HLA-DP, HLA-DQ, and HLA-DR have different requirements for invariant chain and HLA-DM.

The MHC is central to the adaptive immune response. The human MHC class II is encoded by three different isotypes, HLA-DR, -DQ, and -DP, each being highly polymorphic. In contrast to HLA-DR, the intracellular assembly and trafficking of HLA-DP molecules have not been studied extensively. However, different HLA-DP variants can be either protective or risk factors for infectious diseases (e.g. hepatitis B), immune dysfunction (e.g. berylliosis), and autoimmunity (e.g. myasthenia gravis). Here, we establish a system to analyze the chaperone requirements for HLA-DP and to compare the assembly and

Paralytic strabismus: third, fourth, and sixth nerve palsy.

Eye movement abnormalities constitute an important clinical sign that can be a manifestation of dysfunction of cranial nerves III, IV, and VI (the 3 ocular motor nerves). Specific motility deficits often have highly localizing value within the neuroaxis, serving to refine a differential diagnosis and guide management. This article reviews the key anatomic concepts, clinical presentation, differential diagnosis, and management of ocular motor nerve palsies. Dysfunction of an ocular motor nerve must be distinguished from other causes of abnormal eye movements, such as myasthenia gravis or thyroi

[Analysis of severe and threefold atypical and serious course of catatonic-paranoid psychosis].

The paper describes the difficult course of catatonic-paranoid psychosis which began with symptoms similar to the myasthenia. The growing symptoms of catatonia (in this oral mechanisms with the compulsion of mastication, injuring with teeth of the mouth, tongue biting and damage, such as lockjaw) brought about choking which was followed by aspiration pneumonia. The patient had to have pharmacological coma induced, along with muscle relaxation and artificial ventilation in the conditions of the intensive care department. Despite treatment with high doses of neuroleptics, the repeated trials of

Myasthenia gravis requiring pyridostigmine treatment in a national population cohort.

BACKGROUND: Pyridostigmine is the first drug of choice for patients with myasthenia gravis (MG). The drug is not prescribed regularly to any other patient groups. We aimed to determine the prevalence, incidence and gender-specific characteristics of patients with MG needing drug treatment in a well-defined population cohort. METHODS: Data were retrieved from the Norwegian Prescription Database (NorPD) 2004-2007, containing information on all dispensed drugs in Norway. The study population comprised 677 recipients of pyridostigmine who met the following inclusion criteria (one or more): (i) Mo

Using mouse cranial muscles to investigate neuromuscular pathology in vivo.

Neuromuscular pathology is a classic hallmark of many diseases such as muscular dystrophy, myasthenia gravis, amyotrophic lateral sclerosis and spinal muscular atrophy. It is also a feature of many congenital and acquired myopathies and neuropathies such as diabetic neuropathy and toxin-exposure. The availability of experimentally accessible nerve-muscle preparations from rodent models in which pathological events can be studied in nerve and muscle, as well as at the neuromuscular junction, is therefore of fundamental importance for investigating neuromuscular disease. The group of small crani

Detection of poliovirus-infected macrophages in thymus of patients with myasthenia gravis.

BACKGROUND: Genetic and environmental factors are thought to contribute to the etiology of the autoimmune disease myasthenia gravis (MG). Viral involvement has long been suspected, but direct evidence of involvement has not been found. We recently reported that Toll-like receptor 4 (TLR4)-a key activator of innate immunity-was overexpressed in the thymus of some patients with MG, suggesting that thymic infection by pathogens might be involved in MG pathogenesis. We searched for evidence of intrathymic infection in patients with MG. METHODS: Twenty-seven MG thymuses (6 involuted, 7 hyperplasti

Autoimmune mediated neuromuscular junction defects.

PURPOSE OF REVIEW: This review summarizes the recent advances on pathogenesis of antibody-mediated disorders of the neuromuscular junction, and results of studies on clinical assessment and treatments. RECENT FINDINGS: The incidence of myasthenia gravis, particularly in patients older than 50 years, is rising, and this is not solely due to improved disease recognition. It is uncertain how muscle specific tyrosine kinase (MuSK) antibody positive myasthenia gravis results in neuromuscular transmission failure since MuSK antibodies alter neuromuscular junction morphology without altering acetylc

[Neuromuscular diseases - pathophysiology, diagnostics and therapy].

Neuromuscular illnesses are characterized by a motor weakness or unusual muscular fatigue. Besides in some diseases sensory disturbances and pains are prominent. The causes of the disturbed interaction between nerve and muscle are varied and reach from illnesses of the motor anterior horn cells of the spinal cord about multifactorial lesions of peripheral nerves, a disturbance of the neuromuscular junction up to primary and secondary myopathies. In addition to the clinical examination electrophysiological procedures, specific laboratory tests from serum and CSF as well as the nerve or muscle b

Myasthenia and related disorders of the neuromuscular junction.

Our understanding of transmission at the neuromuscular junction has increased greatly in recent years. We now recognise a wide variety of autoimmune and genetic diseases that affect this specialised synapse, causing muscle weakness and fatigue. These disorders greatly affect quality of life and rarely can be fatal. Myasthenia gravis is the most common disorder and is most commonly caused by autoantibodies targeting postsynaptic acetylcholine receptors. Antibodies to muscle-specific kinase (MuSK) are detected in a variable proportion of the remainder. Treatment is symptomatic and immunomodulato

Minimal access mediastinal surgery: One or two lung ventilation?

BACKGROUND: Minimal access mediastinal surgery (MAMS) is usually performed under general anaesthesia with double lumen tubes (DLT). The aim of this study is to evaluate two lung ventilation through single lumen tubes (SLT) during thoracoscopic sympathectomy for hyperhidrosis and thoracoscopic thymectomy for myasthenia gravis. METHODS: In this prospective non-randomized study, MAMS was performed in 58 patients with hyperhidrosis and 42 patients with myasthenia gravis, from January 2002 to December 2008. Patients were intubated with a DLT or SLT, 50 patients in each group. In the DLT group, end

[Endotracheal intubation without the use of muscle relaxants in patients with myasthenia gravis].

INTRODUCTION: Although muscle relaxants have been widely used to facilitate endotracheal intubation, the administration of these drugs in myasthenic patients may be associated with adverse events. MATERIAL AND METHODS: After obtaining Institutional Reviewing Board approval and informed, patient consent, 30 patients with myasthenia gravis were enrolled in a prospective, double-blind, randomized clinical trial. We compared intubating conditions (ease of laryngoscopy, vocal cords, cough, jaw relaxation, limb movement) following fentanyl 2 mg/kg and propofol 2 mg/kg (group PRO, n = 15) vs fentany

Intravenous immunoglobulin therapy in two patients with myasthenia gravis and pemphigus vulgaris.

UNLABELLED: Various forms of pemphigus have been reported to occur with myasthenia gravis (MG), with and without thymoma. We described two cases of pemphigus vulgaris associated with MG without thymoma. Case 1. A 44 year-old woman presented with 3 years history of pemphigus vulgaris. Three years later, she developed myasthenic symptoms with elevated level of anti-acetylcholine receptor (AChR) antibodies - 5.2 nmol/L. She was thymectomised and we revealed only hyperplastic thymus. Case 2. A 64-year-old woman had a general fatigue and intermittent double vision. She was diagnosed as MG three yea

Expression of leucine-rich repeats and immunoglobulin-like domains (LRIG) proteins in human ependymoma relates to tumor location, WHO grade, and patient age.

UNLABELLED: Chyluria, defined as the presence of chyle in urine, is a rare condition caused by an abnormal communication between the lymphatic system and the urinary tract. It can present with massive proteinuria and characteristic milky urine, mimicking nephrotic syndrome. We report the case of a 23-year-old woman with a history of seronegative ocular myasthenia gravis who developed progressive bilateral lower extremity oedema and intermittent nephrotic-range proteinuria despite normal serum albumin and lipid profiles. A renal biopsy, performed to investigate proteinuria, was complicated by a

Glycosylation of proteinase 3 (PR3) is not required for its reactivity with antineutrophil cytoplasmic antibodies (ANCA) in Wegener's granulomatosis.

BACKGROUND: Immunoglobulin E (IgE) is traditionally recognized for its role in allergic response. However, emerging evidence suggests it may also influence cardiovascular, renal, and immune function. Understanding how IgE relates to endothelial function, electrolytes, renal markers, inflammatory mediators, and sociodemographic factors could provide insight into integrated cardiorenal-immune pathways and inform risk stratification people living with or without HIV. METHODS: This cross-sectional study included 221 adults (median age 49 years, 68.3% female, 64.7% HIV-positive) at the medical cli

Seasonal variations of gonadotropins and prolactin in the laboratory rat. Role of maternal pineal gland.

Sexual and gender minority individuals experience higher rates of adverse childhood experiences (ACEs) than heterosexual individuals. Yet, the general ACE framework ignores unique ACEs faced by SGM individuals (i.e., cisheterosexism during childhood). The Sexual and Gender Minority ACE (SGM-ACE; Schnarrs et al., 2022, 2023) framework fills this gap by accounting for ACEs that are unique for SGM individuals resulting from systems of oppression (i.e., cisheterosexism during childhood). However, past studies employing this framework were conducted in the U.S. context, limiting the understanding o
