Serum Mycophenolic Acid Levels and Infection Risk in Myasthenia Gravis.

INTRODUCTION/AIMS: Evidence linking serum mycophenolic acid (MPA) concentrations with safety outcomes in myasthenia gravis (MG) is limited. We evaluated whether single serum MPA concentrations were associated with clinically documented infection. METHODS: This retrospective study included acetylcholine receptor antibody-positive MG patients treated with mycophenolate mofetil (MMF) for at least 1 year who had MPA and mycophenolic acid glucuronide (MPAG) measurement with same-day complete blood count testing. Exact sampling time relative to MMF dosing was not uniformly documented; therefore, va

When Weakness Persists Beyond Its Apparent Cause: A Case Report of Myasthenia Gravis.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction characterised by fluctuating, fatigable skeletal muscle weakness. Its overlap with reversible causes of weakness such as hypokalemia can easily delay recognition. We report the case of a 40-year-old man who presented with one week of profuse diarrhea and vomiting, along with two days of generalized limb weakness. Investigations revealed hypokalemia and hypomagnesemia, and his limb weakness partially improved after fluid and electrolyte correction, which raised suspicion of an additional underlying cause contributing

Spectrum, management, and outcomes of non-obstetric neurological disorders in pregnancy: a descriptive case series from a tertiary centre in India.

BACKGROUND: Neurological disorders unrelated to obstetric pathology are an under-recognised cause of maternal morbidity in pregnancy, particularly in low- and middle-income countries. Evidence describing their clinical spectrum, management, and outcomes remains limited. METHODS: We conducted a prospective descriptive case series study at a tertiary care centre in central India between April 2024 and November 2025. Pregnant and postpartum women (≤6 weeks) diagnosed with neurological disorders of non-obstetric origin were included. Data on clinical presentation, neurological diagnoses, manageme

Therapeutic Plasma Exchange in Pediatric Patients: A Multi-Institutional Landscape Analysis.

Pediatric evidence for therapeutic plasma exchange (TPE) is largely small, single-center cohorts; there is no prior large-scale age-stratified outcomes assessment. Using the TriNetX US Collaborative Network, we identified 3744 patients ≤ 17 years old undergoing an index TPE procedure, stratified into four age groups (≤ 1, 2-5, 6-12, and 13-17 years). ICD-10-CM diagnosis codes recorded 30 days prior to the index procedure were mapped to American Society for Apheresis Categories I-III indications; three high-prevalence nonspecific codes were tracked separately. All-cause mortality at 30 days, 90

Clinical study of eculizumab with or with not tacrolimus in the treatment of generalized myasthenia gravis.

INTRODUCTION: Eculizumab, an antibody that inhibits the complement system, and the immunosuppressant Tacrolimus are both authorized for treating patients with generalized myasthenia gravis (GMG). This study seeks to evaluate and compare the clinical effectiveness of Eculizumab with or with not Tacrolimus in the treatment of generalized myasthenia gravis. METHODS: A retrospective study was performed involving 95 patients with GMG treated with Eculizumab at three hospitals in China. Among them, 80 patients were included in the analysis: 57 received Eculizumab alone, while 23 were treated with a

The effect of FcRn inhibition on overlapping autoimmune comorbidity in AChR-seropositive generalized myasthenia gravis.

INTRODUCTION: Generalized Myasthenia gravis (gMG) is a rare autoimmune disorder of the neuromuscular junction caused by IgG autoantibodies against antigens in the postsynaptic cleft. This study aimed to assess the real-world effectiveness and safety of Efgartigimod (EFG) in AChR-positive gMG with overlapping autoimmune comorbidity. METHODS: EFG was administered as 4 consecutive weekly infusions or injections. Myasthenia Gravis Activity of Daily Living (MG-ADL) and Myasthenia Gravis quantitative (QMG) scales were administered at baseline and after 4 weeks. Clinical data on autoimmune comorbidi

Progressive Dysphagia Revealing Glioma: Diagnostic Overshadowing by Suspected Myasthenia Gravis.

UNLABELLED: Progressive dysphagia in older adults presents a diagnostic challenge with a broad differential including neuromuscular, structural and malignant causes. Dysphagia associated with aspiration may lead to significant respiratory compromise requiring intensive care management. We report the case of a 78-year-old patient with rheumatoid arthritis treated with methotrexate who presented with progressive dysphagia to solids and liquids, weight loss, and cough. She subsequently developed aspiration pneumonia complicated by repeated type 2 respiratory failure and required mechanical ventil

Co-occurrence of myasthenia gravis and neuromyelitis optica spectrum disorder: A case series and literature review.

BACKGROUND: Patients with myasthenia gravis develop other autoimmune diseases in 5-30% of cases and have a higher incidence of demyelinating disorders than the general population. An increased risk of neuromyelitis optica spectrum disorder following thymectomy has also been reported. OBJECTIVE: To describe two adult patients from Bogotá, Colombia, with concurrent myasthenia gravis and neuromyelitis optica spectrum disorder after thymectomy, and to provide a literature review. METHODS: A retrospective literature review of articles published up to October 2024 was performed using PubMed and Em

Suspected Bulbar-Predominant Myasthenia Gravis Exacerbation in an Older Adult With Respiratory Deterioration and Silent Aspiration: A Case Report.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction characterized by fluctuating skeletal muscle weakness. Bulbar-predominant exacerbations may be difficult to recognize in older adults with multiple comorbidities, particularly when respiratory deterioration is initially attributed to cardiopulmonary disease. We report the case of an 83-year-old woman with generalized acetylcholine receptor antibody-positive MG, dementia with behavioral disturbance, moderate chronic obstructive pulmonary disease (COPD), chronic kidney disease, and previous deep vein thrombosis who wa

From Rescue Therapy to Mechanism-Based Sequencing in Refractory Myasthenia Gravis: A Narrative Review with an Illustrative Clinical Case.

Refractory acetylcholine receptor antibody-positive generalized myasthenia gravis (AChR-Ab-positive gMG) is clinically heterogeneous, and evidence on how to select a therapeutic mechanism after incomplete response to a previous targeted class remains limited. This narrative review synthesizes guidelines, randomized trials, extension studies, meta-analyses, observational evidence, and emerging translational data identified through targeted searches updated to 21 June 2026, with a final bibliographic check through 10 August 2026. We examine conventional immunosuppression, rescue therapy, B-cell

Autonomic dysfunction and its association with sexual dysfunction in myasthenia gravis.

PURPOSE: Autonomic dysfunction has been reported in myasthenia gravis (MG); however, its clinical relevance and relationship with sexual dysfunction (SD) remain unclear. This study aimed to evaluate autonomic dysfunction in MG and to investigate its association with SD using objective autonomic tests. METHODS: A total of 44 patients with MG and 50 age- and sex-matched healthy controls (HCs) were included. Autonomic function was assessed with the Composite Autonomic Symptom Score-31 (COMPASS-31), sympathetic skin response (SSR), and R-R interval variability (RRIV). Ocular autonomic involvement

Early high-dose intravenous methylprednisolone is associated with faster symptom improvement in persistent ocular myasthenia gravis: a single-center retrospective study.

BACKGROUND: High-dose intravenous methylprednisolone pulse therapy (HMP) may provide rapid symptom relief in ocular myasthenia gravis (MG), but the clinical impact of its early use as induction immunotherapy remains unclear. We evaluated whether early HMP was associated with more rapid symptom improvement in patients with persistent ocular MG. METHODS: This single-center retrospective study included 36 patients with ocular MG who received corticosteroid-based immunotherapy and remained classified as having ocular MG throughout 2 years of follow-up. Patients were categorized according to their

Treg-associated oligomannose N-glycans trigger immune dysregulation in myasthenia gravis.

Myasthenia gravis (MG) is an autoimmune neurological disease driven by the production of autoantibodies against components of the neuromuscular junction, resulting in fatigable muscle weakness, and in more severe cases, leading to life-threatening complications. This breakdown of immune tolerance has been associated with an impaired suppressive function and altered frequency of Regulatory T cells (Tregs) in autoimmune diseases, including MG. As key regulators of immune tolerance, Tregs play a pivotal role in maintaining immune homeostasis through their ability to suppress other immune cells, c

Extracardiovascular immune-related adverse events in patients with suspected cardiac toxicity associated with immune checkpoint inhibitors.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have transformed cancer therapy but may cause immune-related adverse events (iRAEs). Cardiovascular iRAEs, especially myocarditis, carry high morbidity and mortality. Extracardiovascular iRAEs are included as diagnostic criteria for myocarditis, yet their prevalence, spectrum, and clinical impact in patients with suspected cardiovascular iRAEs are poorly defined. METHODS: We conducted a prospective, single-center observational study between March 2022 and December 2023. Consecutive patients referred to cardio-oncology for suspected ICI-related c

Mitochondrial Myopathy in the Oculoplastic Practice.

Ptosis is a common ocular complaint in the elderly population. While most cases are often due to involutional changes in the eyelid, neurogenic, mechanical or myogenic diseases should remain as important differential diagnoses. We report a case of a 75-year-old Chinese man who presented with chronic progressive ptosis and was initially managed as presumed seronegative ocular myasthenia gravis (MG). Despite positive bedside findings suggestive of MG, serological and electrophysiological investigations were inconclusive, and there was no clinical response to pyridostigmine. The diagnosis was ach
