Thermal threshold determination in alcoholic polyneuropathy: an improvement of diagnosis.

Reports on the incidence of alcoholic polyneuropathies are variable depending on diagnostic tools. In this study, 50 chronic alcoholics with positive MALT (Munich Alcoholism Test) and greater than seven years history of excessive alcohol abuse were examined neurologically. Tibial and peroneal motor and sural nerve conduction velocities (NCV) were studied. Warm and cold perception was evaluated in the area behind the internal malleolus using a Somedic-Thermotest. Thresholds were determined by the method of limits. The effect of a slow, medium and fast temperature change rate on thermal percepti

Remission of severe alcoholic polyneuropathy after liver transplantation.

Alcoholic polyneuropathy presents with variable severity. Only few very severely disabled patients showed good clinical and electrophysiological improvement after cessation of alcohol abuse. We report a 55 year old patient with polyneuropathy who underwent orthotopic liver transplantation for decompensated alcoholic cirrhosis. After an initial postoperative exacerbation of her neurological disorder the patient developed an impressive remission of her clinical condition during the 4 year post transplantation follow up period. Nine months after surgery the initially wheel chair bound patient reg

Vibrameter testing facilitates the diagnosis of uremic and alcoholic polyneuropathy.

The diagnostic sensitivity of Vibrameter and tuning fork examination towards uremic and alcoholic neuropathy was tested in 75 patients. In 40 uremic and 35 alcoholic patients, we compared the sensitivity of neurological examination, nerve conduction studies (NCS) and vibration thresholds assessed at the malleoli by means of Vibrameter and scaled tuning fork. Vibrameter results were correlated with NCS. Polyneuropathy was diagnosed in 52 patients, but in 16 patients diagnosis depended upon inclusion of Vibrameter testing in the examination protocol. In uremic patients, Vibrameter (47.5%) showed

Peripheral neuropathy induced by intravenous administration of vincristine sulfate in the rabbit. An ultrastructural study.

The lack of a suitable animal model for the peripheral neuropathy that often follows the systemic administration of the chemotherapeutic agent vincristine sulfate (VCR) has hampered the correlation between experimental and clinical patterns of this neuropathy. New Zealand rabbits have been recently found to develop, after iv injection of a VCR total dosage similar to that used in humans, a peripheral polyneuropathy characterized by electrophysiological changes that overlap those observed in the clinical setting. The present study was aimed at investigating the ultrastructural features of 3 dif

Chemotherapy of advanced inoperable non-small cell lung cancer with paclitaxel: a phase II trial.

Paclitaxel (Taxol; Bristol-Myers Squibb Company, Princeton, NJ) has demonstrated significant antineoplastic activity against different tumor types, notably ovarian and breast carcinoma. Two phase II trials of 24-hour paclitaxel infusions in chemotherapy-naive patients with stage IIIB or IV non-small cell lung cancer (NSCLC) reported response rates of 21% and 24%. Leukopenia was dose limiting: as many as 62.5% of patients experienced grade 4 leukopenia. We investigated the efficacy and toxicity of a 3-hour paclitaxel infusion in a phase II trial in patients with inoperable stage IIIB or IV NSCL

Dietary factors in epidemic neuropathy on the Isle of Youth, Cuba.

An epidemic neuropathy that broke out in Cuba in late 1991 has exhibited clinical manifestations similar to those of other polyneuropathies of nutritional origin. To investigate its possible association with diet, a study was conducted on the Isle of Youth in 1993, at the start of an outbreak there. Thirty-four subjects with cases and 65 controls were interviewed regarding their diets, measured anthropometrically, and evaluated. As a whole, the subjects with cases demonstrated greater weight loss before the onset of disease, lower body mass indexes (BMI), lower percentages of body fat, and mor

A Mailed-questionnaire Survey of Ex-workers: A Tool to Evaluate the Healthy-worker Effect in Cross-sectional Studies.

In order to evaluate a possible healthy-worker effect in a cross-sectional study in a viscose rayon plant, a survey of ex-workers was conducted. From the personnel records of this factory, 475 male, Caucasian, Dutch-speaking, carbon disulfide (CS&inf2;)-exposed workers who had been employed ten years prior to the cross-sectional study were selected for study. Of these, 142 were still employed in the same plant, and 64 were deceased. Retrieval of the causes of death proved impossible. A questionnaire was mailed to the remaining 269 men, of whom 149 completed and returned it. Eighty-nine subject

Is heat hypoalgesia a useful parameter in quantitative thermal testing of alcoholic polyneuropathy?

Detection of thermal hypoaesthesia, hyperalgesia, and paradoxical sensation significantly contribute to the diagnosis of polyneuropathy (PNP). There is controversy about the clinical usefulness of detected heat hypoalgesia. In 50 chronic alcoholic patients we compared the prevalence and diagnostic value of heat hypoalgesia (HPT) to that of cold (CT) and warm (WT) hypoaesthesia using a "Marstock" thermotest. Clinical examination revealed PNP in 56%, cold hypoaesthesia was present in 62%, warm hypoaesthesia in 24%, paradoxical thermal sensation in 10%, cold and heat hyperalgesia in 12%, and heat

Nutrition and alcohol neurotoxicity.

Neurological complications of alcoholism such as Wernicke-Korsakoff syndrome and polyneuropathy often originate from interactive factors involving direct nervous system toxicity of ethanol and nutrient deficiencies associated to heavy drinking. Not all patients are equally susceptible to these disorders and a genetic predisposition to thiamine deficiency has been described in subjects with Wernicke's encephalopathy. At moderate alcohol dosages, nutrient abnormalities may be marginal, inducing no obvious manifestations until other neurotoxic agents are absorbed. Examples are presented illustrat

Postoperative complications in patients receiving suramin therapy.

BACKGROUND: Suramin is an antiparasitic agent that is currently being evaluated for antineoplastic activity. Documented toxicities of suramin include adrenal and renal insufficiency, coagulation factor abnormalities, immunosuppression, and polyneuropathy. These adverse effects have potential for contributing to postoperative morbidity in surgical patients. Because no experience with suramin has been reported in the surgical literature, this 5-year retrospective review of postoperative complications in patients receiving suramin was performed. METHODS: From a review of 171 charts, 14 patients

Suramin in adrenocortical cancer: limited efficacy and serious toxicity.

OBJECTIVE: No satisfactory treatment for adrenocortical carcinoma (ACC) is available. We investigated the efficacy and toxicity of suramin in the treatment of metastatic ACC since suramin has been recently reported to be active as a single agent therapy for patients with ACC and prostatic carcinoma. DESIGN: We collected data on 9 patients with metastatic ACC treated with suramin in four centres in Germany between 1987 and 1992. PATIENTS: Nine patients (5 women, 4 men; age range 32-67 years) were included. Biochemical evidence of steroid excess was found in 6/9, in three leading to clinical s

Sudomotor potential evoked by magnetic stimulation of the neck.

We studied the sudomotor potential in the palm of the hand, evoked by magnetic stimulation of the neck, in 15 normal subjects and five patients with dysautonomia. The potentials consisted of two positive peaks (S1, S2), with mean (+/- SD) onset latencies of 408.5 +/- 31.8 and 619 +/- 61.4 msec. The sudomotor potential was clearly obtained only from the palm and was influenced by stimulus intensity, coil position, current direction in the coil, atropine or pilocarpine iontophoresis, and motor effort. With this method, we estimated the postganglionic sympathetic conduction time and the central r

Neurotoxicity of Taxol.

Neurotoxicity, manifested primarily by a motor and sensory polyneuropathy, is the principal nonhematological side effect of Taxol. Available evidence suggests that Taxol produces a toxic effect involving either axons or ganglion cell bodies, or both, rather than a myelinopathy. As with other toxic polyneuropathies, patients with preexisting peripheral neuropathies, such as those caused by diabetes mellitus or ethanol, appear to be particularly predisposed to developing neurological toxicity. The incidence and severity of the neuropathic manifestations also appear to be related to the Taxol dos

Clinical toxicities encountered with paclitaxel (Taxol).

Although paclitaxel (TAXOL) appears to be one of the most promising antineoplastic agents of the last decade, with demonstrated activity in advanced and refractory ovarian, breast, lung, and head and neck cancers, most clinical oncologists have had little experience with the agent. This is largely the result of the initially limited supply of paclitaxel and other obstacles encountered during early clinical development that restricted the drug's availability to a few investigational centers. Although a high incidence of major hypersensitivity reactions due to the Cremophor EL vehicle used in fo

[Electroneuromyographic study of alcoholic polyneuropathies and the possibilities of using anticholinesterase drugs].

The efficacy of anticholinesterase (ACE) drugs was studied and compared in patients with alcoholic polyneuropathy (AP) subjected to electroneuromyographic (ENMG) studies. ENMG makes it possible to identify AP, to define the degree of the involvement of different components of the neuromotor apparatus into the pathological process. It has been established that ACE drugs may have individual action selectivity in patients afflicted with AP.

[Alcoholic polyneuropathy and myopathy--a contribution to clinico-neurologic diagnosis of alcoholic patients].

Neurological lesions due to alcohol most frequently affect the peripheral nervous system. Alcoholic myopathy or the combination of alcoholic neuropathy and myopathy are less frequent. The features of these disorders are discussed in detail. To support the clinical diagnosis of alcoholic neuropathy and/or myopathy various criterions corresponding to their clinical relevance are set up. It is suggested that a clear diagnosis ordinarily points to alcohol endangering or even to alcohol addiction and it therefore is incompatible with administering a driving license.

[Neurolysis of the posterior tibial nerve in the tarsal canal--a new therapeutic approach in chronic painless plantar ulcers].

The paper reports on 14 cases of perforating plantar disease of whom 11 belong to the so-called nonpainful chronic plantar ulceration by compression of the posterior tibial nerve in the tarsal canal. The authors show clinically, neuroelectrically and by neurography with lipiodol that in the case of a diabetic or alcoholic polyneuropathy, the posterior tibial nerve is more vulnerable to the compression existing in the tarsal canal due to the development of a fibrous sclerotic tissue.
