Riboflavin therapy in complex I deficiency: Two new cases of leukoencephalopathy and a systematic literature review.

OBJECTIVE: Complex I (CI) deficiency, the most common biochemical defect in pediatric mitochondrial diseases, presents with diverse phenotypes, including cardiomyopathy, myopathy, Leigh syndrome, and mitochondrial leukoencephalopathy (ML). No curative therapies exist. Riboflavin, a precursor of CI cofactors FMN and FAD, is a potential treatment, but evidence is heterogeneous and formal guidelines are lacking. METHODS: We retrospectively analyzed two patients with genetically confirmed CI deficiency due to NDUFS1 and NDUFV2 variants, treated with high-dose riboflavin with long-term clinical, b

Total Intravenous Anesthesia With Remimazolam and Remifentanil for Intraoperative Care of a Child With Venezuelan Heritage, at Risk for a Mitochondrial Disorder.

Recent clinical communications have alerted healthcare providers to potential concerns related to a previously undiagnosed disorder of mitochondrial function in children with maternal Venezuelan heritage. These patients have developed severe neurological damage following the administration of an apparently uneventful anesthetic that included the volatile anesthetic agent, sevoflurane. Subsequent analysis of family members and patients has identified a point mutation of the NADH dehydrogenase 4 (ND4) gene (mtND4 m.11232T>C), a subunit of complex I of the mitochondrial electron transport chain.

Mechanism of Action and Therapeutic Potential of Sulforaphane in Skeletal Muscle Diseases: Molecular Pathways and Precision Medicine.

Sulforaphane (SFN), a bioactive isothiocyanate abundant in cruciferous vegetables, has attracted growing interest as a potential nutraceutical intervention for skeletal muscle disorders. This narrative review synthesizes preclinical and early clinical evidence on the mechanisms and therapeutic applicability of SFN in ICU-acquired weakness, diabetic myopathy, sarcopenia, and exercise-induced muscle damage. SFN modulates skeletal muscle pathophysiology through four interconnected axes: regulation of protein homeostasis via mTOR-associated signaling and suppression of ubiquitin-proteasome-mediate

Bis(monoacylglycero)phosphate (BMP) as a circulating biomarker of lysosomal dysfunction in GNE myopathy.

GNE myopathy (GNEM) is a rare neuromuscular disorder caused by pathogenic variants in the GNE gene and traditionally associated with impaired sialic acid biosynthesis. In a previous untargeted lipidomic study, we identified a lipid feature significantly enriched in the serum of patients with GNE myopathy compared with healthy controls. In the present work, we aimed to determine the structural identity of this disease-associated feature and to establish a robust analytical strategy for its quantitative assessment. Analysis of the mass spectrometric data pointed to bis(monoacylglycero)phosphate

Mitochondrial and Epigenetic Drivers of Skeletal Muscle Dysfunction in Chronic Obstructive Pulmonary Disease.

Skeletal muscle dysfunction (SMD) is a critical extrapulmonary comorbidity in chronic obstructive pulmonary disease (COPD), contributing to exercise intolerance, poor quality of life, and increased mortality. Building upon and extending the disuse model, this review synthesizes evidence establishing COPD-induced SMD as a distinct myopathy with intrinsic disease drivers. Its pathophysiology is driven by a self-reinforcing network: mitochondrial energetic crisis featuring bioenergetic failure and dysregulated dynamics, chronic oxidative stress and inflammation fueling catabolic drive via ubiquit

Muscle Dysfunction in Critical Illness: Established Mechanisms and the Potential Contribution of the NLRP3 Inflammasome.

ICU-acquired weakness (ICUAW) is a clinical condition characterized by muscle weakness in critically ill patients that is not directly attributable to the underlying illness. It affects approximately 40% of intensive care unit patients, primarily impairing the limbs and respiratory muscles, and can compromise motor and respiratory function even after recovery from acute illness. ICUAW exhibits heterogeneous phenotypes. In addition, diverse risk factors influence its occurrence. Although this condition is recognized, the underlying mechanisms contributing to critical illness-associated muscle d

The Central Role of the AMPK/SIRT1/PGC-1α Signaling Axis in Skeletal Muscle Physiology and Pathology and Its Targeted Therapeutic Strategies.

Considered by some to be the largest metabolic organ of the body, the functional integrity of skeletal muscle is highly dependent on its exceptional plasticity, which is primarily governed by mitochondrial quality control. The signaling axis composed of AMP-activated protein kinase (AMPK), sirtuin 1 (SIRT1), and peroxisome proliferator-activated receptor gamma coactivator 1-alpha (PGC-1α) serves as a critical hub that senses cellular energy status, coordinates mitochondrial biogenesis, regulates muscle fiber type switching, and maintains protein homeostasis. This review systematically delineat

Consensus guideline from the European Malignant Hyperthermia Group on perioperative management of patients with an inherited primary disorder of skeletal muscle.

Perioperative abnormalities presenting in patients with inherited primary disorders of skeletal muscle can share similar characteristics to those in a malignant hyperthermia crisis. These patients are often referred to malignant hyperthermia units for perioperative advice as clinicians are accessing conflicting and non-current literature. To address this, the European Malignant Hyperthermia Group (EMHG) has conducted a review of the current literature and used a formal consensus process, with review at annual EMHG meetings and workshops, to provide guidelines for the perioperative management o

Spinal Muscular Atrophy in Adult Neurology Services in India.

Spinal muscular atrophy (SMA) research has focused predominantly on paediatric populations, with limited adult data from low- and middle-income countries (LMICs). We prospectively recruited suspected SMA patients (age ≥ 12 years) into a neuromuscular disease cohort at a tertiary centre in northern India, using phenotyping, creatine kinase, electrophysiology and genetic testing. Forty genetically confirmed patients were included (median age 20.5 years; mean symptom duration 14.7 ± 10.9 years). Proximal lower-limb weakness was the commonest presentation (82.5%). Forty-five percent were misdiagno

Multilevel impairment of mitochondrial respiration in inclusion body myositis.

Oxidative phosphorylation (OXPHOS) is a central function and a key indicator of mitochondrial fitness, yet studies in human tissue remain limited. Inclusion body myositis (IBM) is a progressive myopathy that lies at the intersection of aging, inflammation and mitochondrial dysfunction. We aimed to perform a comprehensive profiling of mitochondrial respiration in muscle tissue from patients with IBM. A wide battery of complementary approaches from RNA level to high-resolution respirometry on permeabilized muscle fibers was employed. The relationship between mitochondrial respiration, mitochondr

Idiopathic inflammatory myopathies as a damage-data-endotype continuum: A roadmap to precision trials and computable biomarkers.

Idiopathic inflammatory myopathies (IIM) are heterogeneous immune-mediated diseases characterized by muscle inflammation that coexists with variable trajectories of lung, skin, cardiac involvement, and malignancy association. Although modern classification criteria and serology have improved phenotyping, evidence remains limited in its applicability across cohorts and trials because broad diagnostic labels obscure major differences in autoantibodies, organ-risk trajectories, disease stage, and outcome selection. We propose a community roadmap that redefines IIM as a damage-data-endotype contin

Ferroptosis in skeletal muscle: from molecular mechanisms to therapeutic interventions.

UNLABELLED: Ferroptosis is an iron-dependent, non-apoptotic form of regulated cell death driven by lethal lipid peroxidation and has emerged as a pivotal regulator of skeletal muscle physiology and pathology. This review systematically delineates the core molecular machinery of ferroptosis, including the system Xc--glutathione-GPX4 axis, dysregulated iron metabolism, and lipid peroxidation, together with key regulatory networks involving p53, Nrf2, and AMPK. We further highlight the context-dependent roles of ferroptosis in skeletal muscle: during development and regeneration, transient and mo

Drug-induced autophagic myopathies: Exploring pathophysiology with immunolabelling and proteomics.

Hydroxychloroquine (HCQ), chloroquine (CQ), and colchicine are widely used to treat autoimmune and inflammatory diseases but can cause toxic autophagic vacuolar myopathies. This study investigates the pathological mechanisms of HCQ/CQ- and colchicine-induced myopathies through integrated clinical, histopathological, ultrastructural, and proteomic analyses. Nine patients with clinicopathologically defined toxic myopathy (HCQ/CQ, n = 4; colchicine, n = 5) were included. Muscle biopsies were analyzed using histoenzymology, immunohistochemistry for autophagic and immune markers, electron microscop

Pulmonary Hypertension with a High-Output Hemodynamic Profile in a Child with MELAS: A Case Report.

Pulmonary hypertension (PH) is uncommon in myopathy, encephalopathy, lactic acidosis, and stroke-like (MELAS) and its hemodynamics are poorly described. A 2-year-old Japanese girl was referred to our hospital with PH diagnosed during pneumonia treatment. Cardiac catheterization revealed PH with a high cardiac output. The patient was diagnosed with MELAS based on multiorgan involvement and the detection of an m.3243A>G mutation. PH therapy was partially effective, and oxygen supplementation may help improve PH without worsening the high cardiac output.

Repurposing mitochondrial-targeting drugs for management of ICANS in CAR T-cell therapy: a novel steroid-sparing approach.

Chimeric antigen receptor (CAR) T-cell therapy has transformed the treatment landscape for relapsed and refractory hematologic malignancies. However, immune effector cell-associated neurotoxicity syndrome (ICANS) remains one of the most clinically significant and potentially life-threatening toxicities, limiting the broader applicability of this otherwise promising modality. Current standard-of-care (SOC) management of ICANS relies heavily on corticosteroids, which are not always effective and at high doses and with prolonged exposure may carry substantial risks for infectious complications, s

The Role of Autophagy in the Pathogenesis of Mitochondrial Diseases.

Mitochondrial diseases are a heterogeneous group of inherited disorders caused by defects in the mitochondrial genome or nuclear genes encoding proteins essential for mitochondrial function. These conditions are characterised by progressive dysfunction of tissues with high energy demands, particularly the nervous and muscular systems. In recent years, increasing consideration has been paid to the role of autophagy-the cellular mechanism for the degradation and recycling of intracellular components in the pathogenesis of mitochondrial diseases. This review synthesizes current knowledge on molec

The Role of Muscle Biopsy in the Era of Modern Genomic Medicine-A Review.

This review examines the evolving role of muscle biopsy in the diagnosis of neuromuscular disorders in the era of modern genomic medicine. Historically the cornerstone of myopathy diagnosis, muscle biopsy enabled structural, histochemical, and ultrastructural characterization of muscle diseases. However, the introduction of next-generation sequencing and other genomic technologies has shifted the diagnostic paradigm, with genetic testing now serving as the preferred first-line approach for many hereditary myopathies due to its non-invasive nature and high diagnostic yield. However, muscle biop
