Recombinant human growth hormone (rHGH) for muscle enhancement in knee osteoarthritis: protocol for a pilot, randomised placebo-controlled trial.

INTRODUCTION: The management of active patients with symptomatic knee osteoarthritis (KnOA) who are too young for total knee arthroplasty poses a specific challenge to clinicians. Research studies show that improving quadriceps muscle strength improves pain and function; however, aspects of the disease render it difficult for patients to achieve and maintain improvements. Recombinant human growth hormone (rHGH) is shown to increase the magnitude and duration of muscle growth when combined with exercise treatment in adult populations. Hence, rHGH combined with physical therapy may provide meani

Additional effects of aerobic training to neck exercises for women with chronic temporomandibular disorders: a randomized controlled clinical trial.

BACKGROUND: Neck-specific exercises are commonly used in the management of temporomandibular disorders (TMD); however, the additional role of aerobic training remains unclear. Given the involvement of central pain mechanisms in TMD and the effects of aerobic exercise on pain modulation, its potential adjunctive value warrants further investigation. OBJECTIVE: To evaluate the additional effects of aerobic training to neck exercises on pain intensity, neck disability, and related outcomes in women with chronic TMD. DESIGN: Randomized controlled trial. METHODS: Fifty-eight women with chronic T

Myoepithelioma of the Lung: A Rare Case With Bilateral Pleural Spread and Diagnostic Challenges in a Middle-Aged Female.

Myoepitheliomas are uncommon neoplasms, predominantly arising in the salivary glands, with pulmonary involvement being extremely rare. Primary lung myoepitheliomas account for only a small number of reported cases, while pleural or bilateral presentations are exceptionally uncommon. Their nonspecific radiological appearance often raises suspicion for metastatic disease, necessitating histopathological and immunohistochemical evaluation for accurate diagnosis. We report a diagnostically challenging case of pulmonary myoepithelioma in a 42-year-old female with a history of treated adenoid cystic

Reciprocal regulation between the protein arginine deiminases and mSWI/SNF chromatin remodelers controls skeletal muscle differentiation and regeneration.

Protein arginine deiminases (PADs) post-translationally convert arginine to citrulline on target proteins and serve as regulators of multiple cellular functions. PAD enzymes have been implicated in autoimmune disorders, cancer, and other diseases. Inhibiting PAD activity is currently being pursued clinically for therapeutic purposes. However, little is known about PAD function in normal developmental or homeostatic processes. Here we show that multiple PAD isoforms contribute to primary myoblast differentiation by binding to regulatory regions of target genes. Furthermore, we demonstrate a nov

A Pediatric Case of Stiff-Person Syndrome: Presentation and Comparative Analysis.

INTRODUCTION: Stiff-person syndrome (SPS) is a rare autoimmune neurological disorder characterized by progressive muscle rigidity and spasms. While it predominantly affects adults, pediatric cases are exceptionally rare, often leading to delayed diagnosis due to non-specific early clinical manifestations. CASE REPORT: We report the case of an 11-year-old boy with progressive gait abnormalities and rigidity, initially misdiagnosed as an orthopedic disorder. The patient exhibited a forward-leaning posture, absent lumbar lordosis, and significant muscle tightness, leading to a primary surgical i

Quantitative Understanding of Advanced Novel Imaging Techniques for Fasciitis and Biosignature Yield (Quantify): Protocol for a Cross-Sectional Diagnostic Study.

BACKGROUND: Myofascial pain remains an underdiagnosed contributor to musculoskeletal pain conditions, including plantar heel pain, which is the most common source of foot pain. The current standard for diagnosing myofascial pain is a clinical examination using manual palpation. However, this approach lacks quantitative thresholds for precise assessment of myofascial pain, highlighting the need for validated biomarkers. OBJECTIVE: This protocol describes the development of a diagnostic imaging biosignature of myofascial pain using both ultrasound and magnetic resonance imaging to differentiate

Anatomical refinement matters: ureteropelvic junction location confers survival benefit for patients with upper tract urothelial carcinoma after radical nephroureterectomy.

OBJECTIVE: Whether primary tumor location influences prognosis in upper tract urothelial carcinoma (UTUC) remains contentious. We aim to clarify the prognostic impact of tumor location in upper tract urothelial carcinoma (UTUC) by anatomically subsegmenting ureteral tumors, with specific focus on ureteropelvic junction (UPJ) carcinomas. METHODS: This retrospective cohort study evaluated 2,315 patients with non-metastatic, unifocal UTUC, including tumors localized to the renal pelvis or ureter, who underwent radical nephroureterectomy. Ureteral tumors were rigorously subsegmented into UPJ, pro

Benefits of oral nitrite supplementation on mitochondrial respiration and physical function in older adults.

BACKGROUND: Age-associated decline in mitochondrial oxidative capacity is associated with increased risk of disease, frailty, and disability. Oral nitrite and nitrate supplementation have been demonstrated to improve mitochondrial energetics and physical function in younger adults, but effects in older adults (age ≥70 years) remain unclear. METHODS: We conducted a randomized, placebo-controlled, double-blind, two-arm trial with a parallel group design to examine the effect of 20 mg sodium nitrite supplements administered three times a day for 12 weeks versus placebo in older (age ≥70 years) s

An integrated behavioral treatment for improving nocturia and insomnia symptoms in older adults (MINT): study protocol for a multi-site randomized clinical trial.

BACKGROUND: Nocturia (i.e., waking to void during the primary sleep period) of two or more times per night affects nearly one-third of older adults and can have a severe impact on sleep, contributing to insomnia symptoms. Current treatment approaches for nocturia often overlook non-lower urinary tract factors that may contribute to nighttime awakenings. Nocturia management, for example, may benefit from more effective integration of cognitive behavioral therapy for insomnia (CBT-I) principles that address other factors underlying insomnia symptoms, and early evidence suggests it also reduces n

Utilisation of intramuscular and intermuscular fat to develop a new skeletal muscle grading score which can predict treatment outcomes for locally advanced rectal cancer.

BACKGROUND: Sarcopenia has been widely studied in rectal cancer with increasing evidence to suggest that other body composition parameters, in particular adipose tissue, have an important role. Advances in artificial intelligence (AI) now allow 3D body composition analysis of intermuscular/intramuscular adipose tissue (IMAT) from CT scans. This study aimed to develop and evaluate a skeletal muscle score (SMS), utilising skeletal muscle (SM) and IMAT measurements, to predict treatment response and survival outcomes for rectal cancer patients. METHODS: A retrospective analysis was performed on

Differential Pathophysiological Drivers of Susceptibility to Type 2 Diabetes and Metabolic Dysfunction-Associated Steatotic Liver Disease: Ethnic Differences in Insulin Dynamics, Whole-Body Fat Metabolism, and Organ-Specific Lipid Deposition.

INTRODUCTION: This narrative review explores the epidemiological evidence and potential underlying pathophysiological defects underlying the disproportionately greater risk of Type 2 diabetes (T2D) and cardiometabolic disease in people of South Asian and African Caribbean ancestry compared with White Europeans. Differences in (i) insulin dynamics, (ii) body composition and liver and pancreas triglyceride accumulation, and (iii) dysregulated fat metabolism likely contribute to this obesity-related susceptibility. INSULIN DYNAMICS: Insulin resistance and hyperinsulinemia are key pathophysiologi

Bladder Defect Repair by Polycaprolactone/Gelatin Nanofiber Scaffolds Loaded with Mitomycin Through Anti-Fibrotic Effects.

BACKGROUND: In recent years, bladder defect repair has emerged as a critical issue in urological tissue engineering. Traditional treatment methods, such as autologous tissue transplantation and synthetic material repair, are limited by factors such as scarce donor sources, immune rejection, and postoperative fibrosis. Consequently, the development of nanofiber materials with bionic structures, biocompatibility, and anti-fibrotic capabilities has become a research hotspot. This research addressed the clinical needs associated with tuberculous bladder contracture, chronic cystitis, traumatic bla

Neuronal lipidome in schizophrenia and metabolic syndrome: Potential interventions.

Life is dependent on the separation of various body compartments from each other and the environment. The permeability of biological barriers, including the enhanced barriers of the gastrointestinal and blood-brain, depends on the integrity of cell or mitochondrial membranes and intercellular junctions. Severe mental illness and metabolic syndrome are characterized by premature cellular senescence that alters the permeability of biological membranes. Increased barrier permeability enables microbial migration outside the gastrointestinal tract, eventually reaching the brain, skeletal muscles, a

Thromboxane signalling links immune activation to enhanced glucose uptake in skeletal muscle.

AIMS/HYPOTHESIS: Exercise elicits a spectrum of metabolic and inflammatory responses that are crucial for skeletal muscle adaptation and overall health, particularly in the context of metabolic diseases, yet the contribution of prostanoid signalling to these processes remains unclear. We hypothesised that exercise-induced thromboxane production enhances skeletal muscle glucose uptake and improves whole-body glucose control. METHODS: Plasma prostanoids were quantified in men and women with normal glucose tolerance or type 2 diabetes before, immediately after and 3 h after a single bout of exer

Safety and efficacy of fordadistrogene movaparvovec in ambulatory participants with Duchenne muscular dystrophy (CIFFREO): a phase 3, double-blind, randomised, placebo-controlled study.

BACKGROUND: Gene transfer is a promising therapeutic approach for Duchenne muscular dystrophy as the disease results from mutations in a single gene. Fordadistrogene movaparvovec is an investigational recombinant adeno-associated virus 9 (rAAV9)-based vector encoding a mini-dystrophin transgene protein for Duchenne muscular dystrophy . We aimed to assess the safety and efficacy of fordadistrogene movaparvovec in slowing the functional decline experienced by individuals with Duchenne muscular dystrophy. METHODS: CIFFREO was a phase 3, double-blind, randomised, placebo-controlled study done at

Genetic Bases of Arthrogryposis Multiplex Congenita.

Arthrogryposis multiplex congenita (AMC) is characterized by congenital joint contractures in two or more body areas resulting from reduced or absent fetal movements. AMC exhibits marked phenotypic and genetic heterogeneity, as it is a symptom rather than a disease and may be part of a large number of unrelated conditions. Despite advances in genomic approaches and the increasing number of newly identified genes, disease-gene identification was achieved in fewer than half of cases in several reports, including in a French cohort of 367 AMC patients. The most frequent cause of AMC in these repo

Spinal cord protection by epidural separation during vertebral cryoablation for metastatic spine disease: A proof-of-concept preclinical study.

INTRODUCTION: Cryoablation for spinal metastases is limited by the risk of cryogenic neural injury near the spinal cord. Physical separation between the vertebral body and dura may provide a thermal margin, but the distance-temperature relationship in the epidural space remains insufficiently defined. RESEARCH QUESTION: This exploratory study aimed to map the association between surgically created epidural separation distance and local thermal and neurophysiological changes during vertebral cryoablation in a canine model. MATERIAL AND METHODS: In a non-randomized, unblinded, two-phase pilot
