Not Just for Kids: A Systematic Review of Outcomes of the Thenar Flap.

Fingertip injuries are common, and the thenar flap is a well-described technique used to maintain digital length. However, its use in patients aged 30 years and above is generally discouraged due to concerns regarding postoperative joint contracture. The purpose of this review was to evaluate whether these concerns are substantiated. A search of PubMed, Embase, and SCOPUS (1947-2025) identified 15 studies involving 519 flaps. Case reports and studies lacking functional outcome data were excluded. Bias was assessed using the Methodological Items for Non-Randomized Studies instrument, and result

Assessing the Credibility of ChatGPT on Temporomandibular Disorders.

INTRODUCTION: Large Language Models such as ChatGPT are increasingly used in dental education; however, their credibility in clinical contexts remains uncertain. This study is aimed to analyse the credibility and efficacy of responses given by ChatGPT about temporomandibular disorders among dentists and dental students. MATERIALS AND METHODS: Nine questions related to TMDs were posed to ChatGPT 3.5, and its responses were used to create an online survey. A total of 115 participants (60 dental students and 55 dentists) rated each response on a five-point Likert scale. Additionally, a Delphi pa

Psychological Determinants of Functionality in Chronic Musculoskeletal Pain: A Structural Equation Modeling Approach.

OBJECTIVE: Beyond nociceptive mechanisms, the experience of pain is shaped by emotional, cognitive, and behavioral processes. The main hypothesis of this study is that beliefs about pain and intolerance of uncertainty may influence susceptibility to catastrophic pain, and that catastrophic pain may determine the severity of chronic pain and the limitation of daily life activities. DESIGN: A total of 173 patients with chronic musculoskeletal pain were assessed using the Pain Belief Questionnaire, Intolerance of Uncertainty Scale-12, Pain Catastrophizing Scale, and the Brief Pain Inventory. Increased IL-23 is Associated to Heliotrope Sign in Multivariate Model on Idiopathic Inflammatory Myopathies.

Heliotrope rash is a pathognomonic feature of dermatomyositis (DM); however, the molecular pathways implicated in cutaneous involvement have not been fully explored. Although the IL-23/Th17 axis has been implicated in inflammatory and autoimmune diseases, the clinical significance of circulating IL-23 across the idiopathic inflammatory myopathy (IIM) spectrum, particularly in cutaneous disease expression, remains insufficiently defined. In this cross-sectional study, we aimed to quantify serum IL-23 levels in patients with IIM and evaluate their association with cutaneous involvement, includin

Predicting Progressive Pulmonary Fibrosis in Idiopathic Inflammatory Myopathy-associated Interstitial Lung Disease: A Machine Learning Approach Integrating Quantitative CT and Clinical Features.

BACKGROUND: The prognosis of progressive pulmonary fibrosis (PPF) in idiopathic inflammatory myopathy-associated interstitial lung disease (IIM-ILD) is poor, and current strategies for predicting its development are limited. This study aimed to use machine learning to integrate quantitative computed tomography (QCT) and clinical features to predict PPF in patients with IIM-ILD. METHODS: We retrospectively included 180 IIM-ILD patients, of whom 86 developed PPF. We randomly assigned patients to training and validation sets in a 7:3 ratio. Least absolute shrinkage and selection operator regress

[Gastrointestinal manifestations in idiopathic inflammatory myopathies : Dysphagia, intestinal vasculopathy and pneumatosis cystoides intestinalis].

In idiopathic inflammatory myopathies (IIM), gastrointestinal manifestations are rare and poorly recognized apart from the relatively common dysphagia. Oropharyngeal or esophageal dysphagia in IIM is associated with aspiration and malnutrition and responds well to conventional immunomodulation except for inclusion body myositis. The rare intestinal vasculopathy in nuclear matrix protein 2 (NXP2)-associated dermatomyositis affects both children and adults, causing life-threatening perforation and bleeding from the esophagus to the colon with a high mortality. It responds poorly to standard immu

Simultaneous Carpal Tunnel Release With Forearm Fasciotomy: Are We Doing Enough?

BACKGROUND: Simultaneous carpal tunnel release (sCTR) with emergent forearm fasciotomy in the setting of acute compartment syndrome (ACS) has been reported, but the national incidence and cost of delayed carpal tunnel release (CTR) remain unknown. This large database study aims to evaluate how often sCTR is performed, the frequency of delayed CTR when not performed simultaneously, and the associated costs. METHODS: The PearlDiver Mariner database was queried for patients who underwent forearm fasciotomies between January 1, 2015, and October 31, 2022. Patients were grouped based on CTR timing

Leveraging the Iron-Probe-MurB Interaction: An Enhanced Platform for PET Imaging of Bacterial Infections.

The accurate discrimination of bacterial infections from sterile inflammation is a critical yet unresolved challenge in clinical diagnostics. To address this, we designed and synthesized a series of 68Ga-labeled PET tracers derived from the FDA-approved iron chelator, deferasirox. The probes, denoted [68Ga]Ga-DFC2D, [68Ga]Ga-DFA2D, and [68Ga]Ga-DFA3D, were systematically evaluated for their Fe(III) chelation ability, stability, and bacteria-binding selectivity. In a murine model of bacterial myositis, [68Ga]Ga-DFA2D emerged as the lead candidate, demonstrating optimal pharmacokinetics and, mos

Genotype-phenotype relationship in a cohort of 131 Chinese patients with myotonic dystrophy type 1.

INTRODUCTION: Myotonic dystrophy type 1 (DM1) is an autosomal dominant dynamic mutation disorder characterized by myotonia and muscle weakness. The genotype-phenotype relationship has not been clearly defined in Chinese patients. METHODS: The clinical and genetic characteristics of 131 patients from 105 pedigrees were retrospectively analyzed. Triplet-primed Polymerase Chain Reaction (PCR) and Flanking PCR were performed to analyze the CTG repeats in DMPK gene. RESULTS: Among 131 DM1 patients, 64.9% were male, indicating a higher male admission rate, possibly due to limited diagnostic awaren

Clinician characteristics and the use of nonpharmacological treatment for acute musculoskeletal pain in an active-duty military population.

This study aimed to examine the degree to which facility- and clinician-level information accounts for nonpharmacological treatment (NPT) receipt early in a musculoskeletal pain treatment episode. We examined care provided within the US Military Health System (MHS) from 2016 to 2021, using a cross-sectional retrospective observational design with multivariable logistic regression analysis. The primary outcome was receipt of any nonpharmacological treatment, and the secondary outcome was receipt of nonpharmacological treatments before pharmacological treatment (opioid or muscle relaxant), in bo

Skeletal muscle transcriptomic profile of older adults with high-impact musculoskeletal pain in the study of muscle, mobility and aging (SOMMA).

High-impact musculoskeletal pain (i.e., pain that limits daily activities and function) affects a substantial proportion of older adults and is associated with accelerated mobility decline. While skeletal muscle is central to movement and functional capacity, its molecular contribution to pain-related disability in aging remains poorly understood. We conducted a cross-sectional analysis of baseline data from 742 community-dwelling adults aged 70 years or older from the Study of Muscle, Mobility and Aging (SOMMA). Bulk RNA sequencing was performed on vastus lateralis muscle biopsies. Pain impac

Biomechanical comparison of parallel versus convergent screw fixation in Zadek osteotomy.

PURPOSE: Zadek osteotomy is an increasingly utilized treatment for insertional Achilles tendinopathy (IAT), although an optimal fixation strategy remains unclear. This cadaveric study compared biomechanical outcomes of parallel and convergent screw fixation constructs following Zadek osteotomy. We hypothesized that convergent screw fixation would demonstrate greater stability. METHODS: Five paired fresh-frozen human calcanei (10 specimens) underwent simulated Zadek osteotomy, receiving paired parallel and convergent screw fixations on contralateral limbs. Specimens were tested using a servohy

Pediatric Upper Extremity Compartment Syndrome: An Analysis of Inpatient Outcomes.

Pediatric acute compartment syndrome (PACS) results from increased pressure within a fibro-osseous space, reducing local limb perfusion. It can be traumatic or non-traumatic, often due to fractures, burns, or iatrogenic compression. PACS is challenging to diagnose in children due to insidious onset. While lower extremity PACS is well studied, upper extremity PACS remains underexplored. This study analyzes characteristics and inpatient outcomes of pediatric upper extremity PACS. The National Inpatient Sample (2015 - 2019) was queried for patients under 18 diagnosed with ACS. ICD-10 codes identi

Co-existence of CLCN1 and DNM2 variants in a phenotype overlapping myotonia and myopathy.

BACKGROUND: Myotonia congenita (MC) is characterized by impaired muscle relaxation following contraction. Autosomal dominant centronuclear myopathy (ADCNM) is characterized by centrally located nuclei and muscle weakness. We aimed to identify potential pathogenic variants responsible for a phenotype overlapping severe myotonia and myopathy in an Iranian family. METHODS: We performed whole-exome sequencing on an index patient with clinical/electrical myotonia and myopathic changes on electromyography (EMG). We confirmed candidate genes by Sanger sequencing and performed co-segregation analyses

Progress on Duchenne muscular dystrophy: from molecular mechanisms to therapeutic strategies.

Duchenne muscular dystrophy (DMD) is the most common lethal neuromuscular genetic disorder in children. It is caused by mutations in the DMD gene that abolish the expression of dystrophin, a protein essential for maintaining sarcolemmal integrity during muscle contraction. Its loss renders muscle fibers susceptible to contraction-induced injury, leading to progressive muscle degeneration and, ultimately, fatal cardiac and respiratory failure. In this review, we summarize the diagnostic framework, molecular pathogenesis, and current therapeutic strategies of DMD, with particular emphasis on the

Neurofeedback for Management of Musculoskeletal Pain: A Randomized Control Trial Using DOMS.

INTRODUCTION: Chronic musculoskeletal (MSK) pain is associated with maladaptive neuroplasticity, often reflected in altered alpha activity (8-13 Hz). Neurofeedback may offer a way to modulate this activity, yet most protocols target alpha activity during rest, despite movement being a common pain trigger. This study investigated whether neurofeedback during movement could modulate alpha activity and reduce pain in healthy individuals experiencing induced pain via delayed onset muscle soreness (DOMS). METHOD: This experiment was a randomized, two-armed, parallel-group trial. Twenty-nine partic
