Myocardial Fibrosis and Early Intervention in Asymptomatic Patients With Severe Aortic Stenosis: Insights From the EVOLVED Randomized Clinical Trial.

IMPORTANCE: Myocardial fibrosis burden has been associated with adverse clinical outcomes in symptomatic patients with aortic stenosis. OBJECTIVE: To determine whether midwall myocardial fibrosis burden is associated with adverse clinical outcomes in asymptomatic patients and whether those with more fibrosis derive greater benefit from early intervention. DESIGN, SETTING, AND PARTICIPANTS: This post hoc analysis of a randomized clinical trial was conducted between August 2017 and October 2022. The trial took place at 24 cardiac centers across the United Kingdom and Australia. Participants in

Primary Prostatic Alveolar Rhabdomyosarcoma in Adult, a Diagnostic Pitfall: Report of an Extremely Rare Tumor.

IntroductionPrimary sarcoma of the prostate is extremely rare and accounts for less than 1% of all prostatic malignancies. Alveolar rhabdomyosarcoma arising from the prostate in an adult man is exceedingly rare. Establishing a correct diagnosis is often challenging in a prostatic core biopsy.Patient presentationA 30-year-old man presented to the emergency department after a period of acute urinary retention. Clinical examination revealed an indurated prostatic mass. Serum prostate-specific antigen level was normal. Imaging work-up showed pelvic lymph node and vertebral metastases. A malignant

Rats lacking emerin develop muscle pathologies and molecular alterations found in humans with X-linked EDMD.

Emery-Dreifuss Muscular Dystrophy (EDMD) is a progressive disease characterized by cardiac and skeletal muscle dysfunction. A primary cause of EDMD is loss of function of the X-chromosome gene emerin (EMD). Although emerin mutations were discovered over three decades ago, X-linked EDMD (X-EDMD) remains understudied largely due to the absence of an animal model with pathological features found in humans. Here, we show that rats lacking emerin (EMD(-/y)) develop motor issues and suddenly die, a major risk factor for patients with X-EDMD. Additionally, EMD(-/y) rats present with other hallmarks o

Antimitochondrial Antibody-Negative Primary Biliary Cholangitis in a Patient with a History of Colon Cancer and Chemotherapy: A Case Report.

BACKGROUND: Primary biliary cholangitis (PBC) is a chronic, immune-mediated liver disease with progressive destruction of bile ducts, cirrhosis, and liver failure. Diagnosis is usually based on detecting antimitochondrial antibodies (AMA), found in 95% of patients with PBC. However, some patients test negative for AMA and other antibodies, which complicates the diagnosis process. CASE PRESENTATION: We report a case of a 57-year-old Syrian woman who presented with fatigue, jaundice, pruritus, and abdominal pain, and her medical history included colon cancer and chemotherapy. Laboratory tests s

Glucagon-like peptide-1: a critical link between gut microbiota dysbiosis and degenerative musculoskeletal diseases.

Degenerative musculoskeletal diseases are characterized by the progressive loss of structural integrity and functional capacity in muscles, bones, joints, and other related tissues. These conditions primarily include osteoarthritis, osteoporosis, sarcopenia, and intervertebral disc degeneration. These diseases progress gradually, leading to chronic pain, impaired mobility, and a significantly elevated risk of disability. Given their high prevalence and substantial health burden, degenerative musculoskeletal diseases pose a major challenge in the context of population aging. Although various ty

Skeletal muscle index and hand grip strength as predictors of postoperative recovery in patients with gastrointestinal tumors.

BACKGROUND: Sarcopenia, defined by loss of muscle mass and function, predicts poor cancer outcomes. In gastrointestinal (GI) malignancies, its prognostic role remains underexplored. Skeletal muscle index (SMI) from computed tomography (CT) scans and hand grip strength (HGS) may contribute to preoperative risk assessment. This study aimed primarily to evaluate the association between preoperative SMI and postoperative hospitalization length, and secondarily to assess HGS as a functional measure related to muscle status and its association with SMI and other clinical factors in GI cancer patient

Causal relationship between epigenetic markers and type 2 diabetes in West African populations: a Mendelian randomisation analysis.

AIMS/HYPOTHESIS: Evidence for a causal role of DNA methylation sites (CpGs) in type 2 diabetes and glycaemic traits is limited due to the cross-sectional nature of many epigenome-wide association studies (EWAS). In addition, epigenetic studies in West African populations are particularly sparse, despite the high and rising burden of type 2 diabetes in these populations. Hence, we aimed to identify CpGs causally associated with type 2 diabetes among West Africans by leveraging Mendelian randomisation (MR) analysis and longitudinal data. METHODS: We used the Illumina EPIC DNA methylation array

Systemic Drivers and Molecular Mechanisms of Sarcopenia in Aetiology-Specific End-Stage Liver Disease.

BACKGROUND: Patients with end-stage liver disease (ESLD) often present with sarcopenia, defined as loss of skeletal muscle mass and quality, which is associated with reduced quality of life and increased mortality. However, the molecular mechanisms driving sarcopenia in ESLD are not fully understood and there are currently no therapeutic interventions. This study aimed to identify potential circulating factors contributing to sarcopenia progression in ESLD by assessing their role in driving transcriptomic alterations in skeletal muscle. METHODS: Quadriceps muscle tissue, plasma and serum were

A severe neurodevelopmental syndrome linked to a South Asian founder variant in the UFMylation adaptor CDK5RAP3.

We investigated the pathogenicity of a homozygous intronic variant in CDK5RAP3, a key UFMylation adapter, in three individuals from two unrelated families with a lethal neurodevelopmental disorder. CDK5RAP3 variants have not been linked to human disorders to date; however, murine Cdk5rap3 knockout is embryonic lethal and variants in five other UFMylation components cause severe neurodevelopmental conditions. A segregating homozygous variant, chr17(GRCh38):g.47974691G > A, CDK5RAP3 NM_176096.3:c.334 + 243G > A, was identified by trio whole-genome and proband RNA sequencing in Family A and by tr

Strategic role of calpains in modulation of NF-κB activity in skeletal muscle.

Calpains (CAPN) serve a crucial function in the regulation of NF-κB activity within skeletal muscle by mediating the degradation of IκBα, facilitating NF-κB activation, inhibiting apoptosis through the suppression of c-FLIP, modulating the activity of p65, p52, and p50, and enhancing the expression of its inhibitor, cardiac ankyrin repeat protein, among other roles. Through these coordinated actions, CAPNs fine-tune inflammatory and survival signaling in muscle cells. The lack of CAPNs leads to increased apoptotic myonuclei and a range of pathological conditions, including muscle weakness, imp

End-stage valvular heart disease requiring heart transplantation: risk profile, surgical modifiers, and myocardial pathology.

BACKGROUND: End-stage valvular heart disease (VHD) constitutes a small but high-risk subset of heart transplant (HTx) recipients, and data describing their natural history and determinants of progression to HTx are limited. We aimed to characterize the clinical, genetic, surgical, and pathological substrates of patients undergoing HTx for end-stage VHD and to assess whether timely cardiac valvular surgery (CVS) mitigates progression to end-stage heart failure. METHODS: We retrospectively screened 961 consecutive recipients who underwent HTx for end-stage heart failure between January 2012 and

Anabolic Androgenic Steroids: Emerging Use Disorder, Mechanisms of Action, and Clinical Considerations for Pharmacotherapy.

Anabolic androgenic steroids (AAS) are synthetic versions of testosterone that are frequently used as performance-enhancing drugs. Over the past few decades, these drugs have become more popular and readily available through online clinics and retail outlets. At the same time, increasing evidence has demonstrated their addictive potential and the likely emergence of an AAS use disorder. In addition to their impact on reward pathways, which is similar to that of other addictive substances, AAS have an addiction-related unique mechanism involving hormonal dysregulations of the hypothalamic-pitui

Myosteatosis Predicts Poor Prognosis in Patients With Castration-Resistant Prostate Cancer Treated With Enzalutamide.

OBJECTIVES: Prostate cancer prognostication has traditionally emphasized tumor-related factors (e.g., Gleason score and metastatic burden). Emerging evidence suggests that physical (host) factors influence outcomes. This study evaluated whether physical factors, including myosteatosis and sarcopenia, are prognostic factors in patients with androgen receptor signaling inhibitor-naïve, castration-resistant prostate cancer. METHODS: We retrospectively analyzed 117 androgen receptor signaling inhibitor-naïve patients with castration-resistant prostate cancer who received enzalutamide treatment at

Pharmacological Interventions in the Management of Temporomandibular Disorders: A Systematic Review and Meta-analysis.

INTRODUCTION: Temporomandibular Disorders (TMDs) encompass a set of conditions affecting the masticatory muscles and the temporomandibular joint (TMJ), with an impact on both functionality and individuals' quality of life. Myalgia is a diagnosis of TMD with multifactorial etiology, and treatment strategies aim primarily to relieve pain and restore function. Pharmacological therapy is commonly considered a second-line option, thus complementing conservative measures. OBJECTIVES: Assess the efficacy of pharmacological interventions in the management of myalgia associated with temporomandibular
