Nationwide trends in mediastinal tumor and myasthenia gravis surgery in Japan: a decade claims-based analysis (2014-2023).

BACKGROUND: Mediastinal tumor surgery and extended thymectomy for myasthenia gravis (MG) have undergone significant changes with the advancement of minimally invasive techniques. However, nationwide population-based data capturing these trends in Japan remain limited. This study aimed to characterize national surgical trends for mediastinal tumors and MG using a comprehensive administrative claims database. METHODS: We analyzed data extracted from the National Database of Health Insurance Claims and Specific Health Checkups (NDB), covering over 95% of insured procedures in Japan. We classifie

Low Prevalence and Inconsistency of LRP4-IgG Detection in Suspected Myasthenia Gravis: A Multicenter CBA Comparison.

BACKGROUND AND OBJECTIVES: Myasthenia gravis (MG) is an autoimmune disorder primarily caused by antibodies targeting neuromuscular junction proteins, particularly the acetylcholine receptor and the muscle-specific tyrosine kinase. However, 10%-20% of patients with MG are double-seronegative (dsMG). Antibodies against low-density lipoprotein receptor-related protein 4 (LRP4-IgG) have been found in a variable proportion of dsMG cases, but their frequency and clinical relevance remain unclear because of differences in assay methodologies and study populations. In this study, we assessed the frequ

Combined adapted physical activity and art-therapy intervention in myasthenia gravis: Study protocol for a randomized crossover pilot trial.

BACKGROUND: Myasthenia Gravis (MG) is a rare autoimmune disorder with fluctuating weakness and substantial impact on quality of life (QoL). Despite advances in pharmacological treatment, patients often experience fatigue, anxiety, and reduced daily functioning. Adapted Physical Activity (APA) has shown benefits in chronic neuromuscular disorders, while Art Therapy (AT) has demonstrated positive psychosocial effects in neurological conditions. However, no studies have investigated the potential synergy of APA combined with AT in MG. METHODS: We designed a monocentric, prospective, randomized c

Myasthenia Gravis With Acetylcholine Receptor Antibodies in the Very Old: Treatment Challenges and Diagnostic Pitfalls.

BACKGROUND: In myasthenia gravis (MG) with acetylcholine receptor (AChR) antibodies, epidemiology, disease mechanisms, diagnosis, and treatment depend on age. MG with debut after 65 years has an increasing incidence and prevalence but is underrepresented in clinical studies. METHODS: This is a systematic review of very late onset MG and MG in patients above 65 years with focus on epidemiology, pathogenesis, diagnosis, clinical characteristics, and treatment. RESULTS: Both innate and adaptive immune responses are influenced by age. The increase in MG incidence in the very old is probably caus

Neuropsychiatric Symptoms After Liver Transplant for Wilson's Disease: A US-Based Multicenter Retrospective Cohort Study.

Liver transplantation (LT) is the standard treatment for Wilson disease (WD) with end-stage liver failure, but its use for severe neurological symptoms remains controversial due to reports of paradoxical post-transplant deterioration. This study examines the risk and predictors of neuropsychiatric symptoms after LT for WD compared to other liver diseases. We conducted a retrospective cohort study using the TriNetX US Collaborative Network. WD patients who underwent LT were matched 1:1 with non-WD LT recipients using propensity score matching (PSM) based on demographics, comorbidities, laborato

Are you weary of illness? Coping and existential positioning in life with myasthenia gravis.

This article explores what it means to position oneself existentially in relation to illness weariness in myasthenia gravis. Grounded in phenomenology and informed by autoethnographic principles, the study examines how the body's changing disposition shapes lived meaning and coping in chronic illness. Phenomenological vignettes serve as experiential entry points, followed by reflective interpretation inspired by Havi Carel, Maurice Merleau-Ponty, Drew Leder, and Kay Toombs. Two experiential themes emerge. First, When the body doesn't always follow one's lead anymore shows how myasthenia gravis

High expression of IRF8 in AChR-specific cells regulates the function of B cells.

Myasthenia gravis (MG) is a kind of autoimmune disease. Interferon regulatory factor 8 (IRF8) is an interferon common sequence-binding protein. IRF8 is a key factor involved in the development, maturation, and antimicrobial activity of myeloid cells. Second-generation sequencing of splenocytes at the peak period of disease in experimental autoimmune MG (EAMG) rats and complete Freund's adjuvant (CFA)-treated mice suggested that the expression of IRF8 gene is related to the pathogenesis of EAMG. This study focused on the differential expression of IRF8 in B cell and T cell in EAMG, and the unde

Telitacicept for refractory AChR-positive generalized myasthenia gravis: a retrospective real-world study.

BACKGROUND AND AIMS: Treating refractory acetylcholine receptor-positive generalized myasthenia gravis (AChR+ gMG) remains challenging, especially for patients requiring long-term immunosuppressive therapy. Current treatments often lack specificity and pose significant long-term risks, underscoring the need for alternatives. Telitacicept, a novel dual inhibitor of B lymphocyte stimulator (BLyS) and proliferation-inducing ligand (APRIL), offers a promising targeted therapeutic approach. This study aimed to evaluate the efficacy and safety of telitacicept in the treatment of refractory AChR+ gen

Single-cell profiling of immune reset in patients with refractory generalized myasthenia gravis receiving autologous CD19/BCMA CAR-T cell therapy.

BACKGROUND: Chimeric antigen receptor (CAR) T cells targeting CD19 or B cell maturation antigen (BCMA) hold great promise to treat neuroimmune disorders, but the efficacy of CD19/BCMA dual-targeting CAR-T cells and their impact on systemic immunity are poorly understood. METHODS: In this phase 1 study (ClinicalTrials.gov: NCT06371040), patients with refractory generalized myasthenia gravis (gMG) received autologous CD19/BCMA CAR-T cells without prior lymphodepletion. The primary endpoint was the frequency and severity of treatment-emergent adverse events at week 4. Secondary endpoints include

The Muscle Function Deficit Concept and Inflammaging.

Aging-related muscle dysfunction has been conceptualized through the model of sarcopenia, but it embraces several other characteristics, e.g., dynapenia, myosteatosis, and powerpenia. Our perspective reframes muscle aging from a different point of view, the Skeletal Muscle Function Deficit (SMFD), a unifying approach that integrates muscle quality and mass into a single functional definition. An SMFD score has been adopted in the InCHIANTI study against many geriatric outcomes, such as risk of disability, physical performance, hospitalizations and falls, and incidence of major diseases, highli

Association between vaccination and myasthenia gravis: a systematic review and meta-analysis.

BACKGROUND: Myasthenia gravis (MG) is a rare autoimmune disorder characterized by fluctuating muscle weakness due to impaired neuromuscular transmission. Vaccination remains a cornerstone of infectious disease prevention, yet concerns persist regarding potential autoimmune exacerbation in susceptible individuals. This systematic review and meta-analysis aimed to synthesize available evidence on the association between vaccination and MG, evaluating both vaccine effectiveness and safety in this population. METHODS: Observational studies in cohort or case-control formats were identified through

Early versus late add-on therapy in generalized myasthenia gravis: a multicenter real-world cohort study.

BACKGROUND: This study examined whether the timing of targeted add-on therapy initiation influences clinical outcomes in acetylcholine receptor (AChR) antibody-positive generalized myasthenia gravis (gMG), based on the hypothesis that earlier escalation may improve treatment response by intervening before structural or immunological consolidation occurs. METHODS: In this multicenter, retrospective real-world cohort study, 153 patients with AChR antibody-positive gMG were included from eight German tertiary centers. All received either complement C5 inhibitors (eculizumab, ravulizumab) or the

The Relationship Between Immune Semaphorins and Myasthenia Gravis.

OBJECTIVE: Myasthenia gravis (MG) is an autoimmune disease involving several immune mechanisms. Recently, semaphorins have emerged as potential diagnostic and prognostic biomarkers in autoimmune neurological and non-neurological diseases. This study investigated the role of immune semaphorins, namely semaphorins 3A, 3F, 4A, 4D, and 7A, in the diagnosis and prognosis of MG and their potential as biomarkers. METHODS: Serum levels of semaphorin 3A, 3F, 4A, 4D, and 7A were compared between 41 patients with MG and 39 healthy controls. Patients were grouped according to the Myasthenia Gravis Activi

Triple M syndrome (myasthenia gravis, myocarditis and myositis) following atezolizumab in small cell lung carcinoma.

An older adult in his early 80s with locally advanced small cell lung carcinoma was treated with carboplatin, etoposide and atezolizumab. After two well-tolerated cycles, he developed acute ocular and neuromuscular symptoms. Investigations revealed features consistent with immune checkpoint inhibitor-related myasthenia gravis, myocarditis and myositis (triple M syndrome). The patient responded to high-dose corticosteroids and pyridostigmine but required prolonged rehabilitation. Notably, he demonstrated an excellent radiological response to systemic therapy. This case highlights the need for v

Zilucoplan is effective for ravulizumab-refractory generalized myasthenia gravis with the C5 p.Arg885His variant: a case report.

Complement inhibition is highly effective for complement-mediated diseases such as paroxysmal nocturnal hemoglobinuria (PNH) and generalized myasthenia gravis (gMG). However, certain C5 variants impair target engagement. In vitro, eculizumab and ravulizumab fail to achieve complete terminal complement blockade, and, in PNH, clinical efficacy is actually compromised in carriers. We report an anti-acetylcholine receptor antibody-positive gMG patient harboring C5 c.2654G>A (p.Arg885His) who exhibited persistent disease activity and only partial suppression of hemolytic complement activity (CH50)

Non-Motor Symptoms in Myasthenia Gravis Patients: Evaluation of Auditory and Olfactory Functions.

INTRODUCTION: Myasthenia gravis (MG) is an autoimmune disease characterized by conduction defects at the neuromuscular junction. Recent studies suggest that impairment in acetylcholine neurotransmission occurs not only at the neuromuscular junction but also in the central and peripheral nervous systems. In this context, we aimed to investigate the presence of auditory and olfactory dysfunction, which are non-motor symptoms (NMS), alongside motor symptoms in patients with MG. METHODS: A total of 30 MG patients and 30 healthy controls were enrolled in the study. Demographic characteristics of a
