Myasthenia Gravis Outcomes After Use of Statins and Other Contraindicated Treatments: Results From the French National Insurance Database.

BACKGROUND: Clinical guidelines for treating myasthenia gravis (MG) recommend avoiding certain therapies because of a risk of exacerbating MG. We evaluated use of statins and other classically contraindicated therapies and their impact on healthcare outcomes among patients with MG in France. METHODS: This was an observational, retrospective, longitudinal cohort study using data from the French national health insurance database. Adult patients with MG-related claims from 2013 to 2020 were included. The first MG claim date was the index date, with follow-up until end-of-study or death. Multiva

Immune dysregulation driven by elevated platelet-to-lymphocyte ratio aggravates myasthenia gravis.

ObjectivePrevious studies have suggested a potential association between the platelet-to-lymphocyte ratio and disease activity in myasthenia gravis. However, the immunological mechanisms underlying this association remain insufficiently elucidated.MethodsA retrospective cohort of 229 patients with myasthenia gravis and a single-cell RNA sequencing dataset were analyzed to investigate the relationship between platelet-to-lymphocyte ratio and disease severity. Clinical associations were assessed using the Myasthenia Gravis Foundation of America classification and multivariable logistic regressio

Exploring Pembrolizumab-Induced IM3OS in a Patient With Bladder Cancer.

Pembrolizumab, an immune checkpoint inhibitor (ICI), has transformed cancer treatment across various malignancies, including metastatic urothelial cancer. As the use of pembrolizumab becomes increasingly widespread, understanding its potential adverse effects is essential. Here, we present the case of a 79-year-old male with urothelial cancer of the bladder, who experienced local recurrence after surgery and conventional therapies. He subsequently began pembrolizumab treatment. Following the first infusion, the patient developed a series of complications over a span of 26 days, including acute

Efgartigimod combined with rituximab helps improve the symptoms and reduce the use of corticosteroids in patients with MuSK antibody-positive MG: a single case report.

INTRODUCTION: Myasthenia gravis (MG) is an autoimmune disorder primarily affecting the neuromuscular junction. Muscle-specific receptor tyrosine kinase antibody (MuSK-Ab)-mediated MG often presents with bulbar weakness and is prone to crisis. Currently, conventional treatments show limited efficacy in MuSK-Ab-mediated MG, while rituximab therapy demonstrates favorable outcomes. CASE PRESENTATION: We report a case of a patient who initially achieved symptom control with efgartigimod during an acute exacerbation but experienced symptom recurrence after subsequent rituximab treatment. Owing to p

Aberrant expression of triggering receptor expressed on myeloid cell-1 is involved in the immunopathological mechanism of myasthenia gravis.

Triggering receptor expressed on myeloid cells-1 (TREM-1) and TREM-2, cell surface receptors involved in innate and adaptive immunity, exist in soluble (sTREM-1 and sTREM-2) and membrane-bound forms, with the soluble form serve as biomarkers in many immune diseases. However, the role of TREM-1 and TREM-2 in myasthenia gravis (MG) remains unclear. This study aims to investigate the expression of TREM-1 and TREM-2 in MG, as well as their potential role in the immunopathological mechanism of MG. We enrolled a total of 85 MG patients and 43 healthy controls (HC). Enzyme-linked immunosorbent assay

Gut Microbiota as Neuroimmune Modulators in Myasthenia Gravis: Mechanistic Insights from the Gut-Brain Axis to Therapeutic Innovations.

Myasthenia gravis (MG) is a chronic autoimmune disorder characterized by an immune-mediated attack on neuromuscular junction acetylcholine receptors (AChRs), and its pathogenesis is closely linked to immune dysregulation. Emerging evidence has highlighted the pivotal role of the gut microbiota in the pathophysiology of MG through immunomodulation, microbial metabolite signaling, and gut-brain axis interactions. This review combines 16S rRNA sequencing, metagenomic, and metabolomic data to reveal distinct gut microbial signatures in patients with MG. These signatures include reduced α-diversity

Low-dose IL-2 Ameliorates Experimental Autoimmune Myasthenia Gravis in Rats by Restoring the CD4-positive Helper T-cell Balance Via the JAK/STAT5 Pathway.

Myasthenia gravis (MG) represents a prototypical autoimmune disorder that targets neuromuscular junctions. Emerging insights into the pathogenic role of cellular immunity in MG have intensified investigations of potential immunomodulatory therapies, and low-dose interleukin-2 (LD-IL-2) has garnered particular attention because of its ability to restore immune balance, maintain immune homeostasis, and ensure immune tolerance. To systematically investigate the therapeutic potential of LD-IL-2 in MG and elucidate its underlying mechanisms, we established an experimental autoimmune myasthenia grav

PGVDA: a pathway-aggregated genetic dosage framework for interpretable disease classification using machine learning.

Although neuromuscular junction disorders (NMDs) and inflammatory polyneuropathies (IPNs) are biologically distinct, direct genetic comparisons between them remain limited, suggesting that additional underlying biological differences may yet be uncovered. Few studies have explored whether differences in variant patterns within shared biological pathways can be leveraged to distinguish NMDs and IPNs using machine learning (ML). We propose an interpretable ML framework based on Pathway-based Genetic Variant Dosage Average (PGVDA) to classify NMDs and IPNs and to identify key genes and pathways d

Autoantibody testing in neuromuscular medicine: assay technologies, interpretation, and clinical utility.

Neuromuscular autoantibody testing is an essential component in the diagnosis and management of autoimmune neuromuscular disorders. These immune-mediated diseases target antigens found in nerves, muscles, and neuromuscular junctions, and may occasionally involve the central nervous system, resulting in complex clinical presentations. Developments in antibody identification and validation have provided clinically relevant biomarkers for diagnostic, therapeutic, and prognostic purposes. Among antibody-associated neuromuscular disorders, paraneoplastic onconeural autoantibodies are of particular

Delayed recovery following myasthenic exacerbation and subsequent endplate-protecting treatment.

Endplate-protecting therapies have improved treatment of myasthenia gravis and indicate a rapid onset of their treatment effect. Currently, these substances can be used following myasthenic exacerbation (ME). Clinical worsening due to ME however often results in prolonged recovery. We here evaluated the treatment courses of patients having received endplate-protecting treatment following ME. Among 12 patients with ME described here, seven received endplate-protecting therapies. Our findings indicate that acceptable recovery usually takes nine months. In the lights of recent trials for treatmen

Neurologists' decision-making in the management of generalised myasthenia gravis: Therapeutic inertia and treatment choices.

The evolving landscape of generalised myasthenia gravis (gMG) treatment with new targeted immunotherapy options presents challenges for neurologists and potentially leads to therapeutic inertia (TI), the failure to initiate or intensify treatment when therapeutic goals are not met. This study aimed to assess neurologists' therapeutic decision-making process in gMG and its influencing factors. A cross-sectional, web-based study was conducted, involving 149 neurologists (mean age [standard deviation]: 39.0 [9.4] years; 54.4% male; median MG experience [interquartile range]: 7 [3-15] years).
