Translation of GGC repeats into a toxic polyglycine protein in oculopharyngodistal myopathy type 2.

GGC repeat expansions in the 5' untranslated region of the GIPC1 gene have been implicated in the pathogenesis of oculopharyngodistal myopathy type 2 (OPDM2). To investigate the underlying mechanism, we generated a series of reporter constructs to confirm the translation product of GIPC1 expanded GGC repeats. We also developed a specific antibody targeting the predicted N-terminus of the predominant translation product. Its expression and toxicity were validated in patient-derived induced pluripotent stem cell-derived myotubes and a zebrafish model. Here, we demonstrate that the expanded GGC r

Neuromuscular disorders with evidence of both neuropathic and myopathic phenotype.

The landscape of genetic research in neuromuscular diseases is revealing an increasing number of genes capable of manifesting both neuropathic and myopathic phenotypes, sometimes in the same patient. While including these genes in broader next-generation sequencing panels addresses the clinical and neurophysiological challenges posed by overlapping or atypical features, it also introduces complexities in the interpretation of their results. In this review, we examine genes that can present with a mixed phenotype, classifying them into categories of clinical utility, and propose a clinical-neur

Clinical, morphological, and molecular characterization of patients with X-linked myopathy with excessive autophagy (XMEA).

X-linked myopathy with excessive autophagy (XMEA) is a slowly progressive disease affecting male patients, caused by hemizygous mutations in the VMA21 gene. We studied nine patients from six unrelated French families clinically suspected of having XMEA. Clinical charts were reviewed, and muscle biopsies underwent histological, immunohistochemical, and electron microscopy analysis. Sanger sequencing and next generation VMA21 gene panels were performed, and proteomic profiling was done on muscle extracts from two patients. Clinical onset ranged from childhood to adulthood with most showing proxi

Muscle mitochondrial changes in antisynthetase syndrome and other idiopathic inflammatory myopathies.

Mitochondria are critical for cellular function. Their dysfunction contributes to cell degeneration and death, leading to disease progression. This study examined mitochondrial changes in idiopathic inflammatory myopathies (IIMs) including antisynthetase syndrome (ASyS), dermatomyositis (DM), and inclusion body myositis (IBM). Skeletal muscle biopsies were analyzed using histology, histochemistry, and electron microscopy from patients diagnosed with IIMs, according to the clinico-sero-morphological classification. There was no significant age difference between 16 ASyS and 16 DM patients, but

Targeting autophagy in Duchenne muscular dystrophy: mechanistic insights and emerging therapeutic strategies.

Duchenne muscular dystrophy (DMD) is a severe X-linked myopathy characterised by progressive skeletal and cardiac muscle degeneration, loss of ambulation, respiratory failure and premature mortality. Although corticosteroids and gene therapies have improved disease management, they are limited by significant side effects, mutation specificity and delivery challenges, underscoring the need for an alternative or an adjunctive strategy. Emerging evidence identifies autophagy dysregulation as a critical secondary pathological mechanism in DMD, contributing to impaired clearance of damaged organell

Altered Connectome Gradient and Its Association with Gene Expression Profiles in Patients with MELAS.

BACKGROUND AND PURPOSE: Hierarchy is a fundamental principle of network organization in the human brain. Functional gradient introduces a new perspective in identifying hierarchy alterations by capturing major axes of functional connectivity in low-dimensional space. However, whether this gradient structure is disrupted in patients with mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) and how this disruption is modulated by the gene-expression profiles remain unknown. MATERIALS AND METHODS: Thirty-one patients with MELAS at the acute stage (MELAS-acute) and 3

CTRP1 regulates skeletal muscle differentiation through quality control of mitochondrial dynamics and function.

Mitochondrial dysfunction is a hallmark of myopathies and impaired skeletal muscle differentiation. Here, we demonstrate that C1q/TNF-related protein 1 (CTRP1) is essential for maintaining mitochondrial dynamics and supporting myogenic differentiation. Loss of CTRP1 in myoblasts and in skeletal muscle-specific-knockout (CTRP1 KOΔACTA) mice led to impaired myotube formation, reduced muscle fiber cross-sectional area, and decreased muscle strength. CTRP1 deficiency also shifted the muscle fiber composition from oxidative type IIA to glycolytic type IIB fibers, indicating a compromised mitochondr

Anti-mitochondrial M2 antibody links to cardiac involvement and immune-mediated inflammatory myopathy-like pathology in myositis.

OBJECTIVES: Cardiac involvement in idiopathic inflammatory myopathies (IIM) is rare but potentially severe. Anti-mitochondrial M2 antibody (AMA-M2) has been implicated in cardiac involvement, but the association remains underexplored. This study aims to evaluate the clinical, pathological and prognostic features of AMA-M2 IIM. METHODS: This historic prospective cohort included IIM patients hospitalized at Peking Union Medical College Hospital between 2008 and 2020. Outcomes were prospectively collected through the Prospective Registry Of MyositIS (PROMIS) registry. Cox regression models were

Elamipretide Improves Mitochondrial Function in Mitochondrial Trifunctional Protein-Deficient Mice and Human Fibroblasts.

Mitochondrial trifunctional protein (TFP) deficiency is an inherited disorder of long-chain fatty acid β-oxidation (FAO). TFP is a heteromeric enzyme composed of two α and two β-subunits. Despite early detection and dietary treatment, TFP deficiency patients often develop hypoglycemia, rhabdomyolysis, cardiomyopathy, and peripheral neuropathy. Degenerative retinopathy and milder peripheral neuropathy occur in patients with an isolated deficiency of the αTFP subunit of long-chain 3-hydroxyacyl-CoA dehydrogenase (LCHAD) activity. Triheptanoin treatment improves most complications, but not periph

Aberrant mitochondrial hsp60 expression affects mitochondria homeostasis and results in muscle dystrophy and premature death.

Heat shock protein 60 (HSP60) plays a vital role in maintaining mitochondrial homeostasis and essential functions and requires ATP for its assembly into chaperone complexes. This study aimed to investigate the long-term effects of HSP60 induction on mitochondrial homeostasis at varying doses and durations using HSP60 transgenic mice. In this study, we generated transgenic mice with elevated levels of native HSP60 using the LoxP-Cre system. These mice exhibited impaired postnatal development, skeletal muscle dystrophy, and increased mortality. Initially, excess HSP60 enhanced the mitochondrial

Sera from patients with dermatomyositis and antisynthetase syndrome mediate muscle weakness, impair mitochondrial respiration and induce local cytokine production in muscle tissue.

OBJECTIVES: Idiopathic inflammatory myopathies (IIM) are systemic autoimmune disorders characterized by skeletal muscle weakness and inflammatory cell infiltrates in muscle tissue. Although circulating systemic factors have been implicated in IIM, it is unclear to what extent such factors shape the muscle disease phenotype. Using a model that can isolate the effect of serum from other systemic influences, we aimed to investigate how serum from IIM affects skeletal muscle contractility, mitochondrial function and inflammatory signalling. METHODS: Isolated skeletal muscles (m. flexor digitorum

Coenzyme Q10 in Cardiovascular Medicine: Mechanisms, Clinical Evidence, and Future Integration in Heart Failure and Statin Myopathy.

Coenzyme Q10 (CoQ10), or ubiquinone, is a lipid-soluble antioxidant essential for mitochondrial adenosine triphosphate production and cellular energy metabolism. Its therapeutic potential has been investigated in conditions marked by mitochondrial dysfunction, particularly chronic heart failure and statin-associated muscle symptoms. Robust evidence, including data from the Q-SYMBIO trial, demonstrates that CoQ10 supplementation can improve functional capacity, ejection fraction, and reduce major cardiovascular events in heart failure with reduced ejection fraction. However, studies on its effi
