Unilateral vitreous amyloidosis as the presenting feature of hereditary transthyretin amyloidosis (TTR p.Val50Met): a case report and focused literature review.

BACKGROUND: Hereditary transthyretin amyloidosis (ATTRv) is an autosomal dominant systemic disorder characterized by extracellular amyloid deposition. Ocular manifestations typically present bilaterally, and unilateral vitreous amyloidosis without systemic symptoms or family history is rare. Few reports have documented the laterality pattern of vitreous amyloidosis or its role as an early indicator of systemic disease. CASE PRESENTATION: We present a 67-year-old Chinese man who experienced progressive left eye vision loss over 1 year. Dense vitreous opacities in the left eye initially mimicke

Recurrence of B-cell Lymphoma Manifesting as Progressive Peripheral and Cranial Nerve Neuropathies: A Case Report.

Neurolymphomatosis (NL) is a rare neurological complication of lymphoma, characterised by direct infiltration of peripheral nerves by neoplastic cells. It most commonly occurs in association with non-Hodgkin's lymphoma (NHL) and poses significant diagnostic challenges due to its non-specific clinical presentation. Classically, NL presents as a painful, rapidly progressive, and asymmetric neuropathy, frequently mimicking inflammatory or vasculitic neuropathies. Here, we present the case of a 69-year-old woman with a history of NHL, in remission for five years, who presented with a progressive,

Facial diplegia as the first manifestation of Burkitt lymphoma with a Guillain-Barré syndrome-like presentation: a case report.

BACKGROUND: Guillain-Barré syndrome (GBS) is among the most common causes of acute inflammatory polyneuropathy and may present with cranial nerve involvement, including facial diplegia. Rarely, hematologic malignancies can produce a GBS-compatible or GBS-like neurological phenotype, creating a diagnostic challenge in the acute setting. CASE PRESENTATION: A 50-year-old immunocompetent woman presented with bilateral facial paralysis, dysarthria, dysphagia, areflexia, and mild left upper-limb weakness. Cerebrospinal fluid (CSF) analysis showed albuminocytologic dissociation. Baseline electromyog

Case Report: Schwann cell reprogramming and PDGF-driven nerve hypertrophy in an NF1 patient with CIDP-like autoimmunity.

BACKGROUND AND AIMS: Differentiating neoplastic proliferation from inflammatory fibrosis in peripheral nerve hypertrophy is critical. We report a patient with a neurofibromatosis type 1 (NF1) deletion exhibiting extreme diffuse nerve enlargement and chronic inflammatory demyelinating polyradiculoneuropathy (CIDP)-like autoimmunity. This study aims to elucidate the underlying endoneurial fibrotic mechanism, specifically focusing on the signaling networks between Schwann cells (SCs) and fibroblasts. METHODS: Single-cell RNA sequencing was performed on a biopsied sural nerve to profile the cellu

Neuromuscular ultrasound of the brachial plexus: know-how, know-when.

Neuromuscular ultrasound has become an integral diagnostic tool for brachial plexopathies, complementing electrodiagnostic studies and MRI. Despite the complex anatomy of the brachial plexus, nearly its entire postganglionic course is accessible by ultrasound, providing real-time, high-resolution structural information at the bedside. Characteristic sonographic findings have been described in traumatic plexopathies, neurogenic thoracic outlet syndrome, neuralgic amyotrophy, immune-mediated polyneuropathies, and neoplastic plexopathies. Evidence of moderate-to-high certainty now supports the us

Relapsing Guillain-Barré syndrome associated with Graves' disease and papillary thyroid carcinoma: a case report and literature review.

Guillain-Barré syndrome (GBS) is an acute immune-mediated peripheral neuropathy typically characterized by a monophasic course; however, atypical or relapsing presentations suggest that sustained immune dysregulation contribute to disease heterogeneity. We report a 33-year-old man with progressive limb weakness diagnosed as motor-predominant acute inflammatory demyelinating polyradiculoneuropathy (AIDP), supported by albuminocytologic dissociation and demyelinating features on nerve conduction studies with preserved sensory conduction. Immunological evaluation revealed B lymphocyte expansion,

Effectiveness of coronary sinus reducer implantation in routine clinical practice: 12-month outcomes.

INTRODUCTION: Refractory angina (RA), due to a lack of therapeutic options, remains a significant health care challenge. Coronary sinus reducer (CSR) has been recently introduced as a therapeutic option for this advanced form of coronary artery disease. However, despite growing evidence of its efficacy, data from routine clinical practice are scarce. OBJECTIVES: We report 12‑month observation data on the efficacy and safety of CSR implantation based on the results from the Lower Silesia Sinus Reducer Registry (NCT06288165). PATIENTS AND METHODS: The study included data from 67 consecutive pa

Polymorphisms in Necroptosis Pathway Genes: Novel Prognostic Markers for Multiple Myeloma Treatment Outcomes.

Multiple myeloma is a neoplastic disease characterised by the proliferation of clonal, atypical plasma cells. In cancer cells, the balance between two paths of cell death, necroptosis and apoptosis, is disrupted. The aim of this study was to analyse the occurrence of polymorphisms in genes encoding key proteins for the necroptosis process, i.e., RIPK-1, RIPK-3 and MAPKAPK2. We investigated the potential relations between the occurrence of genetic variability and the clinical course of the disease. We analysed six single-nucleotide polymorphisms in a population of patients with multiple myeloma

Unicentric Castleman disease following POEMS syndrome remission.

POEMS syndrome is a rare paraneoplastic disorder characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, and skin changes. Although Castleman disease (CD) is observed in up to 30% of POEMS cases, it is typically of the multicentric subtype (MCD) and identified during the initial diagnostic workup. Unicentric Castleman disease (UCD), by contrast, is rarely associated with POEMS and has not been previously reported following POEMS remission. We present the case of a 39-year-old woman who was diagnosed with POEMS syndrome based on clinical, radiographic, an

Complex Lower Back Pain: A Case Report.

Lower back pain is a common primary care symptom with musculoskeletal, neurological, neoplastic, infectious, or inflammatory causes. It remains the leading cause of years lived with disability, and it is linked to modifiable factors such as occupational exposure, smoking, and high body mass index (BMI), highlighting the need for preventive, lifestyle-focused strategies. To illustrate these challenges, a 60-year-old man is presented with a 20-year history of type 2 diabetes and a background of alcohol use disorder and suboptimal therapeutic compliance, resulting in poor glycemic control. He pre

Diabetic Striatopathy: A Rare Case of Non-ketotic Hyperglycaemia-Induced Hemichorea.

Diabetes mellitus is a metabolic condition leading to elevated blood glucose levels due to insulin deficiency, insulin resistance, or a combination of both. Chronically raised blood glucose levels can lead to a broad variety of microvascular and macrovascular complications. Neurological disorders are a common manifestation of diabetes mellitus, and poorly controlled diabetes mellitus frequently causes peripheral sensorimotor polyneuropathy and autonomic neuropathy. More rare manifestations include diabetic amyotrophy and mononeuritis multiplex. Hemichorea-hemiballismus (HCHB) syndrome is a mov

"A Sane Island in an Ocean of Madness": A Case of Alternative Organisational Ethics Through Post-Growth Values.

Unprecedented runaway climate change and ecological degradation is argued to be caused by the dominant capitalist mode of production's reliance on endless economic growth and capital accumulation. Businesses and organisations are expected to act in an ecologically and socially ethical way to help avert the crisis. Yet, there has arguably been little progress in this direction. The conventional ethical frameworks are generally subsumed under capitalism's reliance on growth that effectively delegate business ethics to a peripheral and, often, contradictory pursuit, insufficient to influence ecol

Neurolymphomatosis of T-Cell Origin in a Dog.

A 10-year-old mixed-breed dog was presented with progressive neurological deficits suggestive of polyneuropathy. CT and MRI revealed no relevant abnormalities, whereas the CSF analysis showed a marked lymphocytic pleocytosis, immunocytochemically classified as T-cell lymphoma. Chemotherapy ensued with no improvement of clinical signs. Cerebrospinal fluid reanalysis still revealed a mild lymphocytic pleocytosis. MRI of the brain and spinal cord solely showed a reduced ventral subarachnoid and epidural space (Th11-L2) and mild enlargement of the central canal in this area. Postmortem examination

Relationship between specific microRNA expression and radioresistant conditions in HL60 acute myeloid leukemia cells.

BACKGROUND: The efficacy of cervical cancer screening programs is dependent on the participation rate. To increase participation among women not attending cervical cancer screening, self-collected samples for detection of high-risk human papillomavirus (hr-HPV) may be an option. The aims of this study were: to investigate the response rate to sending a self-collected vaginal sample for hr-HPV mRNA detection to long-term non-attendees; the compliance with follow-up among women positive for HPV in the self-sample; the prevalence of cervical dysplasia (high grade squamous intraepithelial lesion (H

Exercise Therapy in Oncology—the Impact on Quality of Life and Side Effects.

BACKGROUND: The diagnosis and treatment of cancer are highly stressful. Exercise therapy is often used to mitigate the adverse effects of treatment. But how good is the evidence base, and what has changed in recent years? In this narrative review, we present the current data and what it implies for the care of adults with cancer. METHODS: This review is based on data from meta-analyses and systematic reviews concerning 16 relevant clinical endpoints (outcomes) of exercise therapy for cancer patients. RESULTS: The literature evaluated for this paper reveals that targeted exercise therapy is f

HERV-W envelope protein is present in microglial cells of the human glioma tumor microenvironment and differentially modulates neoplastic cell behavior.

Gliomas are the most common parenchymal tumors of the central nervous system (CNS). With regard to their still unclear etiology, several recent studies have provided evidence of a new category of pathogenic elements called human endogenous retroviruses (HERVs) which seem to contribute to the evolution and progression of many neurological diseases such as amyotrophic lateral sclerosis (ALS), schizophrenia, chronic inflammatory polyneuropathy (CIDP) and, particularly, multiple sclerosis (MS). In these diseases, HERVs exert effects on cellular processes such as inflammation, proliferation, and mi

Three genes expressed in relation to lipid metabolism considered as potential biomarkers for the diagnosis and treatment of diabetic peripheral neuropathy.

Diabetic neuropathy is one of the most common chronic complications and is present in approximately 50% of diabetic patients. A bioinformatic approach was used to analyze candidate genes involved in diabetic distal symmetric polyneuropathy and their potential mechanisms. GSE95849 was downloaded from the Gene Expression Omnibus database for differential analysis, together with the identified diabetic peripheral neuropathy-associated genes and the three major metabolism-associated genes in the CTD database to obtain overlapping Differentially Expressed Genes (DEGs). Gene Set Enrichment Analysis
