A study of body composition, muscle strength, and arterial stiffness in adolescents with primary and secondary hypertension.

BACKGROUND: Hypertension in adolescents is associated with early cardiovascular risk, yet functional and vascular differences between primary and secondary hypertension remain poorly characterized. This study aims to compare body composition, muscle strength, arterial stiffness, physical activity, and sedentary behavior among adolescents with primary and secondary hypertension compared with normotensive peers. METHODS: Adolescents with primary hypertension (n = 35), secondary hypertension (n = 17), and normotensive controls (n = 21) were assessed for anthropometric characteristics, handgrip s

Protocol for the internet-based lifestyle intervention to eradicate obese frailty in prostate cancer survivors (iLIVE) randomized controlled trial: a type I hybrid effectiveness implementation trial.

BACKGROUND: Obese frailty is a growing national problem as trends towards aging and obesity converge. This problem is especially acute in men with prostate cancer treated with androgen deprivation therapy (ADT). ADT is the backbone of therapy for advanced prostate cancer, a disease which affects mostly older men. But, ADT accelerates muscle loss and gains in adiposity which threaten physical functioning and quality of life. Diet and resistance training may mitigate obese frailty but have rarely been combined in an accessible delivery format. METHODS: The Internet-Based Lifestyle Intervention t

18F-FDG PET/CT and MRI findings of an atypical glomus tumor of the thoracic spine: a rare case report.

BACKGROUND: Glomus tumors (glomangiomas) are rare mesenchymal neoplasms derived from modified smooth muscle cells of the glomus body. Since they most commonly occur in the subungual regions of the extremities, primary involvement of the spine is exceedingly rare, particularly in atypical variants with borderline biological behavior. CASE PRESENTATION: A 61-year-old man presented with a long-standing history of intermittent back pain that had recently worsened. Magnetic resonance imaging revealed a solid lesion involving the T10-T11 vertebral region with intermediate T1 signal intensity, mildl

BPM31510 Increases the CoQ Pool in Chemically Induced CoQ-Deficient Cells, CoQ-Deficient Patient Fibroblasts, and in Metabolically Active Murine Tissues.

Coenzyme Q10 (CoQ10) is a lipid-soluble redox cofactor essential for mitochondrial electron transport, membrane stabilization, and antioxidant defense in its reduced form. Broad clinical utility has been hampered by poor oral bioavailability and low tissue uptake using nutraceutical formulations. BPM31510 is a novel pharmaceutical nanotechnology formulated with oxidized CoQ10 as a lipid nanoparticle designed to enhance systemic exposure and mitochondrial concentration. Using UHPLC-MS/MS, we quantified oxidized CoQ10, reduced CoQ10, and oxidized CoQ9 in BPM31510- and CoQ10-treated SH-SY5Y neuro

Atomoxetine plus oxybutynin for obstructive sleep apnoea: a randomised-controlled trial with detailed pathophysiology.

RATIONALE: Obstructive sleep apnoea (OSA) is a prevalent disorder with major health consequences for which there is no approved pharmacological therapy targeting upper airway muscle pathophysiology. A combination of a noradrenergic (atomoxetine) and antimuscarinic (oxybutynin) (AtoOxy) may meaningfully reduce apnoea-hypopnoea index (AHI), but further repeated-dose intervention data are needed. METHODS: 58 participants with moderate-to-severe OSA were randomised to receive AtoOxy (80/5 mg) and placebo in a 1-month cross-over study. Primary analysis quantified the effect of AtoOxy versus placeb

EV-Mediated Intracardiac Crosstalk Mitigates Doxorubicin-Induced Cardiotoxicity.

BACKGROUND: Extracellular vesicles (EVs) are involved in exercise-induced cardiac protection. However, the effects and underlying mechanisms of tissue-specific molecular cargo packaged within these EVs, including PIWI (P-element induced wimpy testis)-interacting RNA (piRNA), remain poorly understood. In particular, the mechanistic contribution of exercised intracardiac EV-associated piRNAs to doxorubicin-induced cardiotoxicity (DCT) has not been defined. METHODS: Transgenic reporter mice and a cardiomyocyte-specific Rab27a (RAB27A, member RAS oncogene family) knockout strategy were used to in

A combined progressive muscle relaxation and walking intervention for adults with end-stage kidney disease receiving hemodialysis (Fight Fatigue): Study development and protocol.

BACKGROUND: Fatigue has a profound negative impact on health for adults with end-stage kidney disease (ESKD) who receive maintenance hemodialysis. There are currently no standard-of-care therapies to alleviate this fatigue. Progressive muscle relaxation and physical activity have decreased fatigue in other populations. In collaboration with a patient partnership board, we developed an intervention aimed at reducing fatigue for adults receiving maintenance hemodialysis (Fight Fatigue), and we describe the protocol for its feasibility trial. METHODS: Fight Fatigue was informed by our formative

Models of the human heart for biomedical research: Opportunities and challenges.

Model systems that mimic human cardiac structure and function are essential for the development of novel diagnostics and effective treatments for cardiovascular diseases. While non-human vertebrate models, from zebrafish to pig, remain vital to cardiovascular research, the translatability of findings to human patients is often limited. Therefore, animal experiments should be supplemented with human model systems, including human induced pluripotent stem cell-derived cells, 3D engineered constructs, and last but not least, native tissue preparations and isolated primary cardiomyocytes. However,

BRCA1/2 and CHEK2 Pathogenic Variants in Urological Cancers: A Portuguese Single-Center Experience.

Background Germline pathogenic variants (gPVs) in BRCA1, BRCA2, and CHEK2 are well‑established cancer predisposition factors of prostate cancer. However, their contribution across the full spectrum of urological cancers remains insufficiently characterized. The primary objective was to describe the spectrum and outcomes of urological cancers in BRCA1/2 and CHEK2 gPV carriers, while secondary objectives were to assess overall survival (OS) and compare age at diagnosis, stage, and survival outcome by gene affected. Methods This is a retrospective descriptive study including patients diagnosed wi

Mitochondria-Dependent Metabolic Reprogramming Enhances Myofibroblast Differentiation and Aggravates Bleomycin-Induced Pulmonary Fibrosis.

Idiopathic pulmonary fibrosis (IPF) is a progressive interstitial lung disease characterized by irreversible fibrosis. Aberrant cell differentiation plays a crucial role in the development of IPF. Although recent studies have suggested that mitochondrial dysfunction may play a role in IPF, its direct impact on fibrosis remains unclear. This study aimed to clarify the role of mitochondria in lung cell differentiation and pulmonary fibrosis development by employing mito-mice ND6M, in which the activity of respiratory chain complex I is decreased due to a mitochondrial DNA mutation (G13997A). Economic Evaluation of Thymectomy for the Treatment of Nonthymomatous Myasthenia Gravis.

IMPORTANCE: Myasthenia gravis (MG) is a chronic autoimmune disease that significantly impairs patients' quality of life by causing fluctuating muscle weakness and fatigue. While pharmacological treatment with corticosteroids remains a cornerstone of treatment, long-term use is often associated with substantial adverse effects. Although thymectomy is an established and effective alternative and adjunct to pharmacological treatment, its long-term cost-effectiveness within the UK National Health Service (NHS) is unknown. OBJECTIVE: To evaluate the cost-effectiveness of thymectomy in addition to

Triglyceride deposit cardiomyovasculopathy: A new class of cardiovascular disease.

Triglyceride (TG) deposit cardiomyovasculopathy (TGCV) was first identified in Japanese patients with homozygous mutations in PNPLA2 encoding adipose triglyceride lipase. TGs and fatty acids released by lipolytic action are major energy substrates in a normal heart. In TGCV, the defective intracellular lipolysis of long-chain TGs causes cellular steatosis and energy failure mainly in cardiomyocytes and vascular smooth muscle cells. One of the major characteristics is diffuse narrowing TG-deposited coronary atherosclerosis which is distinct from classic cholesterol-induced focal lesions. TGCV c
