Expanding the Phenotypic Spectrum of NDUFS6-Related Disease: From Neonatal Mitochondrial Encephalopathy to Childhood-Onset Axonal Neuropathy.

Biallelic variants in NDUFS6, encoding an accessory subunit of mitochondrial complex I, were initially associated with lethal neonatal mitochondrial encephalopathy and Leigh syndrome. Recent studies have demonstrated that NDUFS6 variants can also cause childhood- or adolescent-onset axonal neuropathy and Charcot-Marie-Tooth (CMT)-like phenotypes, indicating marked clinical heterogeneity. Here, we report a patient with a novel homozygous truncating NDUFS6 variant presenting with a neuropathy-predominant phenotype accompanied by epilepsy, in the absence of neonatal metabolic decompensation. The

Molecular Bases of Myopathies and Their Impact on Clinical Practice: Advances and Future Perspectives.

Myopathies represent a highly heterogeneous group of primary muscle disorders, traditionally classified based on clinical presentation and histopathological findings. Recent breakthroughs in molecular genetics, immunology, and pathophysiology have revolutionized the understanding, diagnosis, and management of these conditions. Both inherited and acquired forms of myopathy, including structural, metabolic, inflammatory, endocrine, and mitochondrial subtypes, are now recognized to arise from diverse pathogenic mechanisms such as impaired calcium handling, mitochondrial dysfunction, chronic infla

Sleep in MELAS syndrome.

The syndrome of mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes (MELAS) is one of the most frequent maternally inherited mitochondrial disorders, and presents numerous clinical manifestations such us stroke-like episodes, dementia, epilepsy, myopathy, headaches, hearing impairment or diabetes. Little attention has been paid to the possible influence of sleep disorders on this pathology. We conducted subjective and objective assessments of sleep in patients with MELAS syndrome, focusing on sleep-disordered breathing and REM sleep behavior disorder (RBD). We obtaine

Dysregulated iron homeostasis Drives mitochondrial Injury and ferroptosis susceptibility in MELAS fibroblasts.

Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) syndrome, caused by mtDNA mutations, disrupts mitochondrial function and triggers oxidative stress. This study explores ferroptosis playing a key role in cell death in MELAS patient-derived fibroblasts harboring low (MELASLow) or high (MELASHi) heteroplasmy levels, subjected to rotenone-induced mitochondrial stress (a Complex I inhibitor). Rotenone crucially curtailed cell viability in MELAS fibroblasts relative to normal human dermal fibroblasts (NHDF), alongside elevated cellular/mitochondrial reactive oxygen

Neuronal intranuclear inclusion disease: a diagnostic pitfall for MELAS.

BACKGROUND: Neuronal intranuclear inclusion disease (NIID) is a rare neurodegenerative disease characterized by eosinophilic hyaluronan inclusions in the nervous system and internal organs. Mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes (MELAS) is the most common neurological mitochondrial disease involving multiple organs. The complex and overlapping clinical manifestations of both diseases pose a significant risk for misdiagnosis. CASE PRESENTATION: We present a case of NIID that closely mimicked the phenotype of MELAS. A 60-year-old Chinese man presented with

Severe 3,4-Methylenedioxymethamphetamine (MDMA)-Associated Rhabdomyolysis in Crohn's Disease: Direct Toxicity and Inflammatory Susceptibility in the Absence of Hyperthermia.

Rhabdomyolysis is a recognized complication of 3,4-methylenedioxymethamphetamine (MDMA), usually driven by severe hyperthermia and agitation. We describe a previously healthy 45-year-old man who developed profound rhabdomyolysis (creatine kinase (CK) 160,000 U/L) and intrinsic acute kidney injury requiring hemodialysis after ingesting 1.5 g of "Molly" (pure MDMA), despite remaining afebrile throughout. In the literature review, the first reported case of severe MDMA-associated rhabdomyolysis without hyperthermia and the third-highest CK values was documented. He later received a new diagnosis

Direct Prediction of VLCADD Severity Using Newborn Screening Analyte Data.

A critical concern of newborn screening (NBS) for very-long chain acyl-CoA dehydrogenase deficiency (VLCADD) is the difficulty of predicting clinical outcomes. To address this, we investigated neonatal C18:2-carnitine concentrations as a possible predictor of VLCADD phenotype. To investigate the impact of sex, gestational age (GA) at birth, sampling day and birth weight on C18:2-carnitine, we analyzed NBS-dried blood spots (DBS) from Dutch newborns born between 2018 and 2020 (n = 209.785). After normalization for resulting confounders, C18:2-carnitine concentrations were investigated in NBS-DB

Effect of early intensive physical therapy on NLRP3 inflammasome activation and muscle atrophy in critical illness myopathy (PT-NLRP3-CIM): a two-centre randomized open-label study protocol.

BACKGROUND: Critical illness myopathy (CIM) is a prevalent cause of intensive care unit-acquired weakness (ICUAW). It is associated with prolonged mechanical ventilation and increased mortality and frequently evolves into postintensive care syndrome with long-term cognitive and physical impairments. Although several molecular mechanisms have been implicated in CIM, including muscle atrophy and altered contractility, its exact etiopathogenesis is not fully understood. Evidence suggests that activation of the NLRP3 inflammasome and upregulation of atrogenes significantly contribute to this condi

Migratory vasodilatation of cerebral arteries in MELAS episodes: a case report and literature review.

Mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes (MELAS) is the commonest inherited mitochondrial disorder. Dilation of the major cerebral arteries is seldom mentioned in MELAS because magnetic resonance angiography (MRA) usually shows normal findings. Here, we described a 24-year-old male patient with MELAS who had migratory vasodilatation of cerebral arteries on MRA and positive antibodies against contactin-associated protein-like-2 in serum during stroke-like episodes (SLEs). Moreover, a total of 18 MELAS cases were included in this literature review. MELAS with

Mitochondrial abnormalities in idiopathic inflammatory myopathies.

OBJECTIVES: Idiopathic inflammatory myopathies (IIMs) are a heterogeneous group of acquired muscle disorders characterised by immune-mediated muscle damage and systemic involvement. Increasing evidence highlights mitochondrial abnormalities as a key contributor to muscle weakness, inflammation, and disease progression. This review aims to summarise current knowledge on the mechanisms, histopathological features, and clinical implications of mitochondrial dysfunction in IIMs, as well as to discuss emerging therapeutic strategies targeting mitochondrial impairment. METHODS: A narrative review o

Effects of Cacao Flavonoids in Long COVID-19 Patients with Chronic Fatigue: FLALOC, a Placebo-Controlled Randomized Clinical Trial.

Background: In the context of long COVID, persistent fatigue is among the most prevalent symptoms that can develop after SARS-CoV-2 infection. Mitochondrial myopathy and endothelial dysfunction, which are triggers of inflammation, have emerged as prominent causes of long COVID-induced fatigue. Interestingly, the intake of flavanols, particularly (-)-epicatechin (EC), has been associated with the positive modulation of endothelial and mitochondrial structure and function. Methods: In this work, we conducted a randomized, double-blind, placebo-controlled clinical trial to determine whether an EC

Multilevel impairment of mitochondrial respiration with sex-specific signatures in inclusion body myositis.

BACKGROUND: Oxidative phosphorylation (OXPHOS) is a central function and a key indicator of mitochondrial fitness, yet studies in human tissue remain limited. Inclusion body myositis (IBM) is a progressive myopathy that lies at the intersection of aging, inflammation and mitochondrial dysfunction. We aimed to perform a comprehensive profiling of mitochondrial respiration in muscle tissue from patients with IBM. METHODS: A wide battery of complementary tests from RNA level to high-resolution respirometry on permeabilized muscle fibers was performed. The relationship between respiration, mitoch

Progressive Myopathy and Respiratory Failure in a 7-Year-Old Boy With m.3251A>G MT-TL1 Mutation.

We report a pediatric case of severe isolated mitochondrial myopathy because of the rare m.3251A>G variant of the MT-TL1 gene. A 7-year-old boy presented to the hospital with acute-on-chronic weakness and respiratory insufficiency. Initial laboratory tests were notable for elevated lactate, aldolase, and lactate dehydrogenase. Despite a negative autoimmune panel, he was presumed to have myositis and treated with steroids and intravenous immunoglobulin. He continued to deteriorate, eventually requiring intubation and ventilation. Muscle biopsy revealed numerous ragged red fibers, abnormal intra

Smarca4 maintains mitochondrial homeostasis and energy metabolism during cardiac development.

Mitochondrial metabolism is fundamental to cardiac and skeletal muscle function due to the high adenosine triphosphate (ATP) demand required for sustained contractility. Although mitochondrial dysfunction is central to metabolic myopathies, the epigenetic mechanisms regulating mitochondrial structure and function remain poorly defined. Here, we identify the SWI/SNF chromatin remodeling ATPase subunit Smarca4 as a critical regulator of mitochondrial homeostasis and cellular energy metabolism. Using a smarca4a-deficient zebrafish model (smarca4aa8-/-), we show that Smarca4 loss causes ventricula

Skeletal Muscle Mass and Quality in Peripheral Artery Disease: New Insights into Clinical Management and Therapeutic Optimization.

BACKGROUND: Peripheral artery disease (PAD) induces chronic limb ischemia that affects downstream skeletal muscle and leads to ischemic myopathy. Chronic ischemia results in mitochondrial dysfunction, oxidative stress, fiber-type shift, proteolysis, and anabolic resistance, contributing to reduced endurance and persistent weakness despite revascularization. METHODS: This narrative review provides a clinically oriented synthesis of PAD-related muscle pathology, pragmatic approaches to assessing muscle mass and quality, and current strategies to preserve or restore muscle health. RESULTS: Chro

First case of schizophrenia and OCD in TK2-related mitochondrial DNA depletion myopathy: a case report.

INTRODUCTION: Mitochondria generate energy in the form of adenosine triphosphate (ATP), integral in regulating cellular activities. Pathogenic variants in mitochondrial DNA occur when excess free radicals are produced during abnormal electron transport, causing cumulative oxidative stress and decline in mitochondrial function. The brain is particularly susceptible to oxidative damage due to its high oxygen consumption and low levels of antioxidant enzymes. This predisposes to neuropsychiatric manifestations, which may be exacerbated by psychotropics that further impair mitochondrial function.

Successful Anesthetic Management of Orthotopic Liver Transplant in a Patient with Deoxyguanosine Kinase Deficiency: A Case Report.

Deoxyguanosine kinase deficiency is a rare autosomal recessive mitochondrial DNA depletion syndrome characterized by severe progressive hepatic failure and neurologic involvement in infancy or early childhood. The disease often progresses to end-stage liver disease, necessitating liver transplant in selected patients. We present the anesthetic management of a 17-year-old female patient with end-stage liver disease due to deoxyguanosine kinase deficiency who was scheduled for liver transplant. Liver transplant is currently the only definitive treatment option for hepatic failure associated with
