Thyrotoxic periodic paralysis (TPP) is a reversible but potentially severe complication of thyrotoxicosis characterized by acute muscle weakness and hypokalemia resulting predominantly from intracellular potassium redistribution. Although classically associated with Asian populations, TPP is increasingly recognized in Latin America. We report a 38-year-old Colombian man with recurrent nocturnal proximal quadriparesis, preserved deep tendon reflexes, serum potassium of 2.3 mEq/L, and biochemical thyrotoxicosis secondary to Graves disease. In parallel, we conducted a structured review of publish

BACKGROUND: Hypokalemic periodic paralysis (HPP) presenting as acute quadriparesis is a neuromuscular emergency. While its etiology is described in general wards, its severe "critical care phenotype" in the intensive care unit (ICU) remains poorly characterized. We aimed to define this phenotype by analyzing the clinical profile, etiological spectrum, and predictors of life-threatening severity.
METHODS: A retrospective study was conducted of 12 patients (nine male, three female; median age: 31.5 years) admitted to a tertiary ICU (2015-2021) with acute quadriparesis and hypokalemia (median po

Background: Increasing evidence suggests that COVID-19 can induce or exacerbate autoimmune disorders, including immune-mediated thyroid dysfunction. The most common autoimmune thyroid diseases are Graves' disease and Hashimoto's thyroiditis; the mechanisms by which viral infections like SARS-CoV-2 trigger these diseases are not fully understood. Objectives: This study aims to systematically review published clinical evidence on the presentation, laboratory characteristics, and outcomes of autoimmune thyroid diseases after COVID-19 infection. Methods: The review followed the PRISMA 2020 framewo

BACKGROUND: Hypokalemic periodic paralysis is a type of periodic paralysis, a rare but life-threatening condition that can cause sudden onset weakness but can be reversed completely with early diagnosis and treatment. Diagnosis of hypokalemic periodic paralysis can be challenging in resource-limited settings where investigations such as genetic testing, Electromyography (EMG) and biochemical tests are not available. We present a case of hypokalemic periodic paralysis in an adult female diagnosed and treated using trigger pattern recognition, basic investigations, exclusion of differential diag

Hypokalemic periodic paralysis (HypoKPP) is a rare channelopathy that typically presents in adolescence with recurrent episodes of muscle weakness triggered by factors that lower serum potassium. We report a 37-year-old Japanese male with genetically confirmed HypoKPP caused by the rare SCN4A p.Arg672Cys variant. Despite late onset (early 30s) and infrequent attacks, the patient presented with classic flaccid paralysis and severe hypokalemia (1.8 mEq/L) triggered by exercise, alcohol consumption, and carbohydrate intake. Genetic testing confirmed the diagnosis after the condition was initially

INTRODUCTION: Thyrotoxic periodic paralysis (TPP) is an uncommon endocrine emergency caused by rapid intracellular potassium redistribution during thyrotoxicosis. Carbohydrate exposure is a recognized precipitant, but direct observation of a first paralytic attack developing during supervised diagnostic glucose testing is unusual.
CASE PRESENTATION: A 29-year-old Indian man underwent a supervised oral glucose tolerance test (OGTT) for borderline fasting dysglycemia and episodic postprandial tremulousness. He had also experienced recent weight loss, heat intolerance, and palpitations but had n

A rare late-onset presentation of episodic weakness associated with hyperkalemia is described in a 72-year-old male with concurrent early-stage chronic lymphocytic leukemia. He experienced frequent episodes of generalized muscle weakness accompanied by loss of speech, shortness of breath, and fatigue, with a documented serum potassium level of 6.5 mmol/L obtained within 30 minutes of an attack. Treatment with acetazolamide and dietary modification was associated with a substantial reduction in episode frequency and improved functional status, although persistent fatigue remained. The clinical

The severe clotting defects associated with the diagnosis of severe haemophilia A and B require a quality management and quality assurance system designed to avoid both bleeding sequelae (such as damaged joints) through early on-demand or prophylactic treatment in a home-care setting, and side-effects such as infectious diseases (hepatitis A-G and human immunodeficiency virus), allergic reactions, haemolysis and if possible inhibitor formation, by using highly purified, virus-inactivated or recombinant products in which the factor VIII and IX proteins are as natural as possible. Antipathogen defense; b-caryophyllene; vector-virus mutualism Plants respond to insect herbivory by producing and releasing distinctive blends of volatile compounds. These herbivore-induced plant volatiles (HIPVs) serve several roles, including direct defense through toxic or repellent effects on herbivores and indirect defense by attracting the herbivores' natural enemies (Bleeker et al. 2009; Veyrat et al. 2016). HIPVs also act as warning signals, informing undamaged tissues within the same plant and nearby plants of a pending insect attack (Heil and Bueno 2007). HIPVs are mostly composed of

Background: Variants in the ryanodine receptor 1 (RYR1) gene have been linked to a range of disorders, from congenital myopathy to adult-onset manifestations, with phenotypes varying from mild to severe. Methods: A retrospective review was conducted on an Israeli cohort of 36 individuals with RYR1 variants, identified through genetic testing as part of a national collaboration among multiple pediatric and adult neuromuscular clinics. Clinical features, molecular data, laboratory results, electromyographic findings, and muscle histology were analyzed. Each variant was classified according to it

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A case of a young Caucasian man presenting to the Emergency Department (ED) with lower limb weakness, anxiety, and sweating is described. Clinical and laboratory evaluations revealed severe hypokalemia (1.4 mmol/L) associated with thyrotoxicosis, leading to a diagnosis of Thyrotoxic Periodic Paralysis (TPP). After initial improvement following potassium infusion, the patient experienced symptom exacerbation. Further investigation linked the recurrence to excessive carbohydrate intake from vending machine snacks. TPP, a complication of hyperthyroidism, is extremely rare in Western countries but

In the field of equine muscle disorders, many conditions have a genetic basis. Therefore, genetic testing is an important part of the diagnostic evaluation. Validated genetic tests are currently available for 5 equine muscle disorders: hyperkalemic periodic paralysis, malignant hyperthermia, glycogen branching enzyme disease, type 1 polysaccharide storage myopathy, and myosin heavy chain myopathy. These diseases should be tested for in the appropriate breeds with clinical signs of disease or as part of breeding management. Genetic testing in veterinary medicine is not regulated, and therefore,

Patients with periodic paralysis have attacks of weakness precipitated by depolarization of muscle. Each form of periodic paralysis is associated with unique changes in serum K+ during attacks of weakness. In hypokalemic periodic paralysis (hypoKPP), the mutation-induced gating pore current causes weakness associated with low serum K+. In hyperkalemic periodic paralysis (hyperKPP), mutations increase a non-inactivating Na+ current (Na persistent or NaP), which causes weakness associated with elevation of extracellular K+. In Andersen-Tawil syndrome, mutations causing loss of Kir channel functi

BACKGROUND: Gitelman syndrome is a rare autosomal recessive disorder that affects the distal convoluted tubules of the kidneys. It often manifests through various symptoms, including muscle weakness, paresthesia, fatigue, or paralysis. Owing to the scarcity of case reports regarding Gitelman syndrome in the Middle East and North Africa region, it is imperative to spread awareness about this syndrome for prompt diagnosis. Consequently, this could drastically decrease the rate of complications and help with its management and prognosis. This case report addresses the lack of awareness surroundin

Hypokalemic periodic paralysis (HPP) is a rare disease. Due to channelopathy caused by mutations in skeletal muscle ion channels, episodes of sudden flaccid muscle weakness and hypokalemia develop as a result of various trigger factors. The present study reports the case of a 25-year-old male patient with HPP admitted with acute onset numbness and paralysis in the extremities accompanying hypokalemia (2.66 mEq/l). The patient became asymptomatic following treatment with a potassium (K) supplement and was diagnosed with HPP. The present study describes this case of HPP in an aim to remind colle

BACKGROUND: Thyrotoxic periodic paralysis (TPP) is an endocrine emergency caused by thyrotoxicosis, manifesting mainly as periodic myasthenia and hypokalemia, and posing a serious threat to the patient's health. Fatigue, strenuous exercise, alcohol abuse, high carbohydrate intake and insulin injections are common triggers of paralysis. This article reports a case of severe TPP induced by insulin injection, elucidates the characteristics and pathogenesis of the disease, analyses the risk factors for triggering TPP, and hopefully provides more clinical data for TPP patients.
