The Alcoholic Polyneuropathy occurs in about 10-30% of alcoholics. It is the second most frequent type of polyneuropathies after the diabetic form. The clinical pattern is a symmetric sensory or symmetric motor sensory manifestation type. In almost all cases there is a pressure pain of the calves. In the beginning the disturbance of the proprioceptive sensation is predominant. Disturbances of the autonomic nervous system deal with the sympathetic as well as the parasympathetic nervous system. Benfogamma efficacy in alcoholic polyneuropathy therapy with pain syndrome and other sensor disorders has been studied. The most frequent consequence of chronic alcohol intake is a toxic polyneuropathy. It results from inadequate nutrition, mainly deficiency of thiamine and other B vitamins. In a randomized double-blind study involving 303 patients with alcoholic polyneuropathy the efficacy and tolerability of the combinations thiamine/pyridoxine, benfotiamine/pyridoxine, and the nucleotides of cytidine and uridine administered orally for 21 days were compared. Pain and paraesthesia, measured on a visual 10-cm-long analogue scale, as also pallaesthesia and the strength of the dorsiflexion and plantar flexors of the foot, clearly improved in all treatment groups. Clinically relevant differences in efficacy were not observed. All the drugs tested were well tolerated.

The efficacy of anticholinesterase (ACE) drugs was studied and compared in patients with alcoholic polyneuropathy (AP) subjected to electroneuromyographic (ENMG) studies. Neurological lesions due to alcohol most frequently affect the peripheral nervous system. Alcoholic myopathy or the combination of alcoholic neuropathy and myopathy are less frequent. The features of these disorders are discussed in detail. To support the clinical diagnosis of alcoholic neuropathy and/or myopathy various criterions corresponding to their clinical relevance are set up. It is suggested that a clear diagnosis ordinarily points to alcohol endangering or even to alcohol addiction and it therefore is incompatible with administering a driving license.
