Development and External Validation of a Multivariable Model to Predict Early Minimal Symptom Expression Response in Adult Generalized Myasthenia Gravis Patients Treated With Efgartigimod.

BACKGROUND: Efgartigimod, a neonatal Fc receptor (FcRn) blocker, is approved for generalized Myasthenia Gravis (gMG), but predictors of early response are unclear. Using minimal symptom expression (MSE) as the therapeutic target, we developed and externally validated a clinical model to predict early MSE response after starting efgartigimod. This efgartigimod-tailored model estimates the baseline probability of achieving early MSE and may assist in individualized treatment selection at therapy initiation. METHODS: We retrospectively analyzed 118 adults with gMG treated at five tertiary center

Naringenin protects zebrafish larvae from BMAA-induced neuromuscular toxicity by regulating myogenic and inflammatory pathways.

β-N-methylamino-L-alanine (BMAA) is an environmental neurotoxin widely detected in aquatic ecosystems and linked to neurodegenerative diseases. While its central neurotoxicity is well documented, its direct effects on skeletal muscle and neuromuscular junctions (NMJs) remain unclear. Naringenin (NAR), a citrus flavanone with antioxidant and anti-inflammatory properties, has shown protective effects in several disease models but has not been evaluated against BMAA-induced toxicity. Here, we used zebrafish larvae to examine the developmental, functional, structural, and molecular effects of BMAA

Impact of corticosteroid use on comorbidities in patients with myasthenia gravis in the US National Veterans Affairs Health Network.

BACKGROUND: Corticosteroid use may increase the risk of common comorbidities in patients with myasthenia gravis (MG); however, limited longitudinal data are available to evaluate this risk over time. METHODS: This retrospective cohort study used electronic medical record data from the National Veterans Affairs Health Care Network database (January 1999-March 2024) to evaluate the impact of corticosteroid use on the development of selected comorbidities in patients with and without generalized MG. Index date was the first MG diagnosis for the MG cohort and a randomly chosen date for a propensi

Single-nucleus RNA sequencing identifies a novel tenogenic heterologous differentiation in endometrial carcinosarcomas: implications for diagnosis and tumor classification.

Carcinosarcomas (CSs) are aggressive biphasic tumors characterized by epithelial and mesenchymal components, whose histogenesis and differentiation dynamics remain poorly understood. We present single-nucleus RNA sequencing (snRNA-seq) analysis of six CSs (five endometrial and one ovarian) and two normal endometrial samples, profiling over 96,298 cells. By integrating transcriptomic data with inferred copy number variations (CNVs), immunohistochemistry (IHC), fluorescence in situ hybridization (FISH), and in situ hybridization (ISH) validation, we resolved the complex cellular architecture of

Evaluating AI Research Quality in Myasthenia Gravis: A Longitudinal Study Using the CLARITY Framework (2020-2024).

Artificial intelligence (AI) is increasingly applied in clinical research to improve diagnostic accuracy, prognostic modeling, and disease monitoring, particularly in complex, data-constrained areas such as rare diseases. In this context, Myasthenia Gravis (MG)—a rare neuromuscular disorder—has seen a growing number of AI-driven studies aimed at addressing its diagnostic and clinical heterogeneity. However, the methodological quality and reporting standards of these studies remain largely unexamined. This study presents the first longitudinal evaluation of AI research in MG using the CLARITY A

Targeting pathogenic Th17.1 cells via JAK-STAT3 pathway: A novel approach with tofacitinib for refractory myasthenia gravis.

There remains a critical unmet need for effective, accessible, and well-tolerated therapies for myasthenia gravis (MG) who are refractory to current immunotherapies. The janus kinase and signal transducer and activator of transcription (JAK-STAT) signaling pathway plays a pivotal role in maintaining immune homeostasis by regulating cytokine-mediated responses. However, insights into the involvement of the JAK-STAT signaling in MG pathogenesis are still preliminary. In this study, we observed elevated levels of JAK2 in MG and further assessed the clinical efficacy of the pan-JAK inhibitor, tofa

Pharmacological and speech-language pathology management of dysphagia in patients with myasthenia gravis.

Myasthenia gravis (MG) is a rare, complex autoimmune neuromuscular disorder characterized by fluctuating muscle weakness. Bulbar involvement, particularly dysphagia (difficulty swallowing), is a common and debilitating symptom that significantly increases the risk of aspiration pneumonia, malnutrition, and dehydration, profoundly impacting quality of life. The management of myasthenic dysphagia requires a sophisticated, interdisciplinary approach. This review aims to provide a comprehensive overview of the current and emerging pharmacological treatments for myasthenia gravis and to detail the

Association of sleep disorder with bulbar weakness and short-term outcomes in myasthenia gravis.

BACKGROUND: Sleep disorder is increasingly recognized in myasthenia gravis (MG). However, its specific link to bulbar muscle weakness and short-term outcomes is not well established. This study aimed to investigate the prevalence of subjective sleep disorder (based on Pittsburgh Sleep Quality Index [PSQI] score) in a cohort of acetylcholine receptor (AChR) antibody-positive MG patients, and to explore its associations with bulbar muscle weakness, disease severity, and its value for the short-term outcome of MG. METHODS: We conducted a prospective cohort study of 163 AChR-MG patients. Subjecti

Fiber-type-specific architecture and pathophysiology of the neuromuscular junction.

The neuromuscular junction (NMJ) is a specialized synapse essential for translating neuronal signals into muscle contraction. This review examines the complex structural, functional, and molecular differences in NMJs that innervate fast- and slow-twitch skeletal muscle fibers. Fast-twitch fibers, optimized for rapid and powerful contractions, possess elaborate NMJs with deep folds, high neurotransmitter turnover, and greater vulnerability to synaptic fatigue and degeneration. In contrast, slow-twitch fiber NMJs exhibit simpler but more stable architectures that support sustained, fatigue-resis

Eculizumab as a Fast-Acting Therapy in a Myasthenic Crisis Patient With Poor Response to Efgartigimod: A Case Report.

INTRODUCTION: Myasthenic crisis (MC) is the most serious manifestation of myasthenia gravis (MG) and it poses a big challenge in clinical management. Plasma exchange and intravenous immunoglobulin are the first-line therapy for MC, but a subset of patients exhibit poor response. Recently, neonatal Fc receptor (FcRn) inhibitors and complement inhibitors showed great efficacy in MC. However, there is no report on whether eculizumab can serve as a rescue therapy for patients who have not responded to efgartigimod. CASE REPORT: This case report describes a Chinese patient with generalized MG who

Surgical Management of Unexpected Thymic Anatomical Variations During Thoracoscopic Thymectomy.

OBJECTIVES: Complete thoracoscopic thymic resection is crucial for achieving sternotomy-comparable outcomes in myasthenia gravis patients. Thymic anatomy varies significantly, presenting features like retro-left brachiocephalic vein (LBCV) horns and thymic ring. Surgeons encountering these variations may miss abnormal thymic tissue or face intraoperative challenges. This research aims to report on the thoracoscopic management of thymic variations, illustrating their perioperative outcomes. METHODS: This retrospective observational study was conducted from September 2016 to September 2024. A t

Double-seronegative myasthenia gravis: clinical characteristics, treatment, and outcomes - a systematic review of case reports and case series.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune condition characterized by muscle weakness caused by antibodies targeting the postsynaptic neuromuscular junction. The majority of patients test positive for antibodies against the acetylcholine receptor (AChR) or muscle-specific kinase (MuSK). A minority of patients test negative for these antibodies and are referred to as double-seronegative MG (dSNMG), a condition that remains inadequately studied and incompletely characterized. METHODS: We conducted a systematic review in accordance with PRISMA guidelines (PROSPERO registration: CRD420223

miRNA-206 in muscle and central nervous system crosstalk during exercise: A double-edged sword with therapeutic potential.

Physical activity triggers complex molecular responses in skeletal muscle, with increasing evidence showing systemic signaling roles for muscle-derived microRNAs (myomiRs). Among these, miR-206 has attracted attention for its dual function: promoting muscle regeneration but potentially harming the central nervous system (CNS). This review examines how miR-206 expression is regulated during exercise and its effects on muscle biology-such as fiber-type specification, mitochondrial changes, and neuromuscular junction (NMJ) repair. It also explores the paradoxical effects of high miR-206 levels in
