Limb-on-a-Chip: An All-Hydrogel Platform for Scalable and Reproducible Engineering of Functional Neuromuscular Tissues.

Diseases or injuries that impact neuromuscular tissues severely impact human health, motivating the development of in vitro models of the motor control system. Current platforms either have limited reproducibility or require complex fabrication protocols that preclude high-resolution imaging and scalable functional analysis. We have developed a one-step method for fabricating an all-hydrogel Limb-on-a-Chip that addresses key challenges of current systems by enabling reproducible and scalable manufacturing of neuromuscular tissues compartmentalized into "spinal cord" and "limb" chambers. Co-Occurrence of Facioscapulohumeral Muscular Dystrophy and Myasthenia Gravis: A Systematic Review and Meta-Analysis of Individual Patient Data.

BACKGROUND: Co-occurrence of facioscapulohumeral muscular dystrophy (FSHD) and myasthenia gravis (MG) is rare but clinically critical, as treatable MG symptoms may be misattributed to FSHD progression. This systematic review and meta-analysis aimed to comprehensively characterize patients with both conditions and to quantitatively synthesize the available individual patient data. METHODS: Following PRISMA 2020 guidelines, we searched four databases until 3 April 2026 for original reports providing individual-patient data on confirmed FSHD and MG co-occurrence. We performed descriptive and qua

EARLY-ALS: a multicentre study on prodromal amyotrophic lateral sclerosis.

BACKGROUND: Neurodegenerative diseases often feature a prolonged phase of subtle changes preceding diagnosis. In amyotrophic lateral sclerosis (ALS), defining this prodromal period is critical for identifying early disease features and intervention windows. METHODS: In this case-control study, 475 patients with ALS and 285 controls from 20 centres in Germany and Switzerland completed a digital questionnaire on prodromal symptoms and health-related factors. Logistic regression included subgroup analyses by sex and clinical strata. FINDINGS: Patients with ALS reported a higher burden of prodro

Real-World Clinical Outcomes and Corticosteroid Tapering in Telitacicept-Based Therapy for Ocular Myasthenia Gravis: An Exploratory Analysis by Prior Treatment History.

INTRODUCTION: Ocular myasthenia gravis (OMG) is an autoantibody-mediated disease characterized by extraocular muscle weakness. Telitacicept has shown potential in treating autoimmune diseases, but its efficacy and corticosteroid-tapering outcomes in OMG remain unexamined. METHODS: This retrospective, multicenter study evaluated the efficacy and corticosteroid-tapering outcomes of telitacicept in 19 OMG patients (11 with refractory OMG and eight with newly/reinitiated treatment OMG). The primary endpoint was the proportion of patients achieving minimal symptom expression (MSE) within 6 months.

A Novel Sequential Regimen of Efgartigimod and Telitacicept as Fast-Acting Treatment for Impeding Crisis of MuSK-MG.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction. The muscle-specific kinase (MuSK) antibody-positive subtype often involves bulbar and respiratory muscles, conferring a high risk of myasthenic crisis. METHOD: We report a very late-onset MuSK-MG case complicated by lung infection and acute heart failure, with a history of peptic ulcer. The patient was sequentially treated with efgartigimod and telitacicept, achieving minimal symptom expression (MSE) within 1.5 months. DISCUSSION: Following the first efgartigimod infusion, QMG and MG-ADL scores improv

Clinical Neurology in Practice: The Shoulder (Part 2).

BACKGROUND: The shoulder's intricate neuroanatomy and biomechanics render it uniquely vulnerable to a broad spectrum of neurological disorders, from central lesions to peripheral nerve injuries. While Part 1 of this series established the anatomic and neurological foundations of shoulder function, Part 2 focuses on the pathologic landscape of neurological shoulder affections. These conditions-ranging from rare central syndromes to common peripheral neuropathies-often present diagnostic challenges due to overlapping clinical features with orthopedic and rheumatological disorders. A systematic u

Infections and the neuromuscular junction: From direct neurotoxicity to immune-mediated dysfunction.

Infectious diseases and disorders of the neuromuscular junction (NMJ) are linked by complex and heterogeneous mechanisms that extend beyond direct pathogen-mediated injury. While some infectious agents can directly impair neuromuscular transmission, others may act as triggers of autoimmunity or contribute to the exacerbation of pre-existing NMJ disorders. However, the mechanisms underlying these interactions remain incompletely understood. In this narrative review, we summarize the current evidence regarding infections associated with NMJ dysfunction. Available evidence suggests that infection

Healthcare costs and burden of myasthenia gravis for patients and society: a systematic review.

BACKGROUND AND OBJECTIVES: New and costly therapies for myasthenia gravis (MG) have been approved during the last years, and many more are in the pipeline. Cost-benefit analyses are needed as part of defining start and stop criteria for such treatments. In this systematic review, we examine healthcare costs associated with MG: METHODS: A systematic literature search combined the word "myasthenia gravis" with "health resources", and "costs". 1374 records were identified initially, and 44 were included in the final review. RESULTS: Direct costs include treatment and follow-up in the primary and

Immune remodeling after thymectomy in patients with anti-AChR antibodies.

BACKGROUND: Thymectomy is an established treatment for thymoma-associated myasthenia gravis (MG). However, a subset of patients develop new-onset MG after surgery. Although thymectomy affects immune homeostasis, its comprehensive impact on the peripheral immune system remains unclear, particularly at the single-cell transcriptomic level. Therefore, we aimed to characterize immune remodeling following thymectomy in anti-acetylcholine receptor (AChR) antibody-positive patients without overt MG. METHODS: Peripheral blood mononuclear cells were collected from three anti-AChR antibody-positive pat

Myasthenia gravis: from recognition of heterogeneity to a paradigm shift toward precision therapy.

Myasthenia gravis (MG) is a heterogeneous neuroimmune disorder in which pathogenic autoantibodies impair neuromuscular junction transmission and produce fluctuating skeletal muscle weakness. Although conventional therapy with acetylcholinesterase inhibitors, corticosteroids, and broad immunosuppressants remains effective for many patients, its limitations are increasingly evident, including delayed onset, cumulative toxicity, and inadequate control in refractory disease. These challenges have accelerated the transition from non-selective immunosuppression to biologic and other targeted therapi

Disease severity as the dominant clinic-available marker of myasthenic crisis occurrence: a single-centre retrospective study.

INTRODUCTION: Myasthenic crisis is the most feared acute complication of myasthenia gravis (MG), yet simple clinic-based approaches to risk stratification remain limited. We examined which routinely recorded clinical features were associated with crisis occurrence across the observed disease course and whether a parsimonious model could provide retrospective group-level stratification. METHODS: This single-centre retrospective study included 86 adults with MG. Myasthenic crisis was defined as respiratory failure requiring mechanical ventilation. Disease severity was graded using the Myastheni

Impact of laparoscopic gastric bypass in a patient with myasthenia gravis and obesity.

We present the case of a 52-year-old woman with grade III obesity, type II diabetes, sleep apnea, and severe myasthenia gravis. Due to prolonged corticosteroid treatment, she had gained weight and experienced difficulty moving. To improve her condition, a laparoscopic gastric bypass was performed with good results. Myasthenia gravis is a rare autoimmune neuromuscular disease that primarily affects women. Although it is uncommon for it to be associated with obesity, weight loss has been shown to improve the symptoms of this condition, owing to the resolution of metabolic syndrome and the optimi

VARS2 deficiency drives seronegative myasthenia gravis pathology via mitochondrial, immune, and neuromuscular dysregulation.

Seronegative myasthenia gravis (SNMG) is a clinically heterogeneous subtype of myasthenia gravis (MG) that lacks the classical pathogenic autoantibodies used in routine diagnosis, leaving the underlying mechanisms poorly defined. Integrative genetic and single-cell transcriptomic analyses identified VARS2 as an MG-associated candidate gene with particular relevance to SNMG. VARS2 dysregulation was predominantly observed in CD4+ T cells, especially in the SNMG subgroup. To evaluate its in vivo relevance to SNMG, we established a zebrafish vars2 knockdown model. vars2 deficiency induced a myasth

Myasthenic syndromes: mistaking genetic for acquired.

Congenital myasthenic syndromes (CMS) are a rare, heterogeneous group of disorders caused by pathogenic variants in genes encoding proteins essential for neuromuscular transmission. DOK7 variants are among the most common causes of CMS and one of the subtypes that may worsen with pyridostigmine. We report two patients who presented in adulthood with fatigable limb girdle weakness, initially diagnosed with seronegative myasthenia gravis, who slowly progressed over time despite escalating treatment and eventually needed intensive care admission. Revisiting the history led to the diagnosis of DOK

A Prospective Clinical Trial of Efgartigimod for New-Onset Generalized Myasthenia Gravis.

INTRODUCTION: Numerous studies have demonstrated that efgartigimod is effective in treating myasthenia gravis (MG) across various patient populations. However, there is limited evidence regarding its use in patients with new-onset acetylcholine receptor antibody-positive generalized MG (AChR-gMG). Therefore, this study aimed to investigate the real-world safety and effectiveness of efgartigimod in Chinese patients with new-onset anti-cholinergic receptor (AChR)- gMG. METHODS: This prospective study was conducted in 29 patients with new-onset AChR-gMG, with a three-month follow-up. The Myasthe

Muscle-Derived BMP4 Regulates Morphology and Function of Endplates on Extrafusal and Intrafusal Muscle Fibers in Adult Mice.

Understanding factors contributing to neuromuscular junction (NMJ) stability postdevelopment will shed light on how this stability is lost during aging and in neuromuscular diseases. Previous work in Drosophila suggests that morphogens within the bone morphogenic protein (BMP) family are potential candidates because the BMP homolog, gbb, along with its receptor, wit, have key roles in NMJ structure, stability, and function. Whether BMPs have similar roles at vertebrate NMJs is currently unknown. To examine this question, we generated doxycycline-inducible, muscle-specific BMP4 null mice, refer

Literature review of myasthenia gravis complicated by myocardial damage.

BACKGROUND: Myasthenia gravis (MG) is a chronic autoimmune disorder predominantly driven by pathogenic autoantibodies directed against postsynaptic acetylcholine receptors, impairing neuromuscular transmission. Although cardiovascular complications remain relatively uncommon among MG patients, their emergence markedly elevates the risk of fatal outcomes. This review synthesizes existing evidence to delineate the clinical manifestations of MG-associated myocardial damage, offering clinicians a comprehensive framework for early detection and evidence-based therapeutic intervention. METHODS: We

Neuromuscular junction formation by compartmentalized culture using human iPS cell-derived motor neurons and skeletal muscle cells.

The neuromuscular junction (NMJ) is a synapse that connects the nerve terminal of a motor neuron to a skeletal muscle fiber. The mechanisms underlying NMJ formation have been elucidated primarily through studies in animal models. However, elucidation of the pathophysiological mechanisms of human neuromuscular diseases and identification of potential therapeutic agents require the establishment of a human NMJ model. In this study, we established a human NMJ model using a compartmentalized culture system with cells derived from human induced pluripotent stem (iPS) cells, which enables quantitati

Chimeric antigen receptor T cell therapy in autoimmune neurological disease: current status of efficacy and safety.

Autoimmune neurological diseases affecting the central nervous system, peripheral nervous system and neuromuscular junction are frequently refractory to conventional B-cell-depleting therapies such as CD20-directed monoclonal antibodies, highlighting a need for more effective treatment approaches. Chimeric antigen receptor T (CAR-T) cell therapy, initially developed for haematological malignancy, has been repurposed to more comprehensively deplete pathogenic B-cell and plasma cell populations in autoimmune disease, raising the possibility of durable, treatment-free remission. This scoping revi
