Recurrent Lower Limb Venous Thrombosis in a Young Adult with Myotonia Congenita.

A 20-year-old male with myotonia congenita presented with recurrent episodes of lower limb deep vein thrombosis. Initial genetic testing confirmed a homozygous mutation in the CLCN1 gene. Although rare, a possible link between congenital myotonia and venous thromboembolism (VTE) is explored in this case. Triple-positive antiphospholipid antibodies confirmed a diagnosis of thrombotic antiphospholipid syndrome, necessitating indefinite anticoagulation. Literature review highlights RNA toxicity, mis-splicing, and coagulation activation in myotonic disorders. This is the first known case correlati

Myotonia: Recognition, Evaluation, and Differential Diagnosis.

Myotonia is a non-neurotypical muscle physiology of sarcolemmal hyperexcitability due to alterations in the structure and/or function of ion channels in the muscle cell membrane. This hyperexcitability can be observed electrically as spontaneous myotonic discharges during needle electromyography. Myotonic discharges consist of continuous sequential fibrillation or positive sharp wave morphology potentials which exhibit unstable, gradually changing, firing frequencies and amplitudes. In myotonic disorders, a clinical correlate of muscle stiffness is often present. Myotonic disorders are broadly

Muscle Stiffness due to Neuromuscular Hyperexcitability.

Neuromuscular hyperexcitability disorders often manifest as muscle stiffness, spasms, or cramps. These uncommon disorders may be due to central or peripheral nervous system dysfunctions and be either subacute in onset or chronic. Their diagnosis requires a high index of suspicion and is often delayed. Stiff-person syndrome, the prototype of central hyperexcitability disorders, is an immune-mediated disorder that is often associated with high titer glutamic acid decarboxylase antibodies and is amenable to symptomatic treatment and immunotherapy. Peripheral nerve hyperexcitability syndromes, inc

Treatment updates in myotonic disorders.

Myotonia is delayed muscle relaxation after forceful contraction. It is due to hyperexcitability of the skeletal muscle membrane. It can arise from primary skeletal muscle ion channel dysfunction, involving chloride or sodium channels, but is also a prominent clinical feature in myotonic dystrophies where altered RNA splicing leads to secondary ion channel dysregulation amongst other systemic manifestations. Clinically, myotonia can range from delayed eye opening to a disabling symptom causing impaired mobility, functional difficulty and sometimes pain. It can also be a "hidden disability" wit

Myotonic Muscle Activity Drives a Persistent Pain State Responsive to NaV1.8-directed Analgesics.

Pain is a common and disabling feature of myotonic disorders, yet its biological basis remains poorly understood and no targeted analgesic therapies currently exist. Here, we demonstrate that skeletal muscle hyperexcitability is sufficient to initiate a persistent pain state independent of inflammation, nerve injury, or overt tissue damage. Using complementary pharmacological and genetic models of myotonia resulting from loss of the voltage-gated skeletal muscle chloride channel ClC-1 function, we show that transient and chronic myotonia produce robust mechanical, thermal, and cold hypersensit

Population-scale repeat expansions elucidate disease risk and brain atrophy.

Pathogenic expansions of short tandem repeats (STRs) cause over 70 neurological diseases1-3. Here we performed a population-scale survey of pathogenic repeat expansions by analysing repeat length in 37 disease-associated STR loci in a diverse set of 1,020,833 samples using short-read sequencing whole-exome and whole-genome data. Consistent with previous findings, we found that the frequency of pathogenic repeats is higher than the prevalence of corresponding diseases for most loci4,5. Associations of repeat length with 7,671 binary traits captured known locus-trait associations, including HTT

Mouse models of non-dystrophic and dystrophic myotonia exhibit nociplastic pain-like behaviors.

Pain is a prominent and debilitating symptom in myotonic disorders, including myotonia congenita and myotonic dystrophy type 1 (DM1). Although patients frequently report chronic pain, its underlying mechanisms remain poorly defined. In both disorders, impaired chloride conductance through voltage-gated CLC-1 chloride channels in skeletal muscle disrupts membrane repolarization, leading to delayed relaxation and persistent muscle hyperexcitability. Here, we investigated the pathophysiology of pain in mouse models of acute and chronic myotonia. In the acute model, a single intraperitoneal inject

Disorders of Muscle Mass and Tone.

Muscle disease has various clinical manifestations that range from exertional and non-exertional rhabdomyolysis, fasciculations, weakness, rigidity, stiffness, gait abnormalities, poor performance, and alterations in muscle mass and tone. Neurogenic disorders and non-neurogenic disorders such as primary muscle disease can cause muscle atrophy and changes in muscle tone. Myotonic disorders can have a genetic (eg, inherited channelopathies) or acquired (eg, electrolyte derangements) origin. Normal muscle enzyme activities do not rule out a myopathic disorder as the underlying cause of muscle atr

Hereditary, non HINT1 related, axonal neuropathy with neuromyotonia.

To date, neuromyotonia in the context of an inherited axonal neuropathy has been linked to autosomal recessive mutations in the histidine triad nucleotide binding protein 1 (HINT1) gene. In this study we describe two unrelated male patients with late-onset, predominantly motor, axonal neuropathy with neuromyotonia, who carried an autosomal dominant c.103G > A mutation in the myelin protein zero (MPZ) gene (NM_000530.8:c.103G > A, p.Asp35Asn), identified by whole-exome sequence analysis (WES).  CASE DESCRIPTIONS: The first patient presented progressive leg muscle weakness and stiffness with dif

Pearls & Oy-sters: Severe Myotonic Crisis Resembling Malignant Hyperthermia.

Patients with myotonic disorders are at risk for severe generalized muscle contraction, referred to as a "myotonic crisis." For those patients with nondystrophic myotonia (NDM), the most common trigger of a myotonic crisis is exposure to succinylcholine. In this case, a 10-year-old female patient with NDM secondary to an SCN4A pathogenic variant developed a severe myotonic crisis in the setting of an upper respiratory infection and asthma flare treated with a beta-adrenergic agonist (ß-agonist). She presented with generalized rigidity and features of hypermetabolism resembling an anesthetic-re

Painful muscle stiffness with markedly elevated serum creatine kinase (CK) levels after twenty weeks of gestation in four patients with myotonic dystrophy type 1 (DM1) and a patient with paramyotonia congenita (PMC).

Four patients with myotonic dystrophy type 1 (DM1) and a patient with paramyotonia congenita (PMC) developed devastating painful muscle stiffness with markedly elevated serum creatine kinase (CK) levels after 20 weeks of gestation. Immediately after delivery, painful muscle stiffness completely disappeared and the CK levels returned to the baseline. In a patient with DM1, muscle biopsy at delivery and skeletal muscle MRI in six days after delivery showed inflammatory changes, which disappeared in MRI on postpartum day 41. Pregnancy-associated aggravation of myotonia has been reported in myoton

Drug treatment for myotonia.

BACKGROUND: Abnormal delayed relaxation of skeletal muscles, known as myotonia, can cause disability in myotonic disorders. The main myotonic disorders are non-dystrophic myotonia and myotonic dystrophy. Non-dystrophic myotonia is a genetic muscle channelopathy predominantly causing myotonia. Myotonic dystrophic is a more systemic neuromuscular disorder causing myotonia as well as progressive myopathy and systemic manifestations, such as arrhythmias and cataracts. Myotonia manifests as stiffness, cramps, locking, pain, and fatigue, and can cause marked morbidity and disability. Sodium channel

Novel SCN4A Variants Associated With Myalgic Myotonic Disorder or Paramyotonia.

BACKGROUND: This study aimed to determine the role of five new rare SCN4A variants suspected to cause paramyotonia or myotonic disorder. METHODS: Ten patients from seven families underwent clinical, neurophysiological, imaging, and muscle biopsy examinations. Genetic studies were performed with targeted sequencing of all known myopathy genes. Functional changes resulting from these variants were studied with HEK293T cells, by using a whole-cell patch clamp. RESULTS: Five SCN4A variants were identified: c.662 T > C p.(F221S), c.2143G > A p.(A715T), c.4352G > A p.(R1451H), c.3610 A > G p.(N120

High-dose flecainide for symptomatic relief in paramyotonia congenita/severe neonatal episodic laryngospasm due to SCN4A G1306E: a case report.

BACKGROUND: Severe neonatal episodic laryngospasm has been previously reported in multiple patients with the heterozygous pathogenic variant G1306E in SCN4A. Treatment can be difficult due to side effects from therapies utilized conventionally for the management of myotonia and paramyotonia congenita. CASE PRESENTATION: We report on two female siblings of Irish, Scandinavian, and German ethnicity aged 7 and 3 years with severe neonatal episodic laryngospasm and paramyotonia congenita due to a paternally inherited heterozygous pathogenic variant of the SCN4A gene and the use of high-dose fleca

Biophysical and structural insights into the SCN4A E452K variant linked to myotonia and paramyotonia congenita.

Myotonia and paramyotonia congenita (PC) are rare neuromuscular disorders characterized by muscle stiffness that intensifies in cold environments. These disorders are associated with variants in the SCN4A gene, that encodes the alpha subunit of the voltage-gated sodium channel Nav1.4. We report here the case of a 36-year-old female who experiences diverse neurological symptoms, including myotonia, cold induced myotonia, resulting in muscle stiffness, and tightness. A whole exome sequencing revealed a missense variant in the SCN4A gene at position c.1354G > A, named p.E452K. We characterized th

Myotonic Dystrophy.

OBJECTIVE: This article reviews the genetic basis, pathogenic mechanisms, epidemiology, clinical presentation, multiorgan involvement, and multidisciplinary management of myotonic dystrophy type 1 (DM1) and type 2 (DM2), as well as the differential diagnosis of myotonic disorders (DM versus nondystrophic myotonic disorders) and electrical myotonia. LATEST DEVELOPMENTS: Due to underdiagnosis, the prevalence of DM is likely higher than currently recognized. Patients with late-onset or mild phenotypes with little or no skeletal muscle involvement may never be evaluated by a neurologist and thus

Real-world evidence supporting orphan drugs approvals for rare neuromuscular disorders in the European Union and the United States: Review of public assessment reports (2015-2025).

Real-world data (RWD) and real-world evidence (RWE) are becoming essential complements to conventional clinical trials for drug assessment, particularly for rare neuromuscular disorders where small patient populations heighten outcome uncertainty. Robust RWE could reduce that uncertainty and inform regulatory decisions on orphan drugs (ODs). To review how the European Medicines Agency (EMA) and the US Food and Drug Administration (FDA) have incorporated RWE into OD approvals for neuromuscular diseases from January 2015 to January 2025. We reviewed all publicly available EMA European Public Ass

Verapamil mitigates chloride and calcium bi-channelopathy in a myotonic dystrophy mouse model.

Myotonic dystrophy type 1 (DM1) involves misregulated alternative splicing for specific genes. We used exon or nucleotide deletion to mimic altered splicing of genes central to muscle excitation-contraction coupling in mice. Mice with forced skipping of exon 29 in the CaV1.1 calcium channel combined with loss of ClC-1 chloride channel function displayed markedly reduced lifespan, whereas other combinations of splicing mimics did not affect survival. The Ca2+/Cl- bi-channelopathy mice exhibited myotonia, weakness, and impairment of mobility and respiration. Chronic administration of the calcium

Contractile properties and magnetic resonance imaging-assessed fat replacement of muscles in myotonia congenita.

BACKGROUND AND PURPOSE: Myotonia congenita (MC) is a muscle channelopathy in which pathogenic variants in a key sarcolemmal chloride channel Gene (CLCN1) cause myotonia. This study used muscle magnetic resonance imaging (MRI) to quantify contractile properties and fat replacement of muscles in a Danish cohort of MC patients. METHODS: Individuals with the Thomsen (dominant) and Becker (recessive) variants of MC were studied. Isometric muscle strength, whole-body MRI, and clinical data were collected. The degree of muscle fat replacement of thigh, calf, and forearm muscles was quantitively calc
