From cordoned consultations to cohesive care: A service evaluation of a one-stop, multi-speciality, hospice-based Duchenne muscular dystrophy clinic, UK.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a multimorbid recessive genetic disorder. In 2020, a novel one-stop multi-disciplinary (MDT) DMD clinic was piloted in Jack's Place Hospice (registered charity number 1002832), Winchester, UK. This service evaluation aimed to assess the level of National Neuroscience Advisory Group guideline adherence [NNAG, 2024], and the patient views of the clinic format. METHODS: Ten semi-structured interviews with patients attending the clinic were recorded, with nine subsequently transcribed and analysed using Braun & Clarke's six-stage reflexive thematic

Statistical correlation between strength and mini-dystrophin expression in skeletal muscle after an rAAV9-mini-dystrophin gene therapy candidate in DMDmdx rats.

Duchenne muscular dystrophy (DMD) is an X-linked recessive disorder resulting from loss-of-function mutations in the dystrophin gene, leading to progressive muscle wasting and premature death. We present the mini-dystrophin protein expression-function relationship from a post-hoc analysis of mini-dystrophin quantification and function (strength) data at 3 and 6 months in biceps femoris and pectoralis muscles from a DMD rat model (DMDmdx) treated with fordadistrogene movaparvovec, an adeno-associated virus serotype 9 gene therapy containing miniaturised dystrophin. Four treatment groups of ~7-9

Decoding common and complex genotypes of facioscapulohumeral muscular dystrophy using methylation profiling and long-read sequencing.

Due to the complexity of the genomic region, the relationship between genotype, phenotype and epigenetics in facioscapulohumeral muscular dystrophy remains incompletely understood, and diagnostics are limited in precision. In this study, we applied Oxford Nanopore Technology long-read sequencing to characterize FSHD1 and FSHD2 patients with common and previously confirmed genotypes (n = 14). This approach enabled the simultaneous determination of allele-specific methylation profiles, haplotypes and repeat lengths. Our results confirm the presence of an asymmetric methylation gradient increasin

Machine Learning Integrating Quantitative MRI and Clinical Features Predicts Loss of Ambulation in Duchenne Muscular Dystrophy.

BACKGROUND: Loss of ambulation (LOA) is a key milestone in Duchenne muscular dystrophy (DMD) with highly variable timing, complicating clinical counselling and trial design. Quantitative MRI non-invasively assesses muscle fat infiltration, yet few models have integrated MRI with clinical variables for individualized LOA prediction. We aimed to develop and validate a machine-learning model integrating quantitative MRI and clinical features to predict time to LOA and to derive interpretable, individualized risk estimates and candidate thresholds. METHODS: This prospective study enrolled 188 boy

Molecular Hybrids of Deflazacort and H2S Donors: From Bench to Potential Treatment for Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is an X-linked neuromuscular disorder characterized by oxidative stress, chronic inflammation, and skeletal muscle degeneration. Although deflazacort is the standard treatment, its long-term use is associated with significant adverse effects. Since impaired hydrogen sulfide (H2S) signaling contributes to DMD pathogenesis, we designed five novel deflazacort-H2S donor hybrids (I-V) through a molecular hybridization strategy combining anti-inflammatory efficacy with controlled sulfide release. All hybrids showed chemical stability under physiological and acidic c

Myotonic dystrophy type 1 as a cancer predisposition syndrome: A case report of quadruple primary malignancies with review of the literature.

BACKGROUND: Myotonic dystrophy type 1 (DM1) is the most common adult-onset muscular dystrophy and is increasingly recognized as a cancer predisposition syndrome, with 2- to 8.5-fold elevated risks of endometrial, ovarian, thyroid, and colorectal cancers. Evidence on the oncological management of DM1 patients-including perioperative care, chemotherapy tolerability, and cancer surveillance-remains scarce. CASE PRESENTATION: A 56-year-old woman with DM1 developed four primary malignancies over 16 years: papillary thyroid carcinoma at age 40, breast carcinoma at age 49, and synchronous endometria

Progress on Duchenne muscular dystrophy: from molecular mechanisms to therapeutic strategies.

Duchenne muscular dystrophy (DMD) is the most common lethal neuromuscular genetic disorder in children. It is caused by mutations in the DMD gene that abolish the expression of dystrophin, a protein essential for maintaining sarcolemmal integrity during muscle contraction. Its loss renders muscle fibers susceptible to contraction-induced injury, leading to progressive muscle degeneration and, ultimately, fatal cardiac and respiratory failure. In this review, we summarize the diagnostic framework, molecular pathogenesis, and current therapeutic strategies of DMD, with particular emphasis on the

Variable Phenotypic Manifestations of Novel Compound Heterozygous CRPPA Variants in Two Iranian Siblings With Limb-Girdle Muscular Dystrophy.

BACKGROUND: Dystroglycanopathies are a clinically and genetically heterogeneous group of muscular dystrophies caused by impaired glycosylation of α-dystroglycan, with variants in multiple genes, including CRPPA, which is associated with a broad spectrum ranging from congenital muscular dystrophy to limb-girdle muscular dystrophy. METHOD: The proband underwent whole exome sequencing, while Sanger sequencing was used for segregation analysis and oligonucleotide array-CGH was performed to confirm the copy number variant. RESULTS: We report two siblings harboring an identical novel compound hete

SMCHD1 loss rewires MYOD1 enhancer nexuses and chromatin accessibility landscapes in muscle cells.

SMCHD1 is a chromatin architectural protein linked to heterochromatin repression. Loss of function mutations of SMCHD1 cause facioscapulohumeral muscular dystrophy type 2 (FSHD2) through activation of the DUX4 transcription factor gene. However, it is unknown how SMCHD1 may regulate transcription independently of DUX4. Here, we show that SMCHD1 safeguards enhancer organization within the three-dimensional (3D) genome in human myoblasts. Loss of SMCHD1 leads to widespread gains in chromatin accessibility, aberrant transcription, and selective remodeling of myogenic transcription factor MYOD1 oc

Antimitochondrial antibodies in rheumatic diseases: beyond primary biliary cholangitis.

Antimitochondrial antibodies (AMAs) targeting the E2 subunit of the pyruvate dehydrogenase complex are the serological hallmark of primary biliary cholangitis (PBC), but may also occur across systemic autoimmune rheumatic diseases. This narrative review integrates clinical and experimental evidence on AMAs in rheumatic diseases, delineating disease-specific patterns and clinical implications beyond the hepatobiliary context. AMAs and other mitochondrial autoantibodies have been reported in systemic sclerosis, systemic lupus erythematosus, rheumatoid arthritis, Sjögren disease, and inflammatory

Hyperglycemia-induced metabolic rewiring is associated with skeletal muscle loss in a fructose-streptozotocin-induced diabetic rat model.

AIMS: This study investigated the impact of hyperglycemia on systemic and skeletal muscle metabolite profiles. METHODS: Serum and gastrocnemius metabolites of fructose-fed, streptozotocin-injected rats (T2D) were analyzed by 1H NMR and related to markers of energy metabolism and mitochondrial remodeling. RESULTS: T2D rats exhibited hyperglycemia and impaired insulin secretion without obesity or dyslipidemia. Hyperglycemia induced a generalized catabolic state, reflected by reduced body weight and relative gastrocnemius mass. T2D rats presented increased phosphorylated AMP-activated protein k

[Duchenne muscular dystrophy-associated cardiomyopathy: advances in diagnosis and management].

Duchenne muscular dystrophy (DMD) is an X-linked recessive neuromuscular disorder in which cardiac involvement is relatively insidious, and heart failure has become one of the major determinants of prognosis. Cardiac magnetic resonance imaging and speckle-tracking echocardiography provide a basis for the precise assessment of DMD-associated cardiomyopathy. At present, glucocorticoid therapy and cardiovascular drug therapy are the basic treatment approaches, and gene therapy, cell therapy, and molecularly targeted drugs are continually evolving. This review summarizes pathogenic mechanisms, dia

A Recurrent 3'UTR Variant in CAPN3 Highlights the Diagnostic Impact of Non-Coding Variants in Limb-Girdle Muscular Dystrophies.

OBJECTIVE: A substantial proportion of individuals with suspected genetic conditions, including neuromuscular disorders, lack a definitive molecular diagnosis. Non-coding variants, particularly those in the 3' untranslated region (3´UTR), which regulate mRNA stability, translation, and localization, may underlie part of this gap. We investigated the clinical, genetic, and functional relevance of a novel 3'UTR variant in CAPN3 in individuals with persistent hyperCKemia, most of whom also presented with proximal muscle weakness. METHODS: Clinical assessment, including muscle biopsy and muscle m

A late-onset slowly progressive dystrophic-myasthenic phenotype expands the clinical spectrum of TOR1AIP1-related disease.

The TOR1AIP1 gene encodes the nuclear envelope protein LAP1; pathogenic variants cause distinct phenotypes, from muscular dystrophy to multisystem involvement. This recessive disease presents from in utero to late adulthood. Survival depends on phenotype. A 63-year-old woman first presented at 48 years of age with progressive proximal lower limb weakness since age 28, recently involving the upper limbs. There was no consanguinity, but familial gait impairment was reported. She had lumbar hyperlordosis, proximal tetraparesis, and waddling gait. CK was normal and neurophysiological studies revea

Laminin-α2 deficiency is associated with altered multiorgan redox signatures.

Muscle diseases often involve redox imbalance, driven by elevated ROS levels, impaired antioxidant defenses, mitochondrial dysfunction, and metabolic rewiring. In several muscular dystrophies caused by extracellular matrix gene mutations, increased oxidative stress in skeletal muscle is a defining feature. One such condition is LAMA2-congenital muscular dystrophy (LAMA2-CMD), resulting from mutations in LAMA2, which encodes the laminin-α2 chain. Although oxidative stress has been implicated in the muscle pathology in LAMA2-CMD, whether this effect extends beyond skeletal muscle remains unclear

Brain imaging in dystrophinopathies.

Duchenne and Becker muscular dystrophies (DMD, BMD) are caused by genetic variants in the DMD gene coding dystrophin proteins. In addition to the impairment of muscular function, these patients also present frequent cognitive and neuropsychiatric disorders due to impaired expression of several brain dystrophins: Dp427, Dp140 and Dp71. Cerebral imaging has shed new light on the impact of dystrophinopathies on brain structure, function, and connectivity. We review the brain imaging results obtained in patients and divers animal models, showing presence of mild cortical atrophy, slight modificati

Beyond Ocular Symptoms: Chronic Progressive External Ophthalmoplegia With Coexisting Facial Palsy and T-wave Inversion.

Chronic progressive external ophthalmoplegia (CPEO) exists on a wide phenotypic continuum. The systemic propagation of cellular metabolic impairment frequently precipitates a heterogeneous spectrum of neurogenic, myopathic, and cardiovascular manifestations, clinically designated as CPEO-plus. Beyond the highly susceptible extraocular muscles, other metabolically intensive muscular tissues remain vulnerable to compromised function. Diagnosing CPEO remains challenging because its ocular signs overlap significantly with other conditions. Furthermore, CPEO-plus must be distinguished from Kearns-S

Evolution of daytime PaCO2 in patients with neuromuscular disorders and OSA using CPAP: a retrospective study.

BACKGROUND: Neuromuscular disorders (NMD) are frequently complicated by obstructive sleep apnea (OSA), which may add an obstructive ventilatory load and contribute to CO2 retention. While non-invasive ventilation (NIV) is recommended for established hypoventilation, the impact of continuous positive airway pressure (CPAP) on daytime PaCO2 in NMD remains poorly documented. METHODS: We conducted a retrospective single-center cohort study in a tertiary neuromuscular referral center. Consecutive adults with NMD undergoing respiratory assessment were included if they had (i) OSA treated with CPAP
