New understanding and treatment of adult mitochondrial dysfunction: an example of personalized precision medicine.

The principal cellular energy-generating pathways of mitochondria used to produce adenosine triphosphate (ATP) are oxidative phosphorylation and β-oxidation of fatty acids. Under anaerobic conditions, glycolysis in the cytoplasm is an alternative mechanism for production of ATP. Mitochondrial diseases result from one or more of the over 350 mutations in mitochondrial DNA (10%) or nuclear DNA (90%) that cause defective mitochondrial ATP production. The most common manifestations in adults with mitochondrial DNA mutations are diminished vision, myopathy, cardiomyopathy, neuropathy, encephalopath

The pleiotropic impact of chaperone-mediated autophagy on skeletal muscle integrity.

Skeletal muscle is a fundamental tissue as it is found throughout the body, sustains posture, and produces movement. Yet, skeletal muscle disorders, such as myopathies, affect a large percentage of the population, degrading an individual's quality of life. A recent study links myopathy progression to the decline in chaperone-mediated autophagy that occurs during aging. Underscoring the importance of a balanced CMA pathway in maintaining skeletal muscle function and integrity, the study also provides mechanistic insights into the pathways that are dysregulated due to defective CMA and presents

Reengineering statin therapy to protect skeletal muscle: nanocarrier strategies for mitigating mitochondrial dysfunction and myotoxicity.

Statins remain the cornerstone of atherosclerotic cardiovascular disease prevention; however, their long-term clinical utility is frequently limited by skeletal muscle toxicity, ranging from mild myalgia to severe myopathy. Growing evidence implicates mitochondrial dysfunction, oxidative stress, and impaired muscle energy homeostasis as central drivers of statin-induced muscle injury, with important consequences for patient adherence and treatment durability. Emerging nanocarrier-based drug delivery strategies provide an opportunity to address these limitations by reprogramming statin biodistr

iPSC generation from PBMCs of a MELAS patient for mitochondrial dysfunction studies.

Mitochondrial Encephalomyopathy, Lactic Acidosis, and Stroke-like episodes (MELAS) is a multisystemic mitochondrial disorder primarily caused by a heteroplasmic point mutation at mitochondrial DNA (mtDNA) position 3243 (m.3243A > G) in the MT-TL1 gene, which encodes mitochondrial tRNA^Leu(UUR). In this study, we report the successful reprogramming of peripheral blood mononuclear cells (PBMCs) from a male patient diagnosed with MELAS into induced pluripotent stem cells (iPSCs). This patient-specific iPSC platform enables investigation into the relationship between heteroplasmy levels and diseas

Mitochondrial dysfunction-driven inflammation and β-Cell apoptosis in type 2 diabetes mellitus: mechanistic insights and therapeutic implications.

Type 2 diabetes mellitus (T2DM) is a multifactorial metabolic syndrome characterized by chronic hyperglycemia, progressive pancreatic β-cell malfunction, and peripheral insulin resistance. There is growing evidence suggesting that mitochondrial dysfunction and apoptosis play a critical role in the development and advancement of T2DM. Mitochondria are essential for cellular energy metabolism and redox homeostasis. It is the metabolic stress and apoptotic cell death in insulin-producing β-cells and insulin-sensitive tissues induced by mitochondrial dysfunction caused by excessive reactive oxygen

A novel homozygous MICOS13 frameshift mutation causing mitochondrial hepatoencephalopathy and mtDNA depletion: clinical characterization and dynamic structural simulation.

BACKGROUND: Mitochondrial diseases comprise a genetically and clinically heterogeneous group of disorders primarily resulting from defects in oxidative phosphorylation, leading to impaired ATP synthesis and cellular energy deficiency. The functional competence of mitochondria critically depends on the ultrastructural integrity of the inner mitochondrial membrane, notably the cristae, which serve as the platform for respiratory chain complexes. The mitochondrial contact site and cristae organizing system complex (MICOS) is essential for maintaining cristae junction architecture and inner membra

Diagnostic Odyssey of Atypical Long-Chain 3-Hydroxyacyl-CoA Dehydrogenase Deficiency (LCHADD) Explained by Three Allelic Products From Two Pathogenic Variants.

Long-chain 3-hydroxyacyl-CoA dehydrogenase deficiency (LCHADD) is an autosomal recessive mitochondrial defect of long-chain fatty acid β-oxidation, caused by biallelic pathogenic variants in HADHA or HADHB. We report a 22-year-old male with an atypically mild presentation of LCHADD who was referred to the Undiagnosed Diseases Network (UDN). Trio genome sequencing identified a maternally inherited HADHA frameshift pathogenic variant and a paternally inherited noncoding rare HADHA variant. The paternal noncoding variant was predicted by in silico splicing analysis to create a cryptic splice dono

Effects of exercise training on skeletal muscle function in patients with mitochondrial myopathy: a systematic review.

BACKGROUND: Mitochondrial myopathy (MM) is a group of rare, progressive muscle disorders characterized by impaired oxidative phosphorylation due to mitochondrial DNA (mtDNA) or nuclear DNA (nDNA) mutations, leading to exercise intolerance, muscle weakness, and metabolic dysfunction. Although exercise is increasingly recognized for its capacity to enhance mitochondrial function and muscle performance, the specific effects of different exercise prescriptions (in terms of modality, intensity, and duration) on MM and their phenotype-specific outcomes remain heterogeneous. This study systematically

Mild-cerebellar ataxia due to impaired mitochondrial function caused by the MSTO1 variations.

BACKGROUND: MSTO1 encodes a regulator of mitochondrial fusion. Mutations in MSTO1 are linked to a rare mitochondrial disorder characterized by early-onset myopathy and cerebellar ataxia, with 31 cases reported globally to date, which underscores its exceptional rarity. METHODS: We conducted comprehensive clinical, molecular, and biochemical investigations in a patient harboring novel MSTO1 variants. RESULTS: We identified a patient presenting with adult-onset progressive ataxia and cerebellar atrophy who carried two novel compound heterozygous variants in the MSTO1 gene (c.756A>G, p.Glu252Gl

Phenotypic heterogeneity within twins with MELAS with epilepsy: Case report.

RATIONALE: Mitochondrial encephalomyopathy with lactic acidemia and stroke-like episodes (MELAS) syndrome is a maternally inherited mitochondrial disorder caused by mutations in mitochondrial DNA, most commonly the m.3243A>G variant. This mutation impairs oxidative phosphorylation, leading to inadequate cellular energy production, particularly in high-demand tissues such as the brain and muscles. The resultant energy deficit manifests as neurological and muscular dysfunction, including stroke-like episodes, seizures, and lactic acidosis. PATIENT CONCERNS: Twin brothers presented with heteroge

Energy metabolism dysregulation in idiopathic inflammatory myopathies: mechanisms and therapeutic implications.

Idiopathic inflammatory myopathies (IIMs) are being increasingly recognized as disorders driven by profound disturbances in cellular energy metabolism rather than inflammation alone. Recent studies have highlighted mitochondrial dysfunction, oxidative stress, and metabolic reprogramming across glucose, lipid, and amino acid pathways as central mechanisms linking energy metabolism dysregulation to sustained muscle injury. Defective mitophagy, mitochondrial DNA (mtDNA) depletion, and excessive reactive oxygen species (ROS) production create a self-amplifying loop with interferon-driven inflammat

Biallelic MCUR1 nonsense mutation associated with vacuolar myopathy and altered mitochondrial calcium signaling.

During muscle contraction, increased influx of calcium from the myocyte cytosol into the mitochondrial matrix through the mitochondrial calcium uniporter (MCU) links calcium homeostasis with high ATP provision. The MCU is located at the inner mitochondrial membrane and one of its structural components, the mitochondrial calcium uniporter regulator 1 (MCUR1), promotes its activity. Although MCUR1 function has been studied in cell models, mutations have not yet been associated with human disease. Here, we present a patient with proximal muscle weakness and atrophy, showing histological features

MSTO1 modulates RAD51 activity to safeguard mitochondrial DNA integrity and control immune responses.

Pathogenic MSTO1 mutations cause cerebellar ataxia and congenital myopathy, yet their molecular mechanisms remain poorly understood. This study identifies MSTO1 as a crucial regulator of mitochondrial genome integrity through direct interaction with RAD51. MSTO1 deficiency enhances RAD51 binding to mitochondrial DNA (mtDNA), disrupting mtDNA replication and causing mtDNA damage. This interaction, mediated by the conserved FxxA motif, specifically regulates RAD51's mitochondrial activity without affecting its nuclear roles. Loss of MSTO1 impairs mitochondrial membrane potential, reduces mtDNA c
