Postmortem Presentation and Review of Danon Disease: A Rare Metabolic Disorder.

Danon disease is a rare, X-linked dominant inherited lysosomal storage disorder that involves a mutation in the lysosomal-associated membrane Protein 2 gene. Mutation in this gene results in severe disease clinical manifestations that may include skeletal and cardiac myopathies, retinopathies, and intellectual disabilities. This case report explores postmortem findings observed in a 25-year-old male donor who succumbed to heart failure secondary to Danon disease. With no transplant history, postmortem findings include hypertrophy, extensive fibrosis, and vacuolization in cardiomyocytes compare

Progressive Dysphagia Revealing Glioma: Diagnostic Overshadowing by Suspected Myasthenia Gravis.

UNLABELLED: Progressive dysphagia in older adults presents a diagnostic challenge with a broad differential including neuromuscular, structural and malignant causes. Dysphagia associated with aspiration may lead to significant respiratory compromise requiring intensive care management. We report the case of a 78-year-old patient with rheumatoid arthritis treated with methotrexate who presented with progressive dysphagia to solids and liquids, weight loss, and cough. She subsequently developed aspiration pneumonia complicated by repeated type 2 respiratory failure and required mechanical ventil

DNA methylation in motor neuron biology and epigeneticopathy in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS), the 3rd most frequent neurodegenerative disease, is fatal. Skeletal muscles, neuromuscular junctions, and motor neurons in the cerebral cortex, brainstem, and spinal cord that control swallowing, breathing, and movement degenerate, causing paralysis, respiratory arrest, and death. Clinically meaningful disease mechanisms are unknown, and no effective therapies are available. ALS is a polygenic heterogeneous disease seen in 5%-10% of patients as inherited, but most patients have no inheritance pattern. ALS appears to manifest as a complex interplay between m

Visual feedback increases expiratory muscle training effort in Parkinson's disease: a randomised crossover trial.

BACKGROUND: Disorders of airway protection, including impaired cough and swallowing, are clinically important complications of Parkinson's disease. Expiratory muscle strength training (EMST) is an established non-pharmacological intervention targeting these deficits. The aim of this study was to evaluate the acute effect of visual feedback provided by the SpiroGym mHealth app on expiratory muscle activation during EMST in people with Parkinson's disease. METHODS: We conducted a single-center, randomised, two-period crossover trial in people with Parkinson's disease. Participants performed EMS

Impaired Brain Astrocytic Glycolysis in Neuropathic Pain: Insights from Transcriptomic Profiling to Metabolic Intervention.

Neuropathic pain is a chronic disorder marked by sensory hypersensitivity and affective disturbances, yet current treatments often provide limited relief. Research on astrocytes in pain has focused largely on spinal mechanisms. Whether chronic peripheral nerve injury alters brain astrocytic metabolism remains unclear. Here, we isolated astrocytes from whole-brain preparations of adult mice after spared nerve injury (SNI) for transcriptomic profiling and identified coordinated suppression of astrocytic glycolysis, characterized by downregulation of genes (Aldoc, Pfkp, Pfkm, Slc2a12, and Slc2a13

Outcomes of systemic sclerosis with overlapping myositis: a propensity score-matched cohort study.

OBJECTIVES: Systemic sclerosis (SSc) carries the highest disease-specific mortality among rheumatic diseases, with cardiac involvement and interstitial lung disease (ILD) as leading causes of death. We aimed to determine whether overlapping myositis independently contributes to cardiac involvement and mortality in SSc. METHODS: We did a 1:1 propensity score matched retrospective cohort study of 880 SSc patients (2013 ACR/EULAR criteria) at a single centre. SSc myositis required proximal weakness and elevated muscle enzymes without alternative causes. Cardiac involvement used SCTC criteria (pe

Arrhythmias in patients with transthyretin cardiac amyloidosis and the impact of contemporary disease-modifying therapies.

Transthyretin cardiac amyloidosis (ATTR-CM) is an increasingly recognized infiltrative cardiomyopathy characterized by extracellular deposition of misfolded transthyretin fibrils, leading to progressive myocardial stiffening, electrical remodeling, and conduction-system disease. These pathophysiological changes create a complex arrhythmic substrate, resulting in a high burden of atrial, bradyarrhythmia, and ventricular arrhythmias that significantly contribute to morbidity and mortality. Despite advances in disease recognition, the optimal management of arrhythmias in ATTR-CM remains challengi

Hypoxia-induced apoptotic pathways and their contribution to fibrosis progression in wooden breast myopathy of fast-growing broilers.

Fibrotic breast myopathy, including the Wooden Breast (WB) condition, represents one of the most economically and welfare-significant challenges confronting modern commercial broiler production. Despite growing recognition of the syndrome, the cellular and molecular mechanisms linking chronic myofibre hypoxia to the progressive fibrotic pathology remain incompletely defined. This study employed a four-group experimental model using 192 male Ross 308 broilers subjected to chronic hypobaric hypoxia (simulated altitude 3,000 m; 13.0% O₂) with and without pharmacological pan-caspase inhibition via

Mendelian randomization of plasma lipidome and temporomandibular joint disorders.

Mendelian randomization (MR) was used to investigate the possible causal association between lipids (LP) and temporomandibular joint disorders (TMD). To investigate the causal relationship between circulating LP and TMD, we conducted a 2-sample MR analysis. Exposure data comprised summary statistics for 179 LP from the GeneRISK genome-wide association study (N = 7174 samples). Outcome data were derived from the FinnGen genome-wide association study for TMD (N = 185169 samples). Primary causal estimates were generated using inverse-variance weighted regression, with sensitivity analyses includi

Clinical variables and quantitative CT radiomics for mortality risk stratification after acute exacerbation of idiopathic inflammatory myopathy-associated interstitial lung disease.

BACKGROUND: Acute exacerbation (AE) is the most lethal event of idiopathic inflammatory myopathy-associated interstitial lung disease (IIM-ILD). This exploratory study compared clinical, radiomics, and combined predictor strategies across five survival-learning algorithms for survival risk stratification. METHODS: We retrospectively analyzed 78 patients with AE-IIM-ILD and divided them into a training cohort (n = 54; 20 deaths) and a held-out internal testing cohort (n = 24; 9 deaths), stratified by observed mortality event status. Three predictor strategies were evaluated: a clinical strateg

Circulating Natural Killer Cells in Patients With Type 2 Diabetes and Peripheral Neuropathy Are Altered in Their Frequency and Exhibit Impaired Functions and Angiogenic Activities.

Patients with type 2 diabetes mellitus (T2DM) and critical limb-threatening ischemia (CLTI), a severe vascular complication associated with amputation and high mortality, exhibit chronic inflammation and endothelial dysfunction. We previously reported an increased frequency of natural killer (NK) cells in advanced CLTI; however, whether NK cell alterations are already present in patients with peripheral neuropathy, which often precedes diabetic foot lesions with or without distal macroangiopathy, remains unclear. We conducted an observational pilot study to assess circulating NK cell frequency
