Unclassified COL6A1 Variant in a Child With Congenital Hypotonia: A Case Report.

We report a 3-year-old male with congenital hypotonia, proximal contractures, and distal hyperlaxity. Exome sequencing identified a novel homozygous COL6A1 variant (c.2821C>T), classified as a variant of uncertain significance. The distinct phenotype strongly supports Ullrich congenital muscular dystrophy, expanding the gene's mutational spectrum in consanguineous populations.

Postmortem Presentation and Review of Danon Disease: A Rare Metabolic Disorder.

Danon disease is a rare, X-linked dominant inherited lysosomal storage disorder that involves a mutation in the lysosomal-associated membrane Protein 2 gene. Mutation in this gene results in severe disease clinical manifestations that may include skeletal and cardiac myopathies, retinopathies, and intellectual disabilities. This case report explores postmortem findings observed in a 25-year-old male donor who succumbed to heart failure secondary to Danon disease. With no transplant history, postmortem findings include hypertrophy, extensive fibrosis, and vacuolization in cardiomyocytes compare

Visual feedback increases expiratory muscle training effort in Parkinson's disease: a randomised crossover trial.

BACKGROUND: Disorders of airway protection, including impaired cough and swallowing, are clinically important complications of Parkinson's disease. Expiratory muscle strength training (EMST) is an established non-pharmacological intervention targeting these deficits. The aim of this study was to evaluate the acute effect of visual feedback provided by the SpiroGym mHealth app on expiratory muscle activation during EMST in people with Parkinson's disease. METHODS: We conducted a single-center, randomised, two-period crossover trial in people with Parkinson's disease. Participants performed EMS

Development of Italian centile curves for the North Star Ambulatory Assessment and timed items.

Centile curves may improve the interpretation of motor outcomes in specific populations. This study aimed to develop Italian age-specific centile curves for NSAA, rise-from-floor velocity (RFFV), and 10-meter walk/run velocity (10MWRV) in boys with Duchenne muscular dystrophy (DMD) and to assess their agreement with existing UK reference curves. Data from 627 patients (5080 assessments) with an age between 3-18 years were extracted from the Italian DMD Registry. GAMLSS models were used to estimate centile curves. Agreement with UK data (restricted to ages 5-16 years and who are actively on ste

Nationwide Implementation of Newborn Screening for Spinal Muscular Atrophy in Canada.

BACKGROUND AND OBJECTIVES: Newborn screening (NBS) for spinal muscular atrophy (SMA) and reimbursement of disease-modifying therapies (DMTs) are managed provincially in Canada, creating inequities in diagnosis and treatment. We describe NBS methodology, reporting practices, and DMT reimbursement across Canada and report the first national birth prevalence of SMA. METHODS: We conducted a cross-sectional survey of laboratory directors from each provincial NBS program, validated against publicly available government sources (censored December 31, 2025). Birth prevalence was calculated using a Po

Genetic evaluation in living liver donors: beyond individual risk to system responsibility.

PURPOSE OF REVIEW: Living donor liver transplantation (LDLT) increasingly depends on biologically related donors, particularly in Asia and the Middle East, where deceased donation remains limited. This review evaluates genetic risks in living donors and proposes a system-based approach to donor selection. RECENT FINDINGS: Heterozygous carriers exhibit variable functional capacity determined by enzyme thresholds and epigenetic mechanisms such as X-chromosome lyonization. In ornithine transcarbamylase deficiency (OTCD), maternal donors may have insufficient enzymatic activity, resulting in dono

Prospective Cardiac MRI study comparing Recovered COVID-19 patients with Recovered Non-COVID-19 Viral Respiratory Infection Patients (R-COVID-CMR).

OBJECTIVE: Coronavirus disease 2019 (COVID-19) has been associated with subclinical myocardial inflammation. Post-COVID-19 infection, there are concerns about subsequent heart failure and persistent myocarditis. It is unknown how COVID-19 infection compares with non-COVID-19 viral infections in terms of myocardial inflammation based on cardiovascular magnetic resonance (CMR) assessment. This prospective longitudinal study aims to determine and compare the extent and severity of myocardial involvement in patients with COVID-19 infection versus non-COVID-19 viral respiratory infections (control)

Chronic Limb-Threatening Ischemia Shows No Adaptability of Mitochondrial Function After Revascularisation.

Peripheral arterial disease (PAD) impairs limb perfusion, leading to myopathy characterised by altered mitochondrial content and respiration. Because ischemic muscle appears to adapt differently across PAD stages, we examined stage-specific skeletal-muscle mitochondrial function before and after successful revascularisation. Muscle biopsies of gastrocnemius and vastus lateralis muscle were obtained from 32 patients-21 with intermittent claudication (IC) and 11 with chronic limb-threatening ischemia (CLTI)-before and 6 weeks after successful revascularisation. Mitochondrial respiration was asse

Foot drop secondary to lumbar degenerative disease: a scoping review of recovery patterns, predictors, and research gaps.

BACKGROUND: Foot drop (FD) secondary to lumbar degenerative disease is common and disabling, but reported recovery rates and predictors after surgical decompression vary widely, and prior reviews have not clarified how much of this variability reflects true clinical heterogeneity versus inconsistent methodology. OBJECTIVE: To map the available evidence on recovery from FD following lumbar decompression, characterize methodological and definitional heterogeneity, synthesize reported predictors and recovery timelines, and identify gaps to guide future research. METHODS: A PubMed search using M

Donor-intrinsic regenerative potential outweighs histocompatibility in iPSC-derived skeletal muscle engraftment in nonhuman primates.

Cell-based regenerative therapies represent a promising platform for muscular dystrophies, but the role of histocompatibility for muscle engraftment remains poorly defined. Given their close genetic relationship to humans, nonhuman primate (NHP) models provide an ideal system to investigate this question. Here, we generated myogenic progenitors from induced pluripotent stem cell (iPSC) lines derived from five major histocompatibility complex (MHC)-typed Mauritian-origin cynomolgus macaques (MCMs). Following extensive characterization, one iPSC clone from each donor was selected for transplanta

Ketogenic Dietary Therapies and β-Hydroxybutyrate as Modulators of Mitochondrial Function: From Molecular Mechanisms to Clinical Translation.

Ketogenic dietary therapies (KDTs) have been used for a century in drug-resistant epilepsy, and β-hydroxybutyrate (BHB), the major ketone body, is recognized as a signaling metabolite. This review examines how KDTs and BHB modulate mitochondrial function and evaluates evidence supporting these mechanisms. BHB is oxidized through D-β-hydroxybutyrate dehydrogenase 1 and succinyl-CoA:3-oxoacid-CoA transferase and alters redox balance and oxidative phosphorylation. It also inhibits class I histone deacetylases, serves as the substrate for lysine β-hydroxybutyrylation, activates hydroxycarboxylic a

Image-based assessment of muscle contractile performance in facioscapulohumeral muscular dystrophy: protocol of the MUSCLE+ observational study in the Netherlands.

INTRODUCTION: Facioscapulohumeral muscular dystrophy (FSHD) is a hereditary muscular dystrophy with a highly variable and slowly progressing disease course. This variable natural history complicates the identification of biomarkers of disease progression and treatment effects, while there is a growing need for measures which can capture changes within a trial timeframe. METHODS AND ANALYSIS: This study presents the protocol of the MRI UltraSound Comparison Load Evaluation study, a prospective observational muscle imaging biomarker study where Dutch adult patients with FSHD type 1 or type 2 (n

Mitochondrial Myopathy in the Oculoplastic Practice.

Ptosis is a common ocular complaint in the elderly population. While most cases are often due to involutional changes in the eyelid, neurogenic, mechanical or myogenic diseases should remain as important differential diagnoses. We report a case of a 75-year-old Chinese man who presented with chronic progressive ptosis and was initially managed as presumed seronegative ocular myasthenia gravis (MG). Despite positive bedside findings suggestive of MG, serological and electrophysiological investigations were inconclusive, and there was no clinical response to pyridostigmine. The diagnosis was ach

Physiological Reference Mapping Reveals Context-Dependent Dysregulation of Regenerative Programs in Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) involves chronic degeneration and regeneration, causing dystrophic muscle stem cells (MuSCs) to differ from uninjured wild-type tissue. Conventional disease-versus-resting-control comparisons may therefore conflate dysregulation with regeneration. We developed a physiological reference-mapping framework integrating normal regeneration trajectories with replicated dystrophic datasets to estimate disease-associated deviations relative to regenerative state and spatial context. We applied it across single-cell/single-nucleus, bulk, and spatial RNA-sequencing data
