Combined Effect of Hypnotic-Cognitive Therapy With Therapeutic Exercise on Quality of Life in Individuals With Fibromyalgia: A Randomized Controlled Trial Protocol.

BACKGROUND: A multidisciplinary approach is viewed as the primary treatment strategy for fibromyalgia (FM). Therapeutic exercise is an important component of this approach. Hypnotic cognitive therapy (HYP-CT), a relatively new intervention strategy, has not traditionally been incorporated in FM treatment, although it has shown beneficial effects in other chronic pain conditions. This study aims to investigate the potential benefits of a combined approach that incorporates HYP-CT with therapeutic exercise in the rehabilitation of individuals with FM. MATERIALS AND METHODS: One hundred and four

Responsiveness and clinical correlates of continuous turns-amplitude analysis measures in myositis: A prospective longitudinal study.

OBJECTIVE: Turns-amplitude analysis (TAA) is a rapid quantitative electromyographic technique used in neuromuscular disorders, but its categorical interpretation (normal, myopathic, neurogenic) limits the granularity needed for longitudinal monitoring. We investigated whether continuous TAA-derived metrics could reflect longitudinal clinical change in idiopathic inflammatory myopathies (IIM). METHODS: In this prospective single-center study, 30 adults with IIM underwent TAA of biceps brachii, quadriceps, and iliopsoas at baseline, 6, and 12 months. From each cloud, we extracted descriptors of

Complex presentation of neonatal arthrogryposis multiplex congenita: insights into a multifactorial disorder.

This case report details a neonate diagnosed with arthrogryposis multiplex congenita (AMC), specifically the amyoplasia subtype, presenting with bilateral talipes equinovarus, elbow contractures, ulnar deviation of the wrists and clinodactyly. Additional clinical findings included mild developmental dysplasia of the hips and a small atrial septal defect. Genetic analysis revealed heterozygous variants of uncertain significance in the SYNE1 and CHD4 genes, both of which were inherited from asymptomatic parents. While SYNE1 is associated with autosomal recessive AMC type 3 and CHD4 with Sifrim-H

Reflections on 20 Years of Research on Tai Chi for Chronic Musculoskeletal Pain: An Updated Review.

PURPOSE OF REVIEW: Chronic musculoskeletal pain (CMP), including osteoarthritis (OA), rheumatoid arthritis (RA), fibromyalgia, and chronic low back pain (cLBP), contributes substantially to disability and reduced quality of life. Contemporary pain science supports biopsychosocial, multimodal management addressing nociceptive, neuropathic, and nociplastic mechanisms. Tai Chi is a multicomponent mind-body exercise integrating gentle movement, breathing, relaxation, balance, and attentional focus. This narrative review summarizes approximately 20 years of research on Tai Chi for CMP and considers

Patients know best: DESTINIES Patient Panel appraisal of a novel, risk-stratified approach to COVID-19 disease surveillance in the adult immunosuppressed.

The immunosuppressed are poorly represented in disease surveillance and pharmacovigilance dataflows, proving especially consequential during the COVID-19 pandemic. This mixed methods study recruited a panel of immunosuppressed patients (DESTINIES Patient Panel) to quality assure a new digital phenotype for COVID-19 risk in the adult immunosuppressed (DESTINIES phenotype) prior to its implementation in real-world surveillance. Applying their lived experiences, the DESTINIES Patient Panel met to critique the strategy and structure of the provisional phenotype before voting on its adoption in UK

Managing growth and puberty concerns in individuals with Duchenne muscular dystrophy: Updated guidance and future directions.

Long-term glucocorticoid therapy is standard of care for individuals with Duchenne muscular dystrophy (DMD) but often results in impaired growth and delayed or incomplete puberty, impacting their physical and psychosocial health. Although DMD endocrine care has evolved, leading to inclusion in the 2018 Care Considerations, these issues remain prevalent and concerning, and gaps in care persist. The Growth and Puberty Working Group of the OPTIMIZE DMD Consortium, an international multidisciplinary group including endocrinology and neuromuscular specialists with DMD expertise, conducted a literat

Newborn Screening of Primary Carnitine Deficiency and Prenatal Genetic Diagnosis for a Recurrence-Risk Family.

BACKGROUND: Primary carnitine deficiency (PCD) is a treatable autosomal recessive disorder caused by SLC22A5 gene variants and included in newborn screening widely. However, its prevalence and variant spectrum vary greatly, and prenatal diagnosis for recurrence-risk families remains rare. This study assessed tandem mass spectrometry (MS/MS) combined with genetic testing in NBS and prenatal diagnosis of PCD in Yancheng, China, and described the local prevalence and variant characteristics. METHODS: From 2015 to 2024, free carnitine (C0) levels were measured in neonatal dried blood spot (DBS) s

Rhabdomyolysis Following Robot-Assisted Radical Prostatectomy.

Robot-assisted radical prostatectomy remains a favorable treatment option for locally advanced prostate cancer in patients with obstructive lower urinary tract symptoms. However, the steep Trendelenburg lithotomy position required for this procedure can result in serious complications, particularly in patients with obesity. Here, the case of a 66-year-old man with obesity (BMI, 35 kg/m2), insulin-dependent diabetes mellitus, and multiple comorbidities is reported, who underwent robot-assisted radical prostatectomy (console time, approximately 4 h) for clinically staged cT3a cN1 prostate cancer

Clinical and Laboratory Dynamics of Bear Meat-Associated Trichinellosis in Hokkaido, Japan.

Trichinellosis is a rare infection associated with bear meat consumption found in Japan, where limited access to parasite-specific antibody testing makes definitive diagnosis challenging. In the present study, 19 patients, comprising 16 confirmed and three probable cases, treated at a tertiary care hospital in Hokkaido, Japan, during three independent outbreaks in 2018, 2019, and 2025, were retrospectively analyzed. Patients prepared brown bear meat as a roast (n = 18) or a cutlet (n = 1) and consumed it as a main dish (n = 7) or in smaller portions of a few slices (n = 12). Ten patients consu

Atrial Standstill: From Molecular Genetics to Clinical Management - A Narrative Review.

Atrial standstill (AS) is the complete absence of atrial electrical and mechanical activity caused by diffuse atrial myocardial disease, distinct from sinus node dysfunction. First documented in 1897 and often misdiagnosed as atrial fibrillation, it arises from SCN5A loss-of-function, NPPA, or EMD/LMNA mutations, muscular dystrophies, and infiltrative or idiopathic fibrotic processes. Diagnosis requires absent P waves, absent atrial electrograms, and failure of atrial capture at maximal pacing output, with late gadolinium enhancement MRI and electroanatomic mapping demonstrating diffuse fibros

Clinical Neurology in Practice: The Shoulder (Part 1).

BACKGROUND: The shoulder's complex anatomy and biomechanics render it uniquely susceptible to neurological disorders, spanning radiculopathies, plexopathies, and myopathies. Despite its clinical relevance, the shoulder remains understudied in neurological practice, where its dysfunction often serves as an early marker of underlying pathology. A comprehensive understanding of its anatomic, neural, and vascular foundations is essential for accurate diagnosis and management. REVISION SUMMARY: This review focuses on the shoulder's osteoligamentous architecture, motor and sensory innervation, vasc

Co-Occurrence of Facioscapulohumeral Muscular Dystrophy and Myasthenia Gravis: A Systematic Review and Meta-Analysis of Individual Patient Data.

BACKGROUND: Co-occurrence of facioscapulohumeral muscular dystrophy (FSHD) and myasthenia gravis (MG) is rare but clinically critical, as treatable MG symptoms may be misattributed to FSHD progression. This systematic review and meta-analysis aimed to comprehensively characterize patients with both conditions and to quantitatively synthesize the available individual patient data. METHODS: Following PRISMA 2020 guidelines, we searched four databases until 3 April 2026 for original reports providing individual-patient data on confirmed FSHD and MG co-occurrence. We performed descriptive and qua

Myositis Ossificans Affecting the Rotator Cuff Muscles and Proximal Humerus: A Rare Clinical Entity.

INTRODUCTION: Myositis Ossificans (MO) is a benign condition characterized by the formation of non-neoplastic heterotropic ossification in extra-skeletal soft tissues. It is generally associated with preceding trauma; however, cases of atraumatic MO have also been reported in the literature. While the elbow is the most common site, MO can occur anywhere in the body and is particularly common in young male athletes in the second to third decade of life. MO involving the rotator cuff muscles is an extremely rare entity with its pathology poorly understood till date. In such cases where it origin

Myofascial pain with psychosomatic features in an octogenarian female gastric cancer survivor treated with a psychosomatic approach: a case report.

BACKGROUND: Non-cancer-related pain in cancer survivors can be difficult to assess when symptoms are interpreted as cancer progression and when impaired bodily awareness limits engagement in self-care. Myofascial pain with psychosomatic features accompanied by marked alexisomia has rarely been described in gastric cancer survivors. This case report describes the clinical application of a structured, multi-stage psychosomatic approach for refractory myofascial pain with psychosomatic features in an octogenarian female gastric cancer survivor. CASE DESCRIPTION: An octogenarian woman with recurr

Gua Sha (Scraping Therapy) for Spasticity After Stroke: Systematic Review and Meta-Analysis of RCTs with Integrated Preclinical Mechanistic Synthesis.

Emerging clinical data from Chinese randomized controlled trials (RCTs) suggest Gua Sha (scraping therapy) may deliver adjunct antispastic benefits for spasticity after stroke (SAS) when combined with standard rehabilitation. This study aimed to synthesize clinical efficacy evidence and explore treatment parameters for this intervention. We searched eight electronic databases from database inception to January 1, 2026; two independent reviewers completed study screening, risk-of-bias assessment using Cochrane Risk of Bias 2.0 tool, and data extraction. The Role of TXNIP in Skeletal Muscle Physiology and Related Diseases: Current Perspectives.

Thioredoxin-interacting protein (TXNIP) is a multifunctional regulatory protein of the α-arrestin family that acts as a redox sensor, an energy-responsive molecule, and a transcription-associated regulator. TXNIP has emerged as a key regulator of skeletal muscle homeostasis and has been implicated in the pathogenesis of multiple muscle disorders. Skeletal muscle is a principal site of systemic glucose disposal, accounting for more than 70% of insulin-stimulated glucose uptake in the human body. TXNIP has been implicated in sarcopenia, obesity-related insulin resistance, limb ischemia-induced m

A NON-CANONICAL ROLE FOR NOTCH3 IN BUILDING THE INTESTINAL LYMPHATIC NICHE.

Lymphatic dysfunction drives severe and often intractable human diseases, yet the cellular mechanisms that establish functional lymphatic vasculature remain poorly understood. In the intestine, lacteals are specialized lymphatic vessels that absorb dietary lipids and rely on surrounding villus smooth muscle to propel lymph, forming the muscular-lacteal complex (MLC). How distinct mesenchymal populations coordinate assembly of this functional lymphatic unit remains unknown. By integrating developmental single-cell profiling, genetic lineage tracing, conditional mouse genetics, and functional as
