Treatment patterns, intensive treatment, and hospitalization-related burden of juvenile generalized myasthenia gravis in Japan: a retrospective cohort study using a hospital-based claims database.

OBJECTIVE: Juvenile generalized myasthenia gravis (MG) management aims to control symptoms while minimizing corticosteroid-related toxicity. However, large-scale evidence on real-world pediatric treatment patterns remains limited. METHODS: We conducted a retrospective cohort study using a large Japanese hospital-based claims database (Medical Data Vision). We identified patients aged 0-17 years with juvenile generalized MG between April 2008 and November 2024. We summarized the use of cholinesterase inhibitors, oral corticosteroids, immunosuppressants, and intensive treatments (plasma exchang

Biallelic PIGB Variants Cause Motor Neuropathy with Conduction Blocks and Peripheral Nerve Hyperexcitability.

OBJECTIVE: Glycosylphosphatidylinositol (GPI)-anchored proteins play critical roles in nervous system function. Pathogenic variants in genes involved in GPI-anchor biosynthesis cause early-onset multisystem disorders known as inherited GPI deficiencies. We describe a novel neuromuscular phenotype associated with PIGB deficiency. METHODS: Patients with neuromuscular disease carrying PIGB variants were identified, and clinical data collected. GPI-anchored protein and free-GPI expression were assessed by flow cytometry and variants validated in a PIGB knockout cellular model. RESULTS: Biallelic

Apheresis-Triggered Torsades De Pointes in Congenital Long QT Syndrome Type 2: Citrate-Related Arrhythmia Risk and Safe Modality Switching During Myasthenic Crisis.

Citrate anticoagulation during therapeutic plasma exchange (TPE) chelates divalent cations and can precipitate torsades de pointes (TdP) in patients with pre-existing repolarization vulnerability. We report a 44-year-old woman with genetically determined long QT syndrome Type 2 (LQT2) presenting in myasthenic crisis due to anti-MuSK-positive myasthenia gravis. TPE was initiated urgently before confirmatory antibody results were available, as alternative rapid immunomodulatory options were not immediately accessible. Despite prophylactic electrolyte supplementation, the corrected QT interval (Q

IgA autoantibodies demonstrate a novel mechanism of MuSK myasthenia gravis pathology.

Patients with muscle-specific tyrosine kinase (MuSK) myasthenia gravis (MG) develop muscle weakness due to functionally monovalent immunoglobulin (Ig) G4 autoantibodies (Abs) that target MuSK and disrupt acetylcholine receptor (AChR) clustering. Emerging evidence from other autoimmune conditions suggests that Abs of the IgA class-the archetypal immunoglobulin isotype at mucosal sites-may synergize with IgG Abs, contributing to pathology. In MuSK MG, however, the presence of disease-specific IgA Abs has not yet been recognized, limiting a broader understanding of IgG4-driven autoimmunity. To ad

ALG14 variants contribute to a congenital disorder of glycosylation characterized by congenital myasthenia and epilepsy.

Asparagine-linked glycosylation 14 (ALG14) is a UDP-GlcNAc transferase that catalyzes the second sugar addition in the synthesis of the dolichol-linked oligosaccharide precursor in N-linked glycosylation, ultimately contributing to the glycosylation of a wide array of proteins. Bi-allelic pathogenic variants in ALG14 have been suggested to lead to a congenital disorder of glycosylation (CDG) due to incomplete lipid-linked oligosaccharide synthesis and ultimately hypoglycosylation of N-linked glycoproteins. In a cohort of nine previously unreported individuals with bi-allelic variants in ALG14

Optimising well-being in myasthenia gravis: a multidisciplinary approach.

Although myasthenia gravis (MG) primarily manifests as weakness of oculo-bulbar, axial and appendicular muscles, the disease and its treatments are associated with many comorbid conditions that impact patients' quality of life. Patients frequently experience secondary autoimmunity, cardiovascular and endocrine dysfunction, fatigue, sleep and mental health disorders. MG and its treatments can also be an important source of social and economic burden on patients. This review aims to elaborate on the multi-morbid nature of MG and to provide an approach for recognising and managing relevant comorb

BCMA/CD19 CAR T cell therapy for refractory myasthenia gravis: Proteomic signatures and single-cell transcriptomics of disease flares.

Myasthenia gravis (MG) is an autoimmune disorder characterized by B cell dysfunction. Here, we designed B cell maturation antigen (BCMA)/CD19 chimeric antigen receptor T cell (CAR T cell) therapy for six refractory MGs, demonstrating favorable safety with grade 1 cytokine release syndrome observed. CAR T cell expansion induced profound B cell depletion, a sustained reduction in acetylcholine receptor (AChR) antibody titers, and symptom improvement. Five patients achieved drug-free remission with minimal manifestations by month 6, persisting through the 12-month follow-up despite B cell reconst

Telitacicept as a New Therapeutic Avenue for Generalized Myasthenia Gravis and Thymoma-Associated Myasthenia Gravis.

Generalized myasthenia gravis (gMG) is an antibody mediated autoimmune neuromuscular junction disorder characterized by muscle weakness and fatigue as well as acetylcholine receptor antibody (AChR-Ab) as the main presence. A proportion of patients fail to achieve minimal symptom expression (MSE), furthermore 10-20% of them develop into refractory under conventional immunotherapy. We conducted a retrospective study to explore the effectiveness and safety of telitacicept in gMG and thymoma-associated MG (TAMG) patients. The treatment response was assessed by the variation of QMG, MG-ADL and MG-Q

Urinary dysfunction in myasthenia Gravis: a cross-sectional case-control study.

INTRODUCTION: Myasthenia gravis (MG) is a disorder of neuromuscular transmission that primarily affects skeletal muscles, leading to cranial, truncal, and limb weakness. While motor symptoms are well documented, the prevalence and clinical significance of urinary dysfunction particularly urinary incontinence (UI) and overactive bladder (OAB) symptoms remain underexplored in MG, with previous reports limited to small cohorts. METHODS: In this study, 86 MG patients and 90 age- and sex-matched inpatient controls were evaluated. All participants completed the International Consultation on Incontin

Trends in thymectomy for myasthenia gravis in Japan: analysis of a nationwide surgical registry from 1996 to 2023.

BACKGROUND: Thymectomy for generalized myasthenia gravis (MG) without thymoma and with anti-acetylcholine receptor antibody gained evidence-based support following the publication of the MGTX study in 2016, demonstrating its therapeutic efficacy. Consequently, the number of thymectomies increased in the United States. Trends in thymectomy for this condition in Japan is investigated. METHODS: Thymectomy reported in The Japanese Association for Thoracic Surgery (JATS) Annual Report from 1996 to 2023 was analyzed. Annual numbers of thymectomies were reviewed according to surgical approach, non-th

Phylogenetic analysis of the genus Taphrina: proposal of one new genus and seven new species (Taphrinales, Taphrinomycetes) from China.

AIMS: Lambert-Eaton myasthenic syndrome (LEMS) is a kind of autoimmune disease of the neuromuscular junction that is often misdiagnosed as a peripheral nerve disease or myopathy. For some difficult cases, muscle biopsy examination is useful for differential diagnosis. However, studies about the pathological findings of LEMS patients are rare, especially of patients who were diagnosed with small cell lung carcinoma (SCLC). This study aimed to describe several pathological muscle features in patients with LEMS associated with SCLC. MATERIALS AND METHODS: The 5 patients enrolled in this study we

Gender differences in Myasthenia Gravis in a neuromuscular reference center in Belgium.

BACKGROUND: It is well known that scientific literature has a significant gender gap, as gender differences were not considered until very recently. Women were often excluded from studies, and even when included, the results are rarely analyzed separately by gender. This study aims to investigate gender-specific differences in patients with Myasthenia Gravis (MG) by segregating data from our cohort of patients in Antwerp (Belgium). METHODS: We analyzed the data of our previously published cohort of 163 patients with MG visited in the Antwerp University Hospital between 2019 and 2021, segregat

Does myasthenia gravis impact cognitive functioning? A systematic review and meta-analysis.

BACKGROUND: Myasthenia Gravis, traditionally known as a purely motor disease, has been recently associated also with non-motor symptoms, including psychological and cognitive symptoms. However, the presence of cognitive impairment in Myasthenia Gravis remains unclear due to limited and heterogeneous findings. This systematic review and meta-analysis aims to clarify the relationship between Myasthenia Gravis and cognitive performances. METHODS: Following PRISMA guidelines, we systematically searched PubMed, Web of Science, and SCOPUS for original researches published between January 2000 and D

Analysis of infections and malignancy risks among patients with myasthenia gravis compared with matched controls in a US real-world setting.

IntroductionMyasthenia gravis (MG) is a rare, debilitating autoimmune disease associated with pathogenic immunoglobulin G autoantibodies directed against components of the neuromuscular junction. The autoimmune nature of the disease and long-term immunosuppressive treatment may increase susceptibility to infections and malignancies, but studies investigating the association between MG and infections or malignancies remain scarce.MethodsWe conducted a retrospective, observational study using Optum's deidentified Market Clarity Data (Market Clarity) to evaluate the incidence rate (IR) of infecti

Variations in clinical features and disease burden of myasthenia gravis between racial and ethnic groups: A retrospective cohort study of two national databases.

Previous studies have reported that clinical characteristics and outcomes of myasthenia gravis (MG) vary across racial and ethnic groups. This study leverages two nationwide datasets, the MG Foundation of America Global MG Patient Registry (MGFAPR) and the National Inpatient Sample (NIS) to further examine these reports. We compared clinical characteristics, socioeconomic determinants, therapeutic regimens, and inpatient outcomes across racial/ethnic groups of MGFAPR enrollees between July 2013 and November 2024 that self-reported MG diagnosis by a physician and were US residents over the age

Risk of Infection Following Clearance of Immunoglobulin G by Plasma Exchange in Myasthenia Gravis.

INTRODUCTION/AIMS: Management of myasthenia gravis (MG) includes reduction of pathogenic IgG autoantibodies by intravenous immunoglobulin (IVIg), B-cell depletion, therapeutic plasma exchange (TPE), and neonatal Fc receptor (FcRn) blockers. Concerns exist that chronic IgG reduction could lead to more frequent infections. We assessed the frequency and severity of infection with associated serum IgG levels during infections in MG patients treated with chronic TPE in both inpatient and outpatient settings. METHODS: MG patients treated with chronic TPE at a single center from April 2014 through D
