BACKGROUND: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy and is more prevalent in patients with diabetes. However, the underlying mechanisms linking diabetes and CTS have not been fully elucidated. The advanced glycation end products (AGEs) and the receptor for AGEs (RAGE) are involved in chronic inflammation and diabetic complications, suggesting that this pathway may be relevant to the pathogenesis of CTS in patients with diabetes.
QUESTIONS/PURPOSES: (1) Is RAGE expressed in the flexor tendon synovium in patients with CTS, and if so, is it expressed more in patient

Diabetic distal symmetric polyneuropathy (DDSP) is the most common form of diabetic neuropathy. DDSP, foot ulcerations, and lower limb amputation greatly increase mortality, which is 23% 2 years after a foot ulceration and rises to 71% after 10 years. DDSP is seen in patients with both type 1 and type 2 diabetes, but the clinical features and pathophysiology are distinctly different. DDSP also presents in patients with pre-diabetes and/or obesity alone, indicating that glycaemic control is not the sole contributor to the pathophysiology of DDSP. In the BARI 2D trial, 51% of those with type 2 d

Peripheral neuropathy is a common complication in diabetes, affecting around 50% of the diabetic population. Co-occurrence of diabetic peripheral neuropathy (DPN) and diabetic bone disease has led to the hypothesis that DPN influences bone metabolism, although little experimental evidence has yet supported this premise. To investigate, mice were fed a high-fat diet (HFD) followed by phenotyping of skeletal-innervating neurons and bone architectural parameters. Results showed that HFD feeding resulted in a marked decrease in skeletal innervation (69%-41% reduction in Beta-III-Tubulin-stained ne

BACKGROUND: Leber hereditary optic neuropathy (LHON) is a rare inherited mitochondrial disease that leads to mitochondrial dysfunction, resulting in optic nerve damage and vision loss. Systemic involvement has been reported in several LHON cases, referred to as LHON+ disorders. However, the causes and presentations of such conditions have been poorly studied. It is suggested that 90% of mitochondrial dysfunction is caused by one of three primary point mutations in mitochondrial DNA that affect respiratory complex I (referred to as mtDNA LHON), with unresolved cases of LHON being caused by othe

Charcot-Marie-Tooth (CMT) disease is one of the most common inherited neuropathies, leading progressive muscular weakness, pes cavus, loss of deep tendon reflexes, distal sensory loss, and gait impairment. Charcot Marie Tooth Neuropathy Scale (CMTNS) represents the gold standard to assess and grade the impairment in CMT patients. The aim of this retrospective study was to assess the correlation between CMTNS and functional status in CMT patients. We analyzed data of patients with diagnosis of CMT, collecting the following data: age; gender; BMI; CMT type (Demyelinating- Axonal- Other or mixed

Multiple myeloma is a neoplastic disease characterised by the proliferation of clonal, atypical plasma cells. In cancer cells, the balance between two paths of cell death, necroptosis and apoptosis, is disrupted. The aim of this study was to analyse the occurrence of polymorphisms in genes encoding key proteins for the necroptosis process, i.e., RIPK-1, RIPK-3 and MAPKAPK2. We investigated the potential relations between the occurrence of genetic variability and the clinical course of the disease. We analysed six single-nucleotide polymorphisms in a population of patients with multiple myeloma

POEMS syndrome is a rare paraneoplastic disorder characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, and skin changes. Although Castleman disease (CD) is observed in up to 30% of POEMS cases, it is typically of the multicentric subtype (MCD) and identified during the initial diagnostic workup. Unicentric Castleman disease (UCD), by contrast, is rarely associated with POEMS and has not been previously reported following POEMS remission. We present the case of a 39-year-old woman who was diagnosed with POEMS syndrome based on clinical, radiographic, an

Lower back pain is a common primary care symptom with musculoskeletal, neurological, neoplastic, infectious, or inflammatory causes. It remains the leading cause of years lived with disability, and it is linked to modifiable factors such as occupational exposure, smoking, and high body mass index (BMI), highlighting the need for preventive, lifestyle-focused strategies. To illustrate these challenges, a 60-year-old man is presented with a 20-year history of type 2 diabetes and a background of alcohol use disorder and suboptimal therapeutic compliance, resulting in poor glycemic control. He pre

Small fiber neuropathy (SFN) is a common neurological diagnosis with a multitude of symptoms, including dysautonomia symptoms (e.g., orthostatic dizziness) as well as sensory disturbances, such as a tingling sensation or burning pain. The disease can be due to metabolic, autoimmune, or hereditary factors, and treatment plans stem from SFN etiology. Diabetes, autoimmune disease, infection, vitamin deficiencies, post-vaccination syndromes can all cause SFN. Needle electromyography and nerve conduction studies can be performed to exclude other neuromuscular conditions by ruling out large fiber ne

Diabetes mellitus is a metabolic condition leading to elevated blood glucose levels due to insulin deficiency, insulin resistance, or a combination of both. Chronically raised blood glucose levels can lead to a broad variety of microvascular and macrovascular complications. Neurological disorders are a common manifestation of diabetes mellitus, and poorly controlled diabetes mellitus frequently causes peripheral sensorimotor polyneuropathy and autonomic neuropathy. More rare manifestations include diabetic amyotrophy and mononeuritis multiplex. Hemichorea-hemiballismus (HCHB) syndrome is a mov

BACKGROUND: Sleeve gastrectomy (SG) has become the most commonly performed bariatric procedure due to its favorable weight-loss outcomes and lower complication risks. However, serious neurological complications can still occur. We present a case of severe copper deficiency-related polyneuropathy following SG in a 48-y-old man.
CASE PRESENTATION: A 48-y-old man underwent laparoscopic SG for morbid obesity. Three months postoperatively, he developed poor oral intake. At 7 mo, he presented with bilateral distal paresthesia and gait disturbance. Symptoms transiently improved after vitamin B suppl

BACKGROUND: Charcot-Marie-Tooth disease type 2B (CMT2B) is an autosomal dominant neuropathy associated with pathogenic variants in the RAB7A gene, which encodes a GTPase involved in lysosomal and lipid metabolism. Although altered lipid metabolism has been observed in skin fibroblasts derived from patients with CMT2B, a clinical presentation with abnormal fat distribution has not been reported.
CASE PRESENTATION: We report two sisters with a confirmed pathogenic RAB7A variant (NM_004637.6:c.484G > A, (p.Val162Met)), who presented not only with classic CMT2B features but also with abnormal fat

INTRODUCTION: We determined the semiology, causes, risk factors and outcomes of patients with acute nutritional neuropathies requiring hospitalisation in high-income countries.
METHODS: We searched PubMed, Cochrane Library and Embase for non-alcoholic malnutrition neuropathy cases published since 1990.
RESULTS: Thirty-six studies were identified, including 83 patients (mean age 39.8 years; 70% women) with deficient levels of thiamine (54%), pyridoxine (16%), copper (11%) or folate (7%). Common clinical signs were lower extremity weakness (100%), hypesthesia (63%), hypo-/areflexia (54%) and c

GLP-1 agonists have been a welcome addition to the armamentarium in the treatment of diabetes and obesity. While this class of medications is generally considered safe and effective, potentially severe neurological complications may be associated with rapid weight loss and nutritional deficiencies following GLP-1 agonist use. We present a 37-year-old woman who was prescribed semaglutide for diabetes and weight loss who subsequently experienced significant weight reduction and rapid glycemic control over three months. Thereafter, she developed progressive right leg numbness and weakness, follow

Bariatric surgery is being undertaken more frequently in response to rising levels of obesity but is increasingly also requested as a cosmetic choice. Nutritional deficiencies are a recognised consequence of gastrectomy, with potentially severe and permanent neurological sequelae. We present two cases of acute, severe polyneuropathy following sleeve gastrectomy. Severe thiamine deficiency was considered in both cases but with delayed proof and a significant initial differential diagnosis. Neurologists must have a high index of suspicion for the peripheral as well as central presentations of th

<p>Beriberi is a nutritional disorder caused by thiamine deficiency. Classically, Beriberi presents in two primary clinical forms: "wet" Beriberi, which features heart and circulatory system impairment, and "dry" Beriberi, which causes polyneuropathy. Although it is an easily treatable condition, it is often misdiagnosed and can be life-threatening if not promptly recognized and managed. The diagnosis of Beriberi is performed by the signs and symptoms of the disease and can be confirmed by thiamine deficiency identification or by therapeutic testing. However, considering the costs and the limi

Hereditary spastic paraplegia (HSP) is a rare and genetically heterogeneous neurodegenerative disorder, primarily defined by progressive lower-limb spasticity and weakness. Among the numerous genes implicated, pathogenic variants in the spastic paraplegia 7 (SPG7) gene represent one of the most common causes of HSP, whereas mutations in SCN4A, a skeletal muscle ion channel gene, are typically associated with a diverse spectrum of phenotypes, including hyperkalemic and hypokalemic periodic paralysis, potassium-aggravated myotonia, and congenital paramyotonia. To date, however, the coexistence o

Unprecedented runaway climate change and ecological degradation is argued to be caused by the dominant capitalist mode of production's reliance on endless economic growth and capital accumulation. Businesses and organisations are expected to act in an ecologically and socially ethical way to help avert the crisis. Yet, there has arguably been little progress in this direction. The conventional ethical frameworks are generally subsumed under capitalism's reliance on growth that effectively delegate business ethics to a peripheral and, often, contradictory pursuit, insufficient to influence ecol

OBJECTIVE: Pharyngo-laryngeal herpes zoster (HZ) is a rare but clinically important manifestation of varicella-zoster virus (VZV) reactivation. Unlike the well-described Ramsay Hunt syndrome, its neurological spectrum and outcomes remain poorly characterized. This study aimed to clarify patterns of cranial nerve involvement, recovery outcomes, and diagnostic challenges in pharyngo-laryngeal HZ, representing the largest single-center series to date.
METHODS: We retrospectively reviewed 34 patients diagnosed with pharyngo-laryngeal HZ at a tertiary referral hospital in Japan between July 2007 a
