Chlamydia trachomatis genomes from rectal samples: description of a new clade comprising ompA-genotype L4 from Argentina.

Whole-genome analysis has provided insights into the evolution of Chlamydia trachomatis and, recently, into circulating strains that cause lymphogranuloma venereum (LGV). A large LGV outbreak of a new ompA-genotype, L2b, was first reported in Europe in the early 2000s, primarily affecting men who have sex with men (MSM), and then expanded globally. More recent work shows that this outbreak is diversifying into variants of described ompA-genotypes, with the same L2b genomic backbone. This study extends the investigation of LGV cases to Argentina and Finland. In 2017, an LGV outbreak was describ

Surveying genetic markers of antibiotic resistance and genomic background in Chlamydia trachomatis: insights from a multiplex NGS-based approach in clinical strains from Portugal.

OBJECTIVES: To survey genetic markers of potential antimicrobial resistance (AMR) to macrolides and fluoroquinolones among Chlamydia trachomatis-positive samples from the collection of the Portuguese National Reference Laboratory for Sexually Transmitted Infections (STIs), and explore a multiplex PCR approach coupled with NGS to provide complementary information regarding a strain's genomic backbone. METHODS: A total of 502 C. trachomatis-positive samples, mostly anorectal exudates, were subjected to PCR and sequencing of five targets, including loci potentially driving AMR (23S rRNA, gyrA an

Chlamydia trachomatis: a model for intracellular bacterial parasitism.

Chlamydia comprises a diverse group of obligate intracellular bacteria that cause infections in animals, including humans. These organisms share fascinating biology, including distinct developmental stages, non-canonical cell surface structures, and adaptations to intracellular parasitism. Chlamydia trachomatis is of particular interest due to its significant clinical importance, causing both ocular and sexually transmitted infections. The strain L2/434/Bu, responsible for lymphogranuloma venereum, is the most common strain used to study chlamydial molecular and cell biology because it grows r

Rectal Lymphogranuloma Venereum Mass Mimicking Colorectal Cancer.

Chlamydia trachomatis-lymphogranuloma venereum (LGV) is a sexually transmitted infection (STI) and an uncommon cause of proctitis and rectal mass. When the rectum is affected, patients may experience symptoms such as rectal bleeding, pain, and tenesmus, which can resemble proctitis or proctocolitis and may be mistaken for malignancy. We present the case of a 40-year-old male patient with well-controlled HIV, who initially exhibited symptoms and imaging findings concerning for rectal neoplasia. Following colonoscopy with biopsy and a rectal swab polymerase chain reaction (PCR), Chlamydia tracho

Lymphogranuloma venereum proctosigmoiditis misdiagnosed as inflammatory bowel disease: A case report.

BACKGROUND: There has been a rise in the number of cases diagnosed as lymphogranuloma venereum (LGV), caused by the transmission of Chlamydia trachomatis, specifically serotypes L1, L2, and L3, mostly in men who have sex with men (MSM). LGV can clinically manifest as rectal discomfort, bleeding, and mucoid discharge mimicking inflammatory bowel disease (IBD). Therefore, the role of a wholesome collection of patient history along with gaining patient trust and ease in reporting different elements of their life- whether social or sexual history-could greatly guide physician's investigations lead

Generating Whole Bacterial Genomes from Clinical Samples using a Target Enrichment Workflow.

Obtaining complete genomes of bacterial sexually transmitted infections (STIs) directly from clinical samples is challenging due to the presence of human DNA, microbiota, and very low bacterial pathogen loads. Culture is often not an option, as these species are fastidious, and most samples are taken in transport media that lyse microorganisms, rendering them non-viable for culture. To address these issues, we used a probe panel designed across four species to generate whole-genome sequences of bacterial STIs and performed target enrichment, a two- to three-day hybridization procedure prior to

Lymphogranuloma venereum, an invasive and destructive sexually transmitted infection with many faces.

Lymphogranuloma venereum (LGV) is a serious, invasive, ulcerative sexually transmitted infection caused by Chlamydia trachomatis serovar L1, L2, and L3. Until 2003, we saw LGV among the general population of low- and middle-income countries. Since 2003, we have also seen LGV in high-income countries but exclusively in men who have sex with men. The number of patients with LGV has increased in the last decade, and the proportion in men who have sex with men without HIV has increased dramatically. In low- and middle-income countries, LGV affects women and men and often causes genital complaints.

No Histopathological Evidence of Inflammation Despite Molecular Detection of Schistosoma spp. and Sexually Transmitted Pathogens in Placental Parenchyma Specimens with Limited Membrane Sampling from West African Women with Uncomplicated Pregnancies.

BACKGROUND: Placental infections caused by Schistosoma spp. and sexually transmitted microorganisms can adversely impact pregnancy outcomes. However, the association between molecular detection of these pathogens in placental tissue and corresponding histopathological inflammation remains unclear, particularly in sub-Saharan African populations. METHODS: In this cross-sectional study, placental parenchyma specimens with limited membrane sampling were collected from 103 Ivorian and Ghanaian mothers without known pregnancy or birth complications. Effect of decreasing the frequency of screening for Neisseria gonorrhoeae and Chlamydia trachomatis on the incidence of these infections and antimicrobial use among men who have sex with men using HIV PrEP in Belgium: a retrospective cohort study.

BACKGROUND: There is growing evidence that the benefits of systematic screening for Neisseria gonorrhoeae and Chlamydia trachomatis among men who have sex with men (MSM) using HIV pre-exposure prophylaxis (PrEP) are scarce and that screening leads to high antimicrobial consumption. The PrEP clinic of the Institute of Tropical Medicine (ITM) in Antwerp, Belgium, discontinued systematic screening for these infections in MSM taking PrEP in 2023. In this study, we estimated the effect of reducing the frequency of screening on the incidence of these infections and antimicrobial use among MSM using

Oropharyngeal Lymphogranuloma Venereum: A Clinical Reasoning Challenge and Literature Review.

Lymphogranuloma venereum (LGV) is a sexually transmitted infection typically caused by serovars L1-L3 of Chlamydia trachomatis. These serovars are tissue-invasive with a preponderance for lymphatic spread and can be acquired via unprotected oral, anal, or vaginal sex. We present the case of a 23-year-old with a prior history of syphilis admitted with four weeks of progressively enlarging painful right cervical lymphadenopathy. Extensive testing, including oropharyngeal swabs and microbiological testing, did not yield a diagnosis, nor did extensive pathological and microbial testing of the lymp

Sexually transmitted infections in the non-European Union and European Economic Area of the World Health Organization European Region 2021-2023.

BACKGROUND: Sexually transmitted infections (STIs) continue to be a significant public health challenge and have an impact on sexual and reproductive health. European Union and European Economic Area (EU/EEA) countries report data annually on the number of cases of syphilis, gonorrhoea, chlamydia, and lymphogranuloma venereum (LGV) to the European Centre for Disease Prevention and Control. No similar system exists for the non-EU/EEA countries in the WHO European Region, and therefore, the sub-regional overview was unclear due to the limited data availability and inconsistency.
