Ethanol acutely decreases calcium transients in cultured human myotubes.

Ethanol consumption frequently leads to a number of skeletal muscle disorders, including acute and chronic alcoholic myopathy. Ethanol has been found to interfere with signal transduction mechanisms in cardiac and smooth muscle cells. We studied the effects of ethanol on the intracellular calcium ([Ca2+]i) transients responsible for excitation-contraction coupling in human myotubes from chronic alcoholic patients and healthy controls. Cultured myotubes were loaded with the fluorescent Ca2+ indicator fura-2 and evaluated on a single-cell basis. Following electrical stimulation, ethanol caused a

Natural history of alcoholic myopathy: a 5-year study.

Chronic myopathy is a common complication of alcoholism, but its natural history has not been well described. We, therefore, studied muscle structure and function in a 5-year study of 30 chronic alcoholics who became abstinent and 20 who relapsed, and 40 control subjects. The mean strength of the abstaining alcoholics increased from 18.6 to 23.2 kg; but, after 5 years, they were still substantially weaker than controls. In a subset who showed histological myopathy, the strength of half of the patients remained two standard deviations below that of controls. Alcoholics who relapsed tended to be

Dantrolene sodium reduces the enhanced leakage of creatine kinase caused by ethanol, cocaine, and electrical stimulation in isolated fast and slow muscles of rat.

Binge drinking of alcohol, cocaine overdose, or overexertion can lead to rhabdomyolysis characterized by elevated creatine kinase (CK) and myoglobin in the serum, myoglobinuria, and muscle tenderness. Our previous studies showed that ethanol, cocaine, and electrical stimulation enhanced the leakage of CK from isolated soleus and extensor digitorum longus (EDL) muscles of rat. Dantrolene sodium was reported to reduce the muscle damage and elevated serum CK levels in exercised rats. The present study was aimed at testing whether dantrolene can reduce the enhanced leakage of CK from isolated rat

Alcoholic myopathy: evaluation with magnetic resonance imaging--a case study.

Magnetic resonance imaging (MRI) of thigh and leg muscles was performed in a patient with alcoholic myopathy showing myalgia, hypercreatine kinasemia, and hypermyoglobinemia. High signal intensities in both T1- and T2-weighted images were widely distributed in the affected muscle groups, which most likely reflected lipid accumulation. Although he had hypermyoglobinemia, MRI and muscle biopsy did not show findings of rhabdomyolysis, such as necrosis, regeneration, and edema. We suggest that the high signal intensities in this case may have indicated "prerhabdomyolysis" related to alcohol abuse

[Study of ultrastructural alterations in gastrocnemius muscle of C57 BL10 mice after prolonged ethanol ingestion].

The effects of chronic alcoholism on gastrocnemius muscle of well-nourished mice were morphologically studied to test the direct toxic role of ethanol on skeletal muscle. Thirty male young adult C57BL10 mice were divided in two groups: Group A (control) consisting of ten mice that drank water and Group B (alcoholic) consisting of twenty mice that drank 25% ethanol. All mice were allowed a balanced laboratory chow. The animals were kept on this ad libitum regimen under the same conditions of environment for 48 weeks and were weighed once a week. The daily dietary consumption and caloric intake

Effect of alcohol and electrical stimulation on leakage of creatine kinase from isolated fast and slow muscles of rat.

Binge drinking of alcohol may lead to acute alcoholic myopathy with rhabdomyolysis, which is characterized by skeletal muscle damage, elevated serum creatine kinase (CK), and myoglobinuria. This study was undertaken to test whether alcohol acts directly on the skeletal muscles to enhance the leakage of CK, and to assess the influence of fiber-type composition and repetitive contractions of the muscle on the effect of alcohol. After 4 hr of incubation in normal physiological solution at 37 degrees C, mean leakage of CK was 0.7 units/mg from isolated rat extensor digitorum longus (EDL), which ha

[Acute rhabdomyolytic alcoholic myopathy associated with acute alcoholic hepatitis].

Acute rhabdomyolysis (ARM) is a clinico-biological syndrome characterized by the lesion of the skeletal muscle resulting in a disorder of the permeability at the membrane of the muscle cells, with the release of intracellular substances to the plasma, mainly proteins and ions. We present the case of a chronic alcoholic patient which, after a period of several days in which he increased his consumption of alcohol, developed clinical signs of acute rhabdomyolytic alcoholic myopathy (AREM) associated to acute alcoholic hepatitis (AAH). In the review of the literature, we did not find any descript

Chronic alcoholic myopathy: diagnostic clues and relationship with other ethanol-related diseases.

We report the clinical, laboratory, functional and histological features of 100 male alcoholic patients of whom 44 had chronic alcoholic myopathy (CAM). We evaluated the use of non-invasive tests in detecting CAM, and examined its relationship with other ethanol-related diseases such as cirrhosis and cardiomyopathy. Of the CAM patients, 24 (55%) presented clinical symptoms of myopathy, whereas proximal muscle atrophy was observed in 15 patients (35%). Thirty-seven (80%) had significantly decreased muscle strength by myometric measurement and 27 (60%) had abnormally increased serum muscle enzym

Use of dynamic tests of muscle function and histomorphometry of quadriceps muscle biopsies in the investigation of patients with chronic alcohol misuse and chronic fatigue syndrome.

Ischaemic lactate/ammonia tests, serum carnosinase and creatine kinase assays and percutaneous needle muscle biopsies were performed on 10 patients with chronic fatigue syndrome (CFS), and 10 with chronic alcohol misuse complaining of muscular symptoms. Basal serum lactate levels were significantly elevated in the alcohol misusers compared to the CFS patients, but all were within the reference range. Lactate profiles after ischaemic forearm exercise did not differ significantly for the two patient groups. In one patient previously diagnosed as having CFS, myoadenylate deaminase deficiency was

[Acute alcoholic myopathy].

A case of acute alcoholic myopathy in a subject with a 20 year history of alcohol abuse is described. The condition emerged during an episode of delirium tremens and had a quickly deadly outcome. The rapid evolution of the clinical situation accompanied by variation in the biohumoral indices (CPK, LDH, myoglobin) is stressed and although this prevented from obtaining anatomopathological samples of the rhabdomyolisis, the specific diagnosis was made with reasonable certainty. The onset of the condition was not foreseeable because of the absence of an accurate history of the subject's alcohol ab

Chronic alcoholic myopathy and nutritional status.

To investigate the prevalence of alcoholic myopathy and its relationship to the nutritional status, we performed a muscle biopsy on the vastum lateralis of 60 consecutive hospitalized alcoholic patients using a Tru-Cut needle, processing it for light microscope and ultrastructural analysis. The nutritional status was assessed by anthropometric measurements such as midarm circumference, triceps skinfold and midarm muscle area, and serum albumin. The hallmark of chronic alcoholic myopathy, fiber muscle atrophy, was present in 33% of the patients, necrosis scarcely being observed (1.5%). Ultrastr

Chronic alcoholic myopathy: transcription and translational alterations.

Alcoholic myopathy is characterized by selective atrophy of type II fibers and affected subjects lose up to 20% of the entire musculature. Between one- and two-thirds of all alcohol abusers are affected. In acute studies, defective rates of translation occur and type II fiber-predominant muscles are more adversely affected than type I fiber-predominant muscles. Furthermore, acetaldehyde is also a potent modulator of translation, and contractile and noncontractile proteins are affected equally. In chronic ethanol feeding there is rapid and sustained loss of ribosomal RNA. Recent attention has f

The relationship between muscle fibre atrophy factor, plasma carnosinase activities and muscle RNA and protein composition in chronic alcoholic myopathy.

The relationship between chronic ethanol consumption and muscle biopsy morphometry (i.e. atrophy factor), plasma analytes, including carnosinase activities and tissue composition was investigated. In approximately half of chronic alcohol misusers there was Type II-fibre atrophy, which was correlated with reductions in muscle protein and serum carnosinase activities. The protein composition was also correlated with RNA composition. These results directly implicate defects in protein and RNA turnover as characteristics of chronic alcoholic myopathy and re-affirms the routine diagnostic use of fi

Free radicals and muscle damage.

Muscle tissue is unique in its requirement and ability to undertake very rapid and co-ordinated changes in energy supply and oxygen flux during contraction. Several studies have suggested that this renders the tissue particularly prone to oxygen radical-mediated damage. Free radicals have been postulated to play a role in muscle damage induced by different forms of exercise and in various pathological disorders, such as the muscular dystrophies, malignant hyperthermia and alcoholic myopathy. However, conclusive evidence for a fundamental role for free radicals and protective effect of antioxid

Ribosomal RNA activity and protein in skeletal muscles of chronic ethanol-fed rats.

The effect of prolonged ethanol exposure on ribosomal RNA activity and the content of RNA and protein in skeletal muscles of 15- and 22-25-month-old rats was evaluated. Experimental rats were fed a liquid diet containing 6.7% ethanol for 2, 4 and 6 months, and control rats were pair-fed an isocaloric diet. The in vivo incorporation of [3H]puromycin into nascent peptides on messenger RNA-ribosome complexes was determined to assess muscle ribosomal RNA activity. This activity was significantly reduced in extensor digitorum longus and soleus muscles of rats fed ethanol for 2 months. While the tot
