Hypokalemic periodic paralysis (HPP) is an uncommon but reversible cause of acute flaccid paralysis. It can clinically resemble neurological emergencies such as Guillain-Barré syndrome (GBS), leading to potential misdiagnosis and delays in appropriate treatment. We report a 30-year-old man who presented with sudden-onset quadriparesis following an acute febrile illness with gastrointestinal symptoms characterized by fever, myalgia, arthralgia, and vomiting. Initial neurological assessment demonstrated proximal and distal weakness without sensory involvement. Laboratory tests revealed severe hy

Thyrotoxic periodic paralysis (TPP) is an uncommon but potentially life-threatening complication of hyperthyroidism characterized by sudden, reversible muscle weakness due to an intracellular potassium shift. While hypokalemia is a typical feature, normokalemic presentations are exceedingly rare. We report a 46-year-old Hispanic woman with poorly controlled hyperthyroidism who presented with acute bilateral lower-extremity weakness and areflexia. The initial potassium level was within the normal range. The patient was clinically and biochemically thyrotoxic. She was treated with methimazole, d

Periodic paralyses (PPs) are rare skeletal muscle ion channelopathies caused by mutations in skeletal muscle sodium, calcium, and potassium channel genes. PPs can be divided into primary periodic paralyses (PPPs) and secondary PPs by the aetiology. Secondary PPs are common in hyperthyroidism, primary aldosteronism, renal tubular acidosis, or be related to other causes such as medication intake or potassium loss from the digestive or renal systems. Both conditions are characterized by episodic flaccid muscle weakness. Primary PPs are classified as Hypokalemic Periodic Paralyse, (HypoPP), normok

Hypokalemic periodic paralysis (HypoPP) is a rare disease that consists of attacks of flaccid paralysis that often occur at night or early in the morning. Some patients with HypoPP may develop permanent muscle weakness and permanent myopathy. Four phenotypes of disease have been described: 1) no symptoms; 2) periodic paralysis and absence of permanent weakness; 3) periodic paralysis and permanent paralysis; 4) permanent weakness and absence of periodic paralysis. Vacuolar alterations are present in the muscles of all patients affected by HypoPP. In general, vacuoles are heterogeneous in size a

Thyrotoxic periodic paralysis (TPP) is a rare complication of hyperthyroidism characterized by sudden-onset muscle weakness and hypokalemia due to intracellular potassium shifts. It most commonly affects young Asian men and is rarely seen in older adults or non-Asian populations. We present the case of a 52-year-old Hispanic male patient who experienced episodic bilateral limb weakness and falls over several months, ultimately diagnosed as TPP secondary to previously undiagnosed Graves' disease. Despite normal neurologic imaging and no overt hyperthyroid symptoms, thyroid function testing reve

Thyrotoxic periodic paralysis is an uncommon endocrine emergency marked by sudden muscle weakness linked to hypokalemia in the context of thyrotoxicosis. Cases have increasingly been reported worldwide, even though they are more common in Asian populations. Early detection is crucial because of the risk of life-threatening complications such as arrhythmias and respiratory failure. We present the case of a 42-year-old man with no significant medical history who exhibited sudden palpitations, diaphoresis, and anxiety, subsequently developing rapidly progressive lower extremity weakness, culminat

Mutations in SCN4A gene encoding the skeletal muscle-type voltage-gated sodium channel Nav1.4 are known to cause neuromuscular disorders. Here, we report a previously undescribed variant affecting the DIII-IV cytoplasmic linker, which is a critically important region participating in channel inactivation. The variant p.L1326P was found in heterozygous state in two members of the same family presenting mild symptoms of periodic paralysis and in vivo electrophysiological features of sodium channel myotonia. Functional characterization was performed using Xenopus oocytes co-expressing human Nav1.

Hypokalemic periodic paralysis (HypoPP) is an autosomal dominant neuromuscular disorder characterized by episodic flaccid weakness with hypokalemia. It is primarily caused by mutations in the CACNA1S gene, encoding the voltage-gated calcium channel Cav1.1, and the SCN4A gene, encoding the voltage-gated sodium channel Nav1.4. A unified pathophysiological mechanism proposes that these mutations neutralize positively charged residues in the S4 voltage-sensor segments, generating "gating pore currents" (ω currents). These currents destabilize the resting membrane potential, trigger paradoxical dep

Familial hypokalemic periodic paralysis (HypoPP) is a rare autosomal dominant skeletal muscle channelopathy most commonly caused by pathogenic variants in the CACNA1S gene. It is characterized by recurrent episodes of severe hypokalemia and transient flaccid paralysis resulting from intracellular potassium redistribution rather than true potassium depletion. Despite advances in molecular genetics, familial HypoPP remains underrecognized, particularly in regions where acquired causes such as thyrotoxic periodic paralysis predominate. We report a young adult male presenting with recurrent episod

Andersen-Tawil syndrome (ATS) is a rare, hereditary channelopathy characterized by periodic paralysis, cardiac arrhythmias, and sometimes developmental anomalies. No association with hematologic malignancies has previously been reported. We describe the case of a 27-year-old man with genetically confirmed Type 1 ATS who developed T-cell acute lymphoblastic leukemia. He was treated according to the GRAALL-2014 protocol and achieved sustained complete molecular remission without allogeneic transplantation after 3.5 years of follow-up. Management required careful adaptation to mitigate ATS-relate

Hypokalemic periodic paralysis (hKPP) is a rare neuromuscular channelopathy characterized by transient episodes of muscle weakness or paralysis associated with low serum potassium levels. It has been most commonly linked to autosomal dominant mutations in ion channel genes, specifically CACNA1S and SCN4A, which impair skeletal muscle excitability. Although hKPP typically presents in adolescent males with a positive family history, atypical cases in women may be underrecognized due to milder or less frequent attacks and non-specific symptoms between episodes. We present the case of a 31-year-ol

BACKGROUND: Variegate porphyria is a rare heme biosynthesis disorder caused by pathogenic variants in the PPOX gene. Despite being relatively well characterized, its clinical manifestations are highly variable and can mimic a broad spectrum of neurological and systemic conditions.
CASE PRESENTATION: We report the case of a young woman with recurrent episodes of muscle weakness, sometimes accompanied by respiratory dysfunction. Initially suspected to have myasthenia gravis, laboratory testing for this condition was negative. Whole-genome sequencing identified a heterozygous pathogenic variant

Hypokalemic periodic paralysis (HPP) is a rare neuromuscular disorder characterized by recurrent episodes of acute muscle weakness or paralysis secondary to hypokalemia. Attacks are often triggered by metabolic or physiologic stressors and may require critical care management. We present two cases of HPP, highlighting challenges in management. A 22-year-old male and a 42-year-old male presented with severe hypokalemia and acute flaccid paralysis requiring intensive care unit admission due to profoundly low potassium levels. Both patients required aggressive repletion with symptom resolution. D

Polymorphic ventricular tachycardia (VT), particularly in the absence of structural heart disease, has a strong genetic foundation primarily rooted in mutations affecting cardiac ion channels and associated regulatory proteins. Here, we report two patients with polymorphic VT harboring potassium channel subfamily J member 2 (KCNJ2) gene mutations. The first case is a 13-year-old girl presenting with periodic paralysis, polymorphic VT, and a prolonged QT interval. She was sequentially treated with metoprolol, verapamil, amiodarone, moricizine, and mexiletine, along with three radiofrequency cat

BACKGROUND: Hyperkalemic periodic paralysis is a rare autosomal dominant disorder of sodium channels in skeletal muscles caused by pathogenic variants in the SCN4A gene. Emerging evidence suggests an association between this disorder and mitochondrial dysfunction, observed in mice. To our knowledge, there are still no reported cases demonstrating this link in humans.
CASE PRESENTATION: We presented a case of hyperkalemic periodic paralysis caused by a heterozygous, de novo, and rare variant, NM_000334.4: c.2078T > C (p.Ile693Thr) in the SCN4A gene, reported for the first time in the Arab regi

RATIONALE & OBJECTIVE: Severe hypokalemia requires prompt management and close surveillance. Although artificial intelligence-enabled electrocardiography (AI-ECG) rapidly detects severe hypokalemia, its application for monitoring serum potassium (K+) dynamics during treatment remains unexplored. This study assessed AI-ECG performance in monitoring K+ changes during supplementation.
STUDY DESIGN: Multicenter retrospective cohort study.
SETTING & PARTICIPANTS: 191 adults with severe hypokalemia (laboratory-measured [lab-K+] ≤ 2.5 mmol/L; matched ECG-K+ < 3.5 mmol/L) and ≥1 follow-up paired mea
