[Diagnosis of myasthenia gravis in a patient with polymyositis].

In recent years, there has been an increase in publications on rare cases of combined myasthenia gravis and idiopathic inflammatory myopathies, including polymyositis. Such cases complicate the differential diagnosis and worsen patients' prognosis. This article presents a rare case of a patient with two serious autoimmune diseases: myasthenia gravis and polymyositis. A clinical case of the ocular form of myasthenia gravis in a 60-year-old woman with a long history of polymyositis is presented, with a detailed discussion of diagnostic and treatment stages. Clinically, polymyositis was manifeste

Cloudy or Clear? Readability and Content Analysis of Patient Education Materials in Myasthenia Gravis.

INTRODUCTION/AIMS: Myasthenia gravis (MG) is a chronic autoimmune neuromuscular disorder that requires complex treatment decisions and sustained disease self-management. Health literacy is essential for patient understanding of MG, yet online patient education materials (PEMs) have not been systematically studied. This study aimed to evaluate the readability of MG-related PEMs, assess the inclusion of key topics, and identify characteristics associated with more accessible and comprehensive resources. METHODS: We conducted a cross-sectional analysis of MG-related PEMs identified through Googl

Experience Using Efgartigimod to Treat Juvenile Myasthenia Gravis in China: A Multicenter Retrospective Study.

INTRODUCTION/AIMS: Current therapeutic management of juvenile myasthenia gravis (JMG) predominantly relies on conventional immunosuppressive therapies and expert consensus extrapolated from adult data, creating a critical gap in high-quality, pediatric-specific clinical evidence. The aim of this study was to assess the efficacy and safety of efgartigimod in the pediatric population. METHODS: This multicenter retrospective study enrolled JMG patients (under 18 years), from 12 Chinese centers, who received at least one dose of efgartigimod. Efgartigimod was administered as weekly intravenous in

Astragaloside IV ameliorates experimental autoimmune myasthenia gravis through multi-target regulation of immune-microbiota-metabolism network and ferroptosis inhibition.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disorder targeting neuromuscular junction proteins, with rising incidence and limited therapies. Astragaloside IV (AS-IV), a bioactive saponin from Astragalus membranaceus, exhibits immunomodulatory properties, but its mechanisms in MG remain unclear. METHODS: We investigated the therapeutic efficacy and underlying mechanisms of AS-IV in an experimental autoimmune myasthenia gravis (EAMG) rat model using an integrated multi-omics approach, including 16S rRNA sequencing, metabolomics, and network pharmacology. Immune cell subsets (Th1, Th17,

Isolated dysphagia masking myasthenia gravis diagnosis.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction characterized by fluctuating skeletal muscle weakness. We report a 61-year-old woman presenting with progressive dysphagia as the sole initial symptom for 15 months, leading to diagnostic delay. Esophageal manometry suggested ineffective motility, and electroneuromyography was normal, reinforcing a non-neurological hypothesis. During hospitalization for worsening dysphagia, cervical magnetic resonance imaging (MRI) incidentally revealed an anterior mediastinal nodule consistent with thymoma. Pulse corticosteroid the

Juvenile Myasthenia Gravis: An Indian Perspective.

BACKGROUND: Myasthenia Gravis is an autoimmune disorder of the neuromuscular junction which is divided into three categories in children: neonatal or transient, congenital myasthenic syndromes, and autoimmune or juvenile myasthenia gravis (JMG). JMG differs from adult MG in many aspects. Characteristics of JMG also vary between children of various ethnic groups. OBJECTIVE: Although common, there is scant data about JMG in India. This study was performed to determine the characteristics of JMG in Indian children. MATERIALS AND METHODS: We did a retrospective chart analysis of all patients who

Indirect Comparison of Nipocalimab Versus Efgartigimod and Rozanolixizumab in the Treatment of Generalized Myasthenia Gravis.

INTRODUCTION: Nipocalimab, efgartigimod, and rozanolixizumab (the last two cyclically dosed) are approved neonatal Fc receptor (FcRn) blockers for treating generalized myasthenia gravis (gMG). No trials have directly compared these therapies; hence, indirect treatment comparisons (ITCs) were conducted to evaluate their relative efficacy. METHODS: Matching-adjusted indirect comparisons (MAICs) and Bucher ITCs were used to compare nipocalimab vs. efgartigimod and rozanolixizumab for changes from baseline (CFB) in Myasthenia Gravis Activities of Daily Living (MG-ADL) total score observed in thei

Myasthenic Crisis Unmasking Myocardial Infarction with Nonobstructive Coronary Arteries in a Patient with Undiagnosed Myasthenia Gravis.

We present a case of a patient with undiagnosed myasthenia gravis presenting in myasthenic crisis. In this case, the patient presented primarily with myocardial infarction with nonobstructive coronary arteries and respiratory failure and was later diagnosed to be in myasthenic crisis. The myasthenic crisis was treated with intravenous immunoglobulin (IVIg) and corticosteroids. Given the inadequate response to IVIg and glucocorticoids, the patient was subsequently administered an anti-CD20 monoclonal antibody in the form of rituximab. The patient responded well to rituximab, and her cardiac fun

Targeting PGAM5-driven mitochondrial integrated stress response slows ALS progression across subtypes.

Amyotrophic lateral sclerosis (ALS) is genetically and clinically heterogeneous, yet convergent pathogenic mechanisms remain poorly defined. A CRISPR-Cas9 screen identified phosphoglycerate mutase-5 (PGAM5) as a common mediator of ALS pathogenesis. PGAM5 activates the mitochondrial integrated stress response (mtISR) via dephosphorylation of metallopeptidase OMA1 at Ser223 and Ser237, thereby driving neuromuscular junction disruption and motor deficits. We show that PGAM5 is a substrate of valosin-containing protein (VCP) and is consistently elevated in spinal cords from sporadic ALS patients,

Estimating the minimal clinically important difference for the Myasthenia Gravis Quality of Life revised scale (MG-QOL15R).

PURPOSE: To estimate the minimal clinically important difference (MCID) for the MG-QOL15R. METHODS: Data from two multi-country myasthenia gravis studies were used: the ADAPT RCT (n = 157) and the MyRealWorld-MG (MRW-MG) survey (n = 92). MCIDs were estimated using four anchor-based methods: change difference (CD), receiver operating characteristic (ROC) curve with Area Under the Curve (AUC), linear regression, and equipercentile linking. MG-Activities of Daily Living (MG-ADL) was the anchor. MCIDs were applied to the ADAPT RCT to assess the impact of different thresholds. Odds ratios (ORs) and
