Anaesthethic management on a pediatric patient with Sengers syndrome. Case report.

Sengers Syndrome is an autosomal recessive mitochondrial myopathy caused by mutations of the acylglycerol kinase gene, which leads to an abnormal musculoskeletal and myocardial lipid and glycogen accumulation. It is characterized by congenital cataracts, hypertrophic cardiomyopathy, skeletal myopathy and lactic acidosis (especially under stress condition). Therefore, cataract surgery might be indicated. Currently there is no treatment or management formalised for this disease, meaning it is a challenge in clinical practice. We introduce a case of perioperative anaesthetic management on a patie

Deficient Cardiolipin Remodelling Alters Muscle Fibre Composition and Neuromuscular Connectivity in Barth Syndrome.

BACKGROUND: Barth syndrome (BTHS) is a rare X-linked mitochondrial disorder caused by mutations in the TAFAZZIN gene, which disrupts cardiolipin (CL) remodelling and mitochondrial function. While cardiac manifestations of BTHS are well characterized in male patients, the mechanisms underlying skeletal muscle weakness and fatigability are poorly understood. METHODS: We investigated neuromuscular and mitochondrial alterations in a novel murine model (TazPM) carrying a patient-derived D75H point mutation knocked into the Tafazzin locus. This mutation preserves protein abundance but abolishes enz

m.10010T>C Mitochondrial Disease: A Case Report With Hypoparathyroidism and Review of the Literature.

Mitochondria are essential intracellular organelles that play a critical role in cellular metabolism, including the regulation of intracellular calcium signaling. Advances in genomic sequencing have facilitated the identification of rare pathogenic mitochondrial DNA (mtDNA) genetic variants in patients with unexplained endocrine disorders. We present a case report of a woman diagnosed with the rare mtDNA variant m.10010T>C. The case report includes a detailed clinical evaluation, heteroplasmy measurements across several tissues, and a review of previously published cases of patients heteroplas

TSPO-PET highlights an atypical mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes (MELAS) phenotype.

BACKGROUND: The m.10158T > C mutation in the mitochondrial gene (MT-ND3) is a rare cause of adult-onset mitochondrial encephalopathy, typically presenting as MITOCHONDRIAL ENCEPHALOMYOPATHY WITH LACTIC ACIDOSIS AND STROKE-LIKE EPISODES (MELAS) with predominant cortical involvement. Early cerebellar onset and progressive atrophy are atypical. To investigate the underlying pathophysiology, we innovatively applied TSPO-PET to visualize neuroinflammation and MITOCHONDRIAL PATHOLOGY linked to mitochondrial encephalopathies. CASE PRESENTATION: A 32-year-old woman had a 20-year progressive disorder s

Phenotypic characterization of dominantly inherited distal nebulin myopathy.

Nebulin is a large mammalian protein crucial for skeletal muscle sarcomere function. While NEB variants typically cause recessively inherited muscle disorders, a dominantly inherited distal myopathy due to a large multi-exon deletion in NEB was described in 2019 in a single family. We identified 6 unrelated South African families with dominantly inherited distal myopathy associated with a large heterozygous deletion involving exons 14-77 of NEB and collected phenotypic data from 17 affected individuals. The median age at onset was 14 years (range 1 to 48 years), and the phenotype appeared to b

Protein co-expression network analysis explores biological mechanisms associated with meat quality variability in broilers affected by breast myopathies: A focus on wooden breast.

Growth-related myopathies affecting fast-growing broilers still represent a challenge for the poultry industry, and the need for comprehensive and additional knowledge useful to mitigate the occurrence of these myopathies persists. This study aimed at exploring the association between the proteome profiles and the meat quality traits (i.e., weight, morphometric parameters, pHu, color, and compression force) of broiler breast fillets affected by the white striping (WS), wooden breast (WB), and spaghetti meat (SM), as well as the normal ones (NB), by applying the WGCNA approach (N = 12). Clinical and biochemical characterization of amyotrophic lateral sclerosis in a CHCHD10 R15L family.

Familial forms of ALS are potential candidates for gene-directed therapies, but many recently identified genes remain poorly characterized. Here, we provide a comprehensive clinical, neuropathological, and biochemical description of fALS caused by the heterozygous p.R15L missense mutation in the gene CHCHD10. Using a cross-sectional study design, we evaluated five affected and nine unaffected individuals from a large seven-generation pedigree with at least 68 affected members. The pedigree suggests a high (68 - 81%) but incomplete disease penetrance. Through cloning of the disease-allele from

Elevated Kallistatin Induces Myosteatosis and Exercise Intolerance by Antagonizing AdipoR1-Mediated AMPK Signalling.

BACKGROUND: Pathological intramuscular lipid deposition (myosteatosis) and exercise intolerance are hallmarks of metabolic disorders, including diabetes and metabolic dysfunction-associated steatotic liver disease (MASLD), yet their underlying mechanisms remain unclear. Our previous work has confirmed that hypertriglyceridemia-driven kallistatin (KAL) elevation is present in the peripheral blood of patients with MASLD and diabetes and is a causative factor in hepatic steatosis and MASH pathogenesis. Here, we aim to evaluate this elevated KAL on myosteatosis and exercise function. METHODS: We

Sarco/endoplasmatic reticulum calcium ATPase activity in healthy muscle and Brody disease.

The sarco-/endoplasmatic reticulum calcium ATPase (SERCA) is a calcium transporter that plays a key role in the excitation-contraction-relaxation cycle by enabling muscle relaxation. Pathogenic variants in the SERCA-encoding ATP2A1 gene cause Brody disease, a rare recessive myopathy with exercise-induced muscle stiffness as the main feature. Measurement of SERCA activity is an important in vitro functional assay to aid in confirming the pathogenicity of variants in this gene. Furthermore, measurement of SERCA activity is used to study physiological and pathological excitation-contraction (de)c

Transforming the natural course of infantile onset thymidine kinase 2 deficiency through early nucleoside replacement therapy.

BACKGROUND: Thymidine kinase 2 (TK2) deficiency is an ultra-rare, severe mitochondrial myopathy caused by pathogenic variants in TK2 and characterized by a wide range of ages at onset. The infantile form, presenting before 2 years of age, is the most rapidly progressive and is associated with a high risk of early mortality. We describe the clinical outcomes of early nucleoside therapy in a series of children with infantile-onset TK2 deficiency. METHODS: We retrospectively reviewed four children with genetically confirmed infantile-onset TK2 deficiency treated with oral deoxycytidine/deoxythym

Loss of Myofilaments in Gastrointestinal Smooth Muscle: A Novel Pathological Finding in MELAS-Associated Chronic Intestinal Pseudo-Obstruction.

BACKGROUND AND AIMS: Chronic intestinal pseudo-obstruction (CIPO) occurs in up to 40% of patients with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes (MELAS). While enteric nervous system abnormalities have been documented, 15%-38% of CIPO cases show normal histology, suggesting alternative pathogenic mechanisms. We aimed to investigate smooth muscle pathology in MELAS-associated CIPO. METHODS: Comprehensive pathological examination including light and electron microscopy was performed on autopsy material from a 52-year-old male with MELAS (m.3243A > G mutat

Perilipin 5 enhances glycogen storage and mitigates excessive fatty acid oxidation in skeletal muscle to alleviate mitochondrial impairment.

Perilipin 5 (Plin5) is a lipid droplet-associated protein that regulates lipid hydrolysis and mitochondrial oxidative metabolism, and is highly expressed in skeletal muscle. Its role in skeletal muscle glucose metabolism and its relevance to mitochondrial myopathy (MM) remain unclear. We used Plin5-knockout (Plin5-KO) mice in exhaustive swimming tests to assess endurance and anaerobic exercise capacity. Glucose uptake was measured using 2-NBDG; glucose, lactate, NADH/NAD+ ratio, and fatty acid oxidation (FAO) rate were determined with commercial kits. Mitochondrial ultrastructure was evaluated

Immunoprecipitation-mass spectrometry reveals known and novel (NatC, BCS1L) antinuclear antibodies in rheumatoid arthritis.

OBJECTIVE: In a fraction of patients with rheumatoid arthritis (RA), antinuclear antibodies (ANA) are present. However, in most ANA-positive samples from patients with RA, no autoantigens can be identified. We aimed to characterise the ANA landscape in RA and to identify known and novel autoantigens using an unbiased immunoprecipitation-mass spectrometry (IP-MS) approach. METHODS: Sera from 51 ANA-positive, DMARD-naïve patients with RA were analysed by IP-MS using HeLa nuclear or cytoplasmic extracts (depending on the ANA staining pattern). Immunoprecipitated proteins were digested and identi

Blockage of autophagy causes severe skeletal muscle disruption in a mouse model for myofibrillar myopathy 6.

Myofibrillar myopathy 6 is a rare, autosomal-dominant neuromuscular disorder caused by an amino acid exchange Pro209Leu in the co-chaperone BAG3, which disrupts muscle protein turnover and causes severe muscle weakness and shortened lifespan. We generated transgenic mice overexpressing the human mutant BAG3P209L-GFP, which rapidly develop skeletal muscle weakness unlike controls expressing BAG3WT-GFP. Here we show that mutant mice exhibit sarcomere breakdown, inflammation, protein aggregates, centralized nuclei and mitochondrial defects in their skeletal muscles, thereby reducing contraction f

Loss of TMEM65 in mice causes mitochondrial disease mediated by mitochondrial Ca2.

Transmembrane protein 65 (TMEM65) depletion in a patient caused severe mitochondrial encephalomyopathy, highlighting its clinical importance. Recent studies show TMEM65 acts as a mitochondrial Na+/Ca2+ exchanger in vitro. Here, we generated conditional Tmem65 knockout mice to define its role in neuromuscular tissues in vivo. Both whole-body and nervous system-specific Tmem65 knockouts exhibited severe growth retardation and seizure-associated sudden death at ~3 weeks, establishing TMEM65 as indispensable for neuronal function. Additionally, skeletal muscle-specific knockout produced adult-onse

From Initiation to Elongation: eIF3 as a Dual-Phase Guardian of Mitochondrial Integrity and Protein Homeostasis in Skeletal Muscle.

Skeletal muscle is the largest organ by mass in the human body, and its functional capacity depends on the precise coordination of protein synthesis, mitochondrial bioenergetics, and regenerative potential. Eukaryotic translation initiation factor 3 (eIF3), a 13-subunit complex (~800 kDa) best known for its multifaceted roles in cancer, is now emerging as a key translational regulator in skeletal muscle physiology and disease. Here, we present a perspective that synthesizes recent advances into a unifying "dual-phase guardian" model. In the first phase, eIF3f acts at the level of translation i

Loss of STARD7 Impairs Mitochondrial Phospholipid Homeostasis and Contributes to Mitochondrial Myopathy.

Mitochondria are composed of phospholipid bilayers rich in phosphatidylcholine (PC). StAR-related lipid transfer domain-containing protein 7 (STARD7) functions as a lipid transfer protein that plays a crucial role in maintaining mitochondrial PC homeostasis. In this study, we investigated the physiological role of STARD7 in skeletal muscle using muscle-specific knockout (mKO) mice. STARD7 expression was markedly higher in the soleus, a mitochondria-dense slow-twitch muscle, compared with fast-twitch fibers. Although muscle fibers from mKO mice exhibited no apparent structural abnormalities, th
