Physiotherapist's Management of Suspected Cauda Equina Syndrome in the United Kingdom: A National Survey.

BACKGROUND AND PURPOSE: Cauda Equina Syndrome (CES) is a rare but serious spinal condition requiring urgent diagnosis and management. Physiotherapists in UK musculoskeletal (MSK) services increasingly encounter suspected CES cases, but little is known about their clinical decision-making and referral practices. This study aimed to examine the clinical decision-making processes of UK physiotherapists when presented with three clinical vignettes of suspected CES and to identify themes in referral handover methods when emergency care is deemed necessary. METHODS: An online survey featuring three

Synovial Biopsy as a Diagnostic Clue for Transthyretin Amyloidosis With the Rare Tyr98Phe Variant Presenting Predominantly With Severe Musculoskeletal Manifestations.

Hereditary transthyretin amyloidosis (ATTRv), the most common form of hereditary amyloidosis, results from autosomal dominant mutations in the TTR gene. Clinical manifestations of ATTRv are typically driven by extracellular amyloid deposition in multiple organs, most notably the peripheral nerves, heart, gastrointestinal tract, and musculoskeletal (MSK) system. We report the case of a 69-year-old man presenting with a two-year history of paresthesias and hypoesthesia in the lower limbs and a complex MSK history, including bilateral carpal tunnel syndrome, multiple trigger finger surgeries, rig

Clinical spectrum and burden of extrahepatic manifestations in chronic hepatitis B: A multinational study from 2000 to 2025.

Chronic hepatitis B virus (HBV) infection is associated with both hepatic and extrahepatic complications. While liver-related outcomes are well characterized, the burden, demographic patterns, and survival associated with extrahepatic manifestations (EHMs) remain incompletely defined. This study aimed to evaluate the spectrum, incidence, and survival impact of EHMs in a large, geographically diverse cohort of patients with chronic HBV infection. We conducted a retrospective cohort study using the TriNetX Global Research Network, including 2,32,820 patients aged 0 to 90 years with chronic HBV i

Potential Vitamin B6 Depletion During Carbidopa/Levodopa Therapy in Parkinson's Disease: A Scoping Review of Direct Evidence.

Vitamin B6 depletion during carbidopa/levodopa therapy has been linked to polyneuropathy, seizures, and anemia, but the direct evidence is heterogeneous, and causality remains uncertain. We conducted a focused scoping review of direct empirical evidence on vitamin B6 biology during carbidopa/levodopa exposure and on vitamin B6-related clinical consequences supported by biochemical assessment. We also included studies of biomarker-guided monitoring, supplementation, and direct carbidopa-vitamin B6 chemistry. PubMed, Scopus, and Web of Science were searched without date or language limits on Jul

Reduced somatosensory innervation alters the skeletal transcriptome at a single cell level in a mouse model of type 2 diabetes.

Peripheral neuropathy is a common complication in diabetes, affecting around 50% of the diabetic population. Co-occurrence of diabetic peripheral neuropathy (DPN) and diabetic bone disease has led to the hypothesis that DPN influences bone metabolism, although little experimental evidence has yet supported this premise. To investigate, mice were fed a high-fat diet (HFD) followed by phenotyping of skeletal-innervating neurons and bone architectural parameters. Results showed that HFD feeding resulted in a marked decrease in skeletal innervation (69%-41% reduction in Beta-III-Tubulin-stained ne

Extraocular features of Leber hereditary optic neuropathy: A scoping review.

BACKGROUND: Leber hereditary optic neuropathy (LHON) is a rare inherited mitochondrial disease that leads to mitochondrial dysfunction, resulting in optic nerve damage and vision loss. Systemic involvement has been reported in several LHON cases, referred to as LHON+ disorders. However, the causes and presentations of such conditions have been poorly studied. It is suggested that 90% of mitochondrial dysfunction is caused by one of three primary point mutations in mitochondrial DNA that affect respiratory complex I (referred to as mtDNA LHON), with unresolved cases of LHON being caused by othe

Multidimensional evaluation of clinical and functional impairment in a large population of patients with Charcot-Marie-Tooth.

Charcot-Marie-Tooth (CMT) disease is one of the most common inherited neuropathies, leading progressive muscular weakness, pes cavus, loss of deep tendon reflexes, distal sensory loss, and gait impairment. Charcot Marie Tooth Neuropathy Scale (CMTNS) represents the gold standard to assess and grade the impairment in CMT patients. The aim of this retrospective study was to assess the correlation between CMTNS and functional status in CMT patients. We analyzed data of patients with diagnosis of CMT, collecting the following data: age; gender; BMI; CMT type (Demyelinating- Axonal- Other or mixed

Complex Lower Back Pain: A Case Report.

Lower back pain is a common primary care symptom with musculoskeletal, neurological, neoplastic, infectious, or inflammatory causes. It remains the leading cause of years lived with disability, and it is linked to modifiable factors such as occupational exposure, smoking, and high body mass index (BMI), highlighting the need for preventive, lifestyle-focused strategies. To illustrate these challenges, a 60-year-old man is presented with a 20-year history of type 2 diabetes and a background of alcohol use disorder and suboptimal therapeutic compliance, resulting in poor glycemic control. He pre

Myoedema: a forgotten sign in acute colchicine myopathy.

Colchicine myopathy typically presents acutely to subacutely with progressive limb weakness. The patients may not be on high doses of colchicine but almost always have acute kidney injury. Dehydration from colchicine-induced diarrhoea is often a precipitating factor. The concomitant neurotoxicity may produce mild sensory complaints. This combination of acute neurological symptoms preceded by diarrhoea prompts the diagnosis of Guillain-Barre syndrome (GBS). The absence of cranial nerve deficits, raised creatine kinase and myotonic discharges on electromyogram may help in differentiating this co

Predicting long-term trends in inflammatory neuropathy outcome measures using latent class modelling.

Immunoglobulin (Ig) is used to treat chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) and multifocal motor neuropathy with conduction block (MMNCB). Regular infusions may be used for symptom control. Disease activity is monitored with clinical outcome measurements. We examined outcome measure variation during clinically stable periods in Ig-treated CIDP and MMNCB patients. We explored utility of serial outcome measurement in long-term outcome prediction. Retrospective longitudinal analysis of a single neuroscience centre's Ig-treated CIDP and MMNCB patients, 2009-2020, was perf

The Effects of COVID-19 on Skeletal Muscles, Muscle Fatigue and Rehabilitation Programs Outcomes.

Background and Objectives: Consequences due to infection with SARS-CoV-2 virus can have a direct impact on skeletal muscle, due to the fact that both cardiac and skeletal muscle tissue show robust ACE2(angiotensin-converting enzyme 2) expression, suggesting a potential susceptibility to SARS-CoV-2 infection in both types of tissues. From the articles analyzed we concluded that the musculoskeletal damage is firstly produced by the inflammatory effects, cytokine storm and muscle catabolism. However, myopathy, polyneuropathy and therapies such as corticoids were also considered important factors

Rasch-built overall disability scale for POEMS syndrome (POEMS-RODS).

Patient-reported outcome measures engage patients in disease severity measurement and the metrics reported can be meaningful to their lives. The Polyneuropathy, Organomegaly, Endocrinopathy, Monoclonal protein and Skin changes syndrome (POEMS) is a complex multisystem disorder with disabling neuropathy which is distinct from other acquired inflammatory neuropathies. No current POEMS-specific validated disability scales exist. To address this, we have produced a Rasch-built overall disability scale (RODS) specific to POEMS. Premotor Parkinson's disease: Overview of clinical symptoms and current diagnostic methods.

Parkinson's disease (PD) is characterized by typical motor symptoms. However, recent studies show several non-motor features that may precede the development of the motor symptoms of PD. The best known premotor symptoms include hyposmia, REM sleep behavior disorder (RBD), constipation, and depression; other symptoms are excessive daytime somnolence, orthostatic hypotension and symptomatic hypotension, erectile or urinary dysfunction, musculoskeletal symptoms, pain, and global cognitive deficit. In this review, we summarize currently available diagnostic methods for these symptoms. We also brie

Functional Impairments and Rehabilitation Outcomes of Patients With Immunotherapy-Induced Acute Inflammatory Demyelinating Polyradiculoneuropathy, Myasthenia Gravis, and Myositis.

Immunotherapy has led to a higher survival rate among different oncological disease groups but also associated with adverse-related events in multiple organ systems. Immunotherapy-related musculoskeletal weakness often results in a loss of cancer survivors' physical function, ultimately impacting their independence and quality of life. This is a retrospective study of 24 cancer patients who were treated with immunotherapy either alone or in conjunction with other oncological treatments. Twelve subjects (50%) were found to have acute inflammatory demyelinating polyradiculopathy/Guillain-Barré s

Multiple myeloma: a review of atypical imaging features and other distinct plasma cell disorders that demonstrate similar imaging features.

Multiple osteolytic lesions are a well-recognized and typical imaging feature of multiple myeloma as well as several other plasma cell disorders. Given the high volume of imaging studies obtained of multiple myeloma patients, radiologists will likely encounter a subset of multiple myeloma patients with less common or "atypical" findings during their practice. These atypical findings include osteosclerotic lesions, extramedullary lesions, and amyloid deposition. Similar imaging findings that are considered atypical for multiple myeloma can also be detected in other plasma cell disorders that ar

Acute Aortic Thrombus Presenting as Cauda Equina Syndrome.

BACKGROUND: Occlusive abdominal aortic thrombus is a rare but critical clinical emergency with life-threatening consequences. Clinical presentation may mimic other diagnoses, resulting in a delay in the appropriate investigations for this condition. Spinal arterial involvement is a recognized complication of aortic thrombus and can result in pain, lower limb weakness, and loss of continence. These symptoms are usually associated with local spinal compression or stenosis manifesting as cauda equina syndrome (CES): a clinical finding of disrupted motor and sensory function to the lower extremiti
