Unilateral vitreous amyloidosis as the presenting feature of hereditary transthyretin amyloidosis (TTR p.Val50Met): a case report and focused literature review.

BACKGROUND: Hereditary transthyretin amyloidosis (ATTRv) is an autosomal dominant systemic disorder characterized by extracellular amyloid deposition. Ocular manifestations typically present bilaterally, and unilateral vitreous amyloidosis without systemic symptoms or family history is rare. Few reports have documented the laterality pattern of vitreous amyloidosis or its role as an early indicator of systemic disease. CASE PRESENTATION: We present a 67-year-old Chinese man who experienced progressive left eye vision loss over 1 year. Dense vitreous opacities in the left eye initially mimicke

Sex-related phenotypic differences in myotonic dystrophy types 1 and 2: a nationwide registry study from Serbia.

BACKGROUND: Myotonic dystrophies (DM) are autosomal dominant, multisystem disorders with variable clinical expressions. DM1 and DM2 share overlapping features, such as myotonia and muscle weakness, but differ in age at onset, severity, progression and multisystem features. Emerging evidence suggests that biological sex influences disease manifestations, progression, and outcomes. OBJECTIVE: To assess sex-specific differences among patients with genetically confirmed DM1 and DM2. PATIENTS AND METHODS: This cross-sectional study included 587 adult patients (391 DM1, 196 DM2) from the Serbian R

Recurrence of B-cell Lymphoma Manifesting as Progressive Peripheral and Cranial Nerve Neuropathies: A Case Report.

Neurolymphomatosis (NL) is a rare neurological complication of lymphoma, characterised by direct infiltration of peripheral nerves by neoplastic cells. It most commonly occurs in association with non-Hodgkin's lymphoma (NHL) and poses significant diagnostic challenges due to its non-specific clinical presentation. Classically, NL presents as a painful, rapidly progressive, and asymmetric neuropathy, frequently mimicking inflammatory or vasculitic neuropathies. Here, we present the case of a 69-year-old woman with a history of NHL, in remission for five years, who presented with a progressive,

Combined intervention: hydroxysafflor yellow A acts synergistically with aerobic exercise to alleviate cerebral ischemia-reperfusion injury via the HIF-1α/CREB pathway.

BACKGROUND: Clinically amyopathic dermatomyositis (CADM) is a rare subtype of idiopathic inflammatory myositis often linked with the presence of autoantibodies targeting melanoma differentiation-associated protein 5 (MDA5). Patients with CADM are at increased risk of developing rapidly progressing interstitial lung disease, which significantly increases both morbidity and mortality compared to other forms of inflammatory myopathies. While there is no standardized treatment regimen, current therapeutic strategies are generally focused on combination immunosuppressive therapies. Despite early di

Significance of aberrant nerve conduction in hereditary transthyretin amyloidosis.

BACKGROUND: In hereditary transthyretin (ATTRv) amyloidosis, axonal neuropathy is the main pathologic basis of polyneuropathy. However, whether nerve conduction slowing resembling chronic inflammatory demyelinating polyneuropathy (CIDP), observed in some patients, reflects primary demyelination or secondary changes related to axonal damage remains unclear. METHODS: Electrophysiologic findings were compared between 96 patients with V30M ATTRv amyloidosis from nonendemic areas and 175 patients with nutritional or alcoholic axonal neuropathies. Myelinated fiber density was assessed in sural nerv

Association of corneal nerve parameters with nerve abnormalities and neuropathic pain in prediabetes and type 2 diabetes: the Maastricht Study.

AIMS/HYPOTHESIS: Corneal confocal microscopy is a valuable technique for assessing neuropathy; however, whether it can distinguish painful from painless neuropathy remains uncertain and existing evidence is based on the results of smaller studies. This study assessed the association of corneal nerve parameters with abnormalities identified by electromyography (EMG) and neuropathic pain in a large population with and without (pre)diabetes. METHODS: In this study we included cross-sectional data for 3425 participants from the Maastricht Study. Wide-field corneal confocal microscopy (WF-CCM) was

Targeting risk factors and strategies for prevention of peripheral neuropathy: focus on B-vitamin deficiencies and metabolic diseases.

BACKGROUND: Polyneuropathy (PN) is a common neurological disorder characterized by sensory, motor, and autonomic dysfunction of peripheral nerves. Its incidence is increasing, especially among the elderly population, making PN a significant and growing public health concern worldwide. MAIN BODY: This review highlights the key risk factors contributing to the development of PN, with a particular focus on nutritional and metabolic etiologies. These include B-vitamin deficiencies, prediabetes, diabetes mellitus, obesity, and treatment-induced neuropathies. The complex interplay of these factors o

A Large Retrospective Observational Study of Nonpharmacologic Treatment Use Among German Patients Receiving Long-Term Opioid Therapy for Chronic Noncancer Pain.

INTRODUCTION: Evidence-based practice guidelines recommend combining the long-term use of opioids for chronic noncancer pain (CNCP) with suitable nonpharmacologic treatments. Whether long-term opioid therapy (LTOT) in Germany aligns with these recommendations is unknown so far. Therefore, our objective was to determine the use of nonpharmacologic treatments among patients receiving LTOT for CNCP. METHODS: The retrospective observational analysis was based on administrative claims data of the German statutory health insurance fund DAK-Gesundheit. Patients with LTOT and no cancer diagnosis were

Intensive care unit acquired weakness: an update in the context of respiratory virus infection.

INTRODUCTION: Intensive care unit-acquired weakness (ICUAW) is one of the most common neuromuscular impairments featuring symmetric and generalized weakness, and typically occurs in critically ill patients, particularly those with sepsis. Mechanistically, ICUAW shares overlapping immunological networks with respiratory viruses. While accumulating evidence supports an association between Guillain-Barré syndrome (GBS) and respiratory viruses, the causal relationship between ICUAW and these pathogens has not been sufficiently documented. AREAS COVERED: This review summarizes current knowledge on

First-Generation TTR Silencing Therapies in Hereditary Transthyretin Amyloidosis With Polyneuropathy: Real-World Insights From a German Single-Referral Center.

BACKGROUND: Hereditary transthyretin (ATTRv) amyloidosis is a rare, genetically heterogeneous disease leading to an unstable configuration of transthyretin (TTR) and hence irregular deposition of TTR amyloid fibrils. The disease predominantly affects the peripheral nervous system, resulting in a progressive sensorimotor polyneuropathy (ATTRv-PN), but can also involve the cardiac and other organ systems. The TTR mRNA silencers patisiran and inotersen effectively reduce serum TTR levels, thereby improving neurologic disability and quality of life in patients with ATTRv-PN. This real-world study

Risk factors for distal symmetric polyneuropathy in patients with type 2 diabetes mellitus: A retrospective study.

The pathophysiology of distal symmetric polyneuropathy (DSPN) is multifactorial, involving chronic hyperglycemia, oxidative stress, accumulation of advanced glycation end products, impaired microvascular perfusion, metabolic toxicity, and nutritional deficiencies. Although previous studies have identified various risk factors, substantial heterogeneity exists across regions and populations, and most investigations have not comprehensively integrated metabolic markers, microvascular damage, and electrophysiological data. Therefore, a multidimensional assessment of DSPN risk factors is essential

Determinants of Spousal Loneliness Among Caregivers for Patients with Proliferative Diabetic Retinopathy: A Cross-Sectional Study.

OBJECTIVE: Proliferative diabetic retinopathy (PDR), the most severe form of diabetic retinopathy, imposes overwhelming caregiving demands on family caregivers, with spouses typically assuming the primary caregiving role. However, their psychosocial well-being, particularly loneliness-a major modifiable risk factor for adverse health outcomes-remains severely understudied. Guided by the relational model of stress, this study aimed to assess the prevalence of loneliness and identify its multilevel determinants among spousal caregivers of patients with PDR. METHODS: In this cross-sectional stud
