Cholinergic reflex control of inflammation: mechanistic and translational advances in transcutaneous auricular vagus nerve stimulation across rheumatic, metabolic, and postoperative disorders.

Transcutaneous auricular vagus nerve stimulation (taVNS) has recently emerged as a focal noninvasive neuromodulatory approach in anti-inflammatory therapeutics. The vagus nerve functions as a critical neuroimmune interface, tonically suppressing proinflammatory cytokine release via the cholinergic anti-inflammatory pathway (CAP). This mechanism provides substantial therapeutic potential across a spectrum of inflammatory disorders, including postoperative systemic inflammation. Clinical trials have demonstrated the anti-inflammatory efficacy of taVNS, supporting its expanded use in rheumatoid a

Peripheral Neuropathy: A Review.

IMPORTANCE: Peripheral neuropathy, defined as damage to peripheral nerves, affects approximately 1% of adults worldwide. More than 200 causes of peripheral neuropathy exist, with symptoms ranging in severity from mild toe numbness to debilitating symptoms that can require a wheelchair. Diabetes is the most common cause of neuropathy, affecting approximately 206 million people worldwide. OBSERVATIONS: Peripheral neuropathy is typically length-dependent, which means that symptoms appear in the longest nerve axons (toes) and progress proximally over time. Peripheral neuropathy is typically symme

Functional role of the enteric nervous system in poultry intestinal health and production.

Like mammals, the avian intestinal epithelium is innervated by three neuronal pathways: vagal and sympathetic terminals, which originate from ganglia outside the gut wall and send information to the brain to modulate visceral sensitivity, appetite, and gut homeostasis; and the enteric nervous system (ENS), a complex network embedded within the gut wall that functions independently from the brain. The ENS coordinates essential GI physiological functions, such as intestinal motility, peristalsis, digestion, and absorption of nutrients and water. Recent studies conducted in mammals have shown tha

A randomized clinical trial assessing the efficacy of vitamin B prophylaxis in attenuating paclitaxel-induced neuropathy and the imperative use of gabapentin in diabetic ovarian cancer patients.

Paclitaxel being an effective treatment for ovarian cancer, presents one of the most critical toxicities; peripheral neuropathy (PN), a debilitating side effect that might limit continuation of chemotherapy. Vitamin B was found to significantly improve PN and Gabapentin is debatably used in chemotherapy induced peripheral neuropathy (CIPN). The aim of this study was to assess the efficacy of vitamin B prophylaxis in reducing the severity of CIPN, particularly in diabetic patients with the need for Gabapentin as additional therapy and the potential impact on disease response. A clinical trial o

Peripheral nerve block in patients with medication-overuse headache: systematic review and meta-analysis.

BACKGROUND: Medication-overuse headache (MOH) is a disabling condition, particularly in settings with limited access to advanced preventive therapies. Peripheral nerve blocks (PNB) may offer a pragmatic option to reduce headache burden during MOH management. We aimed to assess the efficacy and safety of PNB in adults with MOH. METHODS: We conducted a systematic review in January 2025 using Scopus, PubMed, Embase, Web of Science, and the Cochrane Library. Randomized clinical trials comparing PNB with standard of care or other active interventions in adults with MOH were eligible. Primary outcom

Microvascular vasomotor dysfunction is more prevalent in female versus epicardial vasomotor dysfunction in male ANOCA patients.

BACKGROUND: Coronary vasomotor dysfunction, diagnosed by coronary function testing (CFT), has only recently been recognized as important contributor to chronic coronary syndromes, exposing knowledge gaps on sex and age differences. Therefore, these differences are evaluated in the Dutch national CFT registry (NL-CFT). METHODS: Included were 1008 patients (83% women, 60 ± 9.6 years old) with angina and non-obstructive coronary arteries (ANOCA) undergoing CFT between 2020 and 2024. CFT consisted of coronary vasospasm provocation testing and reduced microvascular vasodilatory capacity assessment

Traditional Asian Mind-Body Exercises for Vasomotor Symptoms and Other Menopausal Symptoms in Women Experiencing Peri- and Post-Menopause: A Scoping Review Protocol.

INTRODUCTION: Around 1.3 million US women enter menopause annually, with 75%-80% experiencing vasomotor symptoms (VMS) impacting mood, sleep, and cognition. VMS may contribute to chronic diseases like cardiovascular disease, a leading cause of female mortality. Limited research suggests behavioural interventions, especially Asian mind-body exercises, may alleviate VMS and other menopausal symptoms. This scoping review explores the literature on the effects of Asian mind-body exercises on VMS and related symptoms in this population. OBJECTIVE: This scoping review aims to map the current litera

The Brain Is the Rate-Limiting Organ of Longevity: A Brain-First Systems Framework for Aging.

Longevity research has traditionally emphasized peripheral organ systems, metabolic optimization, and molecular aging pathways, while comparatively neglecting the central nervous system as the primary determinant of healthspan. This editorial advances the thesis that the brain functions as the rate-limiting organ of longevity. Drawing on systems neuroscience, clinical neurology, and evidence from neuropsychiatric and neurodegenerative disease, it is argued that progressive disruption of neural networks governs functional decline across multiple physiological systems, regardless of peripheral b

A gel-forming antioxidant eye drop for photoreceptor protection in retinitis pigmentosa.

Retinitis pigmentosa (RP) is a chronic genetic condition that leads to progressive loss of photoreceptor cells and vision. While gene therapy is available for a small subset of patients with specific mutations, developing a therapeutic that broadly targets the cellular stresses that lead to cell death could address a major unmet need. One such option would be utilizing antioxidant therapies to neutralize reaction oxygen species (ROS) in the retina that underlie RP progression. Here, we describe an approach for delivering the antioxidants N-acetyl cysteine (NAC) or N-acetyl cysteine ethyl ester

A wearable system enabling acute stress monitoring and closed-loop mitigation through transcutaneous median nerve stimulation.

BACKGROUND: Acute stress, in moderation, helps to prepare the body to overcome mental and physical challenges. However, excessive bouts of acute stress can be detrimental to the cardiovascular system and are a risk factor for cardiovascular disease and sudden cardiac death. Transcutaneous median nerve stimulation (tMNS) is a promising therapy for the mitigation of acute stress through peripheral neuromodulation, but the optimal delivery of tMNS for stress mitigation may require continuous monitoring of acute stress events for targeted delivery. OBJECTIVE: The purpose of this study was to deve

Curcumin reduces neuroinflammation and oxidative stress in a stroke model by epigenetically regulating ADRB2 methylation through JAK2/STAT3 and Nrf2/HO-1 pathways.

The pathological mechanism of acute ischemic stroke (AIS) is complex, and exploring new diagnostic biomarkers and key molecular pathological targets is crucial for improving patient prognosis. The role of epigenetic regulation, especially DNA methylation, in AIS is receiving increasing attention, but its key target genes and clinical translational value still need to be elucidated. This study included 90 case-control groups and used pyrophosphate sequencing to detect the methylation level of β2-adrenergic receptor (ADRB2) promoter in peripheral blood. Using oxygen glucose deprivation/reperfusi

Screening Value of the I-Douleur Neuropathique 4 Questionnaire for Small Fibre Neuropathy in Patients With Painful Syndromes: Insights From 872 Skin Biopsies.

BACKGROUND: Small-fibre neuropathy (SFN), which is defined by the sole involvement of small sensory fibres, often leads to neuropathic pain. The diagnosis of SFN relies on clinical, neurophysiologic, and morphologic abnormalities. A decrease in intraepidermal nerve fibre density (IENFD) on a skin biopsy specimen is considered useful in the diagnosis of SFN in the appropriate clinical context. The self-administered Douleur Neuropathique 4 (I-DN4) questionnaire, designed to differentiate neuropathic from somatic pain, shows high sensitivity and specificity but has not been specifically investiga

Prediction of Optic Disc Edema Progression During Spaceflight.

IMPORTANCE: There are limited means to predict which crewmembers are at risk for developing optic disc edema (ODE) during spaceflight. Identifying crewmembers who have a low probability of developing ODE will allow for reduced monitoring and countermeasure resources, thereby enabling alternate mission activities. OBJECTIVE: To determine if magnitude of ODE early in flight is associated with magnitude of ODE later in flight. DESIGN, SETTING, AND PARTICIPANTS: This cohort study involved crewmembers on board the International Space Station. Participants were astronauts who had optical coherence

The brain-lung axis: Bridging neurological and respiratory disorders via neural-immune-microbial dialogue.

The human brain maintains intricate interconnections with various peripheral organs. Recent scientific inquiry has substantiated the existence of the gut-brain axis; nevertheless, emerging evidence suggests that the brain and lungs engage in bidirectional communication through multiple pathways, thereby giving rise to the conceptualization of a brain-lung axis. Studies indicate the presence of crosstalk between the central nervous system and the lungs, mediated by the lung microbiome, neural pathways, metabolite signaling, and immune pathways. This bidirectional communication between the brain

Proprioceptive limit detectors contribute to sensorimotor control of the Drosophila leg.

Many animals possess mechanosensory neurons that fire when a limb nears the limit of its physical range, but the function of these proprioceptive limit detectors remains poorly understood. Here, we investigate a class of proprioceptors on the Drosophila leg called hair plates. Using calcium imaging in behaving flies, we find that a hair plate on the fly coxa (CxHP8) detects the limits of anterior leg movement. By reconstructing CxHP8 axons in an electron microscopy dataset, we found that they are wired to excite posterior leg movement and inhibit anterior leg movement. Consistent with this con

Roles and Mechanisms of TRIM Family Proteins in Inflammation in the Brain and Beyond.

Neurodegeneration is closely linked to neuroinflammation and is frequently accompanied by comorbidities with inflammatory features. Tripartite motif (TRIM) proteins are known to play an important role in innate immunity and inflammatory signaling in various tissues and organs of the body, including the central nervous system. Among the main cell types of the brain, TRIMs' functions in microglia are largely associated with the regulation of intracellular inflammatory signaling, while in neurons they mainly relate to cell survival and oxidative stress. Data concerning TRIMs' activity in astrocyt
