Management of Small Fiber Neuropathy: A Clinical Perspective.

Small fiber neuropathy (SFN) is a common neurological diagnosis with a multitude of symptoms, including dysautonomia symptoms (e.g., orthostatic dizziness) as well as sensory disturbances, such as a tingling sensation or burning pain. The disease can be due to metabolic, autoimmune, or hereditary factors, and treatment plans stem from SFN etiology. Diabetes, autoimmune disease, infection, vitamin deficiencies, post-vaccination syndromes can all cause SFN. Needle electromyography and nerve conduction studies can be performed to exclude other neuromuscular conditions by ruling out large fiber ne

Severe copper deficiency-related polyneuropathy following laparoscopic sleeve gastrectomy: A case report and literature review.

BACKGROUND: Sleeve gastrectomy (SG) has become the most commonly performed bariatric procedure due to its favorable weight-loss outcomes and lower complication risks. However, serious neurological complications can still occur. We present a case of severe copper deficiency-related polyneuropathy following SG in a 48-y-old man. CASE PRESENTATION: A 48-y-old man underwent laparoscopic SG for morbid obesity. Three months postoperatively, he developed poor oral intake. At 7 mo, he presented with bilateral distal paresthesia and gait disturbance. Symptoms transiently improved after vitamin B suppl

Acute non-alcoholic nutritional neuropathies in high-income countries - a systematic review.

INTRODUCTION: We determined the semiology, causes, risk factors and outcomes of patients with acute nutritional neuropathies requiring hospitalisation in high-income countries. METHODS: We searched PubMed, Cochrane Library and Embase for non-alcoholic malnutrition neuropathy cases published since 1990. RESULTS: Thirty-six studies were identified, including 83 patients (mean age 39.8 years; 70% women) with deficient levels of thiamine (54%), pyridoxine (16%), copper (11%) or folate (7%). Common clinical signs were lower extremity weakness (100%), hypesthesia (63%), hypo-/areflexia (54%) and c

Neurological complications associated with rapid weight loss and nutritional deficiencies following GLP-1 agonist use: a case report.

GLP-1 agonists have been a welcome addition to the armamentarium in the treatment of diabetes and obesity. While this class of medications is generally considered safe and effective, potentially severe neurological complications may be associated with rapid weight loss and nutritional deficiencies following GLP-1 agonist use. We present a 37-year-old woman who was prescribed semaglutide for diabetes and weight loss who subsequently experienced significant weight reduction and rapid glycemic control over three months. Thereafter, she developed progressive right leg numbness and weakness, follow

Beriberi following sleeve gastrectomy.

Bariatric surgery is being undertaken more frequently in response to rising levels of obesity but is increasingly also requested as a cosmetic choice. Nutritional deficiencies are a recognised consequence of gastrectomy, with potentially severe and permanent neurological sequelae. We present two cases of acute, severe polyneuropathy following sleeve gastrectomy. Severe thiamine deficiency was considered in both cases but with delayed proof and a significant initial differential diagnosis. Neurologists must have a high index of suspicion for the peripheral as well as central presentations of th

An Overview of Beriberi.

<p>Beriberi is a nutritional disorder caused by thiamine deficiency. Classically, Beriberi presents in two primary clinical forms: "wet" Beriberi, which features heart and circulatory system impairment, and "dry" Beriberi, which causes polyneuropathy. Although it is an easily treatable condition, it is often misdiagnosed and can be life-threatening if not promptly recognized and managed. The diagnosis of Beriberi is performed by the signs and symptoms of the disease and can be confirmed by thiamine deficiency identification or by therapeutic testing. However, considering the costs and the limi

Genetic analysis of a family with skeletal muscle ion channelopathy and hereditary spastic paraplegia type 7 caused by SCN4A and SPG7 double mutations.

Hereditary spastic paraplegia (HSP) is a rare and genetically heterogeneous neurodegenerative disorder, primarily defined by progressive lower-limb spasticity and weakness. Among the numerous genes implicated, pathogenic variants in the spastic paraplegia 7 (SPG7) gene represent one of the most common causes of HSP, whereas mutations in SCN4A, a skeletal muscle ion channel gene, are typically associated with a diverse spectrum of phenotypes, including hyperkalemic and hypokalemic periodic paralysis, potassium-aggravated myotonia, and congenital paramyotonia. To date, however, the coexistence o

Outcomes of cranial nerve disorders in pharyngo-laryngeal herpes zoster: A retrospective study of 34 cases at a single institution.

OBJECTIVE: Pharyngo-laryngeal herpes zoster (HZ) is a rare but clinically important manifestation of varicella-zoster virus (VZV) reactivation. Unlike the well-described Ramsay Hunt syndrome, its neurological spectrum and outcomes remain poorly characterized. This study aimed to clarify patterns of cranial nerve involvement, recovery outcomes, and diagnostic challenges in pharyngo-laryngeal HZ, representing the largest single-center series to date. METHODS: We retrospectively reviewed 34 patients diagnosed with pharyngo-laryngeal HZ at a tertiary referral hospital in Japan between July 2007 a

Vutrisiran: a new drug in the treatment landscape of hereditary transthyretin amyloid polyneuropathy.

INTRODUCTION: Hereditary transthyretin (ATTRv) amyloidosis is a progressive, fatal disorder caused by mutations in the transthyretin (TTR) gene leading to deposition of the misfolded protein in amyloid fibrils. The main phenotypes are peripheral neuropathy (PN) and cardiomyopathy (CM). AREAS COVERED: Gene silencing therapy, by dramatically reducing liver production of TTR, has transformed ATTRv-PN patient care in the last decade. In this drug discovery case history, the authors discuss the treatment history of ATTRv-PN and focus on the latest siRNA therapy: vutrisiran. Vutrisiran is chemicall

Real-life experience with inotersen at CEPARM, Hospital Universitário Clementino Fraga Filho, Universidade Federal do Rio de Janeiro.

BACKGROUND: Hereditary transthyretin amyloidosis (ATTRv) is an inherited, progressive, and fatal disease still largely underdiagnosed. Mutations in the transthyretin (TTR) gene cause the TTR protein to destabilize, misfold, aggregate, and deposit in body tissues, which makes ATTRv a disease with heterogeneous clinical phenotype. OBJECTIVE: To describe the long-term efficacy and safety of inotersen therapy in patients with ATTRv peripheral neuropathy (ATTRv-PN). METHODS: Patients who completed the NEURO-TTR pivotal study and the NEURO-TTR OLE open-label extension study migrated to the present

Electrophysiological Screening to Assess Foot Drop Syndrome in Severe Acquired Brain Injury in Rehabilitative Settings.

BACKGROUND: Foot drop syndrome (FDS), characterized by severe weakness and atrophy of the dorsiflexion muscles of the feet, is commonly found in patients with severe acquired brain injury (ABI). If the syndrome is unilateral, the cause is often a peroneal neuropathy (PN), due to compression of the nervous trunk on the neck of the fibula at the knee level; less frequently, the cause is a previous or concomitant lumbar radiculopathy. Bilateral syndromes are caused by polyneuropathies and myopathies. Central causes, due to brain or spinal injury, mimic this syndrome but are usually accompanied by

Long-term complications in acute porphyria.

New treatment options and low attack-related mortality have changed the life expectancy of patients with acute porphyria (AP) to that of the general population. Clinicians should therefore be aware of the long-term complications of AP, which typically include chronic neuropathy and encephalopathy, high blood pressure and porphyria-associated kidney disease. Patients have an increased risk of primary liver cancer (PLC), but no increased risk of non-hepatic cancers. Chronic pain occurs in patients with recurrent attacks, combined with chronic fatigue and nausea, leading to poor quality of life.
