in median, ulnar, and peroneal nerves. In relation to ganglioside treatment, there was no significant change in the conduction velocities or in the distal latencies of these nerves, nor was the

[Polyneuropathies. Clinical aspects and experimental principles].

Polyneuropathies are diffuse diseases of the peripheral nervous system. Pathological and anatomical investigations of the nerve fibres show that alterations of the peripheral axon and myelin are most significant at the distal end of the nerve fibres. Alcoholic polyneuropathy primarily affects the axon; during the chronic course of the disease, however, the myelin sheath is involved into the pathological process. Polyneuropathies are characterized by a deficiency and metabolic disturbance of the vitamins B1, B6 and B12. The vitamins of the B-group not only play an important role in the intermed

[Alcoholism, polyneuropathy, and liver disease. A prospective electrophysiologic study].

A prospective clinical and electrophysiologic study was performed in 50 alcoholic patients in order to assess the incidence and clinical manifestations of alcoholic polyneuropathy, the value of the electrophysiologic parameters studied, and the influence of liver disease on the appearance of neuropathy. The proposed criteria for polyneuropathy were fulfilled by 24% of the patients. Abolition of the Achillean reflexes was the most frequent clinical finding, while reduced amplitude of muscular potential was the most sensitive electrophysiologic parameter. There were no differences in alcohol con

Carbon disulfide neuropathy in rats. A morphological and ultrastructural study of degeneration and regeneration.

The aim of this study was to elucidate the site and detailed nature of peripheral nerve damage induced in the rat by chronic CS2 inhalation exposure in the light of the relationship between pathological and neurophysiological data. Adult male rats were exposed to 700 ppm of CS2 2 h/d, 5 d/week for 12 weeks and then followed-up for 18 weeks. The first alteration observed was a decrease in the nerve conduction velocity, discovered after only 3 weeks of exposure. Pathological lesions were first observed in the 10th week and consisted of a typical "giant axon" axonopathy. Obvious pathological lesi

[Alcoholic polyneuropathy].

A classification of alcoholic polyneuropathy is offered. This classification was developed by the author on the basis of neurological examinations of 354 alcoholics. Combinations of various disturbances of peripheral innervation (within the limits of a polyneuropathy) both between each other and with various cerebral disturbances are described. As a result, the following forms of the alcoholic polyneuropathy are distinguished: vegetative, sensor, mixed, pseudodiabetic, and motor ones, as well as Korsakov's syndrome. Both the cerebral and the peripheral disturbances are transient: on being deve

F-wave conduction velocity in alcoholic polyneuropathy.

We used the F-wave conduction velocities to estimate the motor nerve conduction along the proximal segment (spinal cord to knee) of the axon of tibial and common peroneal nerves in ten patients with alcoholic polyneuropathy and compared these to the motor nerve conduction velocities in ten normal subjects. All the studies so obtained showed a normal conduction in the proximal segments even though in three patients the distal nerve conduction values (knee to ankle) revealed moderate to severe neuropathy. These data support the hypothesis that in alcoholic polyneuropathy, motor conduction in pro

Evidence of vagal neuropathy in chronic alcoholics.

Autonomic function was tested in healthy control subjects and 20 chronic alcoholic patients with varying degrees of alcohol-related peripheral and central neurological damage. The alcoholics were divided into two groups according to the severity of their symptoms and signs. The alcoholic subjects had no postural hypotension. However, heart-rate responses to Valsalva's manoeuvre, deep breathing, change in posture, baroreceptor stimulation, and atropine were lower in those alcoholics with a greater degree of peripheral and central nervous damage than in the less severely affected alcoholics and

[Decompression of individual nerves in anatomical strictures in polyneuropathies].

During clinical and electromyographical examination of one patient with diabetic and one with alcoholic polyneuropathy we found an especially severe lesion of the ulnar nerve at the elbow on one side. As the polyneuropathy generally affects the nerves symmetrically we presumed in addition some other most probably ulnar nerve compression syndrome at the elbow. Both patients were operated and the nerves were transposed (Prof. NIGST). The local finding during the operation, the improvement of the paresis of ulnar nerve and the worsening of the polyneuropathy--found simultaneously at control after

Bulbocavernosus reflex in normal men and in patients with neurogenic bladder and/or impotence.

The bulbocavernosus reflex was investigated electrophysiologically in 14 normal adult male subjects and in 80 patients with neurogenic bladders and/or impotence due to various neurological causes as well as in patients with functional impotence. The glans penis was stimulated superficially by single electrical shocks and the reflex responses were recorded from the bulbocavernosus (BC) and the striated anal sphincter muscles by means of concentric needle EMG electrodes. In all normal subjects, the BC reflex was recorded from the BC muscle as a stable and constant response having a mean latency

Two patterns of results in polyneuropathies investigated with the H reflex. Correlation between proximal and distal conduction velocities.

The Hoffman (H) reflex of the soleus muscle has been used to study peripheral neurological disorders in 79 cases of chronic renal insufficiency prior to the initiation of haemodialysis, and in 94 cases of chronic alcoholism. In both groups, care was taken to ensure that the subjects examined could not be suspected of any other type of neuropathy. The results have been compared with those obtained in 42 normal subjects. The nerve conduction velocity in the reflex pathway was evaluated by an H index formula in which the conduction time (interval between M and H responses) is related to the lengt

Toxic polyneuropathy of shoe-industry workers. A study of 122 cases.

The toxic polyneuropathy observed in a group of shoe-industry workers in Italy was clinically characterised by a symmetrical prevalently distal motor deficit, with occasional marked weakness of pelvic girdle muscles, and frequently by only subjective sensory symptoms; non-specific disturbances usually preceded neurological signs. Subclinical cases of 'minimal' chronic neuropathy, characterised by alterations of a neurogenic type in the EMG without a reduction of motor nerve conduction velocity, were also observed. Worsening of the clinical picture, with further lowering of nerve conduction vel

[Alcohol and malabsorption in the pathogenesis of peripheral and central nerve damage (author's transl)].

Xylose absorption was measured, within ten days of being admitted to hospital, in 54 alcoholics with neurological abnormalities. Small-intestine malabsorption was demonstrated in 19. Classified according to the customary clinical diagnoses, 16 of 49 with alcohol polyneuropathy had abnormal values, with no correlation to nerve conduction velocity. There was also no difference among 14 in predelirium and eight in delirium. However, all four patients with Wernicke's encephalopathy had a malabsorption syndrome. These results are similar, also quantitatively, to those reported in the literature in

[Alcoholic polyneuropathy. Electrophysiological and clinical findings in 85 patients (author's transl)].

Maximum motor and sensory nerve conduction velocities, amplitudes of muscle action potentials (surface electrodes) and of sensory nerve action potentials, and needle electromyograms were studied in 80 patients with alcoholic neuropathy and in 5 chronic alcoholics without clinical signs of neuropathy. The electrophysiological results were compared to the clinical findings. Neurographic criteria compatible with the diagnosis "alcoholic neuropathy" were defined. Neurographic findings contrary to a diagnosis of alcoholic etiology were demonstrated in 3 patients. Only in a smaller group of patients

[Tumor- and drug-induced cutaneous axonal dystrophy. An electronmicrocopy proof of multiple lamellated bodies].

A marked cutaneous axonal dystrophy has been observed electronmicroscopically for the first time in the skin of three patients: (a) lesion of pityriasis lichenoides chronica in a patient with bronchogenic carcinoma, (b) non involved skin of a patient with malignant melanoma and (c) non involved skin of a patient with gout and retinal damage after prolonged use of chloroquine. The affected myelinated and non myelinated axons showed distinct alterations of mitochondria and multiple osmiophilic lamellated bodies (LK). These changes were interpreted as a (poly-)neuropathy, due to the influence of
